1. In-Hospital Guideline-Directed Medical Therapy Intensification and Long-Term Outcomes in Patients Hospitalized for Heart Failure in the Current Clinical Paradigm - Insights From the QHEART-R Study.
PMID:日期:2026-10-03The real-world impact of in-hospital intensification of guideline-directed medical therapy (GDMT) on clinical outcomes in patients with heart failure (HF) remains uncertain. We evaluated the association between in-hospital GDMT intensification and long-term outcomes using data from the multicenter QHEART-R cohort. We analyzed 3,190 patients hospitalized for worsening HF. In-hospital GDMT intensification was quantified by the change in the simple GDMT score from admission to discharge (∆S-GDMT). The primary outcome was a composite of all-cause death or hospitalization for HF (HHF). The mean (±SD) age of the cohort was 77.7±14.0 years, 53% were male, and the mean hospital stay was 20±16 days. The cohort included 1,477 (46%) patients with HF with reduced ejection fraction (HFrEF), 435 (14%) with HF with mildly reduced ejection fraction (HFmrEF), and 1,276 (40%) with HF with preserved ejection fraction. Four-year event-free survival was 32.4% for the primary outcome, 82.5% for cardiovascular (CV) death, and 50.4% for HHF. In patients with HFrEF/HFmrEF, a greater ∆S-GDMT was independently associated with a lower risk of the primary outcome (adjusted hazard ratio 0.90; 95% confidence interval 0.86-0.93; P<0.001), regardless of the number of prior HHF or the simple GDMT score at discharge. In-hospital GDMT intensification was associated with improved long-term outcomes in patients with HFrEF/HFmrEF. Hospitalization represents a critical opportunity to optimize GDMT and improve prognosis.

2. Risk Stratification for Pericardial Effusion After Leadless Pacemaker Implantation - Multicenter Study.
PMID:日期:2026-10-03Postoperative pericardial effusion (PPE) is a known complication associated with the use of leadless pacemakers (LPMs). The risk score (RS) is a useful tool for risk stratification. This study evaluated the utility of risk stratification using the RS in Japanese patients who received the Micra LPM. We retrospectively enrolled 443 patients who received Micra LPMs that were implanted at 4 institutions between August 2017 and August 2024. Patients were risk stratified using the RS into 3 groups: low risk (score ≤0), intermediate risk (score 1), and high risk (score ≥2). PPEs were observed in 9.0% (20/221), 11.7% (9/77) and 17.9% (26/145) of patients in the low-, intermediate-, and high-risk groups, respectively (P=0.041). In multivariable analysis, chronic obstructive pulmonary disease was a significant risk factor (P=0.014) for PPE, whereas prior cardiac surgery significantly reduced the risk of PPE (P=0.023). Older age, being female, and hemodialysis were not identified as risk factors. Subsequently, we modified the RS (mRS). Using the mRS, PPEs were observed in 8.8% (27/306), 16.5% (14/85), and 26.9% (14/52) of patients in the low-, intermediate-, and high-risk groups, respectively (P<0.001). The original RS is useful for PPE risk stratification after Micra LPM implantation. However, the mRS provides better risk discrimination and may therefore be a more suitable tool for use in the Japanese population.

5. Current Status and Barriers to Heart Failure Care in Japanese Geriatric Health Services Facilities - A Nationwide Cross-Sectional Survey.
PMID:日期:2026-09-18As Japan faces a heart failure (HF) pandemic, geriatric health service facilities (GHSFs) are vital for transitional HF care from hospital to home. This study investigated HF care practices, challenges, and support needs in Japanese GHSFs. We conducted a cross-sectional web-based survey of 171 GHSFs. We analyzed current management practices, perceived difficulties, and support needs regarding HF care in GHSFs. Concerns over high medication costs for HF were the primary reason for admission refusal (49.7%). Although daily symptom assessments, including dyspnea and edema were common, weight monitoring was conducted only monthly in 50.3% of facilities. HF care infrastructure was inadequate: 99.4% of GHSFs had no specialized staff (e.g., certified HF educators), only 38.0% had access to expert consultation, and only 56.7% provided HF-specific rehabilitation. Consequently, 44.4% of representatives reported significant perceived difficulty in HF care, primarily in judging consultation timing (75.0%). Facilities reporting low/neutral satisfaction with information desired advance care planning (ACP) and consultation criteria. HF patients had significantly lower home discharge rates than the overall facility residents (median [interquartile range] 20.0% [4.1-34.6%] vs. 34.0% [19.0-49.0%]; P<0.01). Concerns over financial barriers, along with structural and communication barriers in GHSFs, drive admission refusals and complicate HF management, potentially limiting home discharge. Improving transitional HF care requires addressing these financial concerns, providing standardized staff education and specialized support, and bridging information gaps, specifically regarding ACP and consultation criteria, between hospitals and facilities.

