1. An index system for evaluating the data security in trauma centers: a modified Delphi study in China.
1. 创伤中心数据安全评估指标体系:一项基于中国的改良德尔菲研究PMID:日期:2026-08-26In response to the characteristics of frequent data interactions between pre-hospital emergency care and multi-departmental in-hospital systems, along with complex transmission pathways, we established a dynamic assessment methodology covering the entire data lifecycle and analyzed the status of data security risks within China's trauma care system. This study aims to develop a security risk assessment index for trauma data security (TDSI). Evaluation indicators were established through qualitative interviews, literature reviews, and Delphi expert consultations. The weight for indicators was determined using the analytic hierarchy process (AHP). National surveillance employing a multi-stage random cluster sampling method was conducted at 80 trauma centers in 31 provincial-level administrative units in China in 2023. The Delphi expert consultation recruited administrators, clinicians, and data specialists from trauma centers across various regions of China. Twenty experts from various regions of China were recruited for the Delphi study. The response rate for the final survey was 95%. The TDSI system consisted of three dimensions and 36 indicators. The weight was 0.388 for trauma care data management from the clinical department, 0.342 for information security technologies from the IT department, and 0.270 for data security policies from the management department. The overall TDSI across all trauma centers was 60.27, varying from 25.7 to 82.1 across trauma centers. The northern region had the highest TDSI (66.32). This study developed a TDSI using a modified Delphi method. This index system is a scoring framework that focuses on clinical, management, and IT capabilities that determine the prioritization of data security enhancements for trauma centers.

2. Efficacy and safety of Shulie'an capsules in treating chronic prostatitis: A multicenter, prospective, open-label clinical trial.
2. 疏莉安胶囊治疗慢性前列腺炎的有效性和安全性:一项多中心、前瞻性、开放标签临床试验PMID:日期:2026-08-26Chronic prostatitis is a highly prevalent urological disorder affecting millions of men globally and significantly impairing quality of life. The aim of this study was to evaluate the efficacy and safety of Shulie'an (SLA) capsules in patients with chronic prostatitis. In this multicenter, prospective, open-label, self-controlled clinical trial, 100 male patients with clinically confirmed chronic prostatitis were enrolled in China. The participants received oral SLA (two capsules per dose, three times daily) for 28 days. The primary endpoint was the change in the National Institutes of Health Chronic Prostatitis Symptom Index (NIH-CPSI) total score from baseline to endpoint. Secondary endpoints included NIH-CPSI individual domain scores and disappearance rate of traditional Chinese medicine (TCM) symptoms. Safety assessments involved monitoring and recording all adverse events throughout the study period. After 28 days of treatment, SLA capsules exhibited significant clinical efficacy, with an overall response rate of 91.75%. The mean NIH-CPSI total score reduction was 9.2±5.82 and 15.0±6.79 points on the 14th and 28th day, respectively. Approximately 95.88% of the participants achieved more than 4-point reduction in the NIH-CPSI score from baseline after treating for 28 days. All symptom domains showed statistically significant improvements, with symptoms of pain being the most remarkable (complete resolution rate: 41.57%). Besides, after 28 days of treatment, the total TCM syndrome score of the participants significantly improved, and the treatment was considerably safe with no reported serious adverse events. This study demonstrated that SLA capsules are a promising therapeutic option for chronic prostatitis, significantly improving clinical symptoms and quality of life with a favorable safety profile.

