1. Statin-induced myopathy is rare.
PMID:日期:2026-08-18Statin-induced myopathy (SIM) is a rare but serious complication of statin therapy. Immune-mediated necrotising myopathy linked to anti-3-hydroxy-3-methylglutaryl (HMG)-CoA reductase (anti-HMGCR) antibodies requires timely recognition. This study examined the occurrence and documentation of SIM and the use of antibody testing in a regional cohort. This was a register-based study using data from regional registries and medical records (2020-2024). Patients with creatine kinase (CK) > 10 × the upper limit of normal while receiving statin therapy were identified, characterised and compared, with a special focus on anti-HMGCR antibody testing and documentation of statin intolerance. Among 1,003 patients, 32 (3.2%) were considered to have SIM. These patients had a higher median CK than other patients (7,800 versus 3,783 U/l; p less-than 0.001). High-intensity therapy was more frequent in SIM (72% versus 52%; p = 0.02), with a prevalence ratio of 2.34 (95% CI: 1.09-5.00). Anti-HMGCR testing was performed in 56% of SIM cases; antibody-positive patients had lower CK and higher eGFR. Documentation of statin intolerance in the electronic record CAVE field (adverse drug reaction) was present in only 59%. Severe SIM is uncommon (4.3 cases/100,000 patient-years), but documentation and diagnostic testing are inconsistent. Systematic recording and broader antibody testing may improve patient safety. 2. The incidence of thyroid cancer in Danish children, adolescents and young adults.
2. 丹麦儿童、青少年和年轻成人甲状腺癌的发病率PMID:日期:2026-08-18Previous Danish data (1980-2014) showed a significant increase in thyroid cancer incidence among children and adolescents. This study examined whether this trend persisted during the most recent decade. All patients aged 0-24 years diagnosed with thyroid cancer in Denmark between 2015 and 2024 were identified in the Danish Cancer Registry. Crude and age-specific incidence rates (IRs) per 100,000 were calculated for children/adolescents (0-17 years) and young adults (18-24 years), stratified by sex and histological subtype. Trends were analysed using Joinpoint regression. A total of 159 cases were identified; 81% were female. Median age at diagnosis was 16 years in the 0-17-year group and 22 years in the 18-24-year group. Papillary carcinoma was the most common subtype in both groups (68% and 82%). From 2015 to 2024, overall IRs increased non-significantly from 0.65 to 0.94 per 100,000. Across 1980-2024, the incidence increased significantly in young adults (average annual percentage change (AAPC) = 3.4%) and in the total population (AAPC = 2.8%), but not in children/adolescents. IRs increased with age, reaching 4.89 among 24-year-old females and 1.01 among males. The incidence of thyroid carcinoma in Danish children and young adults has remained stable during the 2015-2024 period, with persistent female predominance and papillary carcinoma as the dominant subtype. Long-term trends continue to show an increasing incidence overall, mainly driven by an increasing incidence among young adults. None. Not relevant.

