2. Transarterial Radioembolization for Hepatocellular Carcinoma Beyond the Up-to-Seven Criteria: A Single-Center Retrospective Cohort Study.
PMID:日期:2026-10-03To describe clinical outcomes after transarterial radioembolization (TARE) for hepatocellular carcinoma (HCC) beyond the Up-to-Seven criteria. This retrospective study included 94 treatment-naïve patients with HCC beyond the Up-to-Seven criteria who underwent TARE between November 2021 and December 2025. Outcomes included tumor response, local tumor progression-free survival (LTPFS), progression-free survival (PFS), overall survival (OS), and adverse events. The median maximum tumor diameter was 9.5 cm (interquartile range, 7.1-11.4 cm). A single lesion was observed in 55 (58.5%) patients, 30 (31.9%) had oligonodular disease, and 9 (9.6%) had ≥5 lesions. Index-lesion complete response (CR) and objective response rate (ORR) were 38.3% and 83.0%, respectively; at the patient level, CR and ORR were 38.3% and 81.9%, respectively. Median index-lesion LTPFS, patient-level LTPFS, PFS, and OS were 19.6, 15.7, 11.1, and 30.5 months, respectively. Low tumor-absorbed dose (TAD) was independently associated with worse OS (adjusted hazard ratio, 2.70; p = 0.016). Twenty-one patients (22.3%) subsequently underwent curative-intent surgery after TARE, with a more favorable OS pattern observed in the postsurgery state in an exploratory time-varying analysis (p = 0.004). Three fatal adverse events occurred. In this single-center retrospective cohort of patients with HCC beyond the Up-to-Seven criteria, favorable tumor control and survival outcomes were observed after TARE, and 22.3% of patients subsequently underwent curative-intent surgery. Higher TAD was associated with longer OS, although this finding should be interpreted cautiously given the potential for confounding. Careful dosimetric planning is warranted given the observed fatal radiation pneumonitis events.

3. Wireless Tenzing Crossing of Large Vessel-Occlusions - Feasibility of a Technique After Failed Stroke Thrombectomy with Traditional Approaches.
PMID:日期:2026-10-03To test feasibility of Tenzingplasty as bailout technique for resistant large vessel occlusion (LVO). LVO is primarily treated with endovascular thrombectomy (EVT). Up to 10% of cases fail to achieve reperfusion, often due to resistant clot characteristics, intracranial atherosclerotic disease (ICAD), or technical challenges. This study presents a case-series evaluating a bailout crossing technique-wireless "Tenzingplasty" using the Tenzing delivery catheter-for failed conventional EVT approaches with aspiration catheters and microwire crossing. A retrospective review was performed between October 2025 and February 2026 at a single comprehensive stroke center, including adult patients who underwent EVT within 24 hours of stroke onset who had failed multiple aspiration and microwire crossing attempts. Wireless crossing with Tenzing catheter and subsequent Tenzingplasty was employed as a "last hope" technique to traverse and disrupt resistant occlusions in select cases (all cases included). Clinical, procedural, and outcome data were analyzed. All five cases achieved successful clot disruption and reperfusion, with 60% attaining complete (TICI 3) recanalization. Two patients required adjunctive stenting due to re-occlusion risk. No intraoperative complications, including vessel dissection or perforation, were observed. One patient experienced an asymptomatic subarachnoid hemorrhage. Functional outcomes were favorable: 80% achieving a 90-day modified Rankin Scale (mRS) ≤2 and no mortality reported. Mean NIHSS improved post-procedure. This is a proof-of-concept case-series is a feasibility report that wireless Tenzingplasty may offer a rescue strategy for resistant LVO, particularly in cases involving ICAD or failed wire access. Larger, controlled studies are needed to validate its safety and broader applicability.

