Extensive surgical expertise is required to manage blunt and penetrating thoracic trauma. Because opportunities to acquire and maintain these operative skills are limited during training and routine operative practice, trauma focused short courses help bridge potential procedural performance gaps. Integrating procedural assessment into these courses improves surgeon readiness by identifying and closing performance gaps. However, detailed assessment can increase cognitive load and fatigue for assessors. The purpose of this study was to reduce the assement burden by identifying the most predictive assessed procedural elements of overall thoracic trauma operative performance with > 70% accuracy. A total of 1531 military general surgeons participated in cadaver-based training over a 5-year period performing 11 thoracic procedures. Most participants (57%) had < 5 years of professional experience. Linear regression analyses examined the contributions of each procedure (11 predictors) and procedural element (96 predictors) with aggregate procedural performance in thoracic trauma (criterion) (p < 0.01). Seven of the 11 procedures accounted for 96% of the variance in overall thoracic trauma procedural performance. Of the 96 procedural elements examined, 46 predicted overall thoracic trauma procedural performance accounting for 76% of the variance. These findings demonstrate the potential to substantially reduce the faculty assessment burden while maintaining a high level of predictive validity of overall operative performance. The procedural abilities of surgeons to manage traumatic thoracic injuries is critical for patient safety and quality of care. The findings from this study demonstrate that assessment of a focused subset of procedural elements during trauma focused short courses could predict 76% of the procedural abilities for thoracic trauma and provide surgeons with the information they need to ensure their readiness to care patients. Unfortunately, most trauma-focused short courses currently rely on self-perceived competence rather than direct procedural assessment and therefore miss opportunities to identify and close critical performance gaps.

Articular cartilage has limited intrinsic repair capacity, and osteochondral defects may progress to degenerative joint disease. Ozone has anti-inflammatory properties but is highly reactive and short-lived in gaseous form. This study evaluated whether a stabilized liquid ozone-containing formulation delivered through a hyaluronic acid/poly-D,L-lactide (HA/PDL) scaffold improves early histological repair of osteochondral defects. Forty female Sprague-Dawley rats were randomly allocated to four groups (n = 10 each): healthy reference, untreated defect, HA/PDL scaffold, and HA/PDL scaffold impregnated with the stabilized liquid ozone-containing formulation. A 1-mm-diameter, 3-mm-deep osteochondral defect was created in the right femoral intercondylar region. Animals were euthanized on postoperative day 45. Histological repair was evaluated using the Modified Mankin score (0-14), and residual cartilage damage area was quantified in mm² on the index haematoxylin and eosin (H&E)-stained section using calibrated ImageJ analysis. All animals completed the study. Because the healthy reference group received zero scores for all histological variables, inferential analyses were restricted to the operated groups. Total Modified Mankin scores differed significantly among groups (p = 0.001), with the ozone-loaded scaffold group demonstrating lower scores than both the untreated defect group (p = 0.002) and the scaffold-only group (p = 0.006), whereas no difference was observed between the latter two groups (p = 1.000). Significant group differences were also observed for structure (p = 0.001) and tidemark integrity (p = 0.004). Residual cartilage damage area differed among the operated groups (p = 0.001) and was smaller in the ozone-loaded scaffold group than in the untreated defect group (p < 0.001), whereas the comparison with the scaffold-only group did not reach statistical significance (p = 0.169). The stabilized liquid ozone-containing formulation delivered through an HA/PDL scaffold was associated with improved early histological repair compared with untreated defects and yielded lower histological degeneration scores than scaffold implantation alone. Because the formulation comprised multiple components and no ozone-free vehicle control was included, these findings cannot be attributed specifically to ozone and should be interpreted as evidence of improved early histological repair rather than true cartilage regeneration. Not applicable (Experimental animal study).

