1. Cost-Effectiveness Analysis of Ribociclib plus an Aromatase Inhibitor (AI) Versus AI Alone in the Adjuvant Treatment of Stage II and III Pre- and Postmenopausal Women with HR+/HER2- Early Breast Cancer (EBC): A Canadian Payer Perspective.
PMID:日期:2026-10-02Cyclin-dependent kinase 4 and 6 (CDK4/6) inhibitors have transformed the treatment of hormone receptor-positive (HR+) human epidermal growth factor receptor 2-negative (HER2-) breast cancer. Ribociclib plus an aromatase inhibitor (AI) was approved in Canada as adjuvant treatment for high-risk stage II-III early breast cancer (EBC) based on the phase 3 NATALEE trial, demonstrating improved invasive disease-free survival (iDFS) versus AI treatment alone. This study evaluated the cost-effectiveness of ribociclib plus AI versus AI alone and tamoxifen from a Canadian public payer perspective. A non-homogeneous, semi-Markov cohort model with six health states (iDFS, second primary malignancy, non-metastatic recurrence, remission, distant recurrence [DR], and death) was developed, using Ontario-based costs (Canadian dollars) and a lifetime horizon. Transition probabilities from iDFS were derived from NATALEE trial data; other transitions were informed by published sources. DR was stratified by endocrine therapy (ET) sensitivity based on recurrence timing (≤ 12 months post-ET: resistant; > 12 months: sensitive), with outcomes and costs informed by prior health technology assessment evidence in metastatic disease. Results were reported as incremental cost-effectiveness ratios (ICERs) per quality-adjusted life-year (QALY) gained. Ribociclib plus AI yielded a probabilistic ICER of $45,097/QALY versus AI alone; tamoxifen was dominated by AI alone. Sensitivity analyses showed results were most sensitive to iDFS hazard rates and ribociclib drug costs. In the probabilistic analysis, ribociclib was preferred in 58.5% of simulations at a $50,000 willingness-to-pay threshold. Ribociclib plus AI is cost-effective for HR+/HER2- EBC with an ICER of $45,097, below the $50,000 willingness-to-pay threshold versus AI or tamoxifen alone.

2. Generalized Pustular Psoriasis: A Cost-of-Illness and Health-Related Quality of Life Study in Italy.
PMID:日期:2026-10-02Generalized pustular psoriasis (GPP) is a life-threatening rare disease, with an estimated prevalence of 1-3 cases per million individuals in Italy. Despite its clinical severity, evidence on the economic impact of GPP remains limited. This study aimed to estimate the annual societal economic burden of GPP in Italy and assess the health-related quality of life (HRQoL) of patients with GPP. A prevalence-based, probabilistic cost-of-illness model was developed using a societal perspective. Data were drawn from primary and secondary sources, including published and grey literature, expert opinion, and a socio-economic questionnaire administered to Italian patients with GPP. Patient-reported HRQoL was assessed using the Psoriasis Disability Index (PDI). Parameter uncertainty was addressed through both deterministic and probabilistic sensitivity analyses. Our study estimated that the annual economic burden per GPP patient in Italy ranged from €24,914 to €25,270 (total societal cost: €2.94-€2.98 million/year), substantially exceeding the costs associated with plaque psoriasis. While the majority of costs were attributable to the consumption of healthcare resources, more than one-third of the total burden was due to indirect costs. PDI data revealed a high level of perceived disability among patients with GPP, particularly in relation to daily activities and interpersonal relationships. Our analysis indicates that GPP imposes a considerable societal economic burden in Italy and markedly affects patients' quality of life. The substantial direct and indirect costs of GPP, which far exceed those of plaque psoriasis, underscore the importance of recognizing GPP as a distinct clinical entity rather than a subtype of plaque psoriasis. This distinction could support the development of more targeted management strategies and health policies for GPP (as a rare disease), including disease-specific treatment approvals and reimbursement.

