1. Inpatient Pediatric-to-Adult Transition of Care Consultation Delivery: A Mixed-Methods Study.
PMID:日期:2026-10-05We evaluated an inpatient transition consultation (ITC) for adolescents and young adults (AYAs) with chronic conditions and their caregivers admitted to the hospital to assess ITC content, evaluate how it addressed transition concerns, determine its impact, and identify areas for improvement. We conducted a mixed-methods study at a tertiary academic medical center. Eligible participants were English-speaking patients aged 18 years or older and/or their caregivers who received an ITC during hospitalization. The ITC was conducted by a physician who focused on assessing transition readiness, providing education and counseling, and planning the transition from pediatric to adult models of care. Data collection included semistructured interviews and a modified Adolescent Assessment of Preparation for Transition (ADAPT) survey. Interviews explored perceptions of the ITC, including the content covered, impact and accountability of the consult, and suggestions for improvement. Qualitative data were analyzed using inductive coding; survey responses were summarized descriptively. Twelve participants representing diverse chronic conditions completed interviews and surveys. Five themes were generated: "enhanced understanding of self-management in adult care," "anxiety regarding anticipated changes from pediatric to adult care delivery," "reassurance and reduced anxiety following consultation," "importance of support systems for patients and caregivers undergoing transition," and "centering holistic, person-focused care during transition." An ITC was perceived as educational, reassuring, and patient-centered. Findings suggest that targeted transition counseling during hospitalization is acceptable to patients and caregivers and may help reduce anxiety surrounding health care transition.

2. An Overview of Interrupted Time Series as a Research Method.
PMID:日期:2026-10-05Interrupted time series (ITS) study design is increasingly used to evaluate the impacts of guidelines, policy changes, and quality improvement efforts on clinical outcomes. Repeated observations of an outcome of interest over time are "interrupted" by an intervention, dividing the study into pre- and postintervention time periods. Regression analysis then compares the level change and slope of the 2 time periods to determine intervention effects. The benefits of this study design include (1) use of lower-cost, preexisting large data sets, (2) reduced susceptibility to confounding by individual-level characteristics with the ability to test for potential biases, and (3) presentation of results in straightforward, easy-to-interpret figures. However, ITS methods require high-quality, consistent data over an extended time frame, and researchers must have a firm grasp of potential confounding variables and selection of appropriate statistical analysis methods. When properly employed, ITS provides a rigorous study design to evaluate outcomes in pediatric populations that may otherwise be difficult to investigate. This primer familiarizes pediatric hospitalists with the basics of employing this study methodology, introduces examples of its utility in the field, and directs readers to existing literature for the more advanced statistical underpinnings of conducting such a study.

3. Enhancing Care for Hospitalized Children With Medical Complexity via Targeted Education.
PMID:日期:2026-10-05Children with medical complexity (CMC) and their caregivers face multifaceted challenges during hospitalization. Pediatric residents are often frontline clinicians for these patients but frequently identify feelings of inadequate preparedness to meet their care needs. We assessed the impact of a multimodal curriculum on common acute and chronic medical devices in CMC on caregiver-reported resident communication. We hypothesized that targeted education would translate to improved caregiver perception of family-centered communication. We implemented a novel medical device curriculum for pediatric residents at 2 children's hospitals. Caregivers of hospitalized CMC completed a modified 14-item Communication Assessment Tool pre- and post-curricular intervention. We used generalized estimating equations at the caregiver level and paired t tests at the resident level to evaluate pre-post changes. We compared postintervention scores with those of a cohort of unexposed residents. We collected 253 surveys on 39 pediatric residents. At the resident level, there was a statistical increase in overall mean score following intervention (pre, 4.844/5 [SD = 0.213]; post, 4.934/5 [SD = 0.132]; P = .0328). At the caregiver level, mean survey scores increased, although this was not statistically significant. Four of 14 survey items showed statistical improvement following intervention. Caregiver-level outcomes were consistent in the sensitivity analysis using an unexposed control, indicating robustness of findings. Caregivers of CMC reported modest improvements in resident collaborative communication behaviors following intervention. Ongoing deliberate practice, including expanded educational models and the integration of caregiver feedback into physician evaluation tools, may further support skill development for clinicians caring for hospitalized CMC.

4. The Multidimensional Nature of Pediatric Patient Complexity.
PMID:日期:2026-10-05Patient complexity is defined in a variety of ways and is associated with health care costs and use. In this study, we evaluate whether pediatric inpatient complexity represents a single construct or multiple domains using multiple measures of diagnostic burden, health care use, and intensity. We analyzed administrative billing data from pediatric hospitals in the Pediatric Health Information System (PHIS) including medical and surgical hospitalizations discharged from 2021 to 2023 for patients aged 0 to 17 years with lengths of stay (LOS) of 30 days or fewer. We extracted measures of hospital use and clinical characteristics including LOS, readmissions, diagnosis and procedure codes, billing codes, and clinical classifications. For each discharge, we calculated measures capturing diagnostic burden, overall use and intensity, including unique diagnosis codes, unique items, services, and medications received, and complex chronic organ systems. We performed a principal components analysis (PCA) to examine patterns of shared variation and underlying dimensions among the included measures. We included N = 2 090 139 discharges from 48 hospitals. Over three-quarters (79.5%) were medical admissions and 85% stayed less than 1 week. Our PCA identified 9 factors accounting for 74.1% of the variance. The first 3 factors comprised measures of diagnostic complexity (accounting for 20% of variance), use of medications, supply, and other items or services (17%), and procedural and overall use including LOS and intensive care unit use (10%). Our findings indicate pediatric inpatient complexity is multidimensional, with 9 distinct factors identified by PCA that may have important implications for health services research, quality measurement, and risk adjustment.

