Immune checkpoint inhibitors targeting programmed cell death protein 1 (PD-1) have transformed lung cancer therapy, but their efficacy is often limited in tumors with low programmed cell death ligand 1 (PD-L1) expression and immunologically "cold" tumor microenvironments. This study aimed to evaluate the safety and therapeutic efficacy of combining a RAD001-adjuvanted influenza vaccine with PD-1 blockade and to explore its immunological mechanisms in a murine lung cancer model. C57BL/6 mice bearing Lewis lung carcinoma (LLC) tumors, characterized by low PD-L1 levels (10.28%), were randomized to receive control, anti-PD-1 monotherapy, or anti-PD-1 combined with either a split influenza vaccine, an MF59-adjuvanted vaccine, or a RAD001-adjuvanted vaccine. Primary endpoints included tumor growth kinetics and survival. Safety was assessed via histopathological analysis of major organs and biochemical analyses. Mechanistic changes were evaluated through immunophenotyping of tumor-infiltrating lymphocytes (TILs) and serum cytokine profiling. The RAD001-adjuvanted vaccine synergized with anti-PD-1 therapy, resulting in superior tumor growth inhibition compared to all other groups. This synergy was associated with a remodeled tumor microenvironment, showing increased CD8 T-cell infiltration and a higher CD8/regulatory T-cell ratio. Serum analysis revealed a shift toward a Th1-dominant cytokine profile (a trend toward increased interferon-gamma and interleukin-2). Histopathological and biochemical examination confirmed no treatment-related toxicities in the heart, liver, lungs, kidneys, or spleen. RAD001-adjuvanted influenza vaccination safely enhances the efficacy of anti-PD-1 therapy in PD-L1-low murine lung cancer by promoting a pro-inflammatory immune microenvironment. These findings suggest that strategic vaccine adjuvant selection can broaden the therapeutic reach of immune checkpoint inhibitors.

Tyrosine kinase inhibitors (TKIs) have transformed the management of advanced thyroid carcinoma. However, newer agents and updated long-term evidence from pivotal trials have not been comprehensively synthesized. This systematic review and meta-analysis integrates recent randomized evidence, including donafenib and nintedanib, with qualitative synthesis of updated SELECT (lenvatinib) and COSMIC-311 (cabozantinib) analyses, alongside separate quantitative evaluations of placebo-controlled and dose-comparison trials. PubMed/MEDLINE, Embase, Cochrane CENTRAL, Google Scholar, and reference lists were systematically searched for randomized controlled trials evaluating TKIs in advanced thyroid carcinoma. Eighteen studies were included in the systematic review, of which 14 publications contributed to the meta-analysis. Updated SELECT and COSMIC-311 analyses were synthesized qualitatively to avoid duplication of patient populations. For trials reporting progression-free survival (PFS) and overall survival (OS) in separate publications, the most appropriate publication was selected for each outcome, resulting in risk-of-bias assessment of 13 unique randomized trials using the Cochrane RoB 2 tool. Random-effects meta-analyses were performed using SAS version 9.4. Thirteen randomized controlled trials involving 2,671 patients were quantitatively analyzed. Compared with placebo, TKIs significantly improved PFS (hazard ratio (HR) = 0.36; 95% confidence interval (CI), 0.26-0.50; P < 0.001), reducing the risk of disease progression or death by approximately 64%, although heterogeneity was substantial (I = 83.8%). Separate dose-comparison analyses of three trials demonstrated no significant advantage of higher-dose regimens (HR = 1.18; 95% CI, 0.87-1.60). TKIs also significantly improved OS (HR = 0.78; 95% CI, 0.67-0.91; P = 0.002), corresponding to a 22% reduction in mortality risk, with no heterogeneity (I = 0%). Lenvatinib demonstrated the greatest PFS benefit, followed by cabozantinib. Updated SELECT and COSMIC-311 analyses confirmed durable efficacy across extended follow-up and multiple patient subgroups. No significant publication bias was detected. TKIs significantly improve PFS and OS in advanced thyroid carcinoma, supporting their role in contemporary clinical management. Lenvatinib demonstrated the greatest efficacy, while updated long-term evidence reinforced the durable benefits of lenvatinib and cabozantinib. Although placebo-controlled trials consistently favored TKIs, dose-comparison trials did not demonstrate superiority of higher-dose regimens. Further randomized studies with longer follow-up and biomarker-guided treatment strategies are warranted.