3. Healthcare Utilization and Direct Medical Costs of Hepatocellular Carcinoma Treatment - A German Claims Data Study of 2778 Patients.
PMID:日期:2026-10-02Hepatocellular carcinoma (HCC) is a cancer with rising incidence and economic burden. However, evidence on healthcare resource utilization (HCRU) and associated costs is scarce in Europe. This study aims to determine HCRU and costs of HCC from a German third-party payer's perspective. This longitudinal claims study (BARMER statutory health insurance) included patients with HCC (ICD-10-GM C22.0; 2016-2020). HCRU and costs within 2 years after diagnosis were analyzed descriptively and reported per patient (PP) and per patient per month (PPPM). The cohort included 2778 patients. Mean age was 71.9 years (SD ±9.7); 69% were male. Total cost within 2 years after diagnosis was approximately €105 million, with mean cost PP of €37,970 (SD ±€43,840), primarily due to inpatient stays (67%). Median costs PP were highest for patients receiving liver transplantation (€126,943; interquartile range [IQR]: €104,340-€167,205) and lowest for patients without any tumor-specific therapy (€10,132; IQR: €5875-€19,663). Considering the observation period, costs PPPM were lowest for liver resection (€2434; IQR: €1282-€5425) and thermal ablation (€2349; IQR: €1233-€4316), whereas costs PPPM were highest for patients without tumor-specific HCC treatment (€5984; IQR: €2764-€10,908). Similar patterns were observed in the HCRU assessment. In this German claims data analysis, PP costs and HCRU in patients with HCC were higher among those who underwent liver transplantation and lower among those without tumor-specific treatment. However, after adjustment for follow-up duration, the high costs PP among patients receiving curative-intent treatments were significantly relativized. These findings support structured HCC surveillance to potentially improve clinical and economic outcomes.

4. Health Outcomes, Healthcare Resource Use, and Economic Burden of Invasive Meningococcal Disease in the United States Among Children Aged <11 Years.
PMID:日期:2026-09-21Although uncommon, invasive meningococcal disease (IMD) is a serious and life-threatening illness. In the United States (US), incidence is highest among infants aged <1 year, but research characterizing pediatric IMD burden is limited. This study aimed to evaluate health outcomes and economic burden (healthcare resource utilization [HCRU] and costs [associated with health outcomes and HCRU]) in US individuals aged <11 years with IMD and without IMD, and their caregivers, from the patient and payer (commercial and Medicaid insurance) perspective. Retrospective claims analysis utilizing real-world data (1/1/2005-12/31/2022) from the MarketScan Commercial and Multi-State Medicaid Databases compared health outcomes, HCRU (e.g., inpatient and outpatient admissions, surgical procedures, pharmacy claims), and costs (associated with health outcomes and HCRU) in commercially and Medicaid-insured infants (aged <1 year), toddlers (aged 1-4 years), and children (aged 5-10 years) with IMD versus those without IMD (matched 1:5), and their caregivers. Length of follow-up was variable, divided into acute/post-acute phases for individuals with IMD. Infants, toddlers, and children with IMD versus without IMD generally experienced more negative health outcomes and HCRU, and higher associated costs. Common health outcomes among infants with IMD included neurological, physical, and neurodevelopmental/mental health disorders. In the commercially insured cohort, infants with IMD had more inpatient admissions, specialist office visits, and surgical procedures than those without IMD. In the Medicaid-insured cohort, HCRU was higher for infants with IMD versus without IMD. Costs associated with health outcomes and HCRU were higher for infants with IMD versus those without IMD, regardless of insurance cohort. Health outcomes, HCRU, and HCRU-associated costs did not differ for caregivers to individuals with IMD versus without IMD. In this retrospective claims analysis from a dual US patient and payer perspective, infants, toddlers, and children with IMD experienced substantial negative health outcomes and economic burden, regardless of insurance status. Preventive interventions in childhood may lessen the impact of IMD on individuals, families, and healthcare systems.

