1. A Systematic Reflection on the Challenges in Acupuncture and Chinese Medicine Education and Careers in the United States.
PMID:日期:2026-08-05The relationship between education in acupuncture and Chinese medicine (ACM) and its professional practice can be conceptualized as an integrated "factory-product-market" industrial chain. Within the education sector, ACM schools face a significant predicament. Their core mission is to train practitioners who possess robust clinical skills, meet professional licensing standards, and are capable of genuinely serving patients. In recent years, these institutions have come under mounting financial pressure due to a confluence of factors: rising inflation, increasing operational costs (such as rent, utilities, and insurance), and shifts in international trade policies (e.g., higher tariffs). Consequently, faculty compensation, teaching quality, and clinical resources have failed to keep pace-and in some cases have even declined-creating a vicious cycle. On the student side, limited career pathways and a weak feedback loop to the education system persist. Meanwhile, on the market side, the profession faces disordered industry regulation and potentially shrinking demand. These challenges include nonprofessionals entering acupuncture practice, low practitioner income and declining career attractiveness, as well as chaotic business models and imbalanced pricing structures. The chain connecting ACM education, graduate employment, and societal utilization is now caught in a state of systemic imbalance. The root problem lies not in any single point of failure but in a structural breakdown. Addressing this challenge will require coordinated, multidimensional reforms spanning policy, legislation, insurance, and the education system.

3. Effectiveness of Acupuncture at Feishu and Zusanli on Immunoglobulin E Levels in Chronic Asthma Mice Model.
PMID:日期:2026-04-14Asthma is a chronic inflammatory disease affecting millions of people globally. Chronic asthma is generally resistant to steroid therapy. Acupuncture has anti-inflammatory effects and has been widely used as an adjuvant therapy for asthma. This study aims to evaluate the effects of acupuncture at Feishu (BL-13) only and BL-13 and Zusanli stomach (ST-36) on immunoglobulin E (IgE) levels of the chronic asthma mice model. BALB/c mice were sensitized using ovalbumin (OVA) to induce a chronic asthma mouse model by injecting OVA intraperitoneally on days 0 and 14 and followed by aerosol exposure of OVA 1% three times/week for 6 weeks. The 28 mice were randomized into four groups: Control group, untreated asthma group, asthma + BL-13, and asthma + BL-13 + ST-36. For the treatment group, manual acupuncture (MA) was done three times/week for 6 weeks at BL-13 only or BL-13 and ST-36. At the end of the experiment, blood samples were taken to determine IgE levels. This study found significantly lower IgE levels after MA at BL-13 ( = 0.009) and BL-13 + ST-36 ( = 0.018) compared with the control group. Acupuncture is able to stimulate sympathetic nerves, stimulating Beta-2 (β2) adrenergic receptors. Moreover, acupuncture reduces T helper-2 (Th2) cytokines and restores the balance of Th1/Th2 cytokines, which might affect the IgE level. Acupuncture at BL-13 and BL-13 + ST-36 acupoints is able to demonstrate a lower concentration of IgE in the chronic asthma mice model.

4. Trends in Utilization of Acupuncture in the U.S. Health System, 2016-2022.
PMID:日期:2026-02-19The Military Health System (MHS) has provided acupuncture since 2005 and, in 2014, delivered acupuncture to approximately 15,000 beneficiaries. However, no recent studies have assessed acupuncture in the MHS, despite new guidelines aiming to increase its provision. We identified acupuncture claims from the MHS Data Repository (MDR) for fiscal years (FY) 2016-2022, using bivariate analysis to determine demographics and number of visits. Descriptive statistics showed associated diagnoses, provider specialties, clinical settings, and number of military treatment facilities (MTFs) providing acupuncture. Trend analyses showed numbers of patients and MTFs providing acupuncture by FY. During this period, 152,564 people received acupuncture, with greatest use among Army service, White race, and Senior Enlisted rank, for males in active duty aged 26-35 and females in nonactive duty aged 46-65. A cumulative 93.2% of diagnoses related to musculoskeletal and nervous system issues. The most common setting was family medicine clinics; ∼38% of patients received acupuncture from nonphysician providers. The number of MTFs providing acupuncture increased rapidly from 2016 to 2020, and more slowly from 2020 to 2022, while the number of visits peaked in 2019 before decreasing. Acupuncture use in the MHS expanded ∼2-fold among patients and 3-fold among facilities since 2014. Diagnoses are consistent with pain control, and there was a shift toward nonphysician providers. Promotion of acupuncture in the MHS was likely hampered by care shifts during the COVID-19 pandemic public health emergency. Further research should assess the long-term effect of these guidelines and the effectiveness of acupuncture in promoting service member retention.

