1. Creatine for glucagon-like peptide-1-associated muscle loss.
1. 肌酸用于胰高血糖素样肽-1相关的肌肉流失PMID:日期:2026-09-21Glucagon-like peptide-1 (GLP-1)-based therapies have transformed obesity treatment, yet reductions in lean tissue may accompany pharmacological weight loss. We hypothesize that rapid weight loss and associated reductions in energy and protein intake, mechanical loading, and potentially physical activity may create conditions of increased skeletal-muscle bioenergetic vulnerability. Creatine supplementation represents a biologically plausible adjunct that could support muscle energetics and function in this setting. However, this proposal is hypothesis-generating, and no randomized clinical trial has directly evaluated creatine supplementation during GLP-1-based pharmacological weight loss. Future trials should determine whether creatine, as part of a multimodal muscle-preservation strategy, can help preserve muscle quality and functional capacity during treatment.

2. Diabetes remission 10 years after bariatric surgery: Extension study of a randomized controlled trial.
2. 减重手术后10年糖尿病缓解:一项随机对照试验的扩展研究PMID:日期:2026-09-19Metabolic bariatric surgery (MBS) is highly effective for improving glycemic control in patients with severe obesity and type 2 diabetes (T2D), yet long-term evidence remains limited. Understanding the durability of remission and the mechanisms that sustain metabolic benefits is essential for guiding surgical decision-making. This 10-year extension study follows participants from the DIABETCIR randomized controlled trial, in which adults with severe obesity and T2D were randomized 1:1:1 to Roux-en-Y gastric bypass (RYGB) with long biliopancreatic limb, sleeve gastrectomy (SG), or greater curvature plication (GCP). Clinical, anthropometric, and biochemical data were collected at baseline, 1 year, 5 years, and up to 10 years after surgery. Fasting concentrations of PYY, ghrelin, glucagon, and area under the curve (AUC) of GLP-1, after a standard meal test were determined prior to and at months 1 and 12 after surgery. Outcomes included the identification of predictors of long-term diabetes remission including changes in incretin hormones using logistic regression, and longitudinal trajectories with mixed-effects models. In addition, weight loss (WL), glycemic control, remission and recurrence of T2D and time to remission were compared between surgeries at 10 years. Forty-five patients were randomized and 93% completed the 10-year follow-up. RYGB achieved the greatest WL and superior metabolic control at 1, and 5 years but the differences were attenuated at 10 years. T2D remission rates decreased over time and at 10 years, remission persisted in n = 8/15 (53.3%) of RYGB patients, compared with n = 3/12 (25%) after SG and n = 4/15 (26.7%) after GCP. Higher probability of remission over time was observed after RYGB compared to SG and GCP group. A shorter duration of T2D, early increases in GLP-1 secretion and WL within the first postoperative year, were associated with long-term remission regardless of surgical technique. Although relapse rates were high across procedures (41.7-50.1%), the use of glucose-lowering medications decreased substantially in all treatment groups, especially after RYGB. MBS provides durable improvements in T2D, with RYGB offering a sustained metabolic benefit. Less advanced T2D, greater initial weight loss, and a strong GLP-1 response are associated with long-term remission. These findings highlight complementary contributions of anatomical modification, weight loss, and enteroendocrine changes to long-term surgical success. 3. Lifestyle strategies and nutraceuticals in chronic venous disease: an evidence-weighted synthesis and clinical positioning.
3. 慢性静脉疾病中的生活方式策略与营养保健品:基于证据权重的综合分析与临床定位PMID:日期:2026-09-18Chronic venous disease (CVD), including chronic venous insufficiency and venous leg ulcers (VLU), is common and substantially impairs quality of life. Compression is the cornerstone of conservative care, but lifestyle measures and selected nutraceuticals may provide additional benefit. This review aims to synthesize and clinically position evidence on adiposity, weight management, diet-related factors, and venoactive nutraceuticals across CVD phenotypes. Evidence from epidemiological studies, clinical cohorts, randomized trials, and meta-analyses was summarized using a pragmatic phenotype-aware framework distinguishing early symptomatic disease from advanced phenotypes with oedema, skin changes, or venous ulceration. Excess adiposity shows a consistent, modifiable association with higher CVD prevalence, greater clinical severity, and poorer outcomes, supporting weight management as a foundational conservative strategy. Nutritional epidemiology provides mainly associative signals for dietary patterns and individual foods; these data best inform counselling within broader weight optimization and cardiometabolic risk reduction rather than disease-specific prescriptions. For symptomatic C0 to C4 disease, flavonoid-based venoactive nutraceuticals, particularly micronized purified flavonoid fraction (MPFF) and Vitis vinifera seed extract (VVSE), show the most consistent improvements in heaviness, pain, clinical severity scores, and quality of life. In advanced disease, adjunctive MPFF, Centella asiatica and omega-3 fatty acids may favorably influence VLU healing within compression-based protocols, although heterogeneity limits certainty. A phenotype-stratified approach should prioritise weight control and compression as core interventions, with targeted nutraceuticals considered according to dominant clinical goals: symptom relief, quality of life, oedema control, or ulcer healing. Harmonized trials with clinically meaningful endpoints are needed.