5. Economic Burden of Inappropriate Antibiotic Prescribing Attributable to Dengue in Singapore.
PMID:日期:2026-09-15Dengue has a clinical presentation similar to that of bacterial infections, creating diagnostic uncertainty that may lead to inappropriate antibiotic prescribing. To estimate the magnitude of inappropriate antibiotic prescribing attributable to dengue in Singapore and assess the associated economic burden from healthcare system and societal perspectives over the period 2015-2024. A decision tree model was developed to determine costs associated with 142,525 laboratory-confirmed dengue cases reported in Singapore during 2015-2024. National surveillance data and prescribing rates informed model inputs. Costs were estimated from healthcare system and societal perspectives and expressed in 2025 Singapore dollars (SGD). Deterministic and probabilistic sensitivity analyses were conducted. The model estimated 7055 antibiotic prescriptions, of which 6866 were inappropriate; 79.6% of inappropriate prescriptions occurred in ambulatory care. The cumulative burden associated with the 2015-2024 caseload was SGD 14.32 million from the healthcare system perspective and SGD 16.13 million from the societal perspective. Prolonged hospitalisation was the largest cost component followed by adverse drug events due to antibiotic use. In the probabilistic analysis, the mean societal burden was SGD 16.16 million, with a 95% uncertainty interval of SGD 2.78-49.60 million. In this analysis, inappropriate antibiotic prescribing attributable to dengue was associated with an estimated cumulative economic burden of SGD 14.32 million from the healthcare system perspective and SGD 16.13 million from the societal perspective in Singapore during 2015-2024, expressed in 2025 values. Direct medical costs accounted for most of the burden, and the findings support further evaluation of approaches to reduce unnecessary prescribing.

6. Budget Impact of Prophylactic Letermovir to Prevent Cytomegalovirus Disease in High-Risk Adult Kidney Transplant Recipients.
PMID:日期:2026-09-12Cytomegalovirus (CMV) causes serious health problems in people with weakened immune systems, particularly those undergoing kidney transplantation. CMV can severely impact transplant outcomes through increased complications, graft loss, morbidity, and mortality. Valganciclovir (VGCV) is a commonly used prophylaxis after kidney transplantation for high-risk patients, but is known to induce leukopenia/neutropenia (LN). In the MK-8228-002 phase III trial, letermovir prophylaxis was non-inferior to VGCV for CMV prevention among high-risk adult kidney transplant recipients, but was also associated with significantly lower LN. This study aimed to evaluate the budget impact of introducing letermovir as a CMV prophylaxis drug for kidney transplant patients, with an increasing market share over 5 years compared with a market scenario with VGCV only, from a United States (US) payer perspective. A budget impact analysis was conducted from a US payer perspective, modeling a hypothetical health plan of 1,000,000 members, including 18 high-risk kidney transplant patients per year. The model compared a market scenario with VGCV only (without letermovir) versus a scenario in which letermovir is introduced into a healthcare plan with an increasing market share over 5 years. The model incorporated direct medical costs associated with prophylaxis drug acquisition and administration, LN events, and the downstream effects of LN events (e.g., CMV DNAemia, opportunistic infections, graft rejection/loss). Modeled outcomes included total budget impact and incremental cost per member per month (PMPM, 2025 USD). One-way sensitivity analysis (OWSA) was also conducted. The annual total costs without letermovir were US$1,628,307 for each year and ranged from US$1,604,558 (year 1) to US$1,596,297 (year 5) in the scenario with letermovir. Incremental PMPM ranged from - US$0.0020 in year 1 to - US$0.0027 in year 5, indicating cost savings associated with letermovir in each year. The deterministic sensitivity analysis identified the rate of having LN events on VGCV as the most influential driver of the budget impact results, followed by the annual management cost of LN events and LN-associated events. This analysis provides evidence on the potential impact of introducing letermovir into the US market. While the acquisition cost of letermovir is higher than that of VGCV, there are estimated cost savings, primarily driven by the prevention of LN events and their associated downstream clinical consequences, suggesting that the savings from LN avoidance outweigh the incremental prophylaxis costs of introducing letermovir. However, the overall budget impact remains minimal and should be interpreted in the context of the evolving evidence base.

