1. Telitacicept for primary angiitis of the central nervous system: A real-world retrospective study.
PMID:日期:2026-09-11Primary central nervous system vasculitis (PACNS) is a rare inflammatory disease with limited treatment options. Telitacicept and high-resolution vessel wall imaging (HR-VWI) may offer new strategies for therapy and disease monitoring. To evaluate the clinical efficacy and safety of telitacicept in patients with PACNS and to explore the value of HR-VWI in assessing disease activity. A single-center retrospective study. Eight patients with PACNS who received telitacicept and completed HR-VWI follow-up were retrospectively enrolled. Neurological outcomes assessed by the modified Rankin Scale (mRS), HR-VWI vessel wall enhancement characteristics, and immunoglobulin and complement levels were compared at baseline and at 1, 3, 6, and 12 months after treatment. Relapse and adverse events were recorded during follow-up. Following telitacicept treatment, neurological outcomes progressively improved, with modified Rankin Scale (mRS) scores showing a gradual decline over time. At 12 months, all patients remained at a level of mild functional disability (mRS ≤ 2). HR-VWI demonstrated reduced vessel wall enhancement, shortened enhancement length, and decreased circumferential involvement of the affected vessels after treatment. In some patients, the degree of luminal stenosis improved or remained stable, with no evidence of progressive worsening. Immunological analyses showed a progressive decline in serum IgG and IgM levels, whereas IgA levels changed modestly. Complement C3 and C4 levels remained generally stable throughout treatment. Three patients experienced disease relapse, all occurring within 3 months after treatment initiation; however, the relapse frequency was lower than that observed before telitacicept therapy. One patient developed mild allergic cheilitis that resolved with symptomatic therapy. Telitacicept treatment was associated with improvements in neurological outcomes and reductions in HR-VWI inflammatory parameters in patients with PACNS. HR-VWI may provide valuable information for assessing disease activity and evaluating longitudinal vascular changes during follow-up.

2. Efficacy and safety of interleukin-6 receptor inhibitors in myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD): A systematic review and meta-analysis.
PMID:日期:2026-09-11Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) can follow a relapsing course, yet evidence for effective maintenance therapy remains limited. Interleukin-6 receptor (IL-6R) inhibition has emerged as a potential relapse-prevention strategy. To evaluate the efficacy and safety of tocilizumab and satralizumab in patients with MOGAD. Systematic review and random-effects proportional meta-analysis conducted in accordance with PRISMA 2020. PubMed/MEDLINE, Scopus, Web of Science, and Google Scholar were searched from inception through 14 July 2026. Eligible studies reported outcomes in patients with MOGAD treated with an IL-6R inhibitor. The primary outcome was the proportion remaining relapse-free during treatment. Secondary outcomes included annualized relapse rate (ARR), disability, MRI activity, and infectious adverse events. Proportions were pooled using DerSimonian-Laird random-effects models, and study quality was assessed using the Newcastle-Ottawa Scale. Six observational studies comprising 182 treated patients were included. The pooled relapse-free proportion was . ARR decreased consistently across studies, while disability generally stabilized or improved and MRI inflammatory activity decreased or remained stable where reported. Three studies involving 142 patients contributed safety data, with a pooled infectious adverse-event proportion of . Four studies were rated high quality and two moderate quality. IL-6R inhibition was associated with substantial relapse control in selected patients with MOGAD, supporting its potential role as a maintenance therapy. However, the evidence remains limited by observational designs, heterogeneous treatment regimens and follow-up, concomitant therapies, and the influence of one large cohort.

3. Spinal muscular atrophy: Biology, pathogenesis, and therapeutic advances.
PMID:日期:2026-09-11Recently instituted world-wide newborn screening programs for spinal muscular atrophy (SMA) permit identification of at-risk individuals prior to clinical presentation of the disease. Early intervention with one or more of the available disease-modifying therapies has dramatically changed the clinical course for individuals affected by SMA. These therapies that yield such impressive results in pediatric SMA populations frequently provide more muted responses in adolescent and adult individuals with SMA. Many treated individuals experience plateau effects and pursue combination treatments to hopefully boost functional outcomes. In addition, individuals with SMA are also now living longer and experience altered disease phenotypes and challenges, such as severe and progressive scoliosis, bulbar issues and neurocognitive issues. Thus, although recent years are marked by significant advances in our understanding of disease pathogenesis and in the development and implementation of therapies to treat SMA, there are still ongoing challenges and unmet needs that will require new and innovate solutions to more fully address the needs of individuals with SMA. In this review, we provide an overview of SMA disease, including a discussion of the many diverse primary defects that have been documented in many different tissues of individuals with SMA and models of the disease, and provide an overview of the therapeutics currently available and in development to treat SMA.

