Tuberculosis of the oral cavity is uncommon in this antibiotic era. We report a case of a long standing non-healing ulcer of the right upper maxillogingival sulcus, found to be tuberculosis of a rather unusual site in the oral cavity, the alveolus.

Oxidative stress and recurrent aphthous stomatitis. 氧化应激与复发性阿弗他口炎
This study analyzed the oxidative stress status in patients with recurrent aphthous stomatitis (RAS) in the presence and absence of active ulceration. Oxidative stress was analyzed in peripheral mononuclear cells of 28 RAS patients with active ulceration and 29 controls. A further blood sample was collected from nine subjects randomly selected from the 28 RAS cases, during the period in which the patients did not have active oral ulceration. The reduced glutathione (GSH), malondialdehyde (MDA), and oxidized glutathione (GSSG) levels were measured in these samples. The mean MDA and GSSG levels were significantly higher in patients with active RAS than in the controls, while GSH was lower in the RAS group (p < 0.01). There was a nonsignificant tendency toward higher MDA and GSSG levels in patients with major RAS compared with minor RAS. On comparing the serum findings in the nine RAS patients in the presence and absence of lesions, the presence of ulceration was associated with even higher MDA and GSSG levels and lower GSH concentrations (p < 0.05) CONCLUSIONS: Oxidative stress was detected in our RAS patients.

Pemphigus vulgaris is a rare cause of oral ulceration. A 34 year old male presented with a three week history of severe oral ulceration which was initially treated as aphthous ulceration. However, he failed to improve and a mucosal biopsy was performed. Histology and immunostaining confirmed pemphigus vulgaris.

Clinical assessment of oral lichen planus based on different scales. 基于不同量表的口腔扁平苔藓临床评估
Different oral lichen planus (OLP) scoring scales have been developed, though none has received unanimous acceptance from all authors. This study involves the clinical application of two symptom and sign measurement scales for assessing OLP. One hundred patients were diagnosed clinically and confirmed as having OLP by biopsy, according to the WHO criteria. A visual analog scale (VAS) was applied (0-10) along with two semi-quantitative tests for assessing the symptoms and signs. The Escudier test yielded an overall site score of 3.8 +/- 1.83 (mean +/- SD), with an activity score of 1.6 +/- 2.59 (mean +/- SD), and a total weighted score of 7.4 +/- 4.78. The pain VAS score was 1.9 +/- 2.06. The Malhotra scale (range 1-7) yielded a maximum total score of 2.9 +/- 1.21, corresponding to grade 1. The Spearman correlation coefficient between the Escudier and Malhotra scales was 0.54 - the difference proving statistically significant (P < 0.001). Uniform OLP scoring system measures need to be adopted.

Laryngeal and other otolaryngologic manifestations of Crohn's disease are uncommon and may be subtle. Crohn's disease is a well-known inflammatory bowel disease of unknown etiology marked by relapsing and remitting granulomatous inflammation of the alimentary tract. Extraintestinal manifestations of Crohn's disease may appear anytime during the course of the disease process and may be the initial symptom. Findings are nonspecific, primarily edema and ulcerations, and may be confused with a multitude of other disease processes. Awareness of these manifestations in the head and neck will prevent misdiagnosis or a delay in diagnosis.

Psychological Factors, Serum Trace Elements, and Clinical Features of Factitious Oral Ulcers in Children and Adolescents: A Cross-Sectional Study. 儿童和青少年人为性口腔溃疡的心理因素、血清微量元素及临床特征:横断面研究
This study examined associations between psychological factors, serum zinc/iron levels, and clinical features of factitious oral ulcers in children (3-16 years). A cross-sectional analysis of 69 patients (78.3% male) from two Chinese hospitals assessed ulcer characteristics, pain (VAS), micronutrient levels, and psychological scores (SCARED, DSRSC, CPRS, SDQ). Statistical analysis used nonparametric tests and correlation analysis (SPSS 26.0). Zinc deficiency (37.7%) correlated with longer disease duration (12.0 vs. 6.0 months, p = 0.035) and shorter ulcer-free intervals (60.0 vs. 95.2 days, p = 0.040). Zinc levels inversely linked to emotional symptoms (r = -0.416, p = 0.022) but positively to prosocial behavior (r = 0.526, p = 0.003). Pain intensity associated with anxiety (VAS: 4.0 vs. 3.0, p = 0.025) and psychosocial distress (r = 0.461, p = 0.010). Older children (8-16 years) had longer disease duration (12.0 vs. 3.6 months, p < 0.001) and higher zinc deficiency (47.5% vs. 24.1%, p = 0.048). Iron showed paradoxical associations with emotional symptoms (r = 0.452, p = 0.012) and prosocial behavior (r = -0.382, p = 0.037). Zinc deficiency and psychological factors critically influence the chronicity of FOU. Multidisciplinary care combining micronutrient supplementation and behavioral interventions is recommended.

