1. Capturing affective states in everyday life - Comparing psychometric properties of two affect measures.
PMID:日期:2026-09-15E-diary items and scales face three contradicting challenges, originating from their repeated assessment: To achieve high compliance and to limit participant's burden a minimal number of items is aspired, to achieve reliability on a momentary level more items per scale are advantageous. As psychometric properties and distributions of e-diary scales are important but rarely reported, we used a clinical- (N = 51) and a community-based (N = 353) intensive longitudinal dataset comprising the momentary Multidimensional Mood Questionnaire (mMDMQ) and the Positive and Negative Affect Items (PNAI), both measuring affect, to investigate and compare, in an exploratory approach, distribution indices, psychometric properties, and assumptions for multilevel modeling. In the community sample, all scales of the mMDMQ and the positive affect scale of the PNAI showed very good psychometric properties and distributions. In contrast, the negative affect scale of the PNAI showed right-skewness and floor effects on the item and scale level, and even in the residuals of the multilevel models. In the clinical sample, the overall picture was opposite with mMDMQ again showing reasonable distributions, but this time positive affect of the PNAI revealing problematic properties. Carefully selecting momentary scales and reporting their psychometric properties is essential in e-diary research.

2. Assessing psychotherapy patients' social context through personal networks and experience sampling: A mixed methods case series exploring feasibility.
PMID:日期:2026-09-09This study explored the feasibility and perceived clinical utility of a novel psychotherapy intervention consisting of two components (1) assessment of patients' social context using the experience sampling methodology (ESM) and personal social network data collection (PSN) and (2) a feedback report summarizing the collected data. Four qualitatively driven concurrent embedded mixed-methods case studies were conducted in a large mental health care facility in the Netherlands and included nine young adults receiving either individual or group psychotherapy: six in part-time treatment and three transitioning from inpatient to outpatient care. Feasibility and utility were evaluated using codebook thematic analysis of semi-structured patient and therapist interviews, patient compliance with the intervention components and researchers' reflexive field notes. Overall, the intervention appeared most feasible in individual therapy, in which patient and therapist engaged with it collaboratively. Engagement was more difficult among avoidant patients and when the intervention was experienced as by patients, highlighting the importance of establishing rapport before its use. The intervention yielded varying degrees of insight for patients, depending on their ability to engage with the assessments and feedback-based therapeutic dialogue. The feedback report specifically improved therapists' understanding of patients' social context. Future implementation will require automation, integration into digital clinical infrastructure, and therapist training to support appropriate interpretation of feedback and follow-up action.

3. A national approach in a European context: Multidisciplinary and multiprofessional recommendations on the clinical use of psychedelic substances.
PMID:日期:2026-09-09The therapeutic benefits of psychedelics for neuropsychiatric conditions have gathered significant attention in Europe and around the world, with treatments using psilocybin, MDMA, and ketamine as notable examples. However, the mind-altering effects of these substances, and the complexity of their regulatory frameworks, both within and outside of therapeutic use, imply careful clinical guidance. Across Europe, associations dedicated to therapeutic use of psychedelics are flourishing, and have produced clinical recommendations, while more generalist professional boards have contributed with various clinical reports, statements and guidelines. In Portugal this prompted a unique collaborative movement of professional associations of Pharmacists, Psychologists and Physicians to work with national ethics authorities, scientific societies and both research and clinical stakeholders, to produce a consensual recommendation addressing core aspects of psychedelic treatments. This statement reflects positions regarding informed consent, the determinants of medical and psychological interventions, ethical considerations and regulatory recommendations. The initial document was open for public consultation, inviting other stakeholders to contribute to the final version, published in Portuguese and translated here into English towards broader access. The statement underscores the importance of multidisciplinary and multiprofessional collaboration among psychiatrists, psychologists and pharmacists, towards establishing common guidelines and regulations to ensure safe and effective clinical applications. By addressing these factors, the recommendations aim to foster responsible therapeutic use, while safeguarding patient well-being.