5. Electroacupuncture for Plantar Fasciopathy: A Multisite Randomized Clinical Trial.
PMID:日期:2026-02-19Determine if adding an acupuncture protocol with electrostimulation to a standard exercise program improves pain and function in adult patients with plantar fasciopathy more than a standard exercise program alone. In this randomized, multisite controlled study, patients were randomized to either receive a local electroacupuncture protocol plus home exercise program or a home exercise program alone. Acupuncture treatments were performed every 2 weeks for 6 weeks with a 6-week follow-up for a total study period of 12 weeks. The home exercise program included stretching and strengthening exercises over a 5-week period. The primary outcome was change in the patients' reported Defense and Veterans Pain Rating Scale (DVPRS) scores. The secondary outcome was change in the Revised Foot Function Index-Revised (FFI-R) Short Form questionnaire. Seventy-two subjects were enrolled, with 37 randomized into the acupuncture plus exercise group (58 feet) and 35 randomized into the exercise group alone (59 feet). Demographics were similar between the two groups. Significantly stronger declines over time were observed in the intervention group for both DVPRS pain scores (12 week decline of 2.17 compared with 1.62 in the comparison group, interaction coefficient of -0.07, 95% confidence interval [CI]: -0.13, -0.01) and FFI-R scores (12 week decline of 19.5 compared with 12.9 in the comparison group, interaction coefficient of -0.65, 95% CI: -1.20, -0.10). Overall, 23 out of 25 subjects in the acupuncture plus exercise group reported that they were satisfied with their acupuncture treatment. Acupuncture with electrostimulation plus prescribed exercise decreases pain and improves function in both the short and long term in adult patients with plantar fasciopathy, more than exercise alone. These findings add to evidence that acupuncture is an important adjunctive treatment option in the management of common outpatient clinic problems.Trial Registration: NCT04243343.

6. Localization of Auricular Acupoints for Treating Idiopathic Chronic Low Back Pain Using Pressure Pain Test and Electrical Skin Resistance Test.
PMID:日期:2026-02-04Idiopathic chronic low back pain (icLBP) represents approximately 90% of chronic low back pain and lacks a clearly identifiable pathological origin. It impacts both productivity and overall quality of life. Auricular acupuncture (AA) is a promising emerging therapy for icLBP. To verify if there are areas of both the lateral and medial auricle surfaces with greater incidence of neuroreflex points in subjects with icLBP as compared to a group of healthy controls. A total of 64 subjects with a diagnosis of icLBP and a control group of 64 healthy individuals were evaluated. All sectors of the Romoli Sectogram and the Chinese Standardized Map were examined with the Electrical Skin Resistance Test (ESRT) and with the Pressure Pain Test (PPT) on both the medial and lateral surfaces of each auricle. The icLBP subjects had a significantly higher average number of positive points in sectors 17, 18, and 19 of the Romoli Sectogram and in areas AH9, AH5, TF4, and P1 of the Chinese Standardized Map as compared with the control. In icLBP subjects, the positive points were predominantly grouped into four main areas: area 1 in AH9, area 2 in P1, area 3 in TF4, and area 4 in AH5. In areas 1 and 2, one or more positive points in the vast majority of icLBP subjects were detected. In icLBP, there are four areas of the auricle within which one or more points positive on ESRT and PPT can be detected, identifying potentially specific and effective AA points.

7. Acupuncture for Pain Management in a Geriatric Patient with Colorectal Cancer: A Case Report.
PMID:日期:2026-02-04Colorectal cancer (CC) causes pain, thereby affecting a patient's quality of life and functional status. Pain management with analgesics can cause adverse effects. Acupuncture is a nonpharmacological treatment that effectively manages cancer-related pain and provides a safer alternative. In this case report, battlefield ear acupuncture (BFA) and body acupuncture were used. A 76-year-old female patient with right lower abdominal pain for the past 3 months prior to hospitalization. The pain was stabbing, intermittent, and worsened in the seated position, also bloating, reduced appetite, fatigue, weight loss, constipation, and bloody stools. Physical examination revealed conjunctival pallor, tenderness, and a palpable liver. Laboratory tests revealed anemia, elevated carcinoembryonic antigen tumor marker (CEA), and prolonged clotting times. Computed tomography revealed a malignant mass in the cecum and ascending colon. Histopathology confirmed low-grade adenocarcinoma. Modified BFA protocols were used with a combination of body acupuncture at LI4 (Hegu), ST36 (Zusanli), Sp6 (Sanyinjiao), GB34 (Yang Ling Quan), and LR3 (Taichong) bilaterally using filiform needle. Acupuncture therapy was administered daily with a retention time of 30 min for both body and ear acupuncture sessions. After six therapy sessions, the pain level score (numeric rating scale [NRS]) decreased from 5 to 1, and the patient could sleep at night and also showed improvement for EQ-5D. There was a decrease in the use of paracetamol from a dose of 1000 mg and morphine immediate-release dose of 5 mg when experiencing pain until it was not given in the 6th session. In this case, acupuncture therapy was helpful in managing the preoperative pain in a geriatric patient with colorectal cancer. Acupuncture may be considered to treat patients with pain and contraindications to pharmacologic therapies and can reduce the side effects of pharmacologic therapies. Further investigation of acupuncture therapy for cancer pain is warranted.