Risk stratification in colorectal cancer (CRC) plays an important role in treatment decision-making. As such, prognostic biomarkers that can augment risk stratification have clinical value. Quantitative histologic features from routine hematoxylin and eosin (H&E)-stained whole slide images (WSIs) provide a novel avenue for biomarker discovery. In this study, we explored the potential for a computational histology artificial intelligence (CHAI) platform to develop and validate a prognostic biomarker in CRC. The Cancer Genome Atlas Colorectal Adenocarcinoma project was utilized for this study, with inclusion of all subjects (stage I-IV) with available digitized H&E specimens. The cohort was split into development and validation cohorts by a stratified random split. The previously developed CHAI platform was applied in the development cohort to construct a continuous risk score from histologic features associated with progression-free interval (PFI) that was dichotomized based on an optimized cutpoint for distinguishing PFI into a high risk CHAI (+) and lower risk CHAI (-). PFI was compared between CHAI (+) and CHAI (-) patients in the validation cohort in multivariable Cox proportional hazards models. Time-dependent area under the curve (tdAUC) and C-indices were also calculated for PFI. A total of 583 participants were included in the study, with 409 assigned to the validation cohort. The CHAI biomarker classified 229 participants (56%) as CHAI (+) and 180 (44%) as CHAI (-) in the validation set. CHAI (+) participants had worse PFI in a multivariable analysis adjusting for available clinicopathologic variables (hazard ratio (HR) = 2.65; 95% confidence interval (CI), 1.63-4.30). TdAUC for the CHAI biomarker was 0.60 (95% CI, 0.53-0.67) at 12 months, 0.62 (0.55-0.69) at 36 months, and 0.67 (0.55-0.79) at 60 months; the C-index was 0.62 (95% CI, 0.58-0.67). The CHAI platform was used to develop a prognostic digital pathology biomarker in CRC. This demonstrates the feasibility and potential to apply this artificial intelligence-based digital pathology biomarker platform for risk stratification in CRC and supports its further study.

Breast cancer (BC) survivorship is associated with significant psychological challenges, including anxiety, stress, and depression. Coping strategies (C-Strats) play a crucial role in influencing psychological well-being (Psych-WB) among survivors, but few studies have explored interconnected factors among BC surviving women in Bahrain, a disadvantaged population. This study thus examines the links between psychological coping and well-being solutions as deployed among women after BC in the Kingdom of Bahrain and to explore variations according to selected demographic characteristics. A descriptive cross-sectional design was employed. A total of 104 BC survivors attending oncology clinics in governmental hospitals were recruited using convenience sampling. Data were collected using a demographic questionnaire, the Brief Coping Orientation to Problems Experienced Scale, and the Depression Anxiety Stress Scale. Descriptive and inferential statistics, including -tests, analysis of variance, and Pearson correlation, were performed using SPSS version 28. Participants demonstrated moderate use of C-Strats, with higher reliance on problem-focused and "emotion-focused coping," particularly religious coping, and lower use of "avoidant coping." Psych-WB scores indicated moderate levels of anxiety, stress, and depression. C-Strats did not differ significantly across demographic variables (P > 0.05). However, psychological distress was significantly associated with age (P = 0.035), with younger survivors reporting higher distress levels. A statistically significant moderate negative correlation was found between C-Strats and Psych-WB (r = -0.501, P < 0.001), indicating that higher coping ability was associated with lower psychological distress. The findings highlight the protective role of C-Strats in reducing psychological distress among BC survivors. Interventions aimed at enhancing adaptive coping, particularly among younger survivors, are recommended to improve Psych-WB.