7. Threshold Price of Belantamab for Relapsed/Refractory Multiple Myeloma in China: A Cost-Effectiveness Analysis.
PMID:日期:2026-09-11Multiple myeloma is a life-threatening hematologic malignancy with a rising incidence and significant economic burden, driven largely by costly novel therapies. This study estimated the value-based price of belantamab, a novel agent for relapsed or refractory multiple myeloma, from the perspective of the Chinese healthcare system. A three-state partitioned survival model was constructed to compare belantamab plus bortezomib and dexamethasone (BVd) versus daratumumab plus bortezomib and dexamethasone (DVd) for the treatment of relapsed or refractory multiple myeloma over a lifetime horizon. Efficacy data were based on the DREAMM-7 trial (ClinicalTrials.gov identifier: NCT04246047). Cost inputs included the drug acquisition and administration, subsequent treatment, monitoring for post progression, disease evaluation and laboratory testing, and serious adverse events management. Health utilities were derived from published studies. Both costs and quality-adjusted life years (QALYs) were discounted at an annual rate of 4.5%. Probabilistic sensitivity analysis with 10,000 Monte Carlo simulations was conducted to estimate the threshold price of belantamab at a willingness-to-pay (WTP) threshold of twice China's 2025 GDP per capita (US$29,444 per QALY). The BVd regimen yielded 6.15 QALYs compared with 4.51 QALYs for the DVd regimen. The probabilistic mean threshold price of belantamab was US$5.73/mg (95% CrI 2.16-8.89) at a WTP threshold of US$29,444/QALY. The threshold price of belantamab for BVd to be cost effective over DVd is US$5.73/mg at a WTP threshold of twice China's GDP per capita, indicating a need for a very substantial price reduction of approximately 98% from the US list price. This estimate provides evidence-based reference for future national price negotiation and reimbursement decisions in China.

8. A Cost-Effectiveness Analysis for Treatments of Patients with Early Stage Huntington's Disease in the USA.
PMID:日期:2026-08-28Huntington's disease (HD) is a rare neurodegenerative condition caused by mutations in the huntingtin gene. Emerging therapies such as Tominersen and AMT-130 have potential to treat HD, highlighting the importance of evaluating their cost-effectiveness. This study evaluates the cost-effectiveness of Tominersen and AMT-130 versus standard of care (SoC) for early stage HD in the US. A Markov model with four health states (early, middle, late, and death) was developed to estimate lifetime costs and benefits from a modified societal perspective, with death as an absorbing state. The model used 1-year cycle length with half-cycle correction, and an annual discount rate of 3% was used for all future costs and utilities. Health benefits were measured in quality-adjusted life years (QALYs). All costs were adjusted to 2024 US dollars. As per the incremental cost-effectiveness ratio (ICER) value assessment framework, a willingness-to-pay threshold (WTP) of $500,000/QALY gained was considered given that HD is a rare disease. Scenario analysis assessed the impact of adding value of hope to the base-case utility values. Deterministic and probabilistic sensitivity analyses were conducted to capture uncertainty across all model parameters. Compared with SoC, Tominersen increased costs by $2.11 million and QALYs by 0.93, resulting in an ICER of $2.28 million per QALY gained, whereas AMT-130 increased costs by $1.49 million and QALYs by 5.23, resulting in an ICER of $285,703 per QALY gained in the base case. Probabilistic analysis showed AMT-130 had 80% probability of being cost-effective at $350,000/QALY gained. From a modified US societal perspective, this analysis suggests that AMT-130 may be cost-effective relative to SoC, whereas Tominersen exceeds the WTP threshold of $500,000 per QALY gained. Further evidence on long-term effectiveness and durability is needed to inform policy and reimbursement decisions.