8. "Role of Laser Acupuncture Battlefield Acupuncture in Pain Management in Pediatric Patients with Acute Lymphoblastic Leukemia".
PMID:日期:2026-02-03Pain is one of the most common clinical symptoms in pediatric patients with acute lymphoblastic leukemia (ALL). Analgesics may cause side effects that can impact the child's health. Laser acupuncture is a nonpharmacological therapy that can help manage cancer-related pain in pediatric patients. A case study was conducted on a 14-year-old male patient who presented with a chief complaint of pain in both soles of the feet, which had persisted for 3 months prior to hospital admission. The patient had been diagnosed with and was being treated for ALL. The patient developed pain in the soles of his feet after receiving three courses of vincristine-based chemotherapy. Physical examination revealed bilateral pitting edema. Laboratory tests revealed leukocytosis and elevated levels of serum serum glutamate pyruvate transaminase (SGPT). Electromyography with nerve conduction studies revealed signs of lower extremity motor axonal polyneuropathy. The patient underwent daily routine therapy using laser acupuncture with the RJ-laser pen, Nogier frequency E (36.50 Hz), power output of 50 mW, and an energy density of 0.5 Joules/cm applied for 20 s at auricular points according to bilateral battlefield acupuncture (BFA), and 2 Joules/cm applied for 80 s at LI4, ST36, GB34, SP6, and LR3 points bilaterally. The patient's pain level was assessed using the Numerical Rating Scale (NRS). The patient experienced a reduction in pain after six therapy sessions, as indicated by a decrease in NRS from an initial score of 5 to 1. This case report suggests that laser acupuncture therapy might help reduce some of the neuropathic pain experienced by pediatric patients who are being treated for ALL. Furthermore, research should be done in determining whether laser acupuncture can be more widely recommended as an adjunctive or complementary therapy in these settings.

9. The Logic of Simplicity: From Battlefield to Bedside-Richard Niemtzow and the Future of Translational Acupuncture.
9. 简单的逻辑:从战场到床边Richard Niemtzow和转化针灸的未来PMID:日期:2026-02-01Colonel (Ret.) Richard C. Niemtzow, M.D., Ph.D., M.P.H., transformed the modern perception of acupuncture through his development of Battlefield Acupuncture (BFA) and his visionary leadership as founding Editor-in-Chief of . His work embodied a "logic of simplicity" that joined clinical efficacy, operational practicality, and human compassion. This article reflects on Niemtzow's translational vision-how he bridged Eastern and Western epistemologies, military medicine, and humanitarian service-and offers a personal remembrance of his editorial mentorship and correspondence. His enduring legacy lies not only in a protocol of five auricular points but in a model of clarity, humility, and disciplined compassion that continues to guide acupuncture's integration into contemporary medicine. While mixed randomized evidence invites continued scientific scrutiny, the underlying philosophy of concise, teachable, and field-ready pain relief remains a durable contribution to modern integrative care.

10. Perceived Barriers of Implementing Battlefield Acupuncture in Outpatient Family Medicine Clinics: A Qualitative Study.
PMID:日期:2026-01-26Battlefield Acupuncture (BFA) is a quick, in-office procedure effective in treating pain. Supplies cost between $6.40 and $8.50 per procedure, with up to $116 reimbursement per session. Despite availability in training and implementation in military medicine, BFA is not frequently performed in civilian medical care. We investigated what perceived barriers exist to implementing BFA in civilian practices. We performed a qualitative study by interviewing nine faculty physicians of the University of Nebraska Medical Center Family Medicine Residency program in Omaha, Nebraska. After obtaining demographic data, we utilized a 12-question standardized interview protocol, with clarifying follow-up questions when needed. Interviews were recorded and transcribed. Transcripts were reviewed by investigators independently looking for common themes. Independent results were then evaluated as a group. The most common barrier reported was patients' ability to pay, as BFA is not usually covered by insurance. Other common barriers included the credentialing/privileging process within a large health care system, and lack of patient awareness and physician education on the technique. Time to become trained and the time required to perform the procedure in clinic were less commonly identified as barriers. Pain is universally experienced by patients. BFA has been demonstrated to be safe and cost-effective in reducing acute and chronic pain. Implementing BFA may increase patient satisfaction and quality of life. Solutions to reduce out-of-pocket cost to patients, as well as clarifying the credentialing and privileging processes at individual clinics, are needed to enable physicians to perform this technique.
