This review summarizes current evidence on interventional therapies for hepatocellular carcinoma (HCC) with portal vein tumor thrombosis (PVTT), compares the treatment strategies between East and West, and proposes a practical framework for patient selection in the immunotherapy era of HCC. Although systemic therapy remains the primary treatment for advanced HCC with PVTT, selected interventional therapies may provide additional value in carefully chosen patients with preserved liver function, liver-dominant disease, anatomically targetable PVTT, and adequate or reconstructable portal venous flow. Interventional options include transarterial chemoembolization, hepatic arterial infusion chemotherapy, transarterial radioembolization/selective internal radiation therapy, radiotherapy, portal vein stenting, and iodine-125 seed-based endovascular brachytherapy. Eastern and Western practice patterns differ mainly in the threshold for locoregional intensification, technical platform availability, and interpretation of the available evidence. Eastern and Western approaches increasingly share the principle that systemic therapy is the treatment backbone for advanced HCC with PVTT. The key clinical question is how interventional therapy should be integrated for selected patients rather than whether it should replace systemic therapy. Optimal management should be individualized through multidisciplinary decision-making based on PVTT extent, portal venous flow, liver function, intrahepatic and extrahepatic tumor burden, treatment goals, and institutional expertise. Future studies should standardize reporting of PVTT classification, portal vein patency, collateral circulation, liver reserve, tumor distribution, systemic therapy context, and treatment sequencing.

Acute myeloid leukemia (AML) exemplifies how malignant transformation can be driven by both epigenetic deregulation and genetic mutation. Aberrant DNA methylation, histone modifications, and chromatin remodelling cooperate to block myeloid differentiation, sustain leukemic self-renewal, and promote immune evasion. This review examines the molecular mechanisms underlying epigenetic dysregulation in AML and highlights recent advances in epigenetic and immune-based therapeutic strategies. Recent studies have identified multiple druggable epigenetic regulators, including DNA methyltransferases, TET dioxygenases, histone methyltransferases and demethylases, and histone acetyltransferases and deacetylases. These discoveries have led to the development of hypomethylating agents, mutant IDH1/2 inhibitors, menin-MLL and DOT1L antagonists, EZH2 and LSD1 inhibitors, and class-specific HDAC inhibitors, several of which are approved or undergoing advanced clinical evaluation. Beyond reprogramming leukemic transcriptional networks, epigenetic therapies also remodel the immune microenvironment by influencing antigen presentation and immune checkpoint pathways, providing a rationale for combination approaches with antibody-drug conjugates, bispecific T-cell or NK-cell engagers, aptamer-directed therapeutics, vaccines, and CAR-T/NK cell therapies targeting CD33, CD123, FLT3, CLL-1, and other AML-associated antigens. The integration of epigenetic modulation with immunotherapeutic and precision medicine approaches represents a promising strategy to overcome disease heterogeneity and therapeutic resistance in AML. By bridging mechanistic insights with translational advances, clinical trial evidence, and emerging patent landscapes, this review highlights epigenetic dysregulation as both a fundamental driver of leukemogenesis and a clinically actionable therapeutic interface that may enable more rational treatment sequencing and personalized AML management.

This study aims to identify complications and difficulties in oncology patients undergoing dental implant osseointegration, considering the impact of treatment on the microarchitecture of the craniofacial region, the oral cavity microbiome, and oral cavity inflammation. Surgical resection, radiotherapy, chemotherapy, and targeted therapies significantly affect the condition of bones, mucous membranes, and soft tissues, and consequently the possibilities and limitations of prosthetic rehabilitation. Chemotherapy significantly affects calcium and phosphorus metabolism. It has been observed that there is an increased risk of fractures, as well as changes in their microarchitecture, and a reduction in the resorption of biomaterials after transplantation has been observed, impairing bone repair in critical defects. These changes may also affect the processes of osteogenesis and osteoclastogenesis and lead to osteoblast cell apoptosis. The impact of therapy on the oral microbiota, as well as on increased inflammation and oxidative stress in the oral cavity, is also significant. These changes can lead to difficulties in implant osteointegration, as well as opportunistic infections, which reduce the patient's prognosis for implant acceptance. Complications after implantation in oncology patients may include inflammation of the mucosa around the implant and inflammation of the tissues around the implant, soft tissue ulceration, marginal bone loss, and partial loss of osseointegration. Due to the complexity of the processes occurring in the oral cavity of oncology patients, prosthetic treatment planning should be interdisciplinary and involve an oncologist, surgeon, prosthodontist, and conservative dentist.

