3. Dignity-related distress profiles in burn survivors: A latent profile analysis.
PMID:日期:2026-08-17Burn injuries and their sequelae, such as scarring and disfigurement, can severely compromise survivors' dignity through body image disturbance. Existing research on dignity in burn populations has predominantly employed variable-centered approaches, which may not capture the heterogeneous, multidimensional nature of dignity-related distress. Latent profile analysis (LPA), a person-centered method, allows for the identification of homogeneous subgroups within heterogeneous populations, thereby offering a more nuanced understanding of how distinct dimensions of dignity co-occur and influence patient outcomes. To identify subtypes of dignity‑related distress among burn survivors using latent profile analysis (LPA) and to explore the influencing factors associated with these subtypes. A single-center, cross-sectional observational study, conducted in strict accordance with the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) guidelines. A total of 323 burn survivors from a burn center were assessed using the Patient Dignity Inventory (PDI), the Burn Specific Health Scale‑Brief (BSHS‑B), and the Hospital Anxiety and Depression Scale (HADS). LPA was applied to identify dignity‑based subgroups. Model fit was evaluated using multiple criteria including AIC, BIC, aBIC, entropy, LMR‑LRT, and BLRT. Posterior probabilities and classification accuracy were examined to ensure profile reliability. One‑way ANOVA, chi‑square tests, and multivariate logistic regression were used to examine subgroup characteristics and their associations with sociodemographic variables. A three-profile model provided the best fit, comprising: "Preserved Dignity" (Profile 1, n = 69, 21.36%), "Dependency and Symptom Distress" (Profile 2, n = 135, 41.80%), and "Pervasive Psychosocial and Existential Distress" (Profile 3, n = 119, 36.84%). Gender, burn depth, BICU admission, burn location, quality of life, and depression were significant influencing factors for dignity‑related distress across different profiles of burn patients (p < 0.05). This study offers a novel person-centered perspective on dignity-related distress in burn survivors, identifying heterogeneous profiles via latent profile analysis. These profiles enable stratified, dynamic interventions to alleviate distress, enhance dignity, and support reintegration.

4. The burden of tobacco-free nicotine dependence in burn survivors: Acute complications and long-term morbidity.
[英文摘要] Nicotine use from tobacco-free sources (e.g., electronic cigarettes, oral pouches, and gum) has become prevalent. While tobacco is an established detriment for wound healing, the relationship between tobacco-free nicotine dependence (TFND) and post-burn morbidities has not been defined. This study sought to assess the impact of TFND on short- and long-term complications in burn survivors. A retrospective secondary database cohort study was conducted using electronic health records from multiple health institutions. Patients (18 + years) with burn injury and TFND were identified and compared to burned patients without TFND. Propensity scores were estimated, and 1:1 matching was performed for sociodemographics, comorbidities, burn characteristics, and treatment modalities. Outcomes between matched cohorts included 90-day wound complications and healthcare utilization, with mortality assessed at 90 days and 1 year, as well as new-onset opioid use disorder at 1 year. Statistical significance was set to p < 0.05. Following matching, 20,906 patients were included per cohort. At 90 days, TFND was associated with higher rates of sepsis, wound dehiscence, infection, antimicrobial prescriptions, emergency department visits, and mortality compared with matched controls (all p < 0.05). At 1 year, TFND was associated with elevated rates of mortality and being diagnosed with opioid use disorder (both p < 0.005). Tobacco-free nicotine dependence worsens burn outcomes by increasing the risk of developing wound complications, healthcare utilization, mortality, and opioid use disorder. Routine screening for electronic cigarettes and smokeless nicotine products, paired with early cessation counseling, may enhance survival and burn recovery.

5. Necrotizing soft tissue infections: Challenges and opportunities to improve patient outcomes.
PMID:日期:2026-08-10Necrotizing soft tissue infections (NSTIs) remain associated with high mortality rates. Improvements in patient outcomes have been limited, largely due to persistent challenges in early diagnosis, surgical management, and care organization. NSTI is frequently misdiagnosed at initial presentation, and uncertainty regarding tissue viability often necessitates wide surgical debridement, resulting in large wounds and substantial scarring. In addition, care pathways are frequently fragmented, thereby limiting access to specialized expertise. Nevertheless, recent years have seen promising opportunities emerge to address these challenges. Novel biomarkers, point-of-care ultrasound, and artificial intelligence may assist in earlier and more accurate diagnosis. Skin-sparing debridement, potentially guided by indocyanine green angiography, may reduce wound and scar size, while dermal substitutes may improve scar quality. Furthermore, the establishment of structured clinical networks, including designated expertise centers, could improve the organization and coordination of NSTI care. This narrative review summarizes current challenges in NSTI care and evaluates emerging strategies to improve diagnosis, surgical management, and care organization. Together, advances in these three domains have the potential to improve outcomes for patients with NSTI.

