2. Older people living with HIV: a population with high prevalence and severity of metabolic dysfunction-associated steatotic liver disease ‒ A cross-sectional study.
PMID:期刊:日期:2026-09-15Among People Living With Human immunodeficiency virus (PLWH) chronic liver disease is a major cause of non-HIV-related death. Factors related to HIV and its treatment may amplify the impact of metabolic risk factors and aging, thus leading to a high frequency of Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD) among this population. This study aimed to evaluate the frequency and associated factors of MASLD and its severity in older PLWH, and secondarily, to assess the frequency of MASLD-related fibrosis. This cross-sectional study included older PLWH followed at a referral center in northeastern Brazil. Hepatic steatosis was assessed by abdominal ultrasonography and liver fibrosis by point Shear-Wave Elastography (pSWE). Multivariable Poisson regression was performed to identify factors independently associated with hepatic steatosis and steatosis severity. Ninety-two older PLWH were included. Hepatic steatosis was identified in 53.3% (n = 49) of participants, of whom 29 (53%) had moderate-to-severe steatosis. Significant fibrosis (F2 or greater) was identified in 10.6% of participants with steatosis who underwent elastography. Increased waist circumference, higher body mass index, metabolic syndrome, higher HbA1c and triglyceride levels were associated with hepatic steatosis. In multivariate analysis, increased waist circumference remained the factor most strongly associated with hepatic steatosis (PR = 4.69; 95% CI 1.53-14.38; p = 0.007). Factors associated with moderate-to-severe steatosis included metabolic and social factors, higher Alanine Aminotransferase (ALT) levels, dolutegravir use, absence of darunavir exposure and higher CD4 nadir. After adjustment, lipodystrophy (aPR = 1.63; 95% CI 1.04-2.56; p = 0.033) and higher Alanine Aminotransferase (ALT) levels (aPR = 1.03; 95% CI 1.01-1.04; p = 0.001) remained independently associated with greater steatosis severity. MASLD was highly prevalent among older PLWH, with a substantial proportion presenting moderate-to-severe steatosis and significant fibrosis. Metabolic and adiposity-related factors were independently associated with hepatic steatosis, while lipodystrophy and higher ALT levels were associated with greater steatosis severity.

4. Low awareness of HIV status and high proportion of recent infections among young travestis and trans women in Brazil.
PMID:期刊:日期:2026-06-11This study examined correlates of new HIV diagnosis (i.e., no prior awareness of HIV-positive status and HIV diagnosis at study entry) and identified factors associated with HIV vulnerability among Brazilian young trans women. Baseline assessment of Brilhar e Transcender (BeT ["Shine and Transcend" in English]), an HIV status-neutral intervention single-center study using peer-led digital and in-person systems navigation, between February‒July 2022, in Rio de Janeiro, Brazil. Trans women aged 18‒24 years, living in Rio de Janeiro city or the metropolitan area, were eligible if they were not currently on HIV Pre-Exposure Prophylaxis (PrEP) for those who were HIV-negative, or if they were not on antiretroviral therapy, had a detectable viral load, or were not linked to HIV care for those living with HIV. Among 170 potential participants, 164 young trans women were eligible and enrolled. Among these, 13% (n = 22) were living with HIV, 68% of them (n = 15) were unaware of their HIV status and diagnosed with HIV; 33% (n = 4 of 12) had a recent HIV infection. Overall, 23.1% had current syphilis, 15% rectal chlamydia, and 13% rectal gonorrhea. Younger age (18‒19 years) (adjusted Odds Ratio [aOR=5.62], 95% Confidence Interval [95% CI 1.36‒27.22]), lower educational attainment (aOR = 7.99, 95% CI 1.78‒47.85), and moderate/high risk of crack/cocaine use (aOR = 8.6, 95% CI 1.44‒54.91) were associated with a higher odds of a new HIV diagnosis. HIV disparities among young trans women in Brazil reflect persistent structural and social inequities shaping vulnerability and access. Tailored interventions must extend prevention efforts beyond individual behaviors to address these broader determinants.

