1. A Review of Minimally Invasive Therapies for Men with Overactive Bladder.
PMID:日期:2026-10-03This manuscript aims to provide an overview of minimally invasive treatment for overactive bladder (OAB) in men, highlighting how concomitant bladder outlet obstruction in men can complicate management and lead to underutilization of these therapies in this patient population. The 2024 AUA/SUFU guidelines now allow practitioners to offer minimally invasive therapy as an upfront treatment for OAB. Recent studies have closely examined the efficacy of several key procedures, including intradetrusor onabotulinumtoxin-A (BTX-A), sacral neuromodulation (SNM), and tibial nerve stimulation via percutaneous or implantable devices. All three modalities are effective in terms of improving OAB symptoms, however the evidence base is largely female-derived. This highlights the ongoing need for adequately powered, male-specific clinical trials to address this disparity and help guide individualized treatment planning.

2. Imaging in Upper Tract Urothelial Carcinoma: Diagnostic Accuracy of Current Modalities for Local Tumor Staging.
PMID:日期:2026-10-02To summarize current imaging modalities for upper tract urothelial carcinoma (UTUC) and evaluate their diagnostic performance for tumor staging and risk stratification. CT urography remains the diagnostic standard with excellent accuracy for tumor detection, though its reliability for local staging is limited. MR urography shows promise for assessing muscle invasion but is operator dependent. FDG-PET demonstrates value in nodal and metastatic staging rather than primary tumor evaluation. Retrograde pyelography maintains a role in tumor diagnosis in circumstances where cross-sectional imaging is unable to be performed. Emerging approaches, including radiomics and machine learning, as well as novel modalities such as endoluminal ultrasound, optical coherence tomography, and contrast-enhanced ultrasound show encouraging preliminary results but lack large-scale validation. No single modality provides accurate clinical staging in UTUC. A multimodal approach integrating imaging with clinical and endoscopic data remains essential, while emerging technologies may improve future risk stratification.

3. Timing and Cost Consideration in Surgery for Male Stress Urinary Incontinence For Current Urology Reports.
PMID:日期:2026-09-23Surgery remains the standard of definitive care in male stress urinary incontinence (SUI). This narrative review provides a critical discussion of pertinent aspects of male continence surgery, including cost-benefit analysis and whether continence surgery should be offered earlier. Controversies exist regarding the timing of surgery and whether the male sling (MS) is superior to the artificial urinary sphincter (AUS). Presently, significant gaps exist in the cost analysis and modelling for the most effective treatment in male SUI. Each continence device has its own merit and drawbacks, and patients should be counselled on the procedure, mechanics, expectations and surgical complications, with emphasis on shared decision-making and personalised care. More high-quality studies of surgical interventions, including direct comparison against non-surgical methods, coupled with in-depth evaluation on why patients choose a particular treatment over another, will be useful in preoperative counselling and streamlining decision-making in male SUI.

4. Hydrogel-Based Therapies for Female Stress Urinary Incontinence: From Bulking Agents to Regenerative Platforms-A Narrative Review.
PMID:日期:2026-09-18This review summarizes the procedural techniques, short- and long-term efficacy, and clinical application of polyacrylamide hydrogel (PAHG, Bulkamid®) as a urethral bulking agent for female stress urinary incontinence (SUI); compares PAHG with mid-urethral sling (MUS) surgery; and examines emerging hydrogel-based regenerative platforms, used for mechanical bulking, delivery of bioactive molecules or cells, and extracellular matrix-mimetic scaffolding, together with their prospects for clinical translation. PAHG injection has gained acceptance as a minimally invasive option for patients who have failed conservative management, declined or are intolerant of MUS surgery, or have recurrent SUI. Follow-up studies up to 7-9 years show sustained response rates of 44%-65% with mostly transient complications and improvements in quality of life and sexual function. Randomized trials indicate that although MUS achieves superior cure rates, PAHG is associated with fewer severe complications, aligning with patient preferences for less invasive procedures. In preclinical animal models, injectable hydrogels loaded with bioactive molecules or cells provide mechanical bulking and simultaneously promote tissue regeneration, including angiogenesis, neurogenesis, and modulation of the inflammatory response. PAHG is a well-tolerated, minimally invasive bulking agent that achieves sustained symptomatic relief and meaningful quality-of-life improvements. Dual-function hydrogel systems combining bulking and sustained regenerative repair may represent a future direction for SUI therapy; however, most evidence remains preclinical, and further material optimization, rigorous safety evaluation, and well-designed clinical trials are needed before clinical translation.

5. Renal Imaging Characteristics Predicting Percutaneous Access for Staghorn Stone Management.
PMID:日期:2026-09-18Preoperative planning in staghorn stones treated with percutaneous nephrolithotomy (PCNL) can improve efficiency and minimize complications. We summarized the current evidence on imaging features to guide the selection of optimal access for managing staghorn stones. Although the upper pole may offer the most favorable route for stone clearance, it may carry a higher risk of complications in certain situations. In such cases, the middle-calyx may be a safer and equally efficient alternative. Multiple access may still be needed in certain cases. Care should be taken when analyzing preoperative imaging characteristics, as patient positioning and measurements may not directly correlate with the intraoperative findings. Detailed assessment of preoperative imaging remains essential for access planning and success. Linear stone burden, distribution of the stone and its relationships to the access calyx, nephrolithometry S.T.O.N.E. score, and infundibular width are relevant factors when planning PCNL access.

