1. Emergency management of postoperative airway obstruction following carotid endarterectomy: a systematic review.
PMID:日期:2026-10-04Postoperative airway obstruction after carotid endarterectomy presents a significant challenge to anaesthetists. Airway oedema, with or without haematoma, may rapidly develop with life-threatening consequences. This systematic review aims to inform best practice in the management of postoperative airway obstruction following carotid endarterectomy. A literature search was conducted for manuscripts published between 01/01/2000 and 01/07/2025. Articles describing the anaesthetic management of adult patients who developed postoperative airway obstruction following carotid endarterectomy were included. Reference lists of the reviewed articles were also scrutinised for additional relevant sources. Outcome measures were the presentation of airway obstruction, success rate of airway interventions, duration of tracheal intubation and incidence of airway-related morbidity and mortality. Risk of bias was assessed using the Newcastle-Ottawa Scale. The literature search yielded 284 articles, of which 32 full papers were reviewed. Six papers were deemed pertinent, in addition to five identified from review of references, for a total of 11 included in this systematic review (five case reports, three case series and three retrospective observational studies) detailing the management of airway compromise post-carotid endarterectomy in 84 cases. Clinical signs correlate poorly with anatomical distortion, and laryngeal oedema is generally more severe than anticipated. Prompt haematoma evacuation improved the success rate of airway management, without associated morbidity or mortality. Surgical exploration under local anaesthesia was efficacious and may avoid the need for airway intervention. Awake tracheal intubation was associated with fewer complications than asleep tracheal intubation and should be considered where time and patient cooperation allow. Emergency front-of-neck access should not be delayed for repeated attempts at tracheal intubation with a high chance of failure. Patients' tracheas should remain intubated for sufficient time after surgical exploration, allowing airway oedema to resolve prior to extubation.

2. Elective caesarean birth in a patient with autoimmune hepatitis with cirrhosis and extensive rectus sheath varices.
PMID:日期:2026-10-01Cirrhosis in pregnancy poses significant anaesthetic challenges, particularly when complicated by extensive abdominal wall varices, an exceptionally uncommon and high-risk finding with minimal published guidance. We report a 30-year-old primigravida at 36 weeks' gestation with autoimmune cirrhosis and extensive rectus sheath varices undergoing elective caesarean birth. Given the anticipated potential for major haemorrhage, robust multidisciplinary planning was undertaken with hepatobiliary surgeons for performing modified abdominal entry in a specialist theatre environment. Goal-directed coagulation treatment was guided by serial thromboelastography supporting surgical haemostasis. Postoperative recovery was uncomplicated following an admission to a specialist intensive care unit. This case discusses potential limitations of conventional coagulation tests in cirrhosis and supports viscoelastic testing for individualised targeted therapy. We highlight the importance of adapting anaesthetic planning to the competing principles of major haemorrhage preparedness and obstetric anaesthesia and emphasise the critical role of multidisciplinary coordination and surgical adaptation in reducing risk.

3. Management of intractable emergence agitation with emotional and behavioural disinhibition in a child with anxiety requiring multiple anaesthetics.
PMID:日期:2026-09-22Paediatric oncology patients require multiple anaesthetics for diagnostic and therapeutic procedures. Anxiety due to major life stressors, repeated procedures and other cancer therapy is common among these patients and may contribute to the incidence of emergence agitation. Here, we present the case of a 9-year-old child with procedural anxiety and relapsed acute B-cell lymphoblastic leukaemia requiring multiple anaesthetics for various procedures. The patient experienced severe disinhibition and agitation without delirium upon anaesthesia emergence. After conventional pharmacological and non-pharmacological strategies were unsuccessful, the combination of olanzapine premedication with the ongoing anaesthetic regimen resolved her emergence agitation on multiple subsequent anaesthetics. We suggest a novel approach for the management of intractable emergence agitation and highlight the need for early involvement of mental health providers in the unique context of iterative anaesthesia in children with cancer.

4. Performance of ChatGPT-5 on the official sample questions for the Fellowship of the Royal College of Anaesthetists Final Written examination.
PMID:日期:2026-09-20Large language models are increasingly being used in medical education, but their performance in high-stakes anaesthesia examinations is not well defined. This study evaluated ChatGPT-5 on the official sample questions for the Fellowship of the Royal College of Anaesthetists Final Written examination. ChatGPT-5 was tested using questions from the 'Guide to the Fellowship of the Royal College of Anaesthetists Examination: The Final' publication. They comprised 100 marks of Constructed Response Questions, 900 Multiple True False stems and 60 Single Best Answer questions. Performance was compared with the published pass mark for Constructed Response Questions (62/100) and historical pass ranges for multiple true false (72-75%) and single best answer questions (55-60%). Subgroup analyses were performed by topic. A memorisation effects Levenshtein detector analysis assessed whether answers were likely present in the model's training data. ChatGPT-5 scored 89/100 in constructed response questions, 746/900 in multiple true false questions, and 39/60 in single best answer questions. All scores were above the pass standards. Physiology constructed response questions scored 12/12, higher than the overall constructed response question mean. Regional anaesthesia multiple true false questions scored 42/60, lower than the overall multiple true false mean. ChatGPT-5 achieved passing-level performance across all formats in the sample set. Performance was strongest in constructed response questions and weaker in single best answer questions. Large language models may therefore aid postgraduate revision and exam preparation.