5. Rare clinical presentations of paracoccidioidomycosis: Epidemiological, clinical, and microbiological insights from a Brazilian reference center.
PMID:期刊:日期:2026-06-01Paracoccidioidomycosis (PCM) is an endemic systemic mycosis in Latin America, caused by species of the Paracoccidioides brasiliensis complex. Involvement of the eyelid, genital, and osteoarticular systems in PCM is rare. This study describes the epidemiological, clinical, and microbiological characteristics of rare presentations of PCM involving the eyelids, genital system, and osteoarticular system in patients treated at a reference center in southeastern Brazil. A retrospective cross-sectional analysis was conducted among 33 patients diagnosed with uncommon forms of PCM, proven or probable, between 1973 and 2020. Demographic, clinical, and therapeutic data were retrieved from medical records. Between 1973 and 2020, 33 patients with rare forms of PCM were identified in Espírito Santo, Brazil, involving the eyelid (n = 11), genital (n = 10), and osteoarticular (n = 12) systems. Most patients were male and had rural exposure. Eyelid and genital involvement occurred in middle-aged men with chronic disease and frequent pulmonary association, with ulcerative lesions predominating. Osteoarticular cases affected younger patients with acute/subacute forms and systemic symptoms. Radiological findings mainly included osteolytic lesions. Histopathology showed high diagnostic yield, while serology was frequently positive. Treatment varied by severity, and relapses were associated with irregular therapy. These findings expand the clinical spectrum of PCM and highlight the importance of recognizing atypical localizations that may mimic neoplastic or other infectious diseases. Awareness of these rare manifestations, particularly in endemic areas, can improve diagnostic accuracy and patient outcomes.

PMID:期刊:日期:2025-06-25Since the emergence of the novel coronavirus SARS-CoV-2, public health measures have adapted as the virus evolve and acquired greater transmissibility and escape from the previous immune response. Genomic surveillance is a reliable and decisive tool for monitoring the evolutionary dynamics of the virus and its nucleotide diversity. Rio Grande do Sul is a southern Brazilian state that borders Argentina and Uruguay, and genomic and epidemiological surveillance led to early detection of COVID-19 variants, as seen in P.1 lineage. The study aimed to investigate the genetic characterization of SARS-CoV-2 Omicron sublineages in Rio Grande do Sul, Brazil, during 2023. By obtaining viral RNA from nasopharyngeal swabs positive for SARS-CoV-2, we performed high-throughput sequencing and data were analyzed using bioinformatic approaches. Our results revealed a dynamic change in Omicron sublineages during 2023, with the occurrence of JN.1+JN.1* reads during December 2023, parallel to the first JN.1 official record in Brazil, occurred in Ceará state, which is in the northeast region of Brazil. These data revealed a distinct nucleotide diversity in S gene of JN.1 reads, highlighting the importance of genomic surveillance in Rio Grande do Sul for the early detection of the entry of future SARS-CoV-2 variants into Brazil.

7. Sporotrichosis in the earlobe caused by placing an earing.
PMID:期刊:日期:2025-01-02A 22-year-old woman presented with an ulcer on her right earlobe 2 months ago, with inflammation and enlarged ipsilateral lymph nodes in her neck. She was treated with antibiotics without success and then was referred to an infectious disease specialist. She has a cat at home with sporotrichosis, but without direct contact with the lesion, she did not remember any scratching by the cat. She also mentioned wearing a semi-jewel earring. This is a rare and unusual case of sporotrichosis in the earlobe, probably caused by wearing an earring contaminated by the cat's fungus that was present in the home environment. The delay in diagnosis and treatment led to the worsening of the injury and loss of the earlobe.

8. 重症监护室危重患者念珠菌血症诊断和治疗的循证临床标准PMID:期刊:日期:2024-12-21Candidemia is the predominant form of invasive candidiasis and the most frequently occurring serious fungal infection in critically ill patients in Intensive Care Units (ICU). Studies carried out in Latin America reveal a higher incidence of candidemia and higher mortality rates when compared to North America or Europe. This highlights the need to develop guidelines for correctly diagnosing and treating candidemia in critically ill patients in the ICU. These guidelines are part of the efforts to implement antifungal optimization programs in the region to obtain better clinical outcomes and promote rational antifungal use. This evidence-based clinical standard, established through expert consensus for the Latin American context, contains recommendations and algorithms for diagnosing and treating candidemia in critically ill ICU patients.

