1. Iatrogenic Bilateral Sequential Ocular Sympathetic (Horner Syndrome) Turned Bilateral Parasympathetic Pupil Involvement Associated with Long Coronavirus-Related Dysautonomia: A Case Report.
1. 医源性双侧序贯性眼交感神经麻痹(霍纳综合征)转为双侧副交感神经瞳孔受累并伴长新冠相关自主神经功能障碍:一例病例报告PMID:日期:2026-09-24This report presents an unusual case of sequential, pharmacologically confirmed iatrogenic bilateral Horner syndrome followed by bilateral Adie's tonic pupil, highlighting the spectrum of ocular dysautonomia associated with long coronavirus disease 2019 (COVID-19). A 41-year-old woman with a history of coronavirus disease 2019 reinfection and multiple comorbidities developed pharmacologically confirmed bilateral Horner syndrome after bilateral stellate ganglion blocks. Subsequent worsening of long COVID-19 symptoms alongside systemic dysautonomia emerged, including postural orthostatic tachycardia syndrome, orthostatic hypotension, and mast cell activation syndrome. The patient then presented with bilateral, poorly reactive, enlarged pupils, and light-near dissociation. Bilateral Adie's tonic pupil was pharmacologically confirmed. Neuroimaging and laboratory workup were unremarkable. As more cases of long COVID-19 emerge, physicians should be aware that long COVID-19 can manifest with rare neuro-ophthalmic presentations, including bilateral pupillary autonomic dysfunction.

2. Eye Pain in Dejerine-Roussy Syndrome: A Rare Post-Thalamic Stroke Manifestation - A Case Report.
PMID:日期:2026-09-21Dejerine-Roussy syndrome presents with contralateral neuropathic pain following a thalamic stroke. Previously reported presentations included contralateral upper and lower extremity pain, along with paralysis, spasticity, and other classic post-stroke manifestations. Facial and eye pain following a stroke is infrequent, and previous reports of facial and eye pain following a stroke are limited to brainstem infarctions. A 69-year-old male patient reported severe right-sided facial and eye pain following a previous left-sided thalamic hemorrhagic stroke. The patient is positive for multiple vasculopathic risk factors and is continuing residual right arm and leg spastic paralysis management. The patient's neuropathic pain is being treated with gabapentin, amitriptyline, trazodone, and ibuprofen. Interestingly, his facial and ocular pain resolved following cataract removal. The location of this patients' stroke, and his unique presentation, highlight a rare ocular manifestation clinicians should be aware of for their stroke patients' and their post-stroke management. Further, the unique resolution of this pain prompts more exploration into neuropathic pain treatment.

3. Novel Dual-Device Strategy: Failed PRESERFLO MicroShunt Connected to an Ahmed ClearPath - Case Report.
PMID:日期:2026-08-31The PRESERFLO MicroShunt is an established filtrating surgery for moderate to advanced glaucoma, yet postoperative scarring may occur in up to one-third of cases, often requiring needling or revision. This case report describes a novel surgical approach in which a scar-compromised PRESERFLO MicroShunt was connected to an Ahmed ClearPath 250 mm implant to enhance aqueous outflow without introducing an additional tube into the anterior chamber. A 64-year-old patient with advanced primary open-angle glaucoma and recurrent scarring after PRESERFLO implantation underwent this combined procedure. To revive the failed PRESERFLO MicroShunt, the Ahmed ClearPath was used as an extension to the MicroShunt: the tube of the Ahmed ClearPath was inserted into the PRESERFLO MicroShunt and secured with a cross suture, followed by coverage with a scleral patch graft. Intraocular pressure decreased to 5-9 mm Hg and remained stable under minimal medication (dorzolamide eyedrops 3×/d) at the 3 months postoperative follow-up visit. This is the first case report that describes the outcome of the connection of the PRESERFLO MicroShunt to an Ahmed ClearPath. The described dual-device strategy preserved conjunctival tissue and obviated the need for a second tube in the anterior chamber. This approach represents an effective and tissue-sparing alternative for patients in whom a PRESERFLO MicroShunt is well positioned but rendered insufficient by scarring.

