2. 儿童和青少年数字媒体使用的公共卫生挑战:以健康的数字决定因素作为解释框架PMID:日期:2026-10-04Children and adolescents are growing up in a hybrid world in which digital media has become part of the infrastructure of everyday life and a structural determinant of their health. In the second paper of this Series on the digital determinants of health, we apply the framework introduced in the first paper to one of the most contested questions in public health: how young people's digital media use shapes their health and wellbeing. Using the digital determinants of health framework to interpret the current evidence base, we examine what is known, and what remains poorly understood, about the public health implications of children's and adolescents' digital media use. Evidence suggests that digital media use can be associated with both benefits and harms although it is more consistently associated with adverse mental, behavioural, physical, cognitive, and social outcomes, with developmental stage emerging as an important moderator. Illustrative modelling indicates the potential population burden is substantial, even as its causal basis remains uncertain. Research to date has privileged easily measured proxies, such as screen time, and given insufficient attention to the platform design, algorithmic systems, and commercial incentives that shape young people's experiences. Situating young people's digital media use with a digital determinants of health framework, we conclude that protecting children's and adolescents' health calls for a precautionary, structural response-one that moves beyond individual behaviour change to reshape the digital systems, incentives, and knowledge practices that determine how digital media affects health.

3. Governing the digital determinants of health for children and young people: a global public health imperative.
3. 治理儿童和青少年健康的数字决定因素:一项全球公共卫生的紧迫任务PMID:日期:2026-10-04The digital determinants of health are the structural conditions through which digital infrastructures, business models, and design choices shape health outcomes. Yet these determinants remain largely outside public health policy and remain fragmented across regulatory sectors. Rather than regulating individual technologies in isolation, governance should ensure that digital systems and environments advance population health and reduce inequities. A public health approach shifts attention from individual responsibility-and the tendency to blame users-towards the governance of the structural features that produce health risks and inequities. We identify four governance domains for the digital determinants of health: infrastructure and access, platform design and commercial practices, data governance and surveillance, and institutional accountability and rights. Effectively governing these domains requires intersectoral strategies that tackle problematic concentrations of political and economic power. We argue that the governance of the digital determinants of health should be recognised as a core public health function, with the health of children and young people as a priority and entry point and precaution as a guiding governance principle.

4. The digital determinants of health: a new era for global public health.
4. 健康的数字决定因素:全球公共卫生的新时代PMID:日期:2026-10-04Digital transformations exert increasing influence over the lives of the global population, shaping opportunities and generating harms for health and wellbeing across the spectrum of digital access and engagement. The scale of influence of digital transformations on human and planetary health marks the urgency of attention to the digital determinants of health (DDoH). In this Series paper, we describe a prevailing digital sociotechnological order (DSTO) as the structural source of the DDoH. The DSTO is composed of a set of dominant assumptions that motivate digital practices operating through four features (political norms, economic logics, infrastructures, and personal and social practices) to affect human and planetary health. We describe the implications of the DSTO for understanding, studying, and governing the DDoH, and present recommendations for future work.
