Food insecurity is a lack of physical and/or economic access to sufficient, safe and nutritious food that meets dietary needs and food preferences. Disease-related malnutrition (DRM) occurs in the context of illness or its treatment and is associated with inadequate nutrient intake and the inflammatory processes of diseases or medical conditions. Food insecurity and DRM both pose significant health risks, are prevalent and have similar underlying foundations, yet minimal research has explored these connections. The objective was to explore the complex relationship between food insecurity and DRM to inform system change, research, intervention and education supporting both topics. We conducted online semistructured interviews and focus groups with clinicians, researchers/academics and staff from health systems and community organisations purposefully recruited from across Canada. A codebook supported inductive thematic analysis. Following the interviews/focus groups, participants were sent a list of research questions which they rated by priority level, actionability, feasibility and health equity considerations. Interviews (n=7) and focus groups (2; n=5) were conducted with 12 participants from four provinces. There was an even split between clinicians (n=4), researchers/academics (n=4), management (n=4) or representatives (n=3) from health system or community organisations (n=4). Half (n=6) self-identified as providing more input on DRM, and n=8 on food insecurity. The overarching theme was how structural inequities connected food insecurity and DRM. Participants discussed the need for ethical screening for both, but only if results could connect to services beyond food charity and if screening was unbiased and respectful. The need for system change was reported, alongside opportunities to support practice change through research, intervention and education. A list of research questions was generated. This study reveals that food insecurity and DRM are fundamentally linked by structural inequities, necessitating a shift from food charity toward ethical, system-wide intervention. Addressing these interconnected issues requires integrated research and policy changes to support equitable health outcomes.

Adequate physical activity (PA) promotes health, but whether it also reduces health-related expenditures has not been thoroughly assessed. We investigated the association between PA levels and individual health expenditures and health insurance deductibles. Data from the third follow-up (2018-2021) of a prospective cohort study based in Lausanne, Switzerland. PA was measured via accelerometry and categorised as <or ≥150 min/week of moderate+vigorous PA (MVPA). Information on deductibles was self-reported. Health expenditures were obtained from health insurers. Overall, 858 participants (53% females, 65.4±9.7 years) had deductible data. Participants with ≥150 min/week MVPA were more likely to choose higher health insurance deductibles than the less active group: 29.3% against 17.5%; p<0.001. However, this association was no longer significant after multivariable adjustment. Health expenditures were available for 360 participants (50.3 % females, 66.1±9.7 years). Participants with ≥150 min/week of MVPA had lower expenditures: median and (IQR) 1987 (427-5490) against 4118 (1699-8743) CHF, p<0.001. After multivariable adjustment, a nonsignificant (p=0.073) trend remained. Age, female sex and low deductible choice remained significant predictors of higher expenditures. In sex-stratified analyses, a significant inverse association was found between expenditures and MVPA: β (95% CI) -0.0034 (-0.0065 to -0.0003) in females, but not in males. Although less active individuals tend to choose lower deductibles, the association is mainly driven by clinical and sociodemographic factors. In women, each extra 30 min of MVPA per week was correlated to a 10% annual decrease in individual health expenditures, with a same borderline effect in the full sample.

Malnutrition is common in patients with cancer. Limited data exist on the association between nutritional status, chemotherapy cycles and therapeutic responses in low-resource settings. This study investigated the association between pretreatment nutritional status, chemotherapy cycles and treatment response in Addis Ababa, Ethiopia, providing insights through count data analysis. A multicentre prospective cohort study was conducted from February 2024 to June 2025, involving 400 patients with solid tumours followed from prior to chemotherapy to 1 month after completing treatment. Nutritional status was assessed using the Patient-Generated Subjective Global Assessment, and treatment response was measured using the Response Evaluation Criteria in Solid Tumours (V.1.1). Multivariable logistic and Poisson regression analyses were used to determine the association between malnutrition, treatment response and number of chemotherapy cycles. At baseline, 39% of the patients exhibited severe malnutrition. Of the 400 recruited participants, only 204 (51%) completed six or more chemotherapy cycles, and 19.6% showed progressive disease (PD). In bivariate analysis, severe malnutrition was associated with PD (crude OR = 2.67 (95% CI: 1.12 to 6.1)). In the multivariable logistic regression analysis, low dietary diversity (adjusted OR (AOR) = 3.63 (95% CI: 1.19 to 11.05)) and low performance status (AOR = 4.3 (95% CI 1.35 to 13.67)) were associated with PD. Additionally, based on Poisson regression, patients with severe malnutrition received 17% fewer chemotherapy cycles than well-nourished patients (incidence rate ratio = 0.83 (95% CI: 0.73 to 0.93)). Based on a prospective cohort study, malnutrition may not predict chemotherapy response. However, it was associated with a reduced number of chemotherapy cycles. Therefore, early nutritional assessment and intervention are imperative to reduce treatment dropouts. Patients should also be encouraged to consume a diverse diet rich in essential nutrients to improve their treatment responses.