5. Spinal metastases from choriocarcinoma presenting with neurological symptoms suggestive of neuraxial complication in the postpartum period.
PMID:日期:2026-09-15We present the case of a 28-year-old woman who developed lower back pain and right-sided lower limb paraesthesia 2 weeks after an uncomplicated vaginal birth at term facilitated by epidural analgesia for labour. Initial assessment suggested postpartum nerve injury and a locally developed flow chart for the management of postpartum neurological symptoms was used to guide investigation. Urgent magnetic resonance imaging revealed widespread spinal and systemic lesions consistent with metastatic disease. Subsequent markedly elevated human chorionic gonadotropin confirmed a diagnosis of postpartum choriocarcinoma. Early recognition of red flag symptoms and adherence to a structured pathway enabled rapid diagnosis and timely oncological management.

6. Regional analgesia for chest wall injuries related to cardiopulmonary resuscitation: retrospective service evaluation and injury pattern mapping.
6. 心肺复苏相关胸壁损伤的区域镇痛:回顾性服务评估与损伤模式图谱绘制PMID:日期:2026-09-09Cardiopulmonary resuscitation frequently results in rib and sternal fractures. Regional analgesia is recommended in blunt chest trauma, but its role in injuries related to cardiopulmonary resuscitation is not well defined. This study aimed to characterise injury patterns and evaluate regional analgesia use in patients recovering from cardiac arrest. A retrospective service evaluation was undertaken at a tertiary centre over 2 years. Adults referred to the Acute Pain Service following return of spontaneous circulation were included. Data collected included patient characteristics, imaging, fracture distribution, STUdy of the Management of BLunt chest wall trauma (STUMBL) score, regional analgesia use and ventilatory requirements. Eighty-one patients with chest wall injuries related to cardiopulmonary resuscitation (24% of all cardiac arrests) were referred. Rib fractures were identified in 84% and sternal fractures in 34% of patients; median STUMBL score was 38. Anterolateral fractures and anterior fractures were the most common. Referrals occurred a median of 2 days after the cardiac arrest. Regional analgesia was delivered to 40% of patients by the third day after the cardiac arrest. Fascial plane blocks predominated (92%), with technique selection influenced by operator expertise, fracture distribution and ease of insertion in the supine position. Analgesic effect was recorded as marked or moderate in 90%. Forty-three per cent of the referrals were for patients who were mechanically ventilated, and 54% of them received regional analgesia. Chest wall injuries associated with cardiopulmonary resuscitation at our institution were common and frequently received regional analgesia. Injury patterns favoured anterior techniques, although prospective evaluation of effectiveness is required.

8. Development of a survey evaluating patient Concerns Over Peri-operative Events (COPE).
PMID:日期:2026-09-02Anxiety frequently burdens patients before surgery, potentially leading to poorer recovery. However, standard assessment tools typically quantify the intensity of this distress without understanding its root cause. This often leaves clinicians unaware of the specific fears driving a patient's anxiety. We aimed to address this deficit by developing and validating a questionnaire designed to identify individual pre-operative concerns. An iterative development process resulted in a 14-item questionnaire: Concerns Over Peri-operative Events (COPE). We conducted a prospective evaluation of 52 patients having elective surgery, measuring the questionnaire's performance against the Amsterdam Pre-operative Anxiety and Information Scale. The questionnaire performed well on metrics of validity, reliability and acceptability. Severe pain was the most reported concern. The median (IQR [range]) total COPE score was 30 (22-44 [14-63]). There was a positive correlation between the Amsterdam Pre-operative Anxiety and Information Scale and COPE scores. A classification model to predict Amsterdam Pre-operative Anxiety and Information Scale-positive status demonstrated excellent discrimination with an area under the curve of 0.86. An optimal COPE score of ≥ 34 demonstrated 88% sensitivity and 81% specificity. While future research should focus on generalisability, this novel questionnaire of patient concerns facilitates the identification of specific drivers of pre-operative anxiety. By bridging the gap between clinicians' assumptions and patient experience, the COPE questionnaire may facilitate targeted therapies and improve informed consent.

9. Epidural blood patch for persistent cerebrospinal fluid leak after epidural catheter removal in an infant.
[英文摘要] Persistent fluid leakage following epidural catheter removal is rare in infants and may indicate cerebrospinal fluid leakage, with poorly described clinical manifestations. We report a case of a 10-month-old infant who developed persistent fluid leakage from the epidural insertion site and position-dependent changes in consciousness after the removal of a thoracic epidural catheter following right hepatectomy. Brain computed tomography excluded acute intracranial pathology, and the clinical course suggested intracranial hypotension secondary to suspected cerebrospinal fluid leakage. Bed rest, pressure dressings and intravenous fluids were unsuccessful in improving the leakage or the neurological symptoms. An epidural blood patch was performed by injecting 2.5 ml (0.31 ml.kg) of autologous venous blood into the epidural space. No further leakage was observed from the epidural insertion site and the patient's neurological status rapidly returned to baseline. This case highlights the diagnostic challenges of suspected intracranial hypotension in infants and describes the clinical course following the performance of an epidural blood patch.