6. Point-of-care digital manufacturing workflow for patient-specific 3D-printed burn orthoses in a low-resource setting: A retrospective study.
[英文摘要] Burn injuries cause substantial long-term disability in low- and middle-income countries (LMICs), where access to specialized rehabilitation and patient-specific orthoses is limited. Hospital-based point-of-care (POC) digital manufacturing may enable local production of customized burn orthoses in resource-constrained healthcare settings. This retrospective study evaluated a hospital-based workflow integrating optical three-dimensional (3D) scanning, computer-aided design (CAD), and in-house fused filament fabrication (FFF) 3D printing. Four patients aged 3-16 years represented acute-phase, preoperative, postoperative, and hybrid perioperative orthotic pathways. Feasibility outcomes included workflow completion, technical failures, design iterations, fabrication time, clinical fitting, and delivery time. The workflow was completed successfully in all four cases without technical abandonment, repeat scanning due to technical failure, conversion to conventional fabrication, or print failure. Scanning required 20-25 min, CAD customization 5-8 h, and cumulative printer operation 29-78 h. All orthoses were clinically fitted and delivered within 2-3 days of the clinical request. Three cases were completed after a single design iteration, whereas Case 4 required three iterations because of evolving postoperative anatomy and functional requirements. Hospital-based POC digital manufacturing of patient-specific burn orthoses was technically and operationally feasible in this low-resource clinical setting. The workflow supported orthotic management across multiple stages of burn care and provides a practical implementation framework for resource-constrained healthcare systems. Larger prospective studies are needed to evaluate clinical outcomes, cost-effectiveness, and long-term sustainability.

7. Early fluid resuscitation and 28-day mortality in burns: A mediation analysis via coagulation biomarkers.
PMID:日期:2026-08-04Fluid resuscitation is a critical treatment in the shock phase for burn patients, but excessive fluid infusion has adverse effects. We conducted a multicenter retrospective cohort study involving 12 burn centers across mainland China. Adult patients (age >18 years) with severe burns were included. The primary exposure was the 24-hour fluid volume (ml/kg/%TBSA). The primary outcome was 28-day mortality. Cox proportional hazards models were used to evaluate the association between fluid volume and mortality. Secondary outcomes included changes in coagulation parameters (prothrombin time, activated partial thromboplastin time [APTT], INR, D-dimer, fibrinogen, thrombin time, and platelet count) and incidence of coagulopathy. Mediation analysis was performed to assess whether the effect of fluid rate on mortality was mediated through alterations in coagulation markers. A total of 475 patients were included in the initial cohort for mortality analysis, and 278 patients with complete coagulation data were analyzed in the secondary cohort for mediation. After adjusting for multiple confounders, higher fluid volume was significantly associated with increased 28-day mortality (adjusted HR = 1.74, 95% CI: 1.30-2.32, P < 0.001). Elevated fluid volume was also associated with prolonged APTT, reduced platelet count, and higher incidence of coagulopathy. Mediation analysis revealed that the effect of fluid rate on mortality was partially mediated by decline in platelet count (P = 0.002), whereas other coagulation parameters did not show significant mediating effects. Higher initial fluid volume not only has a direct effect on coagulation but also leads to an increased risk of 28-day mortality. The difference in platelet count statistically mediates the effect of fluid volume on mortality outcomes.