3. Limited value of routine chest X-rays before biologic therapy in CRSwNP.
PMID:日期:2026-08-18Biologic therapies for chronic rhinosinusitis with nasal polyps (CRSwNP) have been approved in Denmark since 2022. Chest X-ray (CXR) was implemented by the Global Airways Steering Group as pre-treatment screening to identify pulmonary conditions and monitor adverse effects. This study evaluates the utility of routine CXR screening before initiating biological therapy in patients with CRSwNP. Baseline data, including chest X-ray, were collected prospectively in the nationwide Global Airways Registry (approved by the Danish Research Ethics Committee R. no.: H-21020685) and analysed in May 2025. A total of 511 patients (335 males, 176 females; mean age 46.5 ± 14.6) years were identified and included in this study; 247 patients were initiated on biologic therapy. In the treatment group, 136 (55%) had normal CXRs, and 12 (5%) had abnormal findings. Among the 264 patients who did not receive biologic treatment, 143 (54%) had normal CXRs and ten (4%) demonstrated abnormal findings. Routine baseline CXR screening before biologic therapy in patients with CRSwNP identified few abnormal findings. Accordingly, routine CXR is likely to offer limited clinical benefit. 4. 酒精戒断症状治疗的争议PMID:日期:2026-08-18With the widespread use of prostate-specific membrane antigen (PSMA)-PET/CT in prostate cancer management, incidental thyroid findings are occasionally detected. However, the malignancy risk associated with focal PSMA-PET/CT-positive thyroid incidentalomas remains unclear, and most patients are therefore referred to a fast-track cancer pathway. This study aimed to determine the incidence and malignancy risk of focal PSMA-PET/CT-positive thyroid incidentalomas and to assess whether routine fast-track referral is justified. In this retrospective cohort study, all PSMA-PET/CTs performed between January 2019 and December 2023 were reviewed. Patients with focal thyroid PSMA uptake were included. Medical records were examined to collect data on imaging and subsequent evaluation at an ear, nose and throat department, including ultrasonography, thyroid function tests, cytology, histopathology and follow-up. The malignancy risk was determined based on cytological and histopathological findings as well as clinical follow-up. Among 1,477 PSMA-PET/CTs, thyroid uptake was identified in 80 (5.4%), of which 42 (2.8%) were focal. Forty patients with focal uptake were included in the study. Malignancy was confirmed in one patient, corresponding to an overall malignancy rate of 2.5%. Histopathology revealed papillary thyroid carcinoma. Focal PSMA-PET/CT-positive thyroid incidentalomas are uncommon and associated with a low risk of clinically relevant malignancy. A less aggressive diagnostic approach, therefore, appears justified. 5. Controversies on the treatment of alcohol withdrawal symptoms.
PMID:日期:2026-08-18Alcohol withdrawal symptoms (AWS) are serious and seen in patients with severe alcohol problems reducing their alcohol intake. AWS is associated with a high morbidity and mortality, and - if untreated - with delirium, seizures, and increased severity of subsequent withdrawals. Benzodiazepines are the drugs of choice for treating AWS, and the dosage should be sufficient, preferably according to symptom scores. Experiencing AWS has no positive effects, and early detection and adequate treatment are crucial to the outcome. Medicine-free treatment of AWS is a thing of the past.

6. Increasing use of azoles combined with corticosteroids in Denmark.
PMID:日期:2026-08-13Superficial fungal infections are of growing concern, especially with the emergence of antifungal-resistant infections. Antifungal stewardship is important for preventing further resistance, and understanding how these medications are used will advise public health policy. Our objective was to determine antifungal prescribing patterns in Denmark's primary care sector. We analysed data from the Danish Health Data Bank Medstat website. We examined the number of individuals per 1,000 who received triazole/corticosteroid combinations, miconazole, topical and oral terbinafine, ketoconazole, fluconazole and itraconazole. Data were analysed for both males and females across the following age groups: 0-24 years, 25-44 years, 45-64 years and > 65 years. When analysing topical medications, we found high rates of triazole/corticosteroid prescriptions in all age groups. Topical ketoconazole was the most prescribed topical monotherapy across all ages; topical terbinafine prescription tended to decrease over time. When analysing oral medications, terbinafine was most commonly prescribed in males until age 65 years, after which fluconazole and terbinafine were prescribed at similar rates. Fluconazole prescriptions were highest among females of all age groups. Antifungal prescribing patterns in Denmark have shifted substantially over time, with persistently high use of azole/corticosteroid combination therapies highlighting the importance of ongoing antifungal stewardship. 7. Geographical variation in key outcomes in new Danish health councils.