4. Treatment patterns, access, and multidimensional burden of myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) in China: A nationwide cross-sectional study.
PMID:日期:2026-09-10Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is a rare demyelinating disorder with evolving diagnostic and treatment landscape. Real-world evidence on treatment patterns, access, and disease burden remains limited, particularly in China. To characterize the patient journey, treatment patterns, healthcare resource utilization (HCRU) and multidimensional disease burden of MOGAD in China. Nationwide, cross-sectional electronic survey. Patients with MOGAD and their caregivers completed an electronic survey capturing diagnostic and treatment history, HCRU, and patient-reported outcomes. Data were stratified by age and treatment phases. Multivariable linear regression was used to identify determinants of health-related quality of life (HRQoL). The cohort (n=336) was predominantly pediatric (69.6%). Corticosteroids remained the most widely used across treatment phases. The use of steroid-sparing immunosuppressants increased during maintenance treatment and showed age-specific patterns (SMD=0.9): children used intravenous immunoglobulin more often (57.7%), whereas adults more frequently received mycophenolate mofetil (54.2%). Barriers to treatment access were reflected by distance to obtain care and profound economic burden with high out-of-pocket expenses (up to 100% of total expenditure), while 66.7% of participants reported lack of reimbursement as a major obstacle. Despite largely preserved physical function, HRQoL remained reduced (SF-6Dv2 0.66) with high productivity loss mainly driven by presenteeism. Withdrawal from education/employment was the strongest predictor of reduced HRQoL (β = -0.252, p<0.001), exceeding clinical and treatment-related factors. Caregiver burden was considerable, with 43% requiring leave from work/school, and their high levels of psychosocial distress were evidenced by anxiety and depression screening scores. Real-world management of MOGAD in China is characterized by persistent use of corticosteroids and barriers of access to steroid-sparing immunotherapies. These constraints are associated with impaired quality of life and substantial socioeconomic burden. Improving access to effective and affordable maintenance therapies and strengthening multidisciplinary support may improve long-term outcomes for patients and caregivers.

5. Modelling risk factors for depressive symptoms in seizure disorders.
PMID:日期:2026-09-05Depression is a common mental health comorbidity among individuals with seizure disorders. The emergence of depression results from the complex interplay of clinical, psychological and social factors. To identify clusters of risk factors associated with depressive symptoms in adults with seizure disorders. This study uses exploratory factor analysis (EFA) to model a range of variables and investigate their interactions and influence on depressive symptoms. Cross-sectional observational study. A retrospective examination of medical records identified 121 patients with seizures and moderate depressive symptoms. Demographic, clinical and psychosocial data were extracted and modelled using EFA. Factor structure was determined by adhering to standard fit criteria: CFI ⩾0.90, TFI ⩾0.90, RMSEA ⩽0.08 and latent factor loadings ⩾0.4. The mean age of the cohort was 31.3 ± 10.0 years (range of 18-64), comprising 68 males (56.2%) and 53 married individuals (43.8%). Qataris constituted the largest nationality at 48 (39.7%), and the most prevalent diagnosis was right temporal lobe epilepsy (28.9%). Moderate depressive symptoms were the most common PHQ-9 classification (42.1%), with an average score of 14. EFA yielded a three-factor solution (χ = 71.9, df = 52, RMSEA = 0.06, CFI = 0.96, TFI = 0.92) with the first latent factor incorporating age and social variables, the second encompassing medications and cognitive abilities, and a final factor encapsulating seizure-related items. Exploratory factor analysis elucidates the intricate relationships between variables that impact depressive symptoms in individuals with seizure disorders. It highlights how clusters of variables emerge, providing insight into how depressive symptoms develop in this population. These findings offer new potential avenues for the management and treatment of depression in people experiencing these debilitating events.

