1. Analytical Appraisal of Validity and Reliability Metrics in Dementia Knowledge Assessment Instruments: Consequences for Clinical Nursing Practice in Vietnam.
PMID:日期:2026-09-22To validate and compare the psychometric properties of the Alzheimer's Disease Knowledge Scale Vietnam Version (ADKS-V), Dementia Knowledge Assessment Tool Version Two (DKAT2-V), and 27-item Dementia Knowledge Assessment Scale Vietnam Version (DKAS-V). A cross-sectional study surveyed 360 community-dwelling older adults and 394 primary care nurses in Ho Chi Minh City. The scales demonstrated strong content validity (.86 to .91), convergent validity ( = .698 to .770), and reliability (Kuder-Richardson 20 [KR-20] = .765 to .820, ω = .793 to .849). Unidimensional confirmatory factor analysis (CFA) fit was poor. In contrast, target-population-specific orthogonal bifactor CFA models confirmed superior fit (comparative fit index = .939 to .981, root mean square error of approximation = .031 to .049) over unidimensional structures. High general factor strength (ωH = .571 to .811) and negligible specific subscale variance (mostly <.15) justified single composite scoring. All three Vietnamese versions are culturally appropriate and robust. We recommend DKAT2-V for nursing education, ADKS-V for community screening, and DKAS-V as a universal epidemiological tool.

2. Frailty, Caregiver Burden, and Depressive Symptoms Among Family Caregivers of Community-Dwelling Frail Older Adults: The Moderated Mediating Role of Social Support, Mastery, and Competence.
PMID:日期:2026-09-22Primary stressors, such as frailty and caregiver burden, are predominant contributors to caregivers' depressive symptoms, but mechanisms underlying these relationships remain unknown. Path analysis was used to test whether mastery and competence mediate the effect of frailty and caregiver burden on depressive symptoms, and whether social support moderates these pathways among 464 caregivers of frail older adults in China. Mastery mediated the relationship between physical frailty and depressive symptoms, and competence mediated the relationship between caregiver burden and depressive symptoms. Social support moderated the relationship between physical frailty and mastery, as well as between caregiver burden and competence. Social support buffered the indirect negative influence of physical frailty on depressive symptoms via mastery, and that of caregiver burden on depressive symptoms via competence. Psychological and social frailty were non-significant. Interventions targeting social support, mastery, and competence enhancement may protect caregivers' mental well-being against care-related stressors.

3. Initial Development and Preliminary Validation of the Caregiver Motivation Appraisal Scale: Assessing Voluntary and Obligatory Caregiving Motivations.
PMID:日期:2026-09-22To introduce, describe, and evaluate the Caregiver Motivation Appraisal Scale (CMAS), an 11-item instrument developed to assess voluntary and obligatory caregiving motivations. Psychometric properties of the CMAS were evaluated in a sample of 222 caregivers. Items were generated from qualitative interviews, and exploratory factor analysis (EFA) using principal axis factoring followed by confirmatory factor analysis (CFA) was used to examine the factor structure. EFA supported a two-factor structure comprising an eight-item voluntary motivation subscale and a three-item obligatory motivation subscale, accounting for 64.07% of the variance. The CMAS demonstrated strong internal consistency (α = 0.83). CFA indicated good model fit (comparative fit index = 0.96, goodness of fit index = 0.91, root mean square error of approximation = 0.06, standardized root mean square residual = 0.08). Findings support the initial development and preliminary validation of the CMAS, contributing to understanding caregiver motivation and its potential implications for caregiver well-being research. Future work should refine the obligatory motivation subscale and validate the instrument across diverse caregiver populations.

5. Effect of Intergenerational Support on Successful Aging Among Older Adults: A Cross-Sectional Study.
PMID:日期:2026-07-14To examine the association between intergenerational support from children and successful aging among older adults. Using data from 6,573 adults (aged ≥60 years) in the 2020 China Health and Retirement Longitudinal Study, intergenerational support was measured across three dimensions, and successful aging was defined according to Rowe and Kahn. Multivariate logistic regression analyzed their association, and a classification and regression tree (CART) model explored multi-factor interactions. Caregiving, higher financial support, and moderate monthly emotional contact were each significantly associated with greater odds of successful aging. CART analysis further revealed that although health and education served as foundational factors, their associations with successful aging were synergistically moderated by intergenerational support. Successful aging is closely linked to the interplay of individual characteristics and intergenerational support. Policies should therefore prioritize health promotion and educational investment while facilitating differentiated intergenerational support strategies.