6. One Step at a Time: Design and Rationale for Stepped-Wedge Cluster Randomized Trials.
PMID:日期:2026-10-05Stepped-wedge cluster randomized trials provide researchers with an opportunity to simultaneously implement and evaluate systemic interventions in real-world settings. This study design may have advantages compared with other cluster randomized trial (CRT) designs by optimizing feasibility and uptake when resources or personnel are limited. We present strengths, limitations, and key considerations when deciding whether to use a stepped-wedge design. Statistical analysis of stepped-wedge trials differs from that of traditional CRTs and should account for changes in outcome measures due to the intervention separate from those due to secular or time-based trends. We conclude with an example of a stepped-wedge study at 26 hospitals that helped to establish the effectiveness and safety of the Eat, Sleep, Console approach to neonatal opioid withdrawal syndrome.

7. Pediatric Institutions' Instagram Response to the 2025 Acetaminophen-Autism Controversy.
PMID:日期:2026-10-02Objective: To describe the timing, frequency, and accessibility of Instagram posts from pediatric health institutions during the 2025 acetaminophen-autism controversy. Methods: Cross-sectional audit of public Instagram accounts of 62 pediatric health institutions from September 22 to November 15, 2025. The a priori sampling frame comprised the top 50 US children's hospitals for pediatric behavioral health, the top 10 Honor Roll children's hospitals (US News and World Report), 5 national medical societies, and health departments of the 10 states with the largest pediatric populations. Posts were extracted with a web-scraping tool and manually verified. Two reviewers independently coded relevant posts for prespecified content and accessibility features. Readability was assessed with two validated measures. Results: Twelve of 62 institutions (19%) posted relevant content. The American Academy of Pediatrics and the American College of Obstetricians and Gynecologists posted on day 0, while hospital time to first post ranged from 3 to 51 days. No state health department posted. Nonresponding institutions remained active: 1 hospital published 108 unrelated posts. The 26 relevant posts had a mean Flesch-Kincaid Grade Level of 11.4 and SMOG Index of 10.2. Among coded features, 88% included resource links, 73% directed audiences to a health care provider, 46% featured a named clinician, 42% discussed risks and benefits, and none were multilingual. Conclusions: Most sampled institutions published no relevant content during an active health controversy despite remaining active. Readability of relevant posts exceeded recommended targets, and none were multilingual. These gaps in institutional communication infrastructure could be addressed.

9. Introduction to Network Meta-Analysis.
PMID:日期:2026-09-28Many treatment decisions in hospital medicine are not divalent choices between 2 options but, rather, decisions among multiple possible interventions. While traditional pairwise meta-analysis can provide information on the effectiveness of 1 or 2 interventions, a different strategy is required to evaluate the relative merits of multiple possible interventions. Network meta-analysis is a robust statistical tool to analyze 3 or more interventions for a single problem, allowing for comparisons of interventions that may not have been directly compared in head-to-head studies. This guide provides an introductory overview of network meta-analysis, focusing on how this analysis differs from traditional pairwise meta-analysis. We focus on key assumptions in network meta-analysis, including transitivity and consistency as well as considerations of evidence strength and bias. This guide can serve as a starting point for a researcher seeking to perform their own network meta-analysis or a guide for a clinician looking to interpret published studies.

10. Perceptions of Secure Messaging and Patient Safety: Results of an Interprofessional Survey.
PMID:日期:2026-09-24Communication technology within pediatric health care systems is rapidly evolving and has the potential to affect safety culture. After implementing a secure messaging communication platform at our quaternary care hospital, we aimed to evaluate the new system across interprofessional groups to inform future quality improvement initiatives. A cross-sectional survey developed by quality and safety experts was distributed via email to physicians, advanced practice providers (APP), and nurses over 3 months at a single children's hospital. Descriptive statistics were used to summarize participant demographics. χ2 analysis with correction for multiple comparisons was used to analyze differences across clinician groups. Four hundred and eleven respondents completed the survey, with nurses being the most common respondents (n = 166, 37.6%). When comparing physicians or APP and nurses, there was agreement that the new platform made it easy to communicate with the appropriate team member (P = .096). Practitioners reported feeling more overwhelmed by the number of messages received (P < .001) and being unable to review messages in a timely manner (P < .001) compared with nurses. When practitioner groups were stratified, trainees had the highest proportion of respondents reporting increased workload (n = 65, 94.2%) and being unable to review messages in a timely manner (n = 51, 73.9%). Although there was a consensus across clinical roles that the new communication system makes it easy to communicate with appropriate members of the care team, discrepancies exist in role-specific workload factors. These findings highlight the need for optimizing communication strategies with input from different clinical roles in the inpatient setting.
