1. The Architecture of Shared Decision-Making in Postmastectomy Breast Reconstruction: A Systematic Review of Concepts, Measures, and Clinical Realities.
PMID:日期:2026-10-03Shared decision-making (SDM) is central to postmastectomy breast reconstruction (PMBR), yet its conceptualization, implementation, and evaluation remain heterogeneous. This study aimed to systematically synthesize the evidence on SDM processes, interventions, measurement approaches, and implementation in PMBR. Following a preregistered protocol (OSF; https://osf.io/mu9hv ), we searched PubMed, Embase, Web of Science, Scopus, Trip, and Cochrane from inception to November 2025. Eligible studies included original quantitative, qualitative, or mixed-methods research evaluating SDM in women considering or undergoing PMBR. Methodological quality was assessed using QualSyst, and findings were narratively synthesized within a thematic framework. Fifty-six studies met the inclusion criteria. Randomized and quasi-experimental studies generally reported improvements in patient knowledge and values clarification following decision-support interventions, although effects on decisional conflict and preference concordance were heterogeneous. PMBR decisions were influenced by patient preferences, cultural factors, and clinical context. Major implementation barriers included time constraints, workflow limitations, and variable clinician engagement. SDM can facilitate informed and preference-concordant PMBR decisions but remains inconsistently implemented. Greater integration of structured decision-support strategies and standardized outcome measurement may strengthen its implementation in routine practice.

2. Lack of Utility of Ki-67 in Axillary Surgical Planning and Prognostication after Neoadjuvant Chemotherapy for Node-Positive Breast Cancer.
PMID:日期:2026-10-03Neoadjuvant chemotherapy (NAC) can downstage the axilla for patients with breast cancer and spare axillary lymph node dissection (ALND). Although tumor biomarkers help predict response to NAC and inform surgical decision-making, the role of Ki-67 in predicting axillary pathologic complete response (pCR) and recurrence is unclear. Consecutive patients with stage II/III node-positive breast cancer treated with NAC from January 2007 to July 2021 were identified from a prospective database. Ki-67 was categorized. Multivariable logistic regression identified predictors of nodal pCR and recurrence. Distant disease-free survival (DDFS) analysis was performed for changes in Ki-67 for patients with residual disease. Of 407 node-positive patients treated with NAC, 172 (42.3%) converted to ycN0. Most patients had cT2 (52.9%) and cN1 disease (90.1%). Median Ki-67 from the biopsy specimen was 43% (interquartile range 31-70); Ki-67 was highest in triple-negative disease. Overall, 105 patients (61.0%) had a nodal pCR; the rate of nodal pCR was significantly different among biomarker subtypes: 32.8% for hormone receptor-positive (HR+ )/human epidermal growth factor receptor 2-negative (HER2-), 73.3% for HR-/HER2-, 75.0% for HR+/HER2+, and 81.8% for HR-/HER2+ (p < 0.0001). There was no association between the initial Ki-67 and nodal pCR for any receptor subtype. On multivariable analysis, Ki-67 did not predict nodal pCR or recurrence; among patients with residual disease, a decrease in Ki-67 after NAC was associated with improved DDFS (hazard ratio 0.28; 95% confidence interval 0.07-1.13; Wilcoxon p = 0.02, log-rank p = 0.057). Ki-67 was not predictive of nodal pCR and is not a useful adjunct for surgical decision-making; however, a decrease in Ki-67 from biopsy to surgical specimen may suggest improved DDFS.

3. Disruptions and Comprehensiveness of Patient Information in Multidisciplinary Virtual Tumor Boards: An Observational Study in Thoracic Cancer Care.
PMID:日期:2026-10-03Virtual multidisciplinary tumor boards (vMTBs) support oncologic care, but their real-world operational dynamics and how interdisciplinary interactions unfold are insufficiently characterized. Flow disruptions may interrupt communication and information exchange, potentially undermining efficient workflow and quality of clinical decisions. This study investigated whether flow disruptions are associated with changes in information comprehensiveness during vMTBs in a thoracic cancer network. In an observational study, three trained observers monitored routine vMTB sessions and applied standardized tools to document, for each patient case, the occurrence and type of flow disruptions and to rate information comprehensiveness. The study used bi- and multivariable analyses, adjusting for relevant case- and session-level confounders, including case length and image presentation. The study included 146 registered patient cases and identified 392 flow disruptions (mean, 3.77 ± 3.08 per case). The most frequent disruption categories were audio quality issues (33.9%), impaired image visualization (27.6%), and difficulties in information retrieval and exchange (24.2%). In multivariable models, flow disruptions due to off-topic questions and issues attributable to the vMTB software were associated with lower information comprehensiveness, whereas flow disruptions related to poor visualization were associated with higher comprehensiveness. Flow disruptions are common in vMTBs, and their impact varies by disruption type. These findings highlight actionable leverage points, particularly optimizing audio/visual clarity and improving software-supported information exchange, to strengthen information completeness relevant to surgical decision-making. Future studies should relate these workflow measures to clinical outcomes to investigate downstream effects of inefficient communication patterns in distributed tumor board settings.

