1. Burden, trends, and inequalities of anxiety disorders in the Western Pacific region, 1990-2023: A systematic analysis of the Global Burden of Disease Study 2023.
PMID:日期:2026-09-21Anxiety disorders (ADs) are a leading non-fatal cause of global disease burden, yet a detailed assessment of their burden, trends, and socioeconomic inequalities in the Western Pacific region is lacking. Using data from the Global Burden of Disease (GBD) Study 2023, we estimated prevalence, incidence, and disability-adjusted life years (DALYs) for ADs across 31 countries/territories in the Western Pacific region from 1990 to 2023. We calculated age-standardized rates (ASRs). Temporal trends were analyzed with joinpoint regression, and socioeconomic inequalities were assessed using the slope and concentration indices based on the sociodemographic index (SDI). A Das Gupta decomposition quantified contributions of population growth, aging, and epidemiological change to DALY changes. In 2023, ADs resulted in 87.1 million prevalent cases, 12.6 million incident cases, and 10.3 million DALYs in the region. The age-standardized DALY rate was 456.8 per 100,000. Burden peaked in ages 10-14 and was consistently higher in females. From 1990-2023, age-standardized rates for prevalence, incidence, and DALYs increased significantly, accelerating after 2010. Population growth accounted for 57.4% of the DALY increase, while epidemiological change contributed 41.7%. Cross-country inequality analyses did not show significant associations with SDI. The burden of ADs in the Western Pacific has risen substantially, especially since 2010, disproportionately affecting adolescents and females. Epidemiological changes are a major driver. Strengthened prevention, surveillance, and diagnostic capacity are urgently needed to address this growing burden.

2. Examining the structure of post-traumatic stress symptoms in South Africa: A latent class approach.
PMID:日期:2026-09-15Posttraumatic stress disorder (PTSD) is a common and debilitating mental health disorder. In contexts with high levels of trauma exposure, such as South Africa, it is crucial to understand the way in which PTSD symptoms manifest. The present study examined the latent class structure of PTSD symptoms in a large sample of South Africans. Latent class analysis (LCA) was performed to identify patterns of DSM-5 PTSD symptoms in a sample of 477 trauma-exposed South African adults. Subsequent multinomial logistic regression was conducted to assess sociodemographic and clinical predictors of class membership. LCA revealed a four-class solution: (1) a low PTSD symptom class (24.7%), (2) a moderate PTSD symptom class (18,4%), (3) an avoidance/hyperarousal PTSD symptom class (24.7%) and (4) a high PTSD/high negative mood and cognitions symptom class (32.1%). Multinomial logistic regression analyses revealed distinctions between classes in PTSD diagnosis, anxiety symptoms, depressive symptoms, functional impairment and education level. Findings have implications for identifying subgroups of individuals with unique patterns of PTSD symptoms in South Africa and informing interventions in accordance with an individual's particular symptom profile.

3. Poor reliability in measuring habituation: Implications for clinical and translational research on exposure and extinction.
PMID:日期:2026-09-10The purpose of this study was to investigate the reliability of change indices in exposure therapy. We analyzed data from 119 adults with elevated social anxiety who participated in studies of public speaking exposures. Participants reported their distress using the Subjective Units of Distress Scale (SUDS) at regular intervals. The reliability of within-session habituation, between-session habituation, and expectancy change measures were estimated. Commonly used measures of between-session habituation (BSH) typically demonstrated only moderate reliability, and commonly used measures of within-session habituation (WSH) typically demonstrated poor reliability. Exploratory tests in expectancy change indices suggests these findings apply to other measures of change during exposure. Our results indicate that measures underlying the study of habituation may be inadequate because of low reliability. This threatens the foundation of clinical and translational research, such that no substantive conclusions can be drawn from them.

4. Investigating the dynamic coevolution of coping self-efficacy and posttraumatic stress symptoms after hurricane florence: An empirical test of self-regulation shift theory.
PMID:日期:2026-09-05Self-regulation shift theory (SRST) provides a nonlinear dynamical systems-based theory of posttraumatic psychological adaptation. SRST has two central tenets: (1) coping self-efficacy and posttraumatic stress symptoms (PTSS) dynamically coevolve, such that changes in one correspond with changes in the other, and (2) that this coevolution produces critical transitions in psychological state for a subset of trauma survivors, including a self-determination violation (a decrease in coping self-efficacy paired with a rapid increase in PTSS) and/or a self-determination resurgence (an increase in coping self-efficacy paired with a rapid decrease in PTSS). Though existing evidence generally supports these tenets, SRST has yet to be robustly evaluated using nonlinear dynamical systems-based analytic approaches. This study evaluated these tenets using six weeks of ecological momentary assessment data from 81 hurricane survivors reporting continued hurricane-related stress 5-8.5 months after Hurricane Florence. Coping self-efficacy and PTSS velocities were estimated using functional data analysis, and generalized additive modelling was then used to evaluate dynamic coevolution. Results suggested that coping self-efficacy and PTSS velocities coevolved for 91-96% of participants. Inspection of gradient plots suggested approximately 6% of survivors exhibited nonlinear transitions consistent with SRST's self-determination violation and an additional 6% exhibited nonlinear transitions consistent with SRST's self-determination resurgence. Results provide preliminary support for SRST's tenets and suggest that interventions targeting either PTSS or coping self-efficacy may generate cascading benefits across the psychological system. It also adds to the growing body of literature suggesting nonlinear dynamical systems-based approaches can offer considerable insight into posttraumatic psychological processes.