Sarcomas are rare and heterogeneous malignancies for which molecular diagnostics increasingly complement histopathology, immunohistochemistry, FISH, and RT-PCR. This review summarizes the main research applications of next-generation sequencing (NGS) in sarcoma, with particular attention to mutation and variant discovery, fusion detection, biomarker development, assay validation, multi-omics integration, and the boundaries between exploratory research and clinical implementation. NGS-based approaches have expanded the detection of recurrent and rare alterations across sarcoma subtypes, including subtype-defining fusions, copy-number changes, actionable kinase rearrangements, and alterations in tumor suppressor and cell-cycle pathways. However, the clinical utility of NGS remains context-dependent and should be interpreted according to tumor subtype, sample quality, available orthogonal methods, multidisciplinary expertise, and current precision-oncology recommendations. NGS is a powerful tool in translational sarcoma research and, in selected clinical scenarios, supports diagnosis, molecular classification, and therapeutic decision-making. Its use should be presented with appropriate caution, acknowledging technical limitations, the need for expert reference-center interpretation, and the complementary role of established diagnostic techniques.

This review highlights the major unmet needs in diagnosis and treatment of digestive neuroendocrine carcinomas (NEC) and describes the current and evolving evidence on these aspects. Improved pathological criteria are needed to accurately diagnose NEC and distinguish this entity from high-grade well-differentiated neuroendocrine tumours as well as from adenocarcinoma. Studies on the molecular mechanisms underlying the extreme aggressiveness of the disease are ongoing. Data are lacking on optimal selection of patients with locoregional disease for primary tumour surgery and use of adjuvant or neoadjuvant treatment. For patients with metastatic disease, there has been no improvement in systemic treatment over the last two decades and immunotherapy has limited benefit. Novel antigen-directed strategies targeting DLL3 are evolving. Digestive NEC are among the most aggressive cancers and prognosis remains poor. There have been only minor improvements over the last two decades. To improve diagnosis and therapy, the unmet needs discussed in this review must be addressed.

Nasopharyngeal carcinoma (NPC) is a malignant tumor characterized by a distinct geographical distribution. Effective surveillance is crucial for the early detection of recurrence or metastasis and for improving patient prognosis. This review systematically examines current NPC follow-up protocols and recent developments to inform individualized precision surveillance. This review focuses on two main aspects. 1) We compare and analyze current major NPC follow-up guidelines, with key discussions covering follow-up frequency, imaging modalities (including magnetic resonance imaging [MRI] and positron emission tomography [PET]), plasma Epstein-Barr virus DNA (EBV-DNA) monitoring, and functional assessments. 2)We elaborate on the application prospects and research progress of genomics, radiomics, and artificial intelligence in NPC surveillance. Studies suggest that risk-stratified, individualized follow-up strategies, such as those based on conditional survival models, can enhance the cost-effectiveness of surveillance. Additionally, emerging technologies, including radiomics and artificial intelligence, show promise for improving recurrence risk assessment, prognostic stratification, and individualized surveillance in NPC. Concurrently, advances in genomics and radiomics offer new opportunities for predicting complications and guiding treatment adjustments. Future efforts should focus on integrating multidisciplinary expertise to develop dynamic monitoring systems that enable precise follow-up and ultimately improve patient survival outcomes.

To examine how biological sex and gender influence the tumorigenesis of neuroendocrine neoplasms (NEN), integrating epidemiological evidence with hormonal, molecular, lifestyle and environmental factors. Recent population-based studies demonstrate sex differences in NEN incidence, pathological characteristics and survival across multiple anatomical sites. Emerging evidence suggests that sex-hormone signalling, epigenetic regulation and the tumour microenvironment may contribute to these differences. Lifestyle and metabolic factors, including smoking and obesity, may further modify NEN susceptibility and interact with sex- and gender-related factors. Biological sex appears to be an important modifier of NEN development rather than simply a demographic characteristic. However, mechanistic evidence remains limited and the contribution of gender is poorly characterised. Prospective, sex-stratified studies integrating hormonal, molecular and environmental data are required to clarify these interactions and their potential clinical relevance.