6. Impact of Neoadjuvant Immunotherapy Combined with Chemotherapy on Surgical Margin Strategy in Central Non-Small-Cell Lung Cancer.
6. 新辅助免疫治疗联合化疗对中心型非小细胞肺癌手术切缘策略的影响PMID:日期:2026-10-02The CheckMate 816 trial showed that neoadjuvant immunotherapy combined with chemotherapy improved pathological complete response and tumor downstaging compared with chemotherapy alone. Whether this permits less extensive resection, such as reduced margins, without compromising survival remains unclear. We included 142 patients with central-type non-small-cell lung cancer (NSCLC) who received neoadjuvant chemoimmunotherapy at three hospitals between 2017 and 2022. All were initially considered candidates for sleeve lobectomy (SLT) based on pretreatment findings. After treatment, patients with residual disease confined to the segmental bronchus and/or distal pulmonary parenchyma were considered eligible for lobectomy (LT). Patients underwent SLT or LT according to pretreatment or post-treatment tumor extent, respectively. Negative margins were confirmed by intraoperative frozen-section pathology. Propensity score weighting (PSW) was used to balance baseline characteristics. Of the 142 enrolled patients, 86 had a radiographic response, with bronchoscopy revealing tumor regression to the segmental bronchus or more proximal airways. Ultimately, 82 patients underwent surgery, seven of whom were lost to follow-up during the follow-up period. Among 75 patients, 30 (40.0%) underwent SLT and 45 (60.0%) converted to LT. Grade II-V complications occurred in one case (2.22%) in the LT group and seven cases (23.3%) in the SLT group (relative risk 0.09; 95% confidence interval 0.01-0.57; p < 0.05). After PSW, baseline characteristics were balanced between groups (p > 0.05). PSW-adjusted event-free survival showed no significant difference between groups (hazard ratio 0.53; 95% confidence interval 0.13-2.22; p > 0.05). For central-type NSCLC initially planned for SLT, conversion to LT is feasible when adequate negative margins are confirmed after neoadjuvant chemoimmunotherapy, potentially avoiding more extensive surgery.

9. Universal versus Guideline-Directed Germline Testing in Breast Cancer: Incremental Diagnostic Yield in a Predominantly Black Population.
PMID:日期:2026-10-01The American Society of Breast Surgeons recommends germline testing (GT) for all patients with a personal history of breast cancer (BC), citing concerns that guideline-based approaches may miss mutation carriers. We evaluated the incremental diagnostic yield of universal testing (UT) compared with National Comprehensive Cancer Network (NCCN)-guided testing in a predominantly Black population. We performed an Institutional Review Board (IRB)-approved retrospective review of female patients with BC without prior GT evaluated at a community-based breast center between July 2024 and October 2025. Outcomes included testing uptake, GT results, pathogenic/likely pathogenic (P/LP) variant yield, and NCCN eligibility. Among 170 patients, 94.1% were non-Hispanic Black. Of 147 patients who underwent GT, 6.1% harbored P/LP variants, 26.5% had variants of uncertain significance (VUS), and 67.4% tested negative. Most patients (91.2%) met contemporaneous NCCN criteria. P/LP detection rates were 6.0% in NCCN-eligible patients and 7.7% in non-eligible patients (p = 0.58). UT identified one additional BRCA2 mutation carrier outside NCCN criteria. In this cohort, NCCN criteria captured the majority of patients undergoing GT, with mutation detection rates comparable to UT. The incremental diagnostic yield of UT was limited, identifying one additional mutation carrier. In resource-constrained settings, guideline-based testing may provide comparable clinical utility to universal testing approaches.