6. Associated Factors of Fall Risk Perception Among Community-Dwelling Older Adults.
PMID:日期:2026-07-14The current study aimed to assess the status and associated factors of fall risk perception among community-dwelling older adults in China, conceptualized within the Theory of Planned Behavior as a determinant of attitudes and intentions toward fall prevention behaviors, which could provide evidence for fall prevention strategies. Between November 2023 and March 2024, a convenience sampling method was used to recruit older adults from a community in Quzhou, Zhejiang Province. Data were collected using a self-designed questionnaire and the Community Elderly Fall Risk Perception Scale. A total of 267 community-dwelling older adults participated in the current study, with a mean fall risk perception score of 62.12 ( = 13.25). Univariate analysis revealed that age, educational level, household location, history of falls within the past year, vision condition, number of medications, use of walking aids, perceived physical health condition, fear of falling, and living situation, were significantly associated with fall risk perception among older adults (all < .05). Subsequent multivariate linear regression analysis identified fear of falling, number of medications, use of walking aids, educational level, and living alone as independent predictors of fall risk perception (all < .05). There were differential effects of these factors on fall risk perception in men and women. Community-dwelling older adults from this sample exhibited a moderate level of fall risk perception, indicating that although they recognize potential for falls, there may still be gaps in their awareness or understanding of personal risk factors. This perception is shaped by a combination of factors, underscoring importance of fall prevention. It is imperative that community health workers assist older adults to gain an accurate understanding of their fall risks and facilitate development of tailored fall prevention strategies.

8. The Influence of Nurse Practitioner Roles in Home-Based Care Models on Patient, Caregiver, and Health System Outcomes for People With Advancing Dementia: A Scoping Review.
PMID:日期:2026-06-23Many people living with dementia (PLWD) with advancing disease are unable to access office-based care for primary and dementia care. Nurse practitioners (NPs) could fill this gap. The objective of the current review was to determine patient, caregiver, and health care system outcomes of home-based dementia care models that use NPs. A scoping review was completed using the Arksey and O'Malley Framework. Two independent reviewers completed title/abstract and full-text screening. Data were extracted using Microsoft Excel. A thematic synthesis was completed to summarize findings. The search yielded 4,410 citations; 17 articles met inclusion criteria, reporting on 10 models. Outcomes included high-quality care, decreased emergency department use, decreased acute care length of stay, delayed facility-based long-term care placement, and high rates of advanced care planning and transition to palliative care. Home-based dementia care models that use NPs for longitudinal care have the potential to support aging in place.

9. Practical Challenges in Assisting With Mealtimes for People With Dementia: A Qualitative Content Analysis of Caregivers' Experiences.
PMID:日期:2026-06-23The current study aimed to explore the practical challenges encountered by family caregivers when assisting community-dwelling people with dementia during mealtime. Using a descriptive qualitative design, 18 family caregivers of people with dementia who experienced feeding difficulties were recruited from the Neurology Outpatient Department and Memory Clinic of the First Affiliated Hospital of Wenzhou Medical University between November 2024 and May 2025. Data were collected through face-to-face semi-structured interviews and analyzed using inductive content analysis. Data analysis identified four interrelated categories describing the day-to-day practical challenges encountered by caregivers during mealtime assistance: functional decline, problematic eating behaviors, communication breakdown, and resistance to care. Mealtime assistance in dementia care involves multifaceted, practice-level challenges that extend beyond nutritional management. Findings provide an empirically derived framework to guide clinical assessments and inform targeted, skill-based caregiver support.

10. Outcomes of a Palliative Care Intervention for Older Adults With and Without Dementia in Skilled Nursing Facilities.
PMID:日期:2026-06-23Older adults living with Alzheimer's disease and related dementias (ADRD) receiving skilled nursing facility (SNF) care have different clinical trajectories and care needs compared to those without ADRD. Little is known about whether palliative care affects these two populations differently. We conducted a secondary comparative cohort analysis of a feasibility pragmatic pilot trial ( = 52) to explore the effect of a primary palliative care intervention among older adults with and without ADRD. SNF nurse practitioners received training in the delivery of a primary palliative care intervention. The Palliative Outcomes Scale version 2 measured quality of life at baseline and 14 to 21 days follow up. No statistically significant intervention effect was found; however, quality of life scores in the ADRD cohort trended toward improvement, whereas the non-ADRD cohort trended toward worsening with a similar effect size. Further research is needed to optimize palliative care interventions for SNF residents with different illness trajectories.