6. Leveraging Skills as a Practicing Urologist into Other Leadership Positions.
PMID:日期:2026-09-12This review examines how the clinical skills, professional traits, and developmental pathways of practicing urologists can translate into broader institutional leadership roles, addressing the persistent underrepresentation of surgeons and urologists in health system decision-making. National data show that physician chief executives remain uncommon and that urologists are rarely represented in senior institutional roles. Yet urology draws from a highly competitive applicant pool and is associated with personality traits, longitudinal patient relationships, and operational habits that align closely with institutional leadership. National data further illustrate how physicians commonly reach durable leadership roles through progressive responsibility, visible institutional service, and sustained operational engagement. Urologists already exercise many competencies required for institutional leadership, including triage under uncertainty, multidisciplinary coordination, regulatory awareness, high-stakes communication, and long-term relationship-building. Intentional pathways built through mentorship, leadership development, visible service, and progressive administrative responsibility can help urologists extend their impact beyond the operating room and strengthen surgical representation in organizational leadership.

7. Advancements in Single-Port Robotic Urologic Surgery: A Comprehensive Review of Contemporary Innovations and Outcomes.
PMID:日期:2026-09-11Single-port robotic surgery has experienced remarkable evolution over the past five years, fundamentally transforming minimally invasive urologic practice. This review examines recent advancements in single-port robotic platforms, surgical techniques, and clinical outcomes across the spectrum of urologic procedures. The introduction of the da Vinci SP surgical system and emerging competitors has catalyzed innovation in urologic surgery. Novel approaches including Retzius-sparing radical prostatectomy, extraperitoneal access techniques, transvesical simple prostatectomy, partial nephrectomy, ureteral reconstruction, and pyeloplasty have demonstrated comparable oncologic and functional outcomes to conventional multiport approaches while potentially offering enhanced cosmesis, reduced postoperative pain, and faster recovery. Single-port robotic urologic surgery has matured significantly, with accumulating evidence supporting its safety, feasibility, and efficacy while utilizing novel surgical approaches. Continued refinement of surgical techniques, expanding platform availability, and growing surgeon experience are driving broader adoption. This review synthesizes contemporary literature to guide clinical practice and identify future research priorities.

8. Difficult Access in Endourology: Anatomical Challenges and Contemporary Management.
8. 腔内泌尿外科的困难通道:解剖学挑战和现代管理PMID:日期:2026-09-05This review provides a comprehensive overview of difficult access in endourology. This focuses on anatomical factors that complicate endoscopic management of upper and lower urinary tract diseases and discussing strategies to manage these challenges. Advances in preoperative imaging, three-dimensional reconstruction, flexible digital ureteroscopes, suction-assisted ureteral access sheaths, thulium fibre laser technology, miniaturized percutaneous techniques, and image-guided navigation have significantly improved management in patients with complex anatomy. Improved understanding of congenital and acquired anatomical variations has facilitated individualized surgical planning. Difficult access is a major determinant of operative success and complications in endourology. Anatomical challenges-including narrow ureters, tortuous ureters, lower pole anatomy, calyceal diverticula, horseshoe kidneys, urinary diversion, obesity, spinal deformity, and previous surgery-require preoperative assessment and selective intraoperative strategies. This review summarizes current evidence, practical technical solutions, emerging technologies, and future directions to optimize access, improve stone-free rates, minimize complications, and enhance patient outcomes in complex endourological cases.

9. Disease-Related Etiologies of Nocturia: A Focused Literature Review.
PMID:日期:2026-08-31Nocturia is a highly prevalent symptom that affects multiple aspects of health. However, it is often underrecognized and undertreated, leading to problems both within the genitourinary system and systemically. The goal of this paper is to further characterize the pathophysiology behind the most common medical factors that contribute to nocturia by performing a focused literature review in a structured format. This will aid clinicians when evaluating and treating patients with nocturia. Based on expert opinion, we recognized six main common pathologies associated with nocturia: obesity, diabetes mellitus, hypertension, obstructive sleep apnea, benign prostatic hyperplasia, and lifestyle habits. Our literature review identified 49 articles including randomized controlled trials, observational studies, systematic reviews, and meta-analyses. Each condition has its own unique pathophysiology that is associated with the development of nocturia. These etiologies also present various ways upon which nocturia can be intervened. These six pathologies are the most common medical etiologies for nocturia based on expert opinion. This paper demonstrates the multifactorial nature of nocturia and highlights assessment and treatment strategies.

10. A Comprehensive Review of Minimally Invasive Surgical Therapies for Voiding Dysfunction in Benign Prostatic Hyperplasia: Past Failures, Contemporary Successes and Future Innovations.
PMID:日期:2026-08-31Treatments for voiding dysfunction secondary to benign prostatic hyperplasia (BPH) have evolved greatly but also attempted to recapitulate existing ideas. This review will detail the MISTs for BPH through three temporal lenses: past failures and their lessons, contemporary therapies with evidence bases and emerging technologies that are potential futures for minimally invasive BPH treatment. Newer minimally invasive surgical therapies (MISTs) either occupy a new technological niche or refine concepts used by earlier, unsuccessful BPH treatments. Historical permanent stents, balloon dilation, prostatic injection therapies, transurethral needle ablation, and transurethral microwave thermotherapy were limited by complications, inconsistent objective improvement, or high retreatment rates. Contemporary options, including prostatic urethral lift, water-vapor thermal therapy, and drug-coated balloon dilation, demonstrate improved symptom and flow outcomes in selected patients, while temporary nitinol devices and histotripsy continue to evolve. Contemporary devices frequently revisit earlier mechanical, thermal, or dilation-based approaches. Improved materials and delivery systems may reduce prior limitations, but they do not eliminate the need to demonstrate durable efficacy, acceptable retreatment rates, preservation of sexual function, and appropriate patient selection through high-quality long-term studies.
