4. Association between the number of pregnancies and chronic kidney disease in Korean women: a population-based cross-sectional study from the Korean Genome and Epidemiology Study-Health Examinee (KoGES-HEXA) cohort.
4. 韩国女性妊娠次数与慢性肾病之间的关联:基于韩国基因组与流行病学研究-健康体检者(KoGES-HEXA)队列的人群横断面研究PMID:日期:2026-09-18Chronic kidney disease (CKD) is a growing public health burden. Repeated pregnancies exert anatomical and physiological stress on the kidneys; however, the association between number of pregnancies and CKD is unclear. This cross-sectional study used data from the Korean Genome and Epidemiology Study-Health Examinee (KoGES-HEXA; 2004-2013). The number of pregnancies was categorized as 0, 1, 2, 3, 4, or ≥5. The primary outcome was CKD, defined as an estimated glomerular filtration rate <60 mL/min/1.73 m2, using the CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) formula, or dipstick proteinuria ≥1+. We evaluated the association between number of pregnancies and CKD and whether it varied by adverse pregnancy outcomes (APOs). We included 100,433 women (age, 52.7 ± 8.0 years [mean ± standard deviation]). Of the parous women (n = 99,654), 3.8% (n = 3,795) had CKD. In the fully adjusted model, women with ≥5 pregnancies had modestly higher CKD odds than women with one pregnancy (odds ratio, 1.20; 95% confidence interval, 1.01-1.43). The association differed by age (p for interaction <0.001), with a significant trend among women aged ≥60 years (p for trend = 0.009). In subgroup analyses, ≥5 pregnancies remained associated with CKD among women without preeclampsia or gestational diabetes mellitus, with no clear evidence of interactions. A higher number of pregnancies was modestly associated with a higher prevalence of CKD in Korean women, including those without APO history. Although residual confounding by age and birth cohort cannot be excluded, the number of pregnancies may be relevant to women's long-term kidney health.

5. Association of isolated systolic and diastolic hypertension with moderately increased albuminuria: the Korea National Health and Nutrition Examination Survey.
5. 单纯收缩期和舒张期高血压与中度增加的蛋白尿的关联:韩国国家健康与营养调查PMID:日期:2026-09-18Studies show variable results on health outcomes of isolated diastolic hypertension (IDH); however, data are scarce on its association with early kidney injury. We analyzed the Korea National Health and Nutrition Examination Survey data (2011-2014 and 2019-2024) in adults ≥19 years. After excluding those taking antihypertensive medication or with established kidney disease, participants were categorized into normal blood pressure (BP [mmHg]; systolic BP [SBP] <120 and diastolic BP [DBP] <80), elevated BP (SBP 120 to <130 and DBP <80), stage 1 hypertension (SBP 130 to <140 or DBP 80 to <90 but not stage 2 hypertension), and stage 2 hypertension (SBP ≥140 or DBP ≥90) according to the 2025 American Heart Association/American College of Cardiology guidelines. Stage 1 and 2 hypertension was further classified as IDH, isolated systolic hypertension (ISH), or systolic-diastolic hypertension (SDH). Moderately increased albuminuria was defined as urine albumin-to-creatinine ratio of 30-<300 mg/g. Among 33,638 participants (mean age, 43.2 years; 47.9% female), multivariable-adjusted ORs (95% CIs) for moderately increased albuminuria were 1.45 (1.18-1.78) for stage 1 IDH, 1.66 (1.21-2.28) for stage 1 ISH, 2.09 (1.60-2.74) for stage 1 SDH, and 3.38 (2.85-4.01) for stage 2 hypertension. In age-stratified analysis, stage 1 IDH showed 1.37- and 1.58-fold higher odds of moderately increased albuminuria in participants aged <50 and ≥50 years, respectively. IDH, even at stage 1, was associated with moderately increased albuminuria, suggesting it may not be benign. Longitudinal studies are needed to determine whether IDH predicts subsequent kidney injury and whether early attention to elevated DBP modifies kidney and cardiovascular outcomes.

7. Advances in the study of sodium-glucose cotransporter 2 inhibitors for peritoneal dialysis.
PMID:日期:2026-09-15Sodium-glucose cotransporter 2 inhibitors (SGLT2is) are a novel class of hypoglycemic agents widely used in clinical practice in recent years. Recent studies have revealed that sodium-glucose cotransporter 2 is expressed in human peritoneal mesothelial and vascular endothelial cells, suggesting its potential involvement in peritoneal injury and ultrafiltration failure during peritoneal dialysis. Treatment with SGLT2is may alleviate peritoneal inflammation, fibrosis, and angiogenesis, improving ultrafiltration failure. Therefore, SGLT2is hold promise as a novel therapeutic approach for mitigating peritoneal injury and enhancing peritoneal function. A growing number of clinical studies are emerging, although most still remain in the early stages. This review highlights the role and mechanisms of SGLT2is in addressing peritoneal injury and ultrafiltration failure, with the aim of offering new therapeutic strategies to prevent complications associated with peritoneal dialysis.

9. Smart medical technologies in peritoneal dialysis: current applications and future perspectives.
9. 腹膜透析中的智能医疗技术:当前应用与未来展望PMID:日期:2026-09-04With the global rise in end-stage kidney disease cases, peritoneal dialysis (PD) has become a critical treatment modality for maintaining patients' quality of life. This narrative review aims to summarize and analyze the current applications and effectiveness of smart medical technologies in PD treatment, focusing on patient monitoring, treatment optimization, and healthcare delivery. This review examines the integration of smart medical technologies, including the Internet of Things (IoT), artificial intelligence, and cloud computing, in the management of PD. Key advancements discussed include: 1) IoT-enabled sensors for real-time monitoring of ultrafiltration volume and dialysate turbidity, enabling early detection of infection risks; 2) machine learning algorithms for personalized dialysis prescription adjustments to preserve residual kidney function; and 3) telemedicine platforms that enhance physician-patient collaboration and reduce the need for hospital visits. These innovations represent a significant step toward precision and intelligent management of PD, improving operational efficiency and selected outcomes. Future research should focus on addressing challenges related to data security, algorithm transparency, and real-world clinical adoption.

10. Comprehensive review of thrombotic microangiopathy: classification, genetics, and management.
10. 血栓性微血管病综述:分类、遗传学和管理PMID:日期:2026-09-01Thrombotic microangiopathy (TMA) comprises syndromes characterized by microangiopathic hemolytic anemia, thrombocytopenia, and organ injury, most commonly affecting the kidneys and brain. Recent advances have reframed TMAs from classical clinical labels (thrombotic thrombocytopenic purpura [TTP], hemolytic uremic syndrome, secondary TMA) toward a pathway‑based view centered on complement dysregulation, severe ADAMTS13 deficiency, and secondary endothelial injury. This review integrates current data on genetics, environmental "triggers," and clinical presentation to support a two‑hit model in which germline or acquired susceptibility is unmasked by infection, pregnancy, drugs, transplantation, or malignant hypertension. Building on this framework, we propose a stepwise diagnostic approach that combines ADAMTS13 testing, targeted evaluation for secondary causes, and rational use of complement and genetic testing, with the goal of assigning each patient to a dominant pathogenic pathway. Finally, we discuss mechanism‑based treatment strategies-including plasma exchange and immunosuppression for ADAMTS13‑mediated TTP, complement inhibition for complement‑mediated TMA, and trigger‑directed management in secondary forms-and outline key uncertainties regarding treatment duration, relapse prediction, and access to advanced therapies.