Early mortality after hip fracture surgery remains high. Existing risk models treat age and frailty as independent predictors and may not reflect how physiologic reserve modifies risk across fixation strategies. We evaluated whether modeling age-frailty interactions improves estimation of 30-day mortality. A retrospective cohort study using the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database (2019-2023) included adults undergoing intramedullary nailing (IMN), hemiarthroplasty, or plate fixation for femoral neck, pertrochanteric, or subtrochanteric fractures. Frailty was measured using a validated 5-item modified frailty index. Generalized additive models with bivariate spline surfaces generated procedure-specific mortality risk estimates. Secondary outcomes were assessed using propensity score-matched cohorts. Among 115,643 patients, 30-day mortality was 5.25%. Predicted mortality varied across age-frailty combinations, ranging from 1.41% to 28.83% for IMN, 1.26%-32.06% for hemiarthroplasty, and 0.48%-20.37% for plate fixation. IMN showed higher transfusion rates, while hemiarthroplasty demonstrated higher cardiopulmonary complication rates. Length of stay did not differ between procedures. Modeling age and frailty as interacting variables produces procedure-specific mortality risk estimates after hip fracture surgery. These estimates support patient counseling and perioperative decision-making using routinely available clinical data.

When patients are positioned supine for percutaneous screw insertion to achieve posterior pelvic ring fixation, the edge of the operating table can impede screw insertion trajectories, thereby increasing the risk of cortical perforation, especially during iliosacral screw (ISS) placement. Multiple screw insertion might also potentially increase the risk of cortical perforation. The aim of this study was to evaluate the feasibility, safety, and efficacy of inserting multiple (≥2) transsacral screw (TSS) and ISS in the supine position via the application of specific surgical strategies and techniques developed in our institution. Patients who underwent stabilization of posterior pelvic ring injuries via percutaneous insertion of multiple TSS and/or ISS in the supine position between May 2018 and December 2025 were included in this study. Distinct screw insertion combination patterns at different vertebral levels, which were primarily determined by the variable anterior-posterior width of the transverse osseous pathway at the first sacrum, were employed for posterior pelvic ring fixation. Wound infection and neurovascular complications associated with screws insertion procedure were recorded. Postoperative CT scans were performed to assess the position of the screws. At follow-up, radiographic evaluations were performed to evaluate the complications including screw loosening, screw breakage, and non-union. A total of 149 large-diameter (≥7 mm) screws, including 123 TSSs and 26 ISSs, were inserted in a total of 69 consecutive patients. No wound infection and neurovascular complication associated with the screw insertion procedure were found postoperatively. Postoperative CT imaging demonstrated that all inserted screws were intraosseous. All patients were followed for an average of 19.5 months (range, 6.6-38 months). Only one screw (S2 TSS) breakage was noted, and no screw loosening and no bone nonunion occurred. Fluoroscopy-assisted percutaneous multiple large-diameter screws (TSS and ISS) insertion for posterior pelvic ring injuries with the patient positioned supine has been confirmed to be a feasible, safe, and effective surgical procedure.

Chondrogenic differentiation of human adipose tissue-derived mesenchymal stem cells (ADMSCs) is a promising strategy for cartilage regeneration, but its regulatory mechanisms remain incompletely understood. MicroRNAs (miRNAs) play critical roles in post-transcriptional control of lineage commitment and tissue development. This study aimed to identify miRNAs differentially expressed during ADMSCs chondrogenesis and explore their functional roles in chondrogenic gene regulation. ADMSCs were induced to undergo chondrogenic differentiation under defined conditions. miRNA expression profiles were analyzed, and significantly dysregulated miRNAs were identified. Bioinformatic and literature-based analyses were used to associate these miRNAs with chondrogenic transcription factors, signaling pathways, and cellular processes. Chondrogenesis was associated with distinct patterns of miRNA regulation. miR-140-5p, miR-520d-5p, miR-127-5p, and miR-218-5p were upregulated and correlated with increased expression of cartilage markers (SOX9, COL2A1, ACAN) and suppression of hypertrophy. These miRNAs were linked to TGF-β signaling, epigenetic regulation, and inhibition of terminal differentiation. Conversely, miR-145-5p, miR-101-3p, miR-194-5p, miR-199a-5p, miR-182-5p, miR-29a-5p, miR-20a-5p, miR-193b-5p, and miR-574-3p were downregulated, potentially relieving repression of transcription factors, signaling components, autophagy-related genes, and extracellular matrix regulators. Chondrogenic differentiation of ADMSCs is governed by coordinated miRNA regulation, involving activation of pro-chondrogenic miRNAs and suppression of inhibitory ones. This regulatory network integrates key molecular pathways required for stable cartilage formation and highlights potential targets for improving ADMSCs-based regenerative therapies.