9. A post-pandemic snapshot of the magnitude of COVID-19 in Brazil: A countrywide study.
PMID:期刊:日期:2024-12-21To outline the features of COVID-19 in Brazil through a countrywide telephone survey. Data from the Telephone Survey of Risk Factors for Chronic Noncommunicable Diseases During the Pandemic (Covitel), a telephone survey of individuals aged 18 years or older from all macro-regions of Brazil, were used. The questionnaire included sociodemographic characteristics and outcomes related to COVID-19 infection, severity, vaccination, and use of masks. Data revealed that 34.7 % (95 %CI 32.4 - 37.1) of the population had been diagnosed with COVID-19, and 10.1 % (95 %CI 7.9 - 12.7) of those required hospital admission. The prevalence of COVID-19 diagnosis increased with education level: <8 years (26.6 % [95 %CI 23.1 - 30.7]), 9-11 years (33.4 % [95 %CI 29.4 - 37.7]), and >11 years (53.2 % [95 % CI 49.7 - 56.8]). Nevertheless, the hospitalization rate of Brazilians with more than eleven years of education was lower (5.8 % [95 %CI 4.3 - 7.6]). In 2023, 92.9 % (95 %CI 90.9 - 94.4) of the Brazilian population was fully vaccinated against COVID-19, but only 37.2 % (95 %CI 33.5 - 40.9) have received the updated vaccinal scheme (two doses and two boosters). During the pandemic outbreak, 81.9 % (95 %CI 79.4 - 84.2) reported always using face masks. However, only 16.1 % (95 %CI 13.5 - 19.0) maintained this practice in 2023. There were inequalities in COVID-19 testing in Brazil. Testing and vaccination policies implemented in the COVID-19 pandemic must be reevaluated by the Brazilian government.

10. Activity of ceftolozane/tazobactam and comparators against gram-negative bacilli: Results from the Study for Monitoring Antimicrobial Resistance Trends (SMART - Brazil), 2018‒2021.
PMID:期刊:日期:2024-12-11Increased spread of antimicrobial resistance by Gram-Negative Bacilli (GNB) poses a global challenge, with exacerbated burden post-pandemic. The aim of this study was to investigate the in vitro activity of ceftolozane/tazobactam and its comparators against the frequently identified GNB isolated from patients admitted to Brazilian medical sites between the year 2018‒2019 and 2020‒2021. The impact of pandemic on antimicrobial resistance and presence of β-lactamase genes were also evaluated. Antimicrobial susceptibility testing and molecular characterization of ß-lactamase encoding genes using Polymerase Chain Reaction (PCR) and DNA sequencing were carried out from GNB isolated mostly from intra-abdominal, respiratory, and urinary tract infections and interpreted following BrCAST/EUCAST guidelines. A total of 3994 GNB isolates were evaluated which mostly included E. coli, K. pneumoniae and P. aeruginosa. Ceftolozane/tazobactam remained highly active against E. coli isolates during both 2018‒2019 (96.0 %) and 2020‒2021 (98.5 %). Among K. pneumoniae, ceftolozane/tazobactam (47.6 % and 43.0 % susceptible during 2018‒2019 and 2020‒2021, respectively) showed poor activity due to bla. Colistin and ceftolozane/tazobactam were the most active β-lactam agents tested against P. aeruginosa in 2018‒2019 (99.3 % and 88.8 %) and 2020‒2021 (100 % and 92.8 %), including ceftazidime and meropenem resistant isolates. β-lactamase encoding gene characterization was carried out and both carbapenemases and Extended-Spectrum β-Lactamase (ESBL) producers were found in E. coli, K. pneumoniae and P. aeruginosa isolates. Ceftolozane/tazobactam documented remarkable in vitro activity against E. coli and P. aeruginosa isolates in Brazil, both pre- and post-pandemic periods and could constitute an effective therapeutic option for the treatment of urinary tract infections, intra-abdominal infections, and respiratory tract infections.