Lipid rafts or detergent-resistant membranes (DRMs) have played a fundamental role during prostate-specific membrane antigen (PSMA) activation by serving as platforms for internalization and cell signaling. In the current study, we aimed to evaluate the clinical significance of PSMA-lipid rafts association in prostate adenocarcinoma. The study was carried out on two normal prostate (NP), 11 benign prostate hyperplastic (BPH), and 10 prostate cancer (PC) samples. Immunohistochemical analysis for PSMA, CD34, and Ki67 was performed. Serum levels of prostate-specific antigen (PSA) were assayed by Immulite autoanalyzer. DRMs were isolated from LNCaP cells, NP, BPH, and PC tissues. Using Triton X-100, PSMA, dipeptidyl peptidase IV (DPPIV), binding immunoglobulin protein (BiP), and Flotillin-2 were analyzed in both soluble and insoluble fractions by Western blot analysis. The findings demonstrate that PSMA is overexpressed in PC compared to normal and benign prostate tissues. The strongest staining of PSMA was membranous, with apical accentuation in neoplastic cells. Furthermore, upregulation of PSMA was concomitant with high expression of CD34 and Ki67 in malignant prostate. Our data indicate that PSMA, DPPIV, and BiP are distributed in the insoluble and soluble fractions of the analyzed PC samples. Nevertheless, the level of these membrane proteins is higher in the soluble fraction as compared to the DRMs in prostate adenocarcinoma. Overall, this preliminary study provides initial insights into the potential clinical relevance of the association between PSMA and lipid rafts in human prostatic adenocarcinoma. The observed co-distribution of PSMA with DPPIV and BiP in the DRM fractions of PC samples suggests a possible association among these membrane proteins that may contribute to PC progression. Although these findings are preliminary, they support the hypothesis that crosstalk between these proteins could occur. Further studies are required to elucidate the underlying molecular mechanisms and to determine whether targeting PSMA and its associated membrane partners may represent a promising therapeutic strategy.

Gastric cancer remains the fifth most common malignancy worldwide and is often diagnosed at advanced stages with limited treatment options. Although immunotherapy targeting the programmed cell death protein-1 (PD-1)/programmed death-ligand 1 (PD-L1) has improved outcomes in several cancers, many patients exhibit resistance, necessitating alternative therapeutic strategies. Sialic acid-binding immunoglobulin-like lectin-15 (Siglec-15), a recently recognized immune suppressor, is overexpressed in tumor cells and tumor-associated macrophages (TAMs), and is largely absent from normal tissues, making it a promising immunotherapy target. This tissue microarray (TMA) study retrospectively collected formalin-fixed paraffin-embedded (FFPE) tissue specimens from 2015 to 2019 for 77 gastric cancer patients from King Hussein Cancer Center. Siglec-15 expression patterns in tumor cells and TAMs were analyzed and compared with clinicopathological features and patient outcomes. Survival was assessed using Kaplan-Meier analysis. Siglec-15 expression was detected in 50.7% of tumor samples and 36.4% of TAMs. Membranous expression was significantly associated with tumor differentiation, with higher expression observed in moderately differentiated tumors (P = 0.048), while cytoplasmic and TAM expression showed no significant associations with clinical variables. Although not statistically significant, positive Siglec-15 expression-both membranous and cytoplasmic-was associated with shorter median overall survival (cytoplasmic: 30.62 vs. 74.92 months, P = 0.1790; membranous: 39.44 vs. 66.07 months, P = 0.5761). Established prognostic indicators such as age, clinical stage, and pT stage were significantly associated with overall survival. Siglec-15 is frequently expressed in gastric cancer and may contribute to immune evasion within the tumor microenvironment. Although no significant association with prognosis was found, its high expression underscores its potential as a novel immunotherapeutic target. Further studies are warranted to validate these findings and explore Siglec-15 clinical utility.

