1. {"_":"An incidental B-lymphoblastic leukemia/lymphoma in a prophylactic salpingo-oophorectomy from a patient with germline mutation.","i":["BRCA1"]}
PMID:日期:2024-09-19B-lymphoblastic leukemia/lymphoma (B-ALL) is a malignant neoplasm of immature B lymphocytes. Occasionally, B-ALL may exhibit morphological and immunophenotypic overlaps with other hematologic malignancies such as B-ALL with isolated myeloperoxidase (MPO) expression which may complicate diagnosis. and are tumor suppressor genes involved in homologous recombination and DNA repair. While germline mutations in these genes are well-established risk factors for breast and ovarian cancers, the association between mutations and hematologic malignancies remains underexplored. We present a rare case of B-ALL with isolated MPO expression, discovered incidentally in prophylactically resected ovaries of a patient with a germline mutation, and review the literature exploring this potential association.

2. Acute promyelocytic leukemia: A rare presentation without systemic disease.
PMID:日期:2024-07-22Acute promyelocytic leukemia (APL) is a subtype of acute myeloid leukemia characterized by an abnormal proliferation of promyelocytes. It is often associated with an aggressive clinical presentation involving complex coagulopathies including disseminated intravascular coagulation, with a significant risk of bleeding and/or thrombosis if treatment with all--retinoic acid (ATRA) is not rapidly initiated. Here we present a unique case of APL which was isolated to femoral bone lesions, without definitive evidence of peripheral blood or bone marrow involvement, and without systemic sequelae.

3. Active pulmonary interstitial fibrosis in a COVID-19 survivor diagnosed by transbronchial lung cryobiopsy six months after onset.
3. 一名新冠肺炎幸存者在发病六个月后经支气管肺冷冻活检诊断为活动性肺间质纤维化PMID:日期:2022-06-27We present a 76-year-old man who visited our center because of persistent dyspnea with hypoxia on exertion 6 months after onset of coronavirus disease 2019. Chest computed tomography showed bilateral extensive subpleural reticulation with traction bronchiectasis predominantly in the lower lung lobes. Transbronchial lung cryobiopsy revealed spatially and temporally intermittent interstitial fibrosis with mild lymphoid cell infiltration. Not only were there membranous fibrotic scars on the alveolar septa, but there were scattered foci of alveolar collapse with young fibroblastic proliferations, suggesting progressive interstitial fibrotic lesions. In addition to fibrotic phase of diffuse alveolar damage, the acceleration of subclinical preexisting interstitial lung abnormalities might have affected his respiratory condition.

5. COVID-19 beyond DAD: Persisting microcirculation thrombosis, hidden infections, and early pulmonary fibrosis as remaining challenges of the disease.
PMID:日期:2022-02-28Despite advances in the management of COVID-19, some events occurring in the course of disease still represent challenges to patient treatment. Here we present a case of severe COVID-19 in which the patient received standard treatment including dexamethasone and prophylactic anticoagulation and died on the 24th day of disease. Autopsy showed exudative and proliferative DAD accompanied by remaining microcirculation thrombosis despite anticoagulation treatment, unperceived fungal infection, concomitantly with the presence of dense acellular fibrotic areas amidst organizing lung lesions. Although improvements were achieved in COVID-19 therapeutics lung microcirculation coagulopathy, unperceived fungus infection and early developing alveolar fibrosis remain unsolved problems associated to the disease.

6. Pulmonary hematoidin deposition in a case of severe COVID19 pneumonia.
PMID:日期:2022-02-14A 59-year-old male with past medical history significant for hypertension, coronary artery disease, atrial fibrillation, hyperlipidemia, obesity, obstructive sleep apnea and deep vein thrombosis developed severe COVID19 pneumonia. His clinical course was noted for progressive respiratory failure that subsequently resulted in bilateral lung transplantation. Examination of the explanted lungs was notable for the presence of extensive grossly apparent bright yellow discolorations that histologically represented hematoidin deposits. The background lung was affected by diffuse alveolar damage, accompanied by infarcts and organizing thrombi. This case suggests that a combination of acute lung injury and thrombotic complications in severe COVID19 pneumonia can facilitate formation of pockets of anoxic environment leading to hematoidin deposition within lung tissue.

7. Hypercoagulopathy as a severe Long-term complication of post SARS-CoV-19 infection.
PMID:日期:2022-02-03Two years since the advent of the COVID-19 pandemic, it is time to discuss the long-term complications post virus infection. We are reporting three autopsy cases from patients who had COVID-19 one to six months before death. All three patients were SARS-CoV-2 negative at admission but expired shortly. At autopsy, the first patient showed subacute diffuse myocardial ischemic injury with microthrombi in pericardial small vessels, whereas the second patient showed catastrophic acute and subacute pulmonary infarctions with hemothorax leading to respiratory failure. The third patient showed subacute severe cerebral infarcts in the left middle cerebral artery region. Our findings suggest the hypercoagulopathy and subsequent vital organ damage may persist beyond the active phase of SARS-CoV-2 infection. It is essential to continue monitoring the COVID-19 patients after recovery, so as to identify those with vital organ injury in a timely manner and to take necessary steps to prevent severe consequences of COVID-19 complications.