3. Asian consensus on diagnostic algorithm for invasive pulmonary fungal diseases in high-burden settings.
3. 高负担地区侵袭性肺真菌病诊断算法的亚洲共识PMID:日期:2026-08-26Invasive pulmonary fungal diseases (IPFD) continue to posing an increasing clinical and public health burden on immunocompromised populations. In many Asian settings, the diagnosis of IPFD faces substantial challenges driven by the high prevalence of comorbidities, including poorly controlled diabetes, tuberculosis (TB), and human immunodeficiency virus (HIV) infection, considerable heterogeneity in local pathogens, and disparities in access to diagnostic resources. To address these unmet needs, experts from 18 countries collaboratively developed this consensus, which provides a diagnostic algorithm tailored to high-burden Asian settings based on existing international and regional guidelines. When clinical manifestations are atypical or diagnostic clues are limited, the algorithm prioritizes evaluation for common and regionally prevalent IPFD, followed by stepwise expansion to other potential fungal pathogens. In addition, this consensus outlines the regional accessibility of different diagnostic modalities across Asia. This consensus focuses exclusively on optimization of the diagnostic algorithm and does not provide specific recommendations regarding antifungal therapy. Notably, improved diagnosis of IPFD through this algorithm may contribute to better patient outcomes and strengthened public health strategies in high-burden regions.

5. Bone marrow mesenchymal stem cells are functionally remodeled after complete remission of acute myeloid leukemia.
PMID:日期:2026-08-24Acute myeloid leukemia (AML) is a type of hematological malignancy, induces the normal bone marrow (BM) microenvironment to supporting AML cells. Although previous work revealed that functional remodeling of BM endothelial progenitor cells after complete remission (CR), as another important component of the BM microenvironment, whether AML-induced BM mesenchymal stem cells (MSCs) can also be functional modified after CR is largely unknown. Patients with de novo AML, patients with AML who achieved complete remission (AML-CR) and healthy controls (HC) were enrolled. Flow cytometry was used to exploring the regulatory effects of BM MSCs on cluster of differentiation CD34 cells or CD3 cells. Immunocytochemistry and senescence-associated β-galactosidase (SA-β-gal) staining was used to detect the number of senescent BM MSCs. Ribonucleic acid sequencing (RNA-seq) analysis and quantitative real-time reverse transcription PCR (qRT-PCR) were used to detect the transcription profile and characteristics of BM MSCs. An AML-CR mouse model was used to verify the remodeling of BM MSCs after CR. Dysfunctional BM MSCs in AML patients were subsequently recovered in AML-CR patients, including their differentiation potential, apoptosis ratio, expression of cytokines and regulatory roles, which were return to near HC MSCs. Moreover, the transcriptomic profile of CR MSCs was also remodeled and transformed to be more similar to that of HC MSCs. Our data further revealed that the expression of senescent-related genes, the number of senescent cells in AML-CR patients were reduced, and that CR MSCs regained the ability to be reinduced to senescence by AML cells. Our findings describe the functional remodeling of BM MSCs among patients with AML, AML-CR patients and HC, and the reduction in the number of senescent BM MSCs associated with BM MSC remodeling. Our work provides a new perspective indicating that AML-modified BM MSCs can be remodeled to support normal hematopoietic stem cells (HSCs) in AML-CR patients, which may contribute to promoting normal hematopoiesis recovery for AML patients.

6. Multidimensional physiological impairments are associated with frailty and survival in older patients with cancer.
6. 多维生理功能损害与老年癌症患者的衰弱和生存相关PMID:日期:2026-08-24Frailty, which is influenced by the intricate interactions between malnutrition, metabolism, and systemic inflammation, is highly prevalent in the elderly and constitutes a significant barrier to the effective treatment of older patients with cancer. Assessing specific biomarker patterns may enhance our understanding of the mechanisms underlying the intricate networks that contribute to frailty in this population. This study aimed to examine the association between frailty and malnutrition, metabolism, and systemic inflammation in older patients with cancer, and to further assess their prognostic value. This multicenter cohort study included 2219 older patients with cancer categorized as non-frail ( = 533), pre-frail ( = 1084), or frail ( = 602) according to the Fried frailty criteria at a single time point. Principal component analysis (PCA) was performed on 18 variables encompassing nutritional, metabolic, and inflammatory biomarkers to derive the principal components (PCs). Cox proportional hazards and logistic regression models were used to assess the associations between PCs, frailty, and overall survival (OS). Three PCs were identified among 18 variables. PC1 represented a malnutrition-associated pattern. PC2 represented a metabolism-associated pattern. PC4 represented an inflammation-associated pattern. PC3 was excluded because of its redundancy with PC1. Frail patients exhibited significantly higher factor scores compared to pre-frail and robust patients in PC1 (median [IQR]: 1.03 [1.82] . -0.12 [1.65] . -0.93 [1.52]; < 0.001), PC2 (0.20 [1.90] . -0.03 [1.29] . -0.17 [1.03]; < 0.001), and PC4 (0.07 [1.27] . -0.04 [0.97] . -0.06 [0.75]; = 0.049). Furthermore, patients with higher factor scores in PC1, PC2, or PC4 were significantly associated with frailty [adjusted OR: 2.13, 95% CI: 1.90-2.39, < 0.001 for PC1; adjusted OR: 1.13, 95% CI: 1.03-1.23, = 0.008 for PC2; adjusted OR: 1.12, 95% CI: 1.03-1.22, = 0.011 for PC4]. Frail patients exhibited significantly worse survival outcomes compared to pre-frail individuals (adjusted HR = 1.67, 95% CI: 1.45-1.92; < 0.001). Frailty is significantly associated with decreased survival in older patients with cancer and coexists with multidimensional physiological impairments, including nutritional deficits, metabolic disturbances, and systemic inflammation.

