To examine how recent injury or illness is associated with employment and income among young adults. Retrospective longitudinal cohort study. Using 2001-2021 National Longitudinal Survey of Youth 1997 cohort data (adults aged 18-41 years), we evaluated associations between self-reported recent injury or illness and employment status as well as log-transformed personal income. 102,642 interviews (8628 respondents) were included, of which 39% involved recent injury or illness. At the most recent interview, 4% of respondents were unemployed, 18% were out of the labor force (OLF), and median personal income was $33,000. Respondents with recent injury or illness were less likely to be unemployed (relative risk ratio [RRR]: 0.91; 95% CI: 0.85, 0.97; p = 0.003) or OLF (RRR: 0.87; 95% CI: 0.83, 0.92; p < 0.001) and had a 67% higher income (β = 0.51; 95% CI: 0.43, 0.59; p < 0.001) than respondents who did not report injury or illness. These associations were explained by controlling for sick leave eligibility (employment outcomes) and lagging the exposure by two years (employment and income outcomes). Recent injuries or illnesses among young adults did not reduce employment or income, although this finding is confounded by availability of sick leave eligibility or other aspects of economic advantage.

To quantify the association between functional limitations and household medical bill payment problems among US adults, to decompose this association into components mediated by health status and insurance coverage, and to examine whether the residual association is explained by individual cost barriers. Repeated cross-sectional analysis of a nationally representative survey. Medical Expenditure Panel Survey Household Component data were pooled across seven survey years between 2014 and 2023 (N = 155,064 adults aged ≥18 years) with complete responses to all six limitation items. Functional limitations were measured using six items (hearing, vision, cognition, mobility, self-care, errands) summed into an index (range 0-6). The outcome was self-reported difficulty paying family medical bills. Survey-weighted sequential logistic regression and Karlson-Holm-Breen decomposition were used. The weighted prevalence of payment problems rose monotonically from 7.6% (no limitations) to 19.8% (three or more). The fully adjusted odds ratio for any functional limitation was aOR 1.58 (95% CI 1.46 to 1.70), with an adjusted risk difference of 3.8 percentage points. Health status and insurance coverage mediated 53% of the total association. The association was robust to SF-12 adjustment (aOR 1.46), stable across pre-COVID and COVID-era periods, and observed in every insurance stratum. Adding individual cost barriers attenuated the association only modestly (aOR 1.64 vs. 1.76 in the 2019-2023 subsample; both p < 0.001), which is consistent with mechanisms that extend beyond individual care-seeking but does not exclude them. Functional limitations are associated with a substantial household financial burden. Approximately half of this burden operates through health and insurance pathways that existing policy levers can address; the remainder is consistent with household financial strain that insurance alone does not eliminate. Policy responses should combine insurance expansion with direct financial protection.

The COVID-19 pandemic exposed profound deficiencies in global health preparedness for infectious disease outbreaks. While progress has been made in creating pandemic preparedness frameworks at various scales, there is a lack of a comprehensive overview of the similarities and differences among these frameworks. This scoping review aims to address this gap by mapping pandemic preparedness frameworks to identify key components. Scoping Review. Using the methodological framework proposed by Arksey and O'Malley, this scoping review mapped the peer-reviewed and grey literature presenting pandemic preparedness frameworks across world regions published between January 2000 and June 2025. A thematic framework was prepared outlining four domains of pandemic preparedness (Predict, Prevent, Detect, and Respond) based on the ten pillars of the World Health Organization (WHO) COVID-19 Strategic Preparedness and Response Plan (SPRP). Braun and Clark's thematic analysis method was used to analyze the results of the included studies. This scoping review identified 41 articles presenting pandemic preparedness frameworks, with most scaled for national implementation (n = 25), followed by global (n = 13) and regional (n = 3) implementation. Key components of the identified frameworks were grouped into themes under the four domains, including multi-sectoral collaboration (n = 13), surveillance programs (n = 13), and social media risk communication (n = 9). Findings revealed that pandemic preparedness is a multidimensional, multisectoral process, as the reviewed frameworks emphasize complementary strategies across prediction, prevention, detection, and response domains. Further research should evaluate the effectiveness and contextual applicability of individual strategies.

The aim of this study was to examine the effect modification by greenness on the associations between short-term exposure to air pollutants and the risk of out-of-hospital cardiac arrest (OHCA). An individual-level time-stratified case-crossover study. We conducted a multi-city individual-level time-stratified case-crossover study including 62,915 OHCA cases in China, from 2020 to 2023. Short-term exposures to air pollutants were assigned based on the onset date and location for each case. Greenness (normalized difference vegetation index, NDVI, and enhanced vegetation index, EVI) was evaluated at case locations using annual maximum values. Effect modification by greenness was assessed using conditional logistic regression with linear interaction terms between air pollutants and greenness. Short-term exposure to air pollutants exhibited significant associations with OHCA, and greenness modified these associations. For 0.1-unit increase in NDVI, the percent changes in the odds of OHCA per 10 μg/m increase in pollutants were -1.62% (95% CI: -2.84% to -0.39%, P = 0.010) for PM, -1.13% (95% CI: -2.01% to -0.24%, P = 0.013) for PM, and -0.74% (95% CI: -1.34% to -0.13%, P = 0.018) for O. For 0.1-unit increase in EVI, the corresponding percent change was -2.47% (95% CI: -4.77% to -0.11%, P = 0.040) per 0.1 mg/m increase in CO. These interactions were consistently reproduced for PM and PM at coarser resolutions (500-m and 1000-m) and across alternative lag periods. Subgroup analyses observed significant greenness modifying effects among older (≥65 years) and cardiac etiology cases, and no significant heterogeneity was detected across age, etiology or any other examined subgroups. Short-term exposure to air pollutants was associated with OHCA, and greenness modified the associations of OHCA with specific air pollutants, including PM, PM, O, and CO. These findings highlight green space as an important contextual factor for understanding population susceptibility to ambient air pollutants, emphasizing the need to consider green infrastructure when assessing or addressing air pollution-related health impacts.