Dietary patterns that limit or avoid animal products contribute fewer greenhouse gases and require less energy than diets containing more animal products. However, quantifications of these outcomes on a vegan diet in real-life conditions still need to be explored. In a randomised controlled trial, a low-fat vegan diet supplemented with soybeans led to a significant reduction in hot flashes in postmenopausal women. In this secondary analysis of that study, we hypothesised that the intervention diet would have a reduced environmental impact, greenhouse gas emissions (GHGEs) and cumulative energy demand (CED). 3-day diet records were analysed at baseline and at the end of the 12-week study, and we linked dietary intakes to environmental footprints. Compared with no change in the omnivorous control group, we found that the vegan group had a decrease in GHGEs (p <0.001) of 35% (-662.7 g COeq/person-day) and a decrease in CED (p =0.002) of 34% (-4956 kJ/person-day). These reductions in environmental impact were primarily due to reduced consumption of meat and dairy products. Adoption of a soybean-supplemented low-fat vegan diet significantly reduced cumulative energy demand and GHGEs, important drivers of climate change. NCT04587154.

Most Indian women have a low folate and vitamin B status that can progress to clinical deficiency and contributes to over 100 000 births annually with neural tube defects (NTDs). A common, widely accepted, Indian food vehicle for fortification with folate and vitamin B has long remained elusive. Earlier, we employed folate and vitamin B-fortified teabags to brew a daily cup of black tea and documented improvement in serum folate and vitamin B concentrations among women from Maharashtra state within 60 days. Because most Indian households prefer hot-brewed ('loose') black tea prepared by the 'crush, tear, curl' (CTC) method in large commercial tea factories, we tested the feasibility of directly fortifying CTC tea with folate and vitamin B and assessed its efficacy in improving the folate and vitamin B status among women from Assam state within 90 days. Two groups of women studying nursing (n=30) or pharmacy (n=30) at Assam Medical College and Hospital consumed a daily cup of hot tea prepared from 2 g of either unfortified CTC tea (control group) or vitamin-fortified CTC tea containing 1 mg folate and 1 mg vitamin B (experimental group) for 90 days. We then compared their pre-interventional versus post-interventional changes in serum folate and vitamin B status, iron status and haemoglobin concentration. At baseline, 89% of all women had low folate status (serum folate ≤5 ng/mL) and 72% had low vitamin B status (serum vitamin B ≤300 pg/mL). After 90 days daily consumption of unfortified CTC tea, the control group had clinically insignificant mean increases in serum folate of 1.3 ng/mL and serum vitamin B of 1 pg/mL. By contrast, the experimental group consuming vitamin-fortified CTC tea exhibited a mean rise in serum folate of 5.3 ng/mL (95% CI 3.9 to 6.8; p<0.001) and serum vitamin B of 194.6 pg/mL (95% CI 154.7 to 234.5; p<0.001). Moreover, 28/30 women receiving vitamin-fortified CTC tea had a post-interventional rise in mean serum folate of 9.2±3.6 (SD) ng/mL, and 25/30 women normalised their serum vitamin B ≥300 pg/mL. However, only a minority achieved the higher serum folate concentration required to reduce their risk of NTDs. Despite borderline low normal iron status in most women, there was no adverse impact of CTC tea-consumed ~2 hours between meals-on either per cent-transferrin saturation or haemoglobin concentration. Consumption of a daily cup of hot brewed folate and vitamin B-fortified CTC tea is feasible, efficacious, and safe to clinically normalise the folate and vitamin B status of Indian women within 90 days in Assam. Therefore, CTC tea is an ideal and eminently scalable food vehicle for fortification with these vitamins. The next challenge is to determine the optimum duration and/or dose of vitamins required to fortify CTC tea sufficient to lower the risk of NTDs among women across India. CTRI/2022/10/046289.

While a balanced diet is crucial for human health, the food system is also a key driver of climate change. The EAT-Lancet Commission developed the Planetary Health Diet (PHD), providing guidelines for a healthy and sustainable diet. The aim of this study was to examine the association of PHD adherence with mortality in Switzerland. In this analysis, data from the cross-sectional national nutrition survey menuCH (2014-2015, n=2057) were linked with Swiss mortality and census data (2015-2019). By applying the multiple source method, habitual food intake was calculated based on the reported dietary intakes of two 24- hour dietary recalls and information on food avoidance obtained through a self-administered questionnaire. PHD adherence was quantified using a score that was used to categorise participants (G1, 0-5 points; G2, 6 points; G3, 7 points; G4, 8-14 points). Regression models were fitted to assess the association of the PHD score with all-cause and cause-specific mortality. Spatial autocorrelation of residuals was evaluated using Moran's I statistic. A higher PHD score was associated with lower mortality from diseases of the circulatory system (per 1-point increment: rate ratio=0.98 (95% CI 0.97 to 1.00); G4 vs G1, 0.94 (0.88 to 1.00)). In female participants, a higher PHD score was associated with a lower risk of mortality from diseases of the circulatory system (per 1-point increment: 0.97 (0.94 to 0.99)). In male participants, a higher PHD score was associated with lower all-cancer mortality risk (per 1-point increment: 0.98 (0.96 to 1.00); G4 vs G1, 0.92 (0.86 to 0.99)). This study generally indicates an inverse association between PHD adherence and mortality risk, especially for diseases of the circulatory system, although the associations varied among subgroups. This study provides further evidence on the potential beneficial effects of sustainable and balanced diets on human health.

