1. Mental health impact of COVID-19 among Spanish health care workers: A 3-year longitudinal study (MINDCOVID).
PMID:日期:2026-09-11We assessed the trajectory of adverse mental health outcomes of Spanish healthcare workers (HCWs) during three years after the COVID-19 pandemic initiation, and their association with distal and proximal risk factors. A prospective cohort of HCWs from 18 Spanish institutions was assessed seven times between May 2020 and June 2023 through institutional web-based surveys. Outcomes included: (1) any probable mental disorder (MDx)-depression (PHQ-8 ≥10), anxiety (GAD-7 ≥10), panic attack, post-traumatic stress disorder (PCL-5 ≥7), or substance use disorder (CAGE-AID ≥2); and (2) any suicidal thoughts or behaviors (STB, CSSRS). Risk factors comprised 11 distal (sociodemographic, professional, health-related) and 13 proximal (COVID-19 infection, work, health, financial, and family stressors) variables. Latent class growth analysis identified trajectories. Four thousand eight hundred nine were followed. Most were women (77.3%), hospital workers (57.6%), and nurses/nurse assistants (44.4%). Prevalence of any MDx declined from 45.5% (May-September 2020) to 31.2% (April-June 2023), while STB increased slightly from 8.4% to 9.4% (not significant). Common risk factors for MDx and STB were previous mental disorders, female gender, nurse assistant role, foreign-born status, primary care work, and physical comorbidities. Four trajectory classes were identified for both MDx and STB: persistent high (19.4%, 5.0%), decreasing from high prevalence (28.5%, 5.2%), increasing from low (5.9%, 4.2%), and persistent low (46.2%, 85.5%). Persistent high classes were associated with interpersonal stress, loved ones' health, lack of preparedness/protective equipment, and ethically challenging care decisions. Adverse mental health outcomes remained substantial among Spanish HCWs during three years of follow-up, about one in five showing persistently high burden. Continued monitoring and support are warranted.

2. Beyond repetitive behaviours: Recognising and treating OCD in autistic individuals.
PMID:日期:2026-09-07Obsessive-compulsive disorder (OCD) is one of the most common and impairing co-occurring conditions in autistic individuals, yet it is frequently under-recognised and undertreated. This clinical perspective examines the challenges of recognising, diagnosing, and managing OCD in autism. We discuss the overlap and important distinctions between autistic repetitive behaviours and OCD compulsions, highlighting the need for careful differential diagnosis to ensure that treatable OCD is identified without pathologising core autistic characteristics. We review the limited clinical trial evidence on autism-adapted cognitive behavioural therapy (CBT) for OCD, arguing that the scarcity of treatment trials is likely to reflect a lack of research rather than a lack of efficacy. We also consider barriers to accessing specialist care and discuss the potential of therapist-supported internet-delivered CBT to expand access to evidence-based treatment. Finally, we outline priorities for clinical practice and research, including improving recognition of OCD in autistic people, increasing their inclusion in treatment trials, and developing accessible, autism-adapted interventions.

9. Two decades of progress in schizophrenia: evolution, current insights, and future directions.
PMID:日期:2026-06-24Schizophrenia research has undergone substantial conceptual and clinical evolution over the past two decades, driven by advances in neuroscience, genetics, neuroimaging, cognition, psychopathology, public health and therapeutics. Despite these developments, major questions regarding the nature, heterogeneity, trajectory, and treatment of schizophrenia remain unresolved. In this viewpoint article, the Spanish Journal of Psychiatry and Mental Health (SJPMH) invited leading experts from different fields within schizophrenia research who have authored influential work and contributed significantly to the understanding and treatment of the disorder in recent years. Through concise and reflective perspectives, the contributors revisit some of the most relevant advances of the last two decades while discussing unresolved challenges and future directions. Together, these viewpoints provide a comprehensive overview of the evolving schizophrenia landscape and offer thoughtful and illuminating perspectives on the future of schizophrenia research and clinical care.