6. Use of subcutaneous natalizumab in routine care - Final results of a non-interventional, observational study (SISTER) in Germany and Austria.
PMID:日期:2026-09-03Natalizumab is a disease-modifying therapy for patients with relapsing-remitting multiple sclerosis (RRMS). In addition to its original intravenous (IV) formulation, a subcutaneous (SC) route of administration (RoA) was approved in 2021. Subsequently, the observational SISTER study was initiated in Germany and Austria to generate real-world data on the utilization, patient preferences, safety, and effectiveness of natalizumab in everyday clinical practice. The primary objective was to compare patients' preference of RoA and satisfaction with SC vs. IV natalizumab at baseline and up to 12 months of treatment. Secondary objectives included drug utilization, effectiveness outcomes, safety, and treatment satisfaction with natalizumab. SISTER was a prospective, multicenter, observational study on RRMS patients who were allocated to 3 parallel treatment cohorts: Patients switching from IV to SC natalizumab (SC switcher) and patients starting natalizumab on either the SC or IV route (starter SC/IV). The final study analyses were based on 310 patients (220 switchers, 44 SC starters, 46 IV starters) with an observation period of 10.8±3.1 months. Patient preference for the initial RoA at baseline was 95.1% among SC starters and 77.5% among IV starters (p=0.045). More IV starters than SC starters or SC switchers at baseline and Month 6 wished to change the initial RoA. Disease activity remained low in all cohorts, and the AE pattern was consistent with the known safety profile of natalizumab. The procedure duration was distinctly shorter on SC treatment than on IV treatment. The final SISTER results suggest a patient preference for the SC vs. IV route and support the outcomes of the NOVA Part 2 trial, in which the preference of the SC route over IV was demonstrated in a cross-over design. The SC route allowed relevant time savings, and its good safety and tolerability argues for possible self-administration of SC natalizumab.

7. Patient SAtisfaction with the TYSabri SC Formulation - A Swiss multi-centriC, prospective non-intervenTIONal study: the SATYSFACTION study.
PMID:日期:2026-09-03Natalizumab (NTZ) is a highly effective disease-modifying therapy for relapsing-remitting multiple sclerosis (RRMS), originally administered intravenously (IV). A subcutaneous formulation (SC_NTZ) became available in Switzerland in 2021, offering potential advantages. To investigate satisfaction and convenience of use of SC_NTZ in MS patients. Swiss, prospective, multicenter study. Consecutive RRMS patients treated with either ⩽6 (starters) or > 6 (long-term users) SC_NTZ injections completed the Treatment Satisfaction Questionnaire with Medication-II (TSQM-II, higher scores indicating greater satisfaction) and the Patient Satisfaction Questionnaire (PSQ) at baseline (Visit 1, V1). Starters were reassessed at the 9th SC_NTZ dose (Visit 2, V2). Non-parametric tests were applied to compare long-term users with starters at V1, and changes from V1 to V2 in starters. One hundred six patients (73.6% female, median age 39.6 years, prior IV_NTZ 81.1%) were enrolled, including 79 long-term users and 27 starters. At V1, in the overall group the median (IQR) TSQM-II effectiveness, side effects, convenience and global satisfaction scores were 83 (67-100), 100 (92-100), 83 (72-89) and 83 (75-100), respectively. On the PSQ, the most commonly cited benefit was short administration time (80.2%). SC_NTZ treatment "never" affected work attendance in 54/87 (62.1%) employed patients. Seventy-nine percent participants reported spending ⩽1 h for SC_NTZ administration. Nonetheless, 76/86 (88.4%) of switchers from IV_NTZ found opportunities for discussions with healthcare professionals to be "similar"/"improved," and 78 (90.7%) preferred SC_NTZ. Findings were consistent across subgroups and time points. This real-world, multicenter Swiss study supports SC_NTZ as a well-tolerated, highly accepted, and patient-preferred alternative to IV_NTZ.

8. Implantable vagus nerve stimulation for the treatment of disorders of consciousness with epilepsy: a retrospective propensity score-matched clinical study.
PMID:日期:2026-08-31Disorders of consciousness (DoC) have limited effective therapies. Implantable vagus nerve stimulation (VNS) is established for epilepsy but remains underexplored for DoC with comorbid epilepsy. To evaluate whether VNS is associated with improved consciousness recovery in DoC patients with epilepsy and to explore factors related to response. Retrospective propensity score-matched cohort study. Ninety DoC patients with epilepsy received implantable VNS or conservative management. The primary outcome was clinically meaningful improvement at 1 year (Coma Recovery Scale-Revised (CRS-R) increase ⩾3 points). Propensity score matching (1:1) balanced baseline characteristics (23 per group). Longitudinal CRS-R trajectories were assessed using linear mixed-effects models with Type III ANOVA. Logistic regression within the VNS cohort explored factors associated with responder status; adverse events were extracted from operative and follow-up records. The 1-year responder rate was higher with VNS than with conservative management (34.78% vs 4.35%; two-sided Fisher's exact test, = 0.022). Longitudinal analyses showed a significant group × time interaction (χ = 43.535, < 0.001) with greater CRS-R gains from 3 months onward in the VNS group. Within the VNS cohort, responder status was associated with baseline minimally conscious state (aOR = 9.750, 95% confidence interval (CI) 1.592-59.695; = 0.014) and better seizure control (McHugh classification; aOR = 22.667, 95% CI 3.140-163.629; = 0.002). Traumatic etiology was not associated with 12-month net CRS-R improvement after adjustment for baseline CRS-R (β = 0.893, 95% CI -1.583 to 3.369; = 0.466). Five patients reported stimulation-related hoarseness/dysphonia, and two had surgical-site complications; no device removal occurred. Implantable VNS was associated with higher 1-year clinically meaningful improvement and greater longitudinal CRS-R gains than conservative management in DoC patients with epilepsy. Prospective controlled studies are warranted.