4. A Twenty-Three-Year Single Center Experience with Elective Transcatheter Coil Embolization of Unruptured Splenic Artery Aneurysms.
PMID:日期:2026-10-03To determine the safety and efficacy of elective coil embolization for unruptured splenic artery aneurysms (SAA). A retrospective review of unruptured SAA treated endovascularly from October 2002 to January 2026 was performed. When the inflow vessel was embolized, primary technical success required both cessation of antegrade flow in the inflow vessel and absence of opacification of the aneurysm sac. When the sac alone was embolized or excluded, primary technical success required absence of sac opacification alone. Risk factors for major splenic infarct were assessed. A total of 156 unique aneurysms were treated in 130 patients. Mean aneurysm size was 2.4 ± 1.1 cm. Primary technical success was achieved in all but one (0.8%) patient over a mean follow-up of 4.1 ± 5.0 years. Secondary technical success was 100% and freedom from sac opacification was 95.1%. Reintervention was required in 4/122 (3.3%) patients with follow-up imaging, all of whom remained successfully treated. Major splenic infarct occurred in 17/122 (13.9%) patients, with portal hypertension as the only independent risk factor for major splenic infarct (adjusted OR 6.07; 95% CI 1.90-19.35; p = 0.002). Eight (6.2%) mortalities occurred, one (0.8%) within 30 days, all unrelated to SAA embolization. Elective coil embolization is a safe and effective treatment for SAA with robust rates of technical success, freedom from rupture, and freedom from sac enlargement.

7. Procedural and Functional Outcomes After Computer Assisted Vacuum Thrombectomy With the 16F Catheter System for Treatment of Pulmonary Embolism: A Subanalysis of the STRIKE-PE Study.
7. 使用16F导管系统进行计算机辅助真空血栓切除术治疗肺栓塞的操作与功能结局:STRIKE-PE研究的亚组分析PMID:日期:2026-09-26This subanalysis of the STRIKE-PE study evaluates use of computer assisted vacuum thrombectomy (CAVT), a form of endovascular therapy for acute pulmonary embolism (PE), with the 16F catheter system. This subanalysis from a prespecified interim analysis reports procedural outcomes and functional outcomes to 90 days from the first 244 patients treated using the 16F catheter system in STRIKE-PE, an ongoing single-arm, prospective, global study evaluating safety, effectiveness, and outcomes after CAVT in patients with acute PE. The primary end points of STRIKE-PE are change in right-to-left ventricular (RV/LV) ratio from baseline to 48-hour follow-up and composite major adverse event (MAE) rate within 48 hours. Mean RV/LV ratio at baseline was 1.34, which decreased to 0.95 at 48 hours postprocedure (27.3% reduction; P < .001). The composite MAE rate within 48 hours was 0.8%. Procedural results included a reduction in mean on-table systolic pulmonary artery pressure (23.6% reduction; P < .001) and a median thrombectomy time of 25 minutes. The functional outcome assessments of Borg dyspnea scale (P < .001) and 6-minute walk test distance (P < .001) progressively improved to 90 days. New York Heart Association (NYHA) class I or none increased from 67.4% of patients at discharge to 82.9% at 90-day follow-up (P < .001). Treatment of PE with CAVT using the 16F catheter system was associated with statistically significant improvement in right heart function, hemodynamics, dyspnea, and distance walked; additionally, NYHA class recovered. The device had a short procedure time and a low composite MAE rate.

8. From Distance Measurement to Anatomic Landmark: The Cranial Tributary-based Flush Ablation Protocol for Anatomic Landmark-Guided Catheter Tip Positioning in Great Saphenous Vein Ablation.
PMID:日期:2026-09-26To evaluate the feasibility, safety, and mid-term outcomes of the Cranial Tributary-based Flush Ablation (CTFA) protocol, using the first effective cranial tributary (FECT) as the primary landmark for catheter tip positioning (CTP). Consecutive patients undergoing radiofrequency (RFA) or microwave ablation (MWA) with the CTFA protocol between June 2023 and March 2024 were retrospectively included. Under transverse ultrasound, the catheter was retracted until the FECT orifice (>5 mm distal to the saphenofemoral junction) disappeared; predefined distance-based positioning served as fallback; all received pharmacological thromboprophylaxis. Primary endpoints were technical success, FECT-guided applicability, endothermal heat-induced thrombosis (EHIT) 2-4, and anterior accessory saphenous vein (AASV) recurrence. RFA and MWA were compared by propensity score matching in the 97-limb follow-up cohort; random forest-based imputation and Kaplan-Meier analysis addressed missing data. Technical success was 100% (159/159). FECT-guided applicability was 80.5% (128/159; Scenario 1: 11.3% [18/159], Scenario 2: 69.2% [110/159]); 31 limbs (19.5%, Scenario 3) required the distance-based fallback. Mean CTP distance was 15.7 mm. EHIT-2 occurred in 1.3% (2/159), with no EHIT-3/4. At 18 months, duplex follow-up was available for 97/159 limbs (61%); crude AASV recurrence was 8.2% (8/97), with a Kaplan-Meier recurrence-free survival of 93.40%. Great saphenous vein occlusion was 100%, 98.6%, and 95.9% at 1 week, 6 months, and 18 months. Stump-length evolution (biphasic) and clinical outcomes were comparable between modalities. Using the FECT as the primary landmark, the CTFA protocol demonstrated feasibility and acceptable mid-term safety; whether it improves procedural consistency requires prospective validation.