9. The Price of Absence: Productivity Costs and Sickness-Absence Episodes Attributable to Mental and Behavioral Disorders in the Brazilian Federal Workforce - A Nationwide Time-Series Study (2012-2024).
9. 缺勤的代价:巴西联邦劳动力中归因于精神和行为障碍的生产力成本和因病缺勤事件——一项全国时间序列研究(2012-2024)PMID:日期:2026-08-24Mental and behavioral disorders (MDs) represent a leading cause of sickness absence among Brazilian public servants, generating substantial social and economic burdens. Despite their impact, comprehensive nationwide assessments of productivity costs attributable to MDs over extended periods remain scarce. The aim of this study was to estimate the productivity costs associated with MDs (ICD-10 Chapter V) among Brazilian federal civil servants from 2012 to 2024 compared with the four other most prevalent non-mental causes of absence. We conducted a retrospective longitudinal study using administrative data from SIAPE-SAÚDE (n = 471,507 workers/year average). Outcomes were classified according to ICD-10, focusing on MDs (F00-F99) and comparing them with the four other most prevalent non-mental causes. Productivity costs were estimated using the Human Capital Approach, calculating lost workdays (22 days per 30-day leave) multiplied by daily remuneration. Annual episode rates, average leave duration, and mean costs were stratified by region, education, gender, and age. All monetary values were adjusted to December 2024. MDs accounted for 3.94 billion Brazilian Real (BRL) (38% of total costs among leading causes), representing 13.9 million days lost. Costs were concentrated in mood disorders (F30-F39: BRL 2.04 billion) and neurotic/stress-related disorders (F40-F48: BRL 1.49 billion). Episode rates increased from 42.3 to 83.8 per 1000 employees (2012-2019), declined in 2020 (39.4), then recovered to 85.8 in 2023. Women showed higher episode rates (particularly those aged ≤ 30 years: ≈ 160 per 1000), while men exhibited longer absences (44.7 vs 37.5 days) and higher per-case costs (BRL 11,720 vs BRL 8905). Marked increases occurred in autism spectrum disorders (+ 1673%) and attention-deficit/hyperkinetic disorder (ADHD) (+ 331%). MDs impose a substantial and persistent burden of productivity costs on the Brazilian federal public service, with rising sickness-leave episode rates over the period and pronounced gender, educational, and regional disparities, requiring targeted, equity-oriented occupational health policies.

10. Cost-Effectiveness of an Artificial Intelligence as a Medical Device (AIaMD) for Triaging Patients Presenting to Primary Care with Concerns that They Have a Skin Cancer: A Modelling Study.
PMID:日期:2026-08-18Health services are struggling to cope with the growing numbers of people coming with skin lesions they are worried could be cancer. Using artificial intelligence (AI) to assist in the triage process is one potential approach. Deep Ensemble for the Recognition of Melanoma (DERM) is one such AI device which has achieved Conformité Européene (CE) Class III regulatory approval in the UK and Europe. The effectiveness and cost-effectiveness of DERM have already been carefully examined post-referral. This study examines the cost-effectiveness of DERM pre-referral in the community setting. We modified a decision-analytic model developed for post-referral evaluation of DERM to the community setting. The model compared standard care (SC) with two DERM-based strategies: with second reading (2R) when discharge was recommended (DERM_2R) and without it (DERM_autonomous). The outputs were numbers of diagnostic outcomes, numbers of key events (GP appointments, DERM assessments, hospital referrals), costs and quality-adjusted life-years (QALY). The cost-effectiveness metrics were incremental cost per QALY gained. Analysis was conducted in the UK from an NHS and personal social services perspective, with a life-time horizon up to a maximum of 100 years, with uncertainty assessed mainly using deterministic sensitivity analyses. We report three base-case results on the basis of different assumptions regarding GP diagnostic sensitivity and specificity. When GPs operate with maximised sensitivity as suggested by the literature, SC is the most cost-effective option. SC dominates DERM_autonomous and DERM_2R has an incremental cost-effectiveness ratio (ICER) relative to SC of £30,370. If GPs operate with maximised specificity, DERM_autonomous dominates SC and DERM_2R has an ICER of £397. If GPs diagnose cancer with a performance back calculated from NHS routine data, the cost-effectiveness of both DERM options dominate SC. The priority should be for better estimates of GP accuracy to be obtained to reduce uncertainty in the estimates of cost-effectiveness. The results presented do, however, show that it is plausible that DERM could be cost-effective justifying use in practice with further data collection.