9. The association between BMI and cognition, disability, and quality of life markers in multiple sclerosis: DISCOMS secondary analysis.
PMID:日期:2026-08-31The association between body mass index (BMI) and cognition, disability, and quality of life (QoL) is controversial in multiple sclerosis (MS). We investigated the association between BMI and cognitive function, disability, and QoL in MS. A cross-sectional secondary analysis of the DISCOMS trial was performed to study the association between BMI and cognition, disability, and QoL. Cognitive function was measured by the symbol digit modalities test (SDMT) and disability was measured based on the expanded disability status scale (EDSS) and patient determined disability steps (PDDS). QoL markers were measured by the Quality of Life in Neurological Disorders. Descriptive and inferential statistics were used to analyze the association between BMI and cognition, disability, and QoL. The study included 253 participants, mostly female (83.8%) and white (89.3%). Their mean BMI was 27.27 (SD = 5.6), and 62% were overweight, obese, or very obese. Controlling for age, gender, and disease-modifying therapy duration, BMI as a continuous or as a categorical variable was not associated with cognitive function as measured by the SDMT or disability as measured by the EDSS and/or the PDDS. EDSS as a dichotomous variable was associated with BMI; 1-unit increase in BMI was associated with a 5.7% increase in the odds of having an EDSS score greater than 4 (β = 0.055, = 0.045). Higher continuous BMI was associated with physical domains dysfunction; worse fatigue ( = 0.009), and worse lower extremities function ( = 0.008), while the very obese group had a higher risk of physical and emotional domain dysfunction than the normal BMI group. The lack of observed associations between BMI and cognitive function or disability in this older MS cohort suggests that the relationship between BMI and clinical outcomes may vary across domains. Nevertheless, the associations identified between BMI and QoL measures emphasize the potential importance of weight management in supporting overall health and well-being among individuals with MS. ClinicalTrials.gov NCT03073603.

10. The impact of concomitant use of rosuvastatin or atorvastatin plus clopidogrel on the platelet inhibition and clinical outcomes in acute large-vessel minor stroke or TIA: A randomized controlled multi-center trial, the ROCATIS-1 trial.
PMID:日期:2026-08-26Although clopidogrel is commonly used for ischemic stroke secondary prevention, 20-50% of patients experienced clopidogrel failure and recurrence of ischemic events. We aimed to evaluate the benefits or hazards of adding atorvastatin or rosuvastatin to clopidogrel on the clopidogrel-induced inhibition of platelet activation and vascular events prevention. Our multi-center, randomized, single-blinded, parallel-group trial was conducted between 10th April 2024 and 10th May 2026. 600 first-ever, large-vessel minor ischemic stroke or TIA patients received 40 mg of atorvastatin or 20 mg of rosuvastatin during the first 24 hours of stroke onset once daily till the 90th day after stroke onset. Both groups received open-label 300 mg loading doses of aspirin and clopidogrel during the first 24 hours after stroke onset, followed by 75 mg clopidogrel and 100 mg aspirin once daily for 3 weeks, then 75 mg clopidogrel only until the end of the follow-up period. We followed up with our patients for 3 months. The change in the percentage of platelet maximum aggregation at 3 months of treatment compared with baseline while using adenosine diphosphate ADP), 5 µM/L as an agonist was 54.4 (51.0-57.1) in the rosuvastatin group compared with 49.2 (45.5-52.3) in the atorvastatin group with P-value 0.003. 22 (7.3%) patients in the rosuvastatin group and 40 (13.3%) patients in the atorvastatin group experienced a composite of a new stroke, MI, or death due to vascular insults (HR 0.52; 95% CI, 0.33-0.81; P-value= 0.004), moreover, 12 (4.0%) patients in the rosuvastatin group and 19 (6.3%) in the atorvastatin group experienced recurrent stroke either ischemic or hemorrhagic, with (HR 0.60; 95% CI, 0.32-1.10; P-value 0.10). Compared with combining atorvastatin with clopidogrel in African large-vessel minor stroke or TIA, combining rosuvastatin with clopidogrel yielded higher rates of clopidogrel-induced platelet aggregation inhibition, significantly lower rates of a composite of recurrent stroke, MI, and death due to vascular events at three months. There were no significant differences between rosuvastatin and atorvastatin regarding drug-related side effects. Our findings are hypothesis-generating and require confirmation in a double-blinded, multinational randomized trial before they can inform practice.