4. From HIV to SARS-CoV-2 associated neurological disorder ("HAND" to "SAND"): Viral infection as a "time-bomb" for the aging brain.
PMID:日期:2026-08-29In 2020, a novel Coronavirus, Severe Acute Respiratory Syndrome-Covid-2 (SARS-CoV-2), spread globally to provoke a pandemic that infected over 750 million people. The associated disorder, Coronavirus-19 (COVID-19), has caused at least 7 million deaths and around a hundred million people have developed a variable yet sometimes incapacitating condition called Long Covid. This multi-organ syndrome, often referred to as "Brain Fog", encompasses symptoms of extreme fatigue, anxiety, poor sleep and, most prominently, cognitive impairment. Long Covid emerges three or more months after SARS-CoV-2 infection and bears a marked resemblance to the suite of neurological symptoms that evolves several years after infection with Human Immunodeficiency Virus (HIV). Accordingly, by analogy to "HIV Associated Neurological Disorder" (HAND), the acronym," SAND" is proposed for SARS-CoV-2 Associated Neurological Disorder. Mimicking HIV infection/HAND, SARS-CoV-2 infection/SAND is on a collision course with aging, leading to their mutual aggravation and an increased risk of disorders like Alzheimer's and Parkinson's disease. In contrast to HIV, where the virus crosses the blood-brain-barrier in T-lymphocytes and monocytes to infect microglia and other cell types, there is evidence that SARS-CoV-2 enters the brain. Nonetheless, in addition to the indirect effects of a somatic hyper-inflammation ("cytokine storm") and the disruption of blood- and CSF-brain barriers, SARS-CoV-2-encoded S1 Spike protein and other virally-encoded proteins enter the brain to interfere with cerebral function. Microglia and astrocytes adopt a pro-inflammatory phenotype, neurotoxic proteins accumulate, mitochondrial energy generation declines, and synaptic transmission is perturbed. These findings for SAND mirror observations seen with aging and dementia. A broad-based programme of "R and D", healthcare and social support would appear desirable to better understand the complex set of interacting mechanisms underlying SAND, to prevent its onset, to alleviate the debilitating symptoms, and to improve our readiness for countering the impact of those novel, brain-damaging viruses that will inevitably arise in the future.

5. Mental health and gut-brain crosstalk: implications for depression and Alzheimer's disease.
5. 心理健康和肠脑串扰:对抑郁症和阿尔茨海默病的影响PMID:日期:2026-07-24Brain health and mental health disorders are increasingly becoming an essential priority for modern societies as they affect different parameters of life, such as brain health, quality of life, and productivity with a heavy societal and economic burden. Among mental health disorders, depression and Alzheimer disease (AD) have the higher impact on mental health globally as they are highly prevalent, cause long-term cognitive and mood deficits while they are deeply interconnected, with chronic stress raising as a risk factor and potential link between both disorders. This review focuses on the gut-brain axis, a bidirectional communication network that links the gut microbiome with the central nervous system, and its role in brain malfunction and pathology related to stress, depression and AD. This intricate gut-brain crosstalk is orchestrated through top-down and bottom-up mechanisms implicating the HPA axis, the enteric nervous system as well as gut microbiota-derived metabolites, neurotransmitters, epigenetic processes and extracellular vesicles/exosomes that can contribute to depression and AD. The current work provides a comprehensive summary of evidence linking gut microbial alterations to these brain pathologies, with particular focus on lifestyle and nutrition (e.g. food, water) as critical regulators. Lastly, we explore the therapeutic potential of microbiome-targeted interventions including pro/pre/post-biotics, and trace elements (e.g. lithium and silicon). Overall, this review highlights the potential of microbiome-centered strategies as novel interventions to support mental health and wellbeing.

6. Protocol for a randomised controlled trial on smartphone- and web-based app for precision dosing and monitoring of lithium («PharmOracle») for improving adherence to therapy in patients with bipolar affective disorder or schizoaffective disorder.
6. 在智能手机和基于网络的应用程序上进行随机对照试验的方案,用于精确给药和监测锂(«PharmOracle»),以提高双相情感障碍或分裂情感障碍患者的治疗依从性PMID:日期:2026-07-21Lithium remains a first-line treatment for bipolar disorder but its use is declining owing to the need for intensive monitoring and concerns about tolerability. Digital decision-support tools that combine model-informed precision dosing, Bayesian therapeutic-drug-monitoring, and adherence support may facilitate safer, more effective lithium therapy and improve long-term adherence. We aimed to evaluate whether adding a mobile application with precision-dosing, monitoring reminders and medication-support functions («PharmOracle») to standard care reduces a composite measure of non-adherence and adverse outcomes over 12 months, compared with standard care alone. We planning an open-label, multicenter, randomized (1:1) superiority trial enrolling adults (≥18 years) with ICD-10 bipolar I/II or schizoaffective disorder who are initiating lithium carbonate. Planned sample size is 210 participants (105 per arm). The primary endpoint is time to first occurrence of a composite outcome: lithium discontinuation for any reason, hospitalization for lithium toxicity (serum lithium ≥1.2 mmol/L with symptoms), or unplanned psychiatric hospitalization/emergency visit for mood relapse assessed continuously and verified at 3, 6 and 12 months. Key secondary endpoints include changes in the Integral Diagnostic Index for Bipolar Disorder composite index for assessing symptoms of depression (HDRS), mania (YMRS), psychosocial functioning (PSP), and quality of life (WHOQOL), self-reported medication adherence (10-item Medication Adherence Report), adherence to recommended laboratory/instrumental monitoring, proportion of patients achieving therapeutic serum lithium (0.6-1.0 mmol/L), and safety outcomes. The intervention comprises a mobile/web platform performing model-informed individualized dose prediction, Bayesian dose refinement from measured troughs, automated monitoring reminders, adverse-event self-reporting, and clinician dashboards. Analyses will follow intention-to-treat principles using survival methods (Kaplan-Meier, log-rank, Cox regression) for the primary outcome and mixed models or appropriate nonparametric tests for longitudinal secondary outcomes. We anticipate that using of mobile application with precision-dosing, monitoring reminders and medication-support functions will reduce the incidence of the composite outcome by improving adherence and accelerating attainment of therapeutic lithium concentrations, while maintaining an acceptable safety profile.