Burn injuries are associated with excessive oxidative stress and inflammation that impair mesenchymal stem cell (MSC) survival and regenerative function. This study investigated whether N-acetylcysteine (NAC) preconditioning could enhance the therapeutic efficacy of bone marrow-derived MSCs (BMSCs) in oxidative stress conditions and burn wound healing. In vitro, BMSCs were preconditioned with NAC (5 or 10 mM) prior to H₂O₂ exposure, and cell viability, proliferation, oxidative stress markers, and regenerative gene and protein expression were evaluated. In vivo, BMSCs isolated from male Sprague Dawley rats were transplanted into second-degree thermal burn wounds induced in female rats, and wound healing was assessed over 21 days using macroscopic, histological, biochemical, and molecular analyses. NAC preconditioning significantly improved BMSC viability, proliferation, and morphology under oxidative stress while reducing LDH release. NAC-treated BMSCs exhibited increased antioxidant enzyme activities (SOD, CAT, and GSH), reduced malondialdehyde (MDA) levels, and enhanced expression of PCNA, SDF-1, CXCR4, VEGF-A, and IGF-1. In vivo, transplantation of NAC-preconditioned BMSCs accelerated wound contraction, improved re-epithelialization, increased hydroxyproline content, and promoted collagen deposition and tissue regeneration. Furthermore, NAC-preconditioned BMSCs enhanced antioxidant defenses and upregulated SDF-1, CXCR4, VEGF-A, and IGF-1 expression in wound tissues. Collectively, these findings demonstrate that NAC preconditioning enhances the survival, antioxidant capacity, paracrine activity, and regenerative potential of BMSCs, resulting in improved burn wound healing. NAC-preconditioned BMSCs may represent a promising strategy for stem cell-based treatment of burn injuries.

Critical long-bone defects remain a major challenge in orthopaedic trauma and reconstructive surgery, particularly following high-energy injury, infection, and extensive segmental bone loss. Their management frequently requires staged reconstruction with temporary skeletal stabilisation between initial debridement and definitive reconstruction. External fixation remains an established strategy during this interval, while temporary internal fixation represents an alternative approach. However, biomechanical evidence regarding purpose-designed modular intramedullary constructs used in conjunction with polymethylmethacrylate (PMMA) spacers remains limited. To evaluate the axial and bending mechanical characteristics of a modular intramedullary temporary stabilisation system used with a PMMA spacer and to compare its mechanical behaviour with selected plate and external fixation constructs in segmental long-bone defect models. Five synthetic long-bone models were evaluated, with one specimen assigned to each experimental configuration. Three 50-mm femoral defect models were stabilised using plate osteosynthesis, external fixation, or modular intramedullary temporary stabilisation. Two additional modular intramedullary configurations comprised a 400-mm femoral defect and a 50-mm humeral defect. Each specimen underwent sequential quasi-static axial compression and bending using a TIRATEST-2151 testing machine, with continuous force-displacement recording and complementary optical displacement assessment. Axial and bending stiffness were analysed descriptively; no inferential statistical comparisons were performed. In the 50-mm femoral defect configurations, measured axial stiffness was approximately 60 N/mm for plate osteosynthesis, 80 N/mm for external fixation, and 90 N/mm for modular intramedullary stabilisation; corresponding bending stiffness values were approximately 1.0, 0.6, and 1.2 N/mm, respectively. The 400-mm femoral modular configuration demonstrated axial and bending stiffness of 155 and 1.2 N/mm, respectively, while the 50-mm humeral modular configuration showed the highest measured values, at 325 and 1.5 N/mm. Given the single-specimen design, these differences were analysed descriptively and no inferential comparisons were performed. The modular intramedullary temporary stabilisation system demonstrated measurable axial and bending stiffness across the tested segmental defect configurations, including the 400-mm femoral model. These findings provide an initial mechanical characterisation of the concept and support further replicated biomechanical evaluation incorporating standardised spacer preparation, additional comparators, and torsional and cyclic loading.