4. Acute Orbital Inflammation following Neridronate Therapy: A Case Report.
PMID:日期:2026-08-13Bisphosphonates are known to occasionally induce ocular and orbital inflammation. While most reported cases involve agents such as zoledronate or alendronate, reports associated with neridronate remain exceedingly rare. Early recognition is vital as this condition can be sight-threatening but typically responds well to treatment. We describe a 69-year-old man who developed fever and myalgia, followed by the abrupt onset of severe unilateral orbital inflammation 72 h after receiving an intramuscular injection of neridronic acid. Clinical examination of the left eye revealed acute visual impairment, marked eyelid edema, erythema, ptosis, proptosis, diplopia, and chemosis. Ultrasonography showed posterior scleral thickening (T-sign), while CT and MRI demonstrated diffuse orbital inflammation and intraconal fat involvement with periarterial enhancement, in the absence of infectious foci. Laboratory tests showed elevated C-reactive protein levels. Following the initiation of high-dose oral prednisone (1 mg/kg/day), the patient exhibited rapid clinical improvement within 48 h and near-complete radiologic resolution by 15 days. This case underscores the importance of recognizing neridronate-associated orbital inflammation as a potential adverse reaction, even following intramuscular injection. Despite its severe presentation, the condition typically demonstrates a rapid and favorable response to systemic corticosteroid therapy.

5. A Rare Septuagenarian Patient with a Mild Form of Mucopolysaccharidosis Type IIIC with Ocular Manifestations: A Case Report.
PMID:日期:2026-08-01We present an atypical case of mucopolysaccharidosis (MPS) type IIIC to contribute to the growing understanding of its clinical heterogeneity and expand the known phenotypic spectrum of the disorder. This report describes one of the oldest known patients with genetically confirmed MPS IIIC. The patient received genetic testing in her seventies, revealing variations in the and genes, two genes associated with MPS III. Her clinical presentation consisted primarily of retinal dystrophy, diagnosed via clinical presentation in addition to OCT and ERG. The patient also showed signs of peripheral neuropathy which were initially presumed to be idiopathic. She did not exhibit the typical behavioral involvement and craniofacial abnormalities expected of MPS type IIIC. This presentation broadens the recognized clinical spectrum of MPS IIIC and may facilitate earlier diagnosis in adults with similar, seemingly idiopathic findings. Improved recognition of such atypical cases may support timely genetic testing and potential therapeutic options.

6. FLVCR1 Variant as a Cause of Retinal and Neurologic Dysfunction in Retinitis Pigmentosa with Coats-Like Vasculopathy: A Case Report.
PMID:日期:2026-07-31We report a unique clinical presentation of a patient with undiagnosed neurologic symptoms in the setting of retinitis pigmentosa (RP) with Coats-like reaction and FLVCR1 variant, expanding the presentation of FLVCR1 variants. A 31-year-old Caucasian female with RP with Coats-like reaction initially presented to our clinic in August 2021 with worsening vision. Genetic testing identified the patient as heterozygous for FLVCR1 c.1092 + 5G>A and FLVCR1 c.1058C>T, p.(Thr353Met). The patient notably had a long-standing history of nyctalopia as well as complex neurologic symptoms without diagnosis. Neurologic presentation over 6 years included migraine headaches, decreased extremity strength, seizure-like episodes, and urinary incontinence. Progressive leg weakness and spasms eventually impaired weight bearing and ambulation. On presentation to the clinic again in February 2025, she reported progressive central vision loss, ocular pain OD, and persistent instability in her legs. OCT revealed cystoid macular edema, attenuated retinal vessels, and a large lipid exudation in the fovea OD. The patient was initiated on anti-VEGF therapy in February. Follow-up OCT 1 month later revealed a reduction in lipid exudation. This case underscores the importance of considering an underlying FLVCR1 variant in patients with retinitis pigmentosa and atypical neurologic symptoms. Despite prior genetic testing, this patient's neurologic findings remained undiagnosed, highlighting the need for greater clinical awareness of neuro-ophthalmic manifestations of rare genetic disorders. Anti-VEGF therapy appears effective in reducing lipid exudation in Coats-like retinopathy.

7. Clinical Insights and Genetic Implications: "A Comprehensive Case Series of Two Siblings with Cohen Syndrome".
PMID:日期:2026-07-27Cohen syndrome is a rare autosomal recessive developmental disorder with various clinical manifestations. These include failure to thrive, hypotonia, intellectual disability, pigmentary retinopathy, myopia, nyctalopia, microphthalmia, acquired microcephaly, truncal obesity, joint hypermobility, intermittent neutropenia, and many other less frequent or subtle features. The condition is associated with mutations in the VPS13B (vacuolar protein sorting 13 homolog B) gene, on chromosome 8. Over 150 mutations in VPS13B have been reported in more than 200 patients with Cohen syndrome. Causative mutations include nonsense, missense, indeland splice-site variants. We report 2 siblings with Cohen syndrome who lacked the typical myopic phenotype and instead demonstrated significant hyperopia: +5.00-1.00*175 right eye (RE) and +5.00-1.50*180 left eye (LE) for the first case, and +3.00-1.00*180 (RE), +3.00-1.00*10 (LE) for her sister. Comprehensive ophthalmologic evaluation was performed, including fundus photography, optical coherence tomography (OCT), and electroretinography. Relevant literature was reviewed and correlated with their clinical features and genetic findings. This observation underscores the importance of reevaluating the ophthalmic manifestations and genetic spectrum of Cohen syndrome. To our knowledge, this is the first report of Cohen syndrome cases presenting with hyperopia rather than myopia. Further studies are necessary to establish the full range of ophthalmic presentations and their genetic implications in Cohen syndrome.