9. Natural products modulate renal metabolic reprogramming in diabetic kidney disease.
PMID:日期:2026-08-12Diabetic kidney disease (DKD) is a major cause of end-stage renal disease worldwide. Its pathogenesis is highly complex and involves dysfunction across multiple renal cell types and metabolic pathways, thereby underscoring the urgent need for novel therapeutic strategies. In recent years, renal metabolic reprogramming has been increasingly recognized as a central driver of DKD progression. This review systematically examines the mechanisms of metabolic reprogramming in different renal cell types and highlights their contribution to renal injury. Particular emphasis is placed on the ability of natural products (including glycosides, terpenoids, flavonoids, alkaloids, and polysaccharides) to confer renal protection by modulating key regulatory nodes of metabolic reprogramming. Through these mechanisms, natural products enhance mitochondrial function, suppress aberrant glycolysis, and improve hypoxia-related responses. Collectively, these findings not only underscore the therapeutic potential of natural products in targeting renal metabolic reprogramming but also provide a theoretical foundation and potential molecular targets for the development of novel DKD treatment strategies.

10. Application of metabolomics in drug-induced liver injury: A systematic review.
10. 代谢组学在药物性肝损伤中的应用:系统综述PMID:日期:2026-08-12Drug-induced liver injury (DILI) is one of the most common adverse reactions in clinical settings, and drug hepatotoxicity is also a primary factor contributing to the suspension and withdrawal of drug development. Currently, there are no specific biomarkers available for the early diagnosis and monitoring of DILI in clinical practice. In recent years, metabolomics has emerged as a powerful tool for identifying small molecule biomarkers in DILI, owing to its unique advantages. In this article, we comprehensively reviewed the metabolomics studies related to DILI to investigate the pathophysiological changes in DILI and to assess the potential application of metabolomics in its prediction and diagnosis. Relevant literature published before 30 April 2024 was retrieved from four online databases (PubMed, EMbase, Cochrane Library, and Web of Science). Research data were systematically collected, and the metabolites involved were analyzed. Metabolic pathway analysis related to DILI was conducted using the online software MetaboAnalyst 5.0. A total of 55 studies were included, identifying significantly altered metabolic pathways and valuable metabolic biomarkers. The most frequently reported biomarkers included glycocholic acid, taurochenodeoxycholic acid, taurocholic acid, creatine, glycochenodeoxycholic acid, taurodeoxycholic acid, glycodeoxycholic acid, and -ketoglutaric acid. The disrupted metabolic pathways were mainly related to bile acid metabolism, lipid metabolism, and the tricarboxylic acid cycle, among which bile acid metabolism showing the most prominent alterations. Lipid metabolism-related pathways included fatty acid biosynthesis, glycerophospholipid metabolism, and sphingolipid metabolism. This article provides a systematic review of metabolomics in DILI, deepening our understanding of the metabolic disturbances caused by DILI.