7. In utero air pollution exposure and pre- and postnatal brain development: a review.
PMID:日期:2026-07-11Air pollution is a major threat to public health globally and may also adversely influence brain development. Key neurodevelopmental processes unfold within narrow and sensitive windows during fetal life and continue through childhood and adolescence, leaving the brain particularly vulnerable to environmental influences. Prenatal air pollution exposure could possibly disrupt fetal neurodevelopmental processes via direct placental crossing of pollutants or through maternal inflammatory processes. This literature review evaluates evidence from ultrasound and MRI studies examining associations of air pollution exposure (particulate matter (PM (≤10 μm), PM (10 - 2.5 μm), PM (≤2.5 μm), PM characteristics (PM composition and oxidative potential)), nitrogen oxides (NO), sulfur dioxide (SO), carbon monoxide (CO), black carbon, and ozone (O)) with structural brain development from the fetal period through adolescence. Findings are summarized per pollutant to evaluate potential adverse effects of individual pollutants in the complex mixture of air pollution. Results indicate associations between air pollution exposure and structural brain outcomes, although no distinct patterns or differences between individual pollutants could be identified. Prenatal ultrasound studies reported negative associations between air pollution exposure levels and the global fetal brain metrics biparietal diameter and head circumference in geographical regions with relatively high air pollution levels. In contrast, childhood and adolescent MRI studies, mainly conducted in relatively low exposure areas, showed no associations with global brain volumes. However, associations with regional brain measures were reported including changes in cerebral cortex regions, subcortical gray matter structures, and global and regional white matter structures, potentially impacting cognitive and behavioral outcomes in childhood. Overall, current evidence supports a link between prenatal air pollution and altered brain development, highlighting the need for longitudinal and mechanistic studies to clarify causal pathways and functional consequences.

8. Outcome strategies for clinical trials in Neuropaediatric rare diseases.
PMID:日期:2026-07-10Neuropaediatric Rare Diseases (NRDs) impose a profound and multidimensional burden on patients, families, and healthcare systems. Persistently low clinical trial success rates reflect an unmet methodological need as much as a therapeutic one. A fundamental bottleneck is the absence of fit-for-purpose clinical outcome assessments. In fact, instruments validated for non-rare or adult populations fail to capture clinically meaningful change in heterogeneous, small, and developmentally complex NRD populations. Building directly on a systematic catalogue of methodological challenges in NRD outcome research published by our group (Acosta et al., 2025), this paper presents a structured set of actionable outcome strategies proposed by a large multidisciplinary expert group convened under the auspices of the European College of Neuropsychopharmacology (ECNP) and the International Society for CNS Clinical Trials and Methodology (ISCTM). Using a structured, iterative expert-opinion approach, each identified challenge served as a prompt for developing one or more candidate strategies, each mapped one-to-one to its corresponding barrier. Strategies are presented across four thematic domains: (1) innovative methodologies to enhance ecological validity and reduce rater context effects; (2) novel or adapted outcomes and endpoints that preserve clinical meaningfulness under conditions of high heterogeneity and limited sample sizes; (3) the purposeful use of natural history resources; and (4) approaches to support comparability and synthesis across programmes. Additional considerations address caregiver expectancy bias and recruitment, stakeholder alignment, maturational confounding, and preclinical-clinical connectivity. Collectively, these strategies constitute a practical, challenge-mapped "living" toolbox for clinical scientists designing NRD trials. Each strategy is already in use or validated in analogous rare-disease contexts. Realising their potential at scale requires institutional programmes, pre-competitive co-validation platforms, systematic stakeholder co-design, and early engagement with regulatory agencies as scientific partners in endpoint development.