5. Daily associations of posttraumatic stress disorder symptoms and sleep health on family functioning among US Asian Indians with intimate partner violence experiences.
PMID:日期:2026-08-29Asian Indians in the United States (US) experience a substantial number of traumas, especially intimate partner violence (IPV), which can contribute to PTSD symptoms and sleep disturbances. PTSD and sleep disturbances following IPV may influence family functioning and relationships over time due to mistrust of others, perceptions of limited social support, additional health consequences, communication problems, detrimental impacts on children and parenting, and isolation. To advance this line of research, the current study examined how daily PTSD symptoms and sleep disturbances following IPV are associated with daily family functioning among US Asian Indians. A convenience sample of 90 US Asian Indians who had experienced IPV in their lifetime (M = 31.03 years, 67.78% female; 73.33% with probable PTSD; 55.56% with moderate to severe insomnia symptoms) were recruited via online methods and in-person events. Participants were asked to complete 7 consecutive daily surveys on PTSD, sleep health (i.e., efficiency, duration, quality, nightmare occurrence), and family functioning (daily survey compliance rate = 84.0%). Multilevel models indicated that nights with better sleep quality than average (β = 0.05, p = .043) and days with lower PTSD symptom severity than average (β's = -0.05 to -0.07, ps <.05 across models) were associated with better daily family functioning. After covarying for previous-day family functioning, most effects became non-significant. Findings suggest that targeting PTSD and sleep health may relate to improved family functioning, which can be important components of culturally informed IPV interventions for Asian Indians.

6. Postpartum obsessive compulsive symptom onset: Modelling vulnerability longitudinally.
PMID:日期:2026-08-28The perinatal period represents risk for the development of postpartum obsessive-compulsive symptoms (POCS) but the variables of risk that lead to symptom development are not well understood. The goal of the current study was to evaluate the longitudinal, cross-lagged associations between obsessive beliefs and subsequent postpartum symptom onset, in the context of genetic and personality vulnerability, from early pregnancy to approximately 20 weeks postpartum. The study also tested cognitive specificity, namely whether obsessive beliefs are uniquely predictive of POCS compared to general mood and anxiety symptoms, and conversely, whether cognitive vulnerabilities thematically congruent to mood and anxiety symptoms are predictive of POCS. In a prospective study with a community perinatal sample (N = 599), women completed a genetic assay and a test battery of measures assessing personality, stress, symptom functioning and cognitive vulnerability in early pregnancy (Time 1), and symptom and cognitive measures again in the early postpartum period (Time 2), and in the later postpartum period (Time 3). The model fit was acceptable, and the results supported the hypothesized cross-lagged associations between elevated obsessive beliefs about inflated responsibility/threat and subsequent POCS development at each phase of the study. Obsessive beliefs were not associated with the development of mood and anxiety symptoms in the postpartum period, nor were thematically congruent cognitive vulnerabilities for depression and anxiety symptoms predictive of POCS onset. Obsessive beliefs focusing on inflated responsibility/threat demonstrate specificity and temporal salience in predicting POCS onset longitudinally.

7. Risk factors of anxiety disorders in Korean children and adolescents: National mental health survey of Korea.
PMID:日期:2026-08-25Anxiety disorders are among the most common psychiatric disorders in youth and are associated with impaired academic, social, and developmental outcomes. Despite their public health importance, nationally representative evidence integrating individual, family, and environmental correlates among Korean youth remains limited. Data were obtained from the National Mental Health Survey of Korea-Child & Adolescent 2022, a nationally representative sample of 6275 participants aged 6-17 years. Anxiety disorders were diagnosed using the Korean Kiddie Schedule for Affective Disorders and Schizophrenia-Computerized Version. Survey-weighted logistic regression models were used to estimate adjusted odds ratios (aORs) and 95% confidence intervals (CIs). Stratified analyses by developmental stage (<12 years and ≥12 years) were performed to examine age-specific differences in risk profiles. The prevalence of anxiety disorder was 10.2%. Female sex (aOR=1.36, 95% CI=1.13-1.64), sleep disruption occurring monthly or more often (aOR=1.83, 95% CI=1.44-2.34), infrequent physical activity (aOR=1.51, 95% CI=1.02-2.24), peer victimization (aOR=2.28, 95% CI=1.73-3.00), urban residence (aOR=1.74, 95% CI=1.18-2.56), parental divorce (aOR=2.70, 95% CI=1.64-4.49), and unknown parental marital status (aOR=4.86, 95% CI=2.92-8.11) were associated with higher odds of anxiety disorder, whereas underweight status (aOR=0.34, 95% CI=0.16-0.71) and larger household size (aOR=0.42, 95% CI=0.24-0.75) were inversely associated with anxiety disorder. In age-stratified analyses, sleep disruption, peer victimization, and urban residence were consistently associated with anxiety disorder across both age groups. These findings highlight the importance of individual, interpersonal, social-environmental, and family/household correlates of anxiety disorder among Korean youth and support the need for multilevel prevention strategies.