To determine the sodium, potassium and iodine excretion and blood pressure (BP) of New Zealand (NZ) school children 8-13 years old and compare to recommended guidelines. A cross-sectional study was undertaken in Auckland and Dunedin schools between July 2022 and February 2023. Sodium and potassium excretion were assessed using 24-hour urine, iodine using urine concentration and BP using standard methods; all values were compared with relevant international guidelines. 75 children from five schools took part (n=37 boys, 29 girls and 9 not stating gender). Mean (SD) 24-hour sodium excretion (n=59 complete samples) was 2420 (1025) mg, potassium excretion was 1567 (733) mg and the sodium-to-potassium molar ratio was 3.0 (1.6). 32% (19/59) of children met the WHO sodium guideline (<2000 mg/day), 2% (1/59) met the potassium guideline (≥3500 mg/day) and none met the sodium-to-potassium molar ratio guideline (<1:0). Median (25th, 75th percentile) urinary iodine concentration (n=55 complete samples) was 129 (90, 163) µg/L with 65% (36/55) above the WHO criteria for adequate iodine nutrition (≥99 µg/L). Mean (SD) systolic and diastolic BP (n=74) were 105 (10) mm Hg and 67 (9) mm Hg, respectively; 32% (21/65) of boys and girls with age and height measures had a systolic and diastolic BP measure >90th percentile for their gender, age and height. This is the largest cross-sectional study using gold-standard methods to assess sodium and potassium excretion in NZ children. Findings suggest many NZ children could be at risk of developing cardiovascular disease later in life, and some are at risk of complications from low iodine intake. A national sodium reduction strategy and policies to increase potassium intake in NZ children are urgently needed, alongside adequate monitoring and a review of bread fortification as the only strategy for increasing children's iodine intake in NZ.

Obesity is a growing global epidemic and is rapidly increasing in low- and middle-income countries like India. The rising rates of obesity and related health problems in India demand urgent, practical interventions involving healthy eating, physical activity and supportive community-based approaches. This study aims to evaluate the impact of nurse-led intervention (NLI) on weight reduction among obese adults in urban Indian communities. An exploratory sequential mixed methods design was adopted, combining qualitative focus group discussions (FGDs) and in-depth interviews (IDIs) with a quantitative randomised controlled trial (RCT). Adults aged 18-50 years were screened for obesity, recruited for the RCT and randomised into either the NLI arm, which was designed by experts using findings from the FGDs, or the general care group, which received routine care through primary health centres. Following RCT, postintervention IDIs were conducted with participants who achieved 5% or more weight loss to explore the factors that facilitated weight loss. Descriptive and inferential statistics were used. Screening of 982 adults revealed a high prevalence of obesity, particularly among women aged 29-50 years. The key barriers identified and addressed included lack of motivation, time and awareness, while the enablers were family support and community resources. At 12 months, 71% of participants in the NLI group achieved 5% or more weight loss, with statistically significant improvements in metabolic, dietary and physical activity outcomes. Post RCT IDIs revealed that motivation, behaviour change and social support were key drivers of sustained weight loss. The NLI guided to meaningful weight loss and improved health indicators among obese adults in Puducherry. The findings highlight the potential of culturally adapted, nurse-led programmes and recommend broader implementation through integrated community health strategies. CTRI/2021/12/038785).

When extracorporeal membrane oxygenation (ECMO) was first introduced for paediatric patients, lipid emulsions were mostly administered directly into the ECMO circuit. However, clinicians have observed that lipid emulsions tend to separate and agglutinate within the circuit, raising concerns about potential complications. Many institutions therefore opted to exclude lipids from parenteral regimens, inadvertently leading to inadequate nutritional support for these critically ill patients. This study measured the caloric deficit resulting from the lack of lipid provision to paediatric ECMO patients in relation to the patients' basal energy expenditure (BEE) and explored the resulting nutritional outcomes. This was a retrospective, single-centre study conducted at a tertiary care hospital. All paediatric ECMO patients receiving parenteral nutrition (PN) from the period of April 2020 to April 2023 were included in the study.Twenty-three patients were identified. Of these, 18 (78.26%) did not achieve the target BEE. The average calorie deficit of all patients was 40.23% of the calculated BEE (SD=17.468%). Of the patients who had a calorie deficit, eight (50%) died. No significant differences were found in patients' weights before and after stopping PN (p=0.3594). This study explored the challenge of meeting the nutritional needs of paediatric ECMO patients when lipids are excluded from PN. Significant caloric deficits require alternative strategies to prevent patient malnourishment without affecting the ECMO circuit's functionality.