The number of High-Level Trauma Centers (HLTCs) (Level I or II) per million people across the United States (US) is variable. The purpose of this study was to describe that variability and to determine if it was correlated with state characteristics. A cross-sectional study of the US was performed utilizing census data. The number of HLTCs per million people in each state was calculated. The correlation between the number of HLTCs per million and the following variables was evaluated for: population density, black and white race, land area, median household income, employment rate, uninsured rate, poverty rate, and public health insurance coverage rate. All fifty states were included. The median number of HLTCs per million was 1.58 (IQR 1.14-2.45) and ranged from 0.47 in New Mexico to 6.28 in North Dakota. The median ratio of level 1 to level 2 trauma centers located in each area was 0.51 (IQR 0.25-1.0) and ranged from 0.0 to 4.0. HLTCs per million was positively correlated with white race (R=0.41, p = 0.002) and employment rate (R=0.32, p = 0.02) and negatively correlated with population density (R=-0.31, p = 0.02) and black race (R=-0.37, p = 0.008). No statistically significant correlations were observed between HLTCs per million and land area, median household income, uninsured rate, poverty rate, disability rate, or public health insurance coverage rate (p > 0.05). The number of HLTCs per million across the United States varied widely and was positively correlated with white race and employment rate and negatively correlated with black race and population density.

The prevalence of chronic health conditions is increasing in New Zealand (NZ) and internationally. This means that more people entering the health system for an acute event, such as major trauma, are likely to have other health conditions (i.e. comorbidities) that may impact their healthcare needs, outcomes and/or cost. This study examines comorbidities among major trauma patients injured in NZ between 2018 and 2022. In this retrospective cohort study, records were linked between Te Rēhita Whētuki o Aotearoa, the New Zealand Trauma Registry and the National Minimum Dataset of hospital discharges for those who experienced a major trauma event (Injury Severity Score (ISS) > 12 or had died in hospital following trauma) in NZ during 2018-2022. Comorbidities were determined using the Multimorbidity Index (M3) using a five-year lookback from the date of discharge. Over 5 years, 11,768 patients were hospitalised for major trauma in NZ. Over half of these (54%) had at least one comorbidity as assessed by the M3. Of those hospitalised with major trauma in 2018, 53% had at least one comorbidity, rising to 56% in 2022. Comorbidities typically resulted in longer length of stay, longer length of ICU stay, longer hours on a ventilator and higher in-hospital and 12-month post-discharge mortality. For example, for the 5143 with serious injury (ISS 16-24), median length of stay was 6 days for those with no comorbidities (M3 =0) and 18 days for those with an M3 > 2; median length of ICU stay was 39 h for those with no comorbidities (M3 =0), and 116 h for those with an M3 > 2. Publicly-funded healthcare costs tended to be higher for those with more comorbidities. The increasing presence of comorbidities has implications for patient care, complexity and healthcare needs of those hospitalised following major trauma. As well as having implications for health service delivery, the findings of this study suggest that it is important to consider prevention, not only with regard to trauma, but also in relation to comorbidities, to reduce the burden on individuals and the healthcare system.