Oral cavity cancer (OCC) is a significant clinical challenge within head and neck oncology. It is characterized by significant morbidity, high recurrence rates, and only modest improvements in long-term survival over recent decades. Recent advances in metagenomics and high-throughput sequencing have revealed that oral microbial dysbiosis contributes to tumorigenesis, highlighting the potential of microbiome-derived biomarkers for early detection and prognosis of OCC. The purpose of the work is to discuss oral microbiome alterations in OCC. The microbiome plays a crucial role in regulating the host's physiological, immunological, and metabolic functions. Changes in the composition of the microbiome can influence our health status of local diseases of the oral cavity and oropharynx, as well as systemic diseases. In recent years, the role of the oral microbiome in oral potentially malignant disorders progression and OCC development has been emphasized. Changes in the oral and gut microbiome can lead to the induction of chronic inflammation, modulation of the immune system, and the production of carcinogenic metabolites. Dysbiosis can lead to critical inflammatory responses and consequently contribute to neoplastic events. Microbiome-modulating therapies tailored to the patient's individual profile and aimed at improving cancer treatment outcomes are being introduced, including the introduction of antibiotics to reduce harmful bacteria. Identification of new biomarkers in oral microbiota communities and development of therapeutic strategies in relation with oral microbiota in the future may be one of the pillars of the oral cancer diagnosis and treatment strategy. Oral cancer prevention protocols should include the improvement of oral hygiene as an element of the prevention and treatment of periodontitis. An introduction of microbiota-targeted interventions, including probiotics, prebiotics, and dietary strategies, may reduce the toxicity of radiotherapy, leading to improved patient outcomes.

To synthesizes evidence on artificial intelligence (AI) performance in neuroblastoma (NB) diagnosis, risk stratification, prognosis, and genomic characterization. A systematic review and meta-analysis was conducted following PRISMA 2020 guidelines (PROSPERO: CRD42024539475) across five databases. Meta-analyses used random-effects models with logit-transformed Area Under the Curve (AUCs) and cluster-robust standard errors. AI models were classified as Machine Learning Models (MLM) or Hybrid Nomograms (HN) based on their construction methodology. Of 3,742 articles identified, 53 were included. MLMs demonstrated higher point estimates than radiologists in differential diagnosis (AUC: 0.87 vs. 0.83), though this difference was not statistically significant and carried substantial uncertainty. HNs achieved stronger performance in risk stratification (AUC: 0.87). AI-derived nomograms (AUC: 0.9) and gene signatures (AUC: 0.8) outperformed conventional prognostic markers descriptively. Chemotherapy response prediction remained below clinical utility thresholds across all model types. Only 33.9% of models reported calibration and 24.5% underwent external validation. AI demonstrates proof-of-concept across multiple NB clinical domains. However, clinical adoption remains premature given persistent gaps in external validation, calibration, dataset size, and pediatric-specific model development. Future studies should test these models prospectively in multicenter pediatric cohorts, ideally through COG or SIOPEN, using shared definitions for diagnosis, risk group, treatment response, and survival outcomes.

Tele-health has evolved from a marginal tool confined to rural populations and selected follow-up programs into a structurally integrated component of modern cancer care. Prior to COVID-19, its adoption was constrained by regulatory fragmentation, non-uniform reimbursement, and licensure barriers. This narrative review evaluates the evolutionary integration of tele-health in oncology post-COVID-19, examines digital disparities across patient populations, and addresses the medico-legal implications of this integration, with the objective of providing a comprehensive and clinically actionable framework for the governance of virtual oncology care. The pandemic acted as a global catalyst, driving telehealth to over 50% of oncology outpatient encounters in some settings, before stabilising post-pandemic at approximately 10-20% of consultations within hybrid care models. Evidence supports meaningful clinical benefits - improved access to specialist services, reduced travel burden, and sustained continuity of care - with outcomes comparable to in-person care in postoperative follow-up, symptom monitoring, and survivorship. However, persistent disparities in device availability, connectivity, and digital literacy disproportionately affect older, rural, and socioeconomically disadvantaged patients, raising the risk that geographic inequalities are replaced by technological ones. From a medico-legal standpoint, the remote modality does not modify the applicable standard of care, yet restricted physical examination and reliance on patient-reported data introduce risks of diagnostic delay and incomplete clinical assessment, with direct implications for professional liability, data protection under HIPAA and GDPR, cross-border licensure, and multi-party accountability across physicians, institutions, and technology providers. Tele-oncology has become a permanent structural feature of modern cancer care, offering demonstrable benefits in access, continuity, and patient satisfaction. Yet its integration has been uneven, its governance remains fragmented, and its medico-legal landscape is still evolving. Realising the full potential of virtual oncology care - equitably and safely - requires coherent regulatory frameworks, sustained investment in digital infrastructure, and explicit attention to the populations at greatest risk of being left behind.