8. Emotion differentiation in social anxiety disorder: A comparison with panic disorder.
PMID:日期:2026-08-20Social anxiety disorder (SAD) is a common and debilitating disorder. Emotion differentiation (ED), the ability to identify and label distinct emotional experiences, is an emotional construct that plays an important role in SAD. Specifically, ED is vital for adaptive emotion regulation and previous research has suggested it may be disrupted in SAD. The present study examined ED among individuals with social anxiety disorder (SAD; n = 44), panic disorder (PD; n = 29), and a control group without SAD or PD (n = 43). Participants were recruited from the community and diagnosed in the lab, and were not treatment-seekers. Mean age was 28.54 (SD = 8.53) and 55.2% of participants were women. Participants completed a 21-day daily diary reporting on eight positive and eight negative emotions each day. ED was calculated separately for positive and negative emotions using intraclass correlation coefficients (ICCs). Consistent with the hypotheses, both SAD and PD groups demonstrated poorer ED of negative emotions compared to controls. Individuals with PD, but not SAD, also showed poorer ED of positive emotions relative to both the SAD and control groups. These findings indicate both shared and disorder-specific patterns of ED across SAD and PD. Implications for emotion-focused interventions are discussed, including the potential to tailor interventions to the distinct emotional profiles of SAD and PD.

9. Trajectories and predictors of anxiety in childhood and adolescence: A 12-year longitudinal study.
PMID:日期:2026-08-18This study identified trajectories of anxiety symptoms from childhood to middle adolescence among children characterised as Behaviourally Inhibited (BI) and Behaviourally Uninhibited (BUI) at age 4. How BI and family environment factors predicted trajectories of anxiety and anxiety diagnosis at age 16 was examined. This longitudinal study recruited 202 children including BI (n = 102) or BUI (n = 100) aged 3-4 years. Temperament and family environment factors were assessed via observation and parent report at baseline. Child anxiety symptoms were assessed when children were 4, 6, 9, 12, and 16 years old. Child anxiety diagnosis was assessed via diagnostic interview at age 16. Growth mixture modelling identified three distinct trajectories of anxiety symptoms: low-stable (72.87%), moderate-stable (19.58%), and high-increasing (7.55%). Number of maternal lifetime anxiety diagnoses significantly predicted the high-increasing and moderate-stable trajectory relative to the low-stable trajectory. Separate regression analyses showed that number of maternal lifetime anxiety diagnoses when children were aged 4 was significantly associated with the number of child anxiety diagnoses at age 16. BI, maternal overinvolvement, mother-child attachment and maternal negativity at age 4 were not significant predictors of trajectory group nor child anxiety disorders at age 16 after correcting for multiple comparisons. Three classes of anxiety symptoms trajectory were identified through childhood and middle adolescence. Maternal anxiety was the most consistent predictor of child anxiety. Targeted prevention programs could be provided for children whose mothers have experienced clinical levels of anxiety.

10. Emotion regulation and anxiety disorder symptoms: Within-person relations from ages 6 to 18 in a birth cohort.
PMID:日期:2026-08-14Emotion regulation difficulties have been implicated as a risk factor for anxiety disorders in youth, but prospective studies disentangling within-person from between-person effects are needed to substantiate such a claim. We therefore tested the bidirectional, within-person relations between emotion regulation and symptoms of generalized anxiety disorder, specific phobia, and social anxiety from 1st grade to late adolescence. Participants were 911 children and adolescents from the Trondheim Early Secure Study, a Norwegian birth cohort sample assessed biennially from ages 6 to 18. Emotion regulation was reported by parents and teachers. Anxiety symptoms were assessed using structured diagnostic interviews. Results from Random Intercept Cross-Lagged Panel Models (RI-CLPM) were contrasted with those from ordinary Cross-lagged Panel Models (CLPM). Age and sex differences were examined. According to CLPM, lower emotion regulation, as reported by both parents and teachers and across ages and sexes, predicted a higher number of anxiety symptoms two years later, and vice versa. However, according to RI-CLPM, apart from two paths in the model of parent-reported emotion regulation with specific phobia, no prospective within-person associations were found. Prospective relations between emotion regulation and anxiety may primarily be due to common factors linking the two, as these associations are no longer significant when time-invariant confounding is accounted for and within- and between-person effects are disentangled. Our results do not support the hypothesis that poor emotion regulation increases the risk of developing anxiety disorder symptoms.