Lung cancer (LC) and venous thromboembolism (VTE) are closely associated, with VTE contributing to morbidity and mortality among patients with LC. We aimed to identify and characterize a mitochondrial-related transcriptomic signature shared between LC and VTE and to explore its association with immune microenvironment features. We applied a multiomics approach focused on mitochondrial-related signaling pathways. Publicly available transcriptomic datasets were analyzed using differential expression profiling and weighted gene co-expression network analysis to identify key regulatory genes. These genes were intersected with a mitochondrial gene set and subjected to functional enrichment analysis. Least absolute shrinkage and selection operator (LASSO) regression was used to identify candidate diagnostic genes validation. Immune cell infiltration was quantified, and associated regulatory mechanisms were explored. Thirty-nine shared crosstalk genes were identified and were primarily enriched in mitochondrial metabolic processes. LASSO regression identified a five-gene candidate signature (, , , , and ). The model exhibited promising discriminatory performance (area under the curve > 0.9 in LC dataset and 0.7-0.9 in VTE dataset). These genes were significantly dysregulated and were associated with altered immune cell infiltration, particularly in dendritic cell and T cell subsets. We identified a mitochondrial-related gene signature reflecting shared transcriptomic correlates between LC and VTE. The signature showed variable performance across disease contexts and correlative associations with immune features, supporting its role as a candidate biomarker for further investigation. Prospective validation in independent clinical cohorts is required before any translational application.

This study aimed to assess the association between sleep disorders and breast cancer (BC) in the National Health and Nutrition Examination Survey (NHANES) and to evaluate potential causal associations using Mendelian randomization (MR), with separate analyses of estrogen receptor-positive (ER+) and estrogen receptor-negative (ER-) BC. This study was based on cross-sectional data from the NHANES 2007-2018, including a total of 8,366 female participants. A weighted multivariable logistic regression model was used to analyze the association between sleep disorders and BC, with stepwise adjustment of covariates. Further subgroup analyses were conducted by covariates. Two-sample MR was applied to evaluate the potential causal association between sleep disorders and BC (including ER+ and ER- subtypes). Analysis of NHANES 2007-2018 cross-sectional data showed that among 8,366 female participants, 33.8% of BC patients reported sleep disorders, which was significantly higher than that in the non-cancer group (27.9%, P = 0.035). However, in the weighted logistic regression model with multivariable adjustment, no significant association was observed between sleep disorders and BC risk (model 3: odds ratio (OR) = 0.814, 95% confidence interval (CI): 0.553-1.196, P = 0.298). Subgroup analysis revealed a significant positive correlation between sleep disorders and BC risk in Mexican Americans (OR = 2.801, 95% CI: 1.302-6.025) and participants with low body mass index (BMI) (OR = 2.705, 95% CI: 1.332-5.494), whereas a significant negative correlation was found in diabetic patients (OR = 0.383, 95% CI: 0.199-0.738). Two-sample MR showed no significant causal effect of sleep disorders on ER+ BC risk (inverse variance weighted (IVW) method: OR = 0.98, 95% CI: 0.94-1.02, P = 0.304). For ER- BC, IVW showed a modest positive association (OR = 1.070, 95% CI 1.013-1.130; P = 0.016), whereas the other MR methods were not statistically significant. No significant bias was detected by heterogeneity test (Cochran's Q test, P > 0.05) or pleiotropy analysis (MR-Egger intercept test, P = 0.662). Leave-one-out sensitivity analysis and funnel plot further confirmed the robustness and symmetry of the results. The cross-sectional analysis found no overall association between sleep disorders and BC. MR provided suggestive evidence of a modest association between genetically predicted sleep disorders and ER-, but not ER+, BC; this finding requires cautious interpretation and further validation.