PMID:日期:2026-08-05Denmark's 2024 health reform established 17 health councils to strengthen cross-sectoral coordination from 2027. The Danish system is often described as homogeneous, but local variation in quality indicators across council areas has not been examined. We aimed to quantify inter- and intra-council variation in cross-sectoral quality indicators before councils become operational. We hypothesised that council averages would mask municipal outliers. This was a national register-based cross-sectional study using aggregated data for all Danes. Indicators included chronic disease burden, general practice (GP) capacity, medication use (polypharmacy, antipsychotics in dementia), preventable admissions, readmissions, primary care contacts, municipal care contacts and primary care costs. Variation was quantified using coefficients of variation within (wCV) and between councils (bCV). Health Council Fyn served as illustrative case. Among 5.98 million inhabitants, GP contacts and primary care costs showed limited variation (bCV 0.8% and 4.7%). Excessive polypharmacy (less-than 65 years) and municipal care contacts had wCVs above 20% and bCVs of a similar magnitude, whereas preventable admissions (≥ 65 years) varied more between than within councils (bCV 12.5% versus 8.2%). Antipsychotic prescribing in dementia had a bCV of 17.5%. In Health Council Fyn, Ærø had antipsychotic prescribing 62% and municipal care contacts 76% above the council mean. Primary care access appears geographically uniform, but cross-sectoral indicators vary within and between councils, and council averages can mask municipal outliers. None. Not relevant.

8. Increasing use of azoles combined with corticosteroids in Denmark.
PMID:日期:2026-08-05Calcinosis cutis (CC), the deposit of insoluble calcium salts in the skin and subcutaneous tissues, is a rare condition that occurs in autoimmune connective tissue diseases, including systemic sclerosis (SSc), dermatomyositis and in non-connective tissue diseases (non-CTD), particularly in patients with calciphylaxis and nephropathy, and in chronic ulcers. CC causes significant morbidity and impacts quality of life. This study evaluated CC management strategies of patients in a clinical setting. A retrospective descriptive study was conducted at a university dermatology department in Denmark. We analysed all patients diagnosed with CC during 2019-2024 regarding CC distribution, treatment and follow-up, with data extracted from medical records. CC was diagnosed in 49 patients and was most prevalent in SSc. Lesions were located on fingers, elbows and other joints - often in areas prone to trauma. In non-CTD, most patients were diagnosed with nephropathy (50%) and chronic ulcers (70%). Sodium thiosulfate (STS) was the most frequently used treatment (87.8%). STS showed a positive effect in 30% of patients, 20% experienced no improvement and 2% experienced an adverse effect, but 48% had missing data. Surgical excision was performed in 13.9% and CO2 laser in 8.3%. CC management remains challenging. In non-CTD cases, CC is associated with nephropathy and chronic ulcers. SSc patients with CC present with multiple lesions, primarily on trauma-prone sites. STS shows promising effects on CC burden and pain, but more robust treatment-efficacy studies are warranted. None. Delays arise from regulatory complexity, economic constraints and scientific debate over novel psychotropics. 10. Potential impact of mandatory helmet when biking.
PMID:日期:2026-08-04Head injuries are a common and severe outcome of bicycle-related incidents, but they are preventable with a helmet. In Denmark, helmet use is not mandatory, and only half of cyclists wear helmets. Estimating the prevention potential of mandatory helmet use may inform evidence-based injury prevention policy. We aimed to estimate the number of preventable head injuries and severe head injuries under mandatory helmet use in 2020 and 2024. We conducted a retrospective study using emergency department data from Odense and Svendborg University Hospital, where helmet use is systematically recorded. Cyclists treated for head injuries (minor or major) following bicycle-related incidents were included. The potential impact of mandatory helmet use was estimated-, assuming a post-legislation helmet use rate of 92%, based on evidence from New Zealand. A switch fraction was applied to account for compliance, and meta-analytic-based relative risk reductions of 48% for minor injuries and 60% for severe head injuries were used to estimate the number of potentially preventable head injuries. In total, 2,393 cyclists were treated for minor or major head injuries during the five-year period (1,366 (57%) unhelmeted). Overall, 57% were male, and the mean age was 38 years (SD 21 years). Severe head injuries were sustained by 511 cyclists, 275 of whom (53%) were unhelmeted. Extrapolation suggests that approximately 5,733 minor and 1,806 severe head injuries could have been prevented. The findings support helmet use as an effective prevention strategy to reduce head injuries among bicyclists. None. Not relevant.