8. Analgesic use and pain burden in adults with major burns: A retrospective analysis.
PMID:日期:2026-07-30Pain control in severe burns remains challenging due to physiological factors affecting drug metabolism and efficacy. This study characterizes the most frequent combinations of pain medication, dosages of opioids and non-opioids over time, and their correlation with pain management in adults. A single-center retrospective analysis of patients ≥ 18 years of age admitted to University of North Carolina Jaycee Burn Center with ≥ 20% total body surface area (TBSA) burn injury between 2020 and 2024 was performed. Records were abstracted for demographics, prescription pain medication management, and numerical rating scale (NRS) pain scores. Generalized linear and mixed models assessed average daily pain scores and morphine milligram equivalents (MME), adjusted for age, TBSA burn, and inhalation injury. 140 patients were included (age: 32 ± 7 years; 72% male; TBSA burn 37 ± 19% [range: 20-98%]; length of stay: 42 ± 54 days; 19% inhalation injury). 88% survived and most injuries were flame-related (76%). Opioid-only regimens accounted for 7% of prescribing medications, non-opioid-only 15%, and multimodal regimens 78%. Acetaminophen and hydromorphone were most frequently administered (17%), followed by acetaminophen, hydromorphone, and oxycodone (14%) and acetaminophen, fentanyl, and oxycodone (13%). Patients receiving multimodal or opioid-only regimens demonstrated lower and more stable pain trajectories than those receiving non-opioids alone. Hydromorphone use increased with burn severity and was highest in patients with > 80% TBSA. Although opioid requirements declined over time, patients with burns > 40% TBSA had significantly higher MME at day 25 compared with moderate burns (451 ± 980 vs 237 ± 388; p = 0.023). Multimodal analgesia predominated and was associated with improved pain stability. Persistent opioid requirements in larger burns highlight the impact of burn-related metabolic alterations and support development of individualized analgesic strategies to optimize dosing and reduce prolonged opioid exposure.

9. Hyper-inflammatory response in burn patients coincides with impaired antibacterial activity of neutrophils in blood and burn tissue.
PMID:日期:2026-07-29Burn injury is frequently complicated by excessive inflammation and bacterial infection, contributing to increased morbidity and delayed recovery. Neutrophils play a central role in antibacterial defense, yet their functional state after burn injury remains incompletely understood. This study investigated how burn injury affects neutrophil phenotype and antibacterial activity. In this prospective observational cohort study, neutrophils were isolated from blood and burn tissue (eschar) of burn patients and compared with blood neutrophils from healthy volunteers. Using multicolor flow cytometry and functional assays, we assessed neutrophil phenotype, phagocytosis of heat-killed FITC-labeled Staphylococcus aureus, migration, reactive oxygen species (ROS) production, and cytokine secretion. Burn tissue neutrophils (15 ± 7 days post burn) displayed increased degranulation, reflected by elevated expression of the activation markers CD11b, CD63, and CD66b, but showed markedly reduced antibacterial activity compared with healthy blood neutrophils. In both circulating and burn tissue neutrophils, phagocytosis of S. aureus strains was reduced by 50%, while intracellular ROS production was reduced by 2-fold (burn tissue) or 10-100-fold (blood). In burn tissue neutrophils, IL-8-induced migration was reduced by approximately 30%. Time-course experiments demonstrated that burn tissue neutrophils produced minimal ROS under both basal and PMA-stimulated conditions, indicating a profound defect in inducible oxidative burst capacity. Cells in burn tissue released inflammatory cytokines including MCP-1, IP-10, IL-8, IL-10, and IL-6, although levels were not significantly different from cells in healthy skin. These findings demonstrate that burn injury induces a dysfunctional neutrophil state characterized by phenotypic activation but impaired antibacterial function in both circulating and tissue-derived neutrophils. This dysfunctional response is likely to contribute to increased susceptibility to S. aureus infection, as well as impaired wound healing after burn injury, highlighting neutrophil dysfunction as a potential target to improve both post-burn infection control and clinical outcomes.

10. Pain trajectories during acute hospitalization after burn injuries.
PMID:日期:2026-07-28Burn pain is complex and challenging to manage. Identifying patterns of pain progression during acute hospitalization may improve pain management strategies and outcomes. This study examined pain trajectory patterns and associated factors among burn patients during inpatient admission. A retrospective chart review was conducted for 442 patients admitted to a regional burn center. Daily pain scores were extracted from electronic medical records, and longitudinal k-means clustering was used to identify distinct pain trajectories and their associations with patient characteristics. Data from 417 patients (43,737 pain scores) were analyzed. Patients were mostly male (67%) and White (57%), with a median age of 52 years, a total body surface area burned (TBSA) of 6%, a length of stay (LOS) of 11 days, and a median pain score of 7. Two distinct pain trajectories emerged: High Pain (n = 249, 59.7%) and Low Pain (n = 168, 40.3%). Compared to the Low Pain group, the High Pain group had younger age (46 vs 59), and higher morphine milligram equivalents (24 vs 20). After adjusting for covariates, pain trajectory group membership was not significantly associated with LOS, while TBSA, age, and burn depth were strong predictors of LOS. Two distinct pain trajectories were identified, suggesting early identification of high-pain patients can assist with targeted interventions to support recovery and mitigate persistent pain.