8. Reactivation of Varicella-Zoster Virus Keratitis after Recombinant Herpes Zoster Vaccination: A Case Report and Literature Review.
PMID:日期:2026-07-23Varicella-zoster virus (VZV) keratitis following recombinant herpes zoster (HZ) vaccination is rare, and its underlying mechanisms remain poorly understood. Sporadic cases of ocular inflammatory reactivation have been reported after vaccination, particularly in patients with a history of HZ infection. A 68-year-old woman with a history of trigeminal HZ without prior corneal involvement developed VZV keratitis 2 months after receiving the recombinant HZ vaccine (Shingrix). The condition progressed to neurotrophic keratitis, requiring advanced ocular surface therapy, including autologous serum and topical insulin, with favorable visual and symptomatic outcomes. Reactivation of VZV keratitis after HZ vaccination is an uncommon but clinically relevant event. Ocular reactivation may occur even in patients without previous corneal involvement and may lead to chronic complications, supporting the need for close ophthalmologic follow-up in patients with a history of HZ infection.

9. Syphilitic Neuroretinitis with Positive Cytomegalovirus Serology in an Immunocompetent Adolescent: A Case Report.
PMID:日期:2026-07-21Neuroretinitis is an inflammatory optic neuropathy characterized by optic disc edema followed by macular star formation. It is most commonly caused by , but other infectious etiologies including syphilis and cytomegalovirus (CMV) should be considered, particularly due to their potential for severe visual morbidity if untreated. A 17-year-old immunocompetent female presented with unilateral visual blurring and headache. Ophthalmologic examination revealed optic disc edema, macular star exudates, and peripapillary hemorrhages in the right eye. Dermatologic examination revealed features consistent with secondary syphilis, and the diagnosis was further supported by laboratory investigations confirming syphilis. Serologic testing also demonstrated positive CMV IgM and IgG antibodies. HIV testing was negative. The patient was treated with intravenous ceftriaxone and ganciclovir followed by oral valganciclovir. Complete anatomical and functional recovery was achieved with restoration of visual acuity. This case highlights the importance of comprehensive infectious evaluation in neuroretinitis, even in immunocompetent individuals. Early diagnosis and appropriate targeted antimicrobial therapy are essential to prevent permanent visual loss.

10. Toxic Anterior Segment Syndrome following XEN Gel Stent Implantation: A Rare Complication in Minimally Invasive Glaucoma Surgery - Case Report.
PMID:日期:2026-07-21Toxic anterior segment syndrome (TASS) is a rare, sterile, acute inflammatory reaction of the anterior segment following intraocular surgery. Although it is most commonly associated with cataract surgery, it has been reported after a variety of ophthalmic procedures. To our knowledge, no case of TASS following XEN Gel Stent Implantation has been previously described in the literature. An 85-year-old pseudophakic woman with primary open-angle glaucoma developed TASS after uneventful XEN63 implantation. Within 24 h postoperatively, the patient presented with diffuse limbus-to-limbus corneal edema, anterior chamber inflammation, and absence of pain or posterior segment involvement. Despite prompt and intensive topical corticosteroid therapy, she developed irreversible corneal decompensation, iris atrophy, and fixed nonreactive mydriasis. No additional cases were identified in the same surgical setting, suggesting a patient- or procedure-specific trigger. Potential causative factors included intracameral agents (lidocaine, acetylcholine, cefuroxime, viscoelastic material), possible migration of mitomycin C through the XEN stent lumen, or an inflammatory response to the gelatin-based device itself. This case demonstrates that TASS can occur following minimally invasive glaucoma surgery, including XEN Gel Stent implantation, and may result in severe and irreversible ocular sequelae. It underscores the importance of heightened postoperative vigilance and strict preventive protocols even in procedures perceived as low risk.