9. MicroRNAs expression changes associated with trauma-focused psychotherapy efficacy in treatment-resistant depression: An exploratory study.
9. 一项探索性研究:与治疗抵抗性抑郁症的创伤集中心理治疗疗效相关的MicroRNAs表达变化PMID:日期:2026-07-09Early life stress (ELS) increases vulnerability to major depressive disorder (MDD) and treatment-resistant depression (TRD) through lasting neurobiological and epigenetic alterations. MicroRNAs (miRNAs), key post-transcriptional regulators of gene expression, are emerging as potential biomarkers linking environmental stressors and depression pathophysiology. This study employed a miRNomic approach to investigate plasma miRNA changes associated with trauma-focused psychotherapy in TRD patients with a history of ELS. Twenty-four patients received either trauma-focused cognitive behavioural therapy (TF-CBT) or eye movement desensitization and reprocessing (EMDR) over eight weeks, with depressive symptoms assessed by the Montgomery-Åsberg Depression Rating Scale (MADRS) at baseline (T0) and post-treatment (T8). Plasma levels of 188 miRNAs were quantified using qRT-PCR. Of these, 148 passed quality control and were included in the analysis. Twenty-two miRNAs were nominally significantly downregulated after psychotherapy, with miR-132-3p remaining significant after correction for multiple testing, and four (let-7f-5p, miR-195-5p, miR-29c-3p, miR-32-5p) showing trends toward significance. Pathway enrichment analyses identified 120 pathways involved in inflammation, neuroplasticity, and stress response, including Hippo, TGF-β, TNF, Wnt, PI3K-Akt, Neurotrophin, glutamatergic synapse, and dopaminergic signaling. Subgroup analyses identified nominal downregulation of miRNAs following EMDR (miR-132-3p and let-7f-5p) and TF-CBT (miR-32-5p and let-7f-5p), with none remaining significant after correction. Correlational analyses between miRNAs changes and treatment outcomes showed, in the whole sample, that miR-139-5p downregulation was nominally associated with greater improvements in overall depressive symptoms, as well as cognitive and neurovegetative symptom domains. Subgroup correlational analyses identified significant nominal associations between miR-125b-5p downregulation and improvements in overall depressive symptoms, mood, and neurovegetative domains following TF-CBT, and between miR-139-5p downregulation and overall depressive symptoms (trend significant) and neurovegetative symptom improvement following EMDR; however, none of these associations survived multiple-testing correction. These findings suggest that trauma-focused psychotherapies may induce peripheral miRNA modulation linked to clinical response, with miR-132-3p, miR-125b-5p, and miR-139-5p emerging as potential biomarkers of treatment response. The results support a role for miRNA-mediated neuroplastic and inflammatory mechanisms in the molecular underpinnings of psychotherapy efficacy in TRD patients exposed to ELS.

10. Investigating behavioral inertia in passively sensed smartphone parameters to differentiate affective episodes in patients with bipolar disorder.
PMID:日期:2026-06-24Bipolar disorders are severe, recurrent mental illnesses characterized by distinct mood states associated with different behavioral patterns. While (emotional) inertia, the tendency of a system to persist or change, has been linked to depressive episodes, research on its role in hypomanic or manic states remains sparse. Moreover, behavioral inertia, assessed via passive sensing using smartphone parameters is equally underexplored. This study investigated the temporal dynamics during (hypo-)manic and depressive episodes in passively sensed data in patients with bipolar disorder, complementing previous research on depression and using autocorrelation (AR) and moving averages (Mov.AVG) as dynamic measures. Data were drawn from the BipoSense study (N = 29), with 10 587 observed patient days including 20 (hypo)manic and 30 depressive episodes. Multilevel logit models were used to analyse AR and Mov.AVG across several parameters of activity, communication, sleep and phone use. During depressive episodes, AR was significantly increased in activity and communication domains, particularly in later depressive weeks, reflecting heightened behavioural inertia. In contrast, Mov.AVG showed consistent decreases across activity, communication and sleep domains. For (hypo-)manic episodes, AR patterns were more heterogenous (both, elevated and decreased values), suggesting inconsistent temporal dynamics. However, Mov.AVG revealed a coherent pattern of increased activity, communication and phone use and reduced sleep, aligning with the clinical picture of (hypo-)mania. Concomitant analyses revealed that AR and Mov.AVG seem to explain different variance in the dynamic of patients experiencing bipolar episodes. This study provides the first insight of behavioural inertia in patients with BD using passive sensing smartphone parameters. It underscores the importance of future research investigating behavioral inertia and especially the dynamics during (hypo-)manic episodes on larger cohorts and longer observation periods.