Crohn's disease (CD) is a chronic, autoimmune inflammatory disease which may affect the entire gastrointestinal tract, from the oral cavity to the anus. Oral-pharyngeal ulcerations may be significant and persistent in patients with established CD, and the use of TNF-α inhibitor has demonstrated to be useful. We report a unique case of an unusual manifestation of oral CD characterized only by multiple, giant, long-lasting, relapsing ulcerations successfully treated with an escalating dose of Adalimumab at 40 mg weekly as a monotherapy.

Prevention of oral mucositis in paediatric patients treated with chemotherapy; a randomised crossover trial comparing two protocols of oral care. 接受化疗的儿科患者口腔黏膜炎的预防;比较两种口腔护理方案的随机交叉试验
This study compared the efficacy of two protocols for oral care using either chlorhexidine or benzydamine as oral rinses to alleviate mucositis in children undergoing chemotherapy. Eligible participants were randomised to receive either protocol for 3 weeks in a two-period crossover design. The occurrence of ulcerative lesions and severity of mucositis were measured at baseline and twice weekly, using the modified Oral Assessment Guide (OAG). Data were continuously analysed by plotting them directly on predefined sequential charts. According to this sequential analysis, the study could be terminated at the 34th within subject comparison, with a statistically significant reduction in ulcerative lesions (P<0.05) and severity of mucositis (P<0.05) in children on the chlorhexidine protocol. These findings suggest that chlorhexidine together with oral care might be helpful in alleviating mucositis when given prophylactically to children on chemotherapy, but the therapeutic benefit needs to be confirmed in a larger trial.

Sarcoidosis presenting as gingival erosion in a patient under long-term clinical control. 长期临床控制患者以牙龈糜烂为主要表现的系统性结节病
Sarcoidosis is an inflammatory granulomatous systemic disease that rarely affects the oral cavity. Gingival involvement has been reported in only a very limited number of cases, occasionally as the first manifestation of the disease. This article reports a case of sarcoidosis affecting the gingiva and alveolar mucosa in a patient previously treated for the systemic disease and considered under long-term clinical control. A 57-year-old white female presented with a chief complaint of gingival pain in the maxillary right area lasting 3 years. Clinical examination revealed an erythematous, ill-defined erosive macule on the buccal aspect of the maxillary right gingiva extending from the canine to the third molar. The medical history included a diagnosis of sarcoidosis 16 years prior to presentation, which was considered to be under clinical control for the last 11 years. An incisional biopsy was obtained. Histopathology indicated the presence of an intense chronic inflammatory infiltrate and focal areas with non-caseating granulomas. Periodic acid-Schiff and Grocott's stains were negative for microorganisms, and the diagnosis was compatible with sarcoidosis. Systemic evaluation showed no involvement of other areas and organs, and the patient was managed with topical steroid therapy using silicone trays in conjunction with conventional periodontal therapy and oral hygiene instructions. Complete response was achieved after 6 months, and the patient remains in clinical follow-up. Although sarcoidosis is a systemic disease that rarely affects the gingiva, the possibility of gingival involvement as the sole manifestation of the disease should be considered in the differential diagnosis of gingival lesions.

COVID-19's psychological toll on oral health: A cross-sectional study in Iranian adults. COVID-19对口腔健康的心理影响:一项针对伊朗成年人的横断面研究
The Coronavirus disease 2019 pandemic increased global psychological distress, emotional distress, and sleep disturbances, all known risk factors for compromised oral health. Despite this, there is limited understanding of the impacts of these psychological factors on oral health in certain populations, including Iranians. Thus, the present study investigates the associations between sociodemographic characteristics, emotional distress, sleep pattern changes, tooth brushing frequency, and oral ulcer reports in a sample of Iranian adults during the Coronavirus disease 2019 pandemic. This cross-sectional, web-based study collected data from Iranian adults between July and September 2022 using respondent-driven sampling. The Mental Health and Wellness questionnaire was used to gather information on sociodemographic characteristics, emotional distress, sleep pattern changes, toothbrushing frequency, and oral ulcer reports. Simple and multiple logistic regression served for statistical analysis. Among the 240 participants, comprising 164 females and 76 males, with a mean age of 35.3 years (±13.3), 28 individuals (11.7%) reported reduced tooth brushing frequency, and 35 individuals (14.6%) reported oral ulcers. Male gender (OR = 2.75, p = 0.016) and sleep patterns changes (OR = 2.93, p = 0.01) increased the likelihood of reduced tooth brushing frequency. Additionally, being younger than 30 (OR = 2.87, p = 0.025) and fearing coronavirus transmission (OR = 3.42, p = 0.009) increased the odds of oral ulcers. Male gender and sleep pattern changes were risk factors for reduced tooth brushing frequency among the present sample of Iranian adults during the Coronavirus disease 2019 pandemic. Additionally, being under 30 and fearing coronavirus transmission were identified as risk factors for oral ulcers in this population. To preserve and promote adults' oral health during public health crises, targeted educational initiatives, public health awareness campaigns, and integrated mental and oral healthcare approaches are encouraged.