4. Genetic architecture of ultra-processed food consumption links neurodevelopmental and metabolic pathways to habitual diet.
4. 超加工食品消费的遗传结构将神经发育和代谢通路与习惯性饮食联系起来PMID:日期:2026-09-18Ultra-processed food (UPF) consumption is rising globally and is implicated in non-communicable diseases including cancer, metabolic disorders, and dementia. Engineered to optimize palatability, UPFs introduce sensory properties that may drive consumption beyond that of traditional foods. However, it remains unclear whether this susceptibility is a product of environmental exposure alone or reflects a distinct, heritable genetic architecture, as is the case in food preferences in general. Here, we present a genome-wide association study of UPF consumption in over 142,000 UK Biobank participants with a polygenic score validation in an independent cohort. We identify 23 genes, most of which cluster within two major chromosomal inversions - 17q21.31 and 8p23.1 - linked to brain development and morphology, as well as FGF21, a gene associated with sugar and alcohol preference. Downstream analyses reveal that UPF-associated genes are preferentially expressed in neurons and oligodendrocyte precursor cells of subcortical brain regions previously implicated in UPF intake. Genetic correlations with metabolic traits, macronutrient intake, and self-control provide insight into potential physiological and behavioural correlates of UPF consumption. These findings highlight neurobiological correlates of UPF intake, generating hypotheses regarding brain-behaviour relationships in diet-related disease.

10. How diet-related follow-up period affects risk of adverse health outcomes.
10. 与饮食相关的随访期如何影响不良健康结果的风险PMID:日期:2026-09-11Regression dilution bias (RDB) occurs when baseline exposure measurements such as 25-hydroxyvitamin D or dietary intake fail to account for changes over time, potentially weakening observed associations in prospective cohort studies (PCS). In this narrative review, we examine how follow-up duration after food-frequency questionnaire (FFQ) completion influences relative risk (RR) estimates in meta-analyses of diet-related chronic disease outcomes. A narrative review and graphical reanalysis of studies in six meta-analyses involving diet-related exposures and outcomes was conducted. RR values from PCS in populations with Western dietary patterns were included and plotted against mean follow-up periods. Linear regression analyses were performed to assess strength and direction of association between follow-up time and RR estimates. Plots were made of RR versus mean follow-up period after completion of FFQs for incidence of type 2 diabetes mellitus from coffee, processed meat, and whole grains; for risk of coronary heart disease and cardiovascular disease from nuts; and for risk of ischemic heart disease from processed and red meat. Linear regression fits to the data were made with and without inclusion of Harvard University's nurses and health-professionals' studies. Those studies last for up to 26 years but have participants complete FFQs every four years. Longer follow-up periods after baseline dietary assessments may introduce RDB, weakening observed associations in nutritional epidemiology. Future studies should consider repeat FFQs every few years and adjust for RDB. Meta-analyses should account for temporal misclassification to avoid underestimating dietary risk estimates.