Strengthening adolescent HPV vaccination intentions and reduction of cervical cancer risk factors through school-based prevention strategies may improve future vaccine uptake and reduce cervical cancer incidence. This pilot study aimed to inform the design of a future definitive trial by evaluating the Senior High School Nurse-led Education (SHiNE) intervention in relation to: (i) feasibility (uptake, engagement and acceptability) among participants; and (ii) potential to improve HPV vaccination intentions and cervical cancer risk-related behaviors among senior high school students in Ghana. Pilot two-arm, cluster non-randomised controlled trial. The study was conducted in four senior high schools (clusters) in the Ashanti Region of Ghana. Two schools were allocated to receive the SHiNE intervention and two were assigned to a control condition (no intervention). The four-week SHiNE intervention included a nurse-led face-to-face session, printed leaflet, website, and gamified mobile application. Primary outcomes were uptake, engagement and acceptability. Secondary outcomes included HPV vaccination intention, condom use and smoking status. There was strong uptake, and high acceptability of the nurse-led face-to-face session and printed leaflet. There was lower uptake of the website and no uptake of the gaming application. There was some indication of potential effectiveness: at one-month follow-up, students exposed to SHiNE had higher odds (adjusted OR = 4.75, 95% CI 1.85-12.18) of reporting strong intentions to receive HPV vaccination compared with students in the control group. A nurse-led, school-based education intervention is feasible and has the potential to improve HPV vaccination intentions among adolescents in Ghana. However, while a non-digital approach may be more feasible in low- and middle-income countries, the implementation of digital interventions in school settings remains constrained in the absence of policies permitting students access to technology devices. The findings indicated that further refinement to optimise uptake and engagement of digital intervention and a full-scale effectiveness trial.

Pregnant women and neonates face a higher risk of adverse outcomes from infectious diseases, such as COVID-19. COVID-19 vaccines were developed rapidly, but we do not fully understand how the disease affected pregnancies and births before these vaccines were available. This study investigated how COVID-19 impact on pregnant women and their neonates before vaccines became widely available. The aim was to investigate the association between maternal COVID-19 infection and pregnancy and neonatal outcomes. This retrospective cohort study included pregnant women and their neonates (n = 3472 each) in Mashhad before the COVID-19 vaccine rollout. Participants were categorized into two groups: those who had COVID-19 and those who did not. Chi-square, t-tests, and Fisher's exact test were used to compare the groups. Linear and logistic regression analyses were used to examine the associations between COVID-19 exposure and different outcomes, both before and after adjusting for potential confounders. Pregnant women with COVID-19, when compared to those without, experienced less weight gain (OR = 0.91, CI 95%: 0.82-0.93) and shorter pregnancy duration (OR = 0.95, CI 95%: 0.92-0.97). COVID-19 infection in pregnant women also showed links to higher odds of cesarean (OR = 1.72, CI 95%: 1.62-1.83), gestational diabetes (OR = 1.11, CI 95%: 1.007-1.22), and unusual newborns that had congenital abnormalities (OR = 1.46, CI 95%: 1.03-2.11). The study indicates that women who had COVID-19 during pregnancy, before vaccines were available, had a higher risk of adverse maternal and neonatal outcomes. This finding can provide useful information to healthcare providers and pregnant women about how COVID-19 vaccination can reduce the severity of illness and adverse maternal and neonatal outcomes during pregnancy and the postnatal period.

Flourishing among children is an emerging indicator of positive health and well-being yet no studies have examined trends and socioeconomic variations spanning pre-and post-pandemic time periods. Repeated cross-sectional analysis of a nationally representative sample of children in the US. Data from seven cycles of the National Survey of Children's Health (2018-2024) with information on the components of the child flourishing index including resiliency, positive emotions, curiosity and emotional connection. A total of 308,255 children ages 6 months to 17 years were included with sociodemographic data (age, sex, race/ethnicity, parental education level, and family income to poverty ratio). Logistic regression models were used to estimate the odds ratios (ORs) associated with flourishing across socioeconomic strata. ORs were estimated to compare flourishing prevalence between the post-pandemic period (2023-2024) with the pre-pandemic period (2018-2019) by age group and family income to poverty ratio (FPR). Flourishing among very young children ages 6 months to 5 years declined from 83.8% (95% CI, 82.0%-85.6%) during 2018 to 79.9% (95% CI, 78.4%-81.3%) in 2024 (P-trend = 0.000). Among school-aged children (aged 6 to 17), flourishing fell from 66.5% (95% CI, 65.1%-67.8%) in 2018 to 60.6% (95% CI, 59.6%-61.9%) in 2024 (P-trend = 0.000). Compared to the pre-pandemic period, the multivariable-adjusted ORs of flourishing were 0.72 (95% CI, 0.64-0.81) for young children and 0.75 (95% CI, 0.71-0.79) for school-aged children. The prevalence of flourishing among US children decreased significantly between 2018 and 2024. There is little evidence of a post-pandemic recovery in the prevalence of child flourishing.