Concurrent chemoradiotherapy (CCRT) for unresectable head and neck cancer (HNC) in advanced stages is associated with a high probability of residual disease (RD). Salvage surgery after completing CCRT often leads to severe complications, and palliative care is required if surgery is not feasible. The aim of this study was to develop a prediction model for RD based on pretreatment clinical features. This retrospective observational cohort study analyzed the medical records of patients with unresectable HNC who received CCRT at the Radiation Oncology Unit of Buddhachinaraj Phitsanulok Hospital, Thailand, between October 2019 and October 2025. Clinical features and laboratory results were collected before treatment. Tumor response was evaluated 8-12 weeks after completion of CCRT using computed tomography or magnetic resonance imaging scans according to the Response Evaluation Criteria in Solid Tumors. Multivariable logistic regression was used to identify predictors of RD. The model performance was evaluated for discrimination using the area under the receiver operating characteristic curve (AuROC) and for calibration using calibration plots. Internal validation was performed using the bootstrap resampling method. Decision curve analysis was also performed. A total of 382 patients with HNC were included. After the completion of CCRT, 180 patients (47.1%) presented with RD. The final model retained primary tumor site (hypopharynx (odds ratio (OR): 3.56; 95% confidence interval (CI): 1.10-11.40) and oral cavity (OR: 3.77; 95% CI: 0.95-14.90)), maximum tumor diameter (OR: 3.37; 95% CI: 2.59-4.39), time to initiation of radiotherapy (OR: 1.01; 95% CI: 1.00-1.02), body mass index (OR: 0.97; 95% CI: 0.88-1.06), neutrophil-to-lymphocyte ratio (OR: 1.02; 95% CI: 0.94-1.10), and hemoglobin level (OR: 0.94; 95% CI: 0.79-1.11). The model demonstrated excellent discrimination, with an AuROC of 0.929 (95% CI: 0.903-0.954). At an RD probability cutoff of ≥ 47.1%, the model yielded a positive predictive value of 81.2% (95% CI: 75.1-86.4) and a negative predictive value of 89.2% (95% CI: 83.8-93.3). This clinical feature-based prediction model accurately predicted the probability of RD in patients with unresectable HNC. Physicians can utilize this model for pre-CCRT treatment planning to identify high-risk patients.

Endometrial cancer is a prevalent gynecologic malignancy, which is predominantly of the histological type known as endometrial adenocarcinoma. Deletion of the tumor suppressor gene contributes significantly to the development of endometrial cancer. Phosphatase of regenerating liver-3 (PRL-3), as a common pro-oncogenic factor, has been shown to promote angiogenesis in endometrial cancer. However, the functional relationship between PRL-3 and phosphatase and tensin homolog deleted on chromosome 10 (PTEN) in endometrial cancer remains unclear. Following the modulation of specific molecule expression, the levels of relevant molecules were assessed by quantitative real-time polymerase chain (qRT-PCR) and Western blot. Endometrial adenocarcinoma cell proliferation and migration were further determined by Cell Counting Kit-8 (CCK-8) assay and scratch assay. PTEN and phosphatase and tensin homolog pseudogene 1 (PTENP1) expression in endometrial adenocarcinoma samples and cell lines was analyzed and found to be closely associated with the malignant biological behavior of tumors. High expressions of PTENP1 and PTEN inhibited the proliferative and migratory capacities of endometrial adenocarcinoma cells, and high expressions of PRL-3, miR-21 and miR-17 enhanced their proliferative and migratory capacities. Bioinformatics analysis predicted that PTEN and PTENP1 were direct targets of miR-21 and miR-17. PRL-3 could downregulate PTENP1 and PTEN through upregulation of miR-21 and miR-17, which in turn could enhance the proliferative and migratory capacities of endometrial adenocarcinoma. PRL-3 may play a pivotal role in promoting the proliferation and migration of endometrial adenocarcinoma by affecting the PTENP1/miR-21 or miR-17/PTEN.