9. Interventions in Acute Venous Thrombosis: Proceedings from the Society of Interventional Radiology Foundation Research Consensus Panel.
PMID:日期:2026-09-24Acute deep vein thrombosis (DVT) is associated with substantial long-term morbidity, primarily through the development of postthrombotic syndrome (PTS). While endovascular strategies aim to clear thrombus and preserve venous patency, randomized evidence has not demonstrated consistent reductions in overall PTS across a broad DVT patient population. New clinical and basic research into individual variability in vein wall inflammation and fibrosis, ability to mount endogenous fibrinolysis, response to antithrombotic therapy, and impact of timing of treatment initiation continue to refine our understanding of overall susceptibility to PTS. Thrombectomy devices remain focused on short-term venous patency, while freedom from PTS is achieved with long-term venous patency, minimization of recurrent thrombotic episodes, and a reduction of perivenous inflammation. Endovascular DVT trial end points remain heterogeneous and frequently emphasize anatomic thrombus reduction without standardized physiologic, biologic, or patient-centered correlates. Recognizing these gaps in evidence, methodology, and end point standardization, the Society of Interventional Radiology (SIR) Foundation convened a multidisciplinary Research Consensus Panel to define research priorities and establish standardized frameworks for outcome determination in acute DVT intervention. Expert panelists representing interventional radiology, vascular surgery, vascular medicine, and hematology delivered structured presentations, followed by moderated discussion and weighted ranking of proposed research initiatives. The resulting agenda emphasizes biologically informed patient selection, standardized anatomic and physiologic metrics, validated acute DVT severity instruments, longitudinal biomarker integration, and incorporation of economic and quality-of-life measures to advance precision and clinically meaningful DVT investigation.

10. Effects of Celiac and Hepatic Sympathetic Denervation on Glucose Homeostasis: A Scoping Review.
PMID:日期:2026-09-22To map the human and animal evidence on the effects of celiac and hepatic sympathetic denervation on glucose homeostasis and identify gaps to inform future trials. PubMed, Embase, Web of Science, and Scopus were searched from inception to December 2025 for studies reporting glucose outcomes after sympathetic denervation of the celiac plexus, celiac/superior mesenteric ganglia (CSMG), or common hepatic artery perivascular nerves in humans or animals. Vagotomy-only studies were excluded. gov and bioRxiv were also searched. Two reviewers screened records; data were charted per PRISMA-ScR methodology, and one preprint was included and identified as such. Twenty-nine studies were included: 3 human studies (2 endovascular denervation [EDN] cohorts, n = 41), 25 animal studies (rat, mouse, dog, and pig), and 1 unpublished preprint (mouse). In patients with type 2 diabetes mellitus (T2DM), percutaneous EDN reduced hemoglobin A1c by 0.9-1.9 percentage points at 6-12 months, with no serious adverse events. In animals, sympathetic denervation broadly improved glucose tolerance and reduced hepatic glucose output across species and denervation paradigms. Counterregulatory responses to hypoglycemia showed species-specific divergence: nearly abolished in pigs after complete denervation, attenuated in rats, and preserved in chronically denervated dogs. One preprint found that CSMG ablation recapitulated the glycemic effects of sleeve gastrectomy in obese mice. Celiac and hepatic sympathetic denervation was associated with favorable glycemic effects across human and animal studies, although the human evidence was limited to 2 uncontrolled cohorts. Counterregulatory responses to hypoglycemia varied by species and extent of denervation.