6. Impact of Right Ventricular Involvement on Bleeding and Mortality in Patients With Right Coronary Artery-Related Myocardial Infarction - Insights From Nationwide Registry.
PMID:日期:2026-09-17Right ventricular involvement (RVI) in acute myocardial infarction (AMI) patients is recognized as a high-risk subset. However, its impact on bleeding and mortality is not well defined in patients with right coronary artery (RCA)-related ST-segment elevation acute myocardial infarction (STEMI) undergoing percutaneous coronary intervention (PCI). Using data from the multicenter, prospective Japan AMI Registry including 3,411 patients, we analyzed 993 patients with RCA-related STEMI who underwent PCI. The incidence of all-cause mortality and major bleeding, defined as Bleeding Academic Research Consortium Type 3 or 5, was compared between patients with and without RVI. RVI was present in 9% (n=92) of patients. During a 12-month follow-up, RVI was independently associated with an increased risk of all-cause mortality (adjusted hazard ratio [HR] 2.05; 95% confidence interval [CI] 1.11-3.79) and major bleeding (adjusted HR 2.28; 95% CI 1.13-4.57) in Cox analysis, although this bleeding association was attenuated in adjusted competing-risk analysis (adjusted subdistribution HR 1.87; 95% CI 0.90-3.89). Among patients with RVI, those who experienced major bleeding had a significantly higher risk of death (HR 4.88, 95% CI 1.63-14.58). In patients with RCA-related STEMI treated with PCI, RVI was associated with increased mortality and appeared to be associated with a higher risk of bleeding.

7. Radial Wall Strain Derived From Coronary Angiography Reflects Plaque Morphology of Future Acute Coronary Syndrome.
PMID:日期:2026-09-16Radial wall strain (RWS) is a potential marker of plaque vulnerability; however, its association with plaque morphology in future acute coronary syndrome (ACS) remains unclear. We retrospectively enrolled 98 patients with ACS who underwent optical coherence tomography-guided percutaneous coronary intervention and had a prior coronary angiography (CAG) 1 month to 5 years before ACS. RWS was calculated as the maximum percentage change in lumen diameter within a cardiac cycle (RWS). Mild-to-intermediate stenotic lesions on the index CAG were classified as culprit lesions (CLs) and non-CLs (NCLs) of ACS. CLs were further categorized as plaque rupture (PR) or erosion (PE). We identified 74 CLs (50 PRs, 24 PEs) and 96 NCLs. Angiographic and physiological parameters were comparable across the groups; however, RWSwas significantly higher in both PRs and PEs than in NCLs (15.8% vs. 12.6%, P<0.001; 14.1% vs. 12.6%, P=0.012, respectively). In Cox regression analysis, RWSwas associated with subsequent PRs (hazard ratio [HR] 1.56; 95% confidence interval [CI] 1.34-1.82; P<0.001) and PEs (HR 1.33; 95% CI 1.05-1.67; P=0.017). Multinomial logistic regression showed that higher RWSfavored PR, lower RWSfavored NCL, and the estimated probability of PE remained low across the RWSspectrum. RWSwas independently associated with subsequent PR and PE, with a higher estimated probability of PR than of PE.

8. Determinants of Disproportionality in Functional Mitral Regurgitation - Importance of Mitral Valve Morphology.
PMID:日期:2026-09-16Disproportionate (d-) functional mitral regurgitation (FMR) is characterized by greater mitral regurgitation (MR) severity than expected from left ventricular (LV) dilatation. Morphological determinants of this imbalance remain unclear. Consecutive patients who underwent transcatheter edge-to-edge repair (TEER) for symptomatic FMR were divided into 2 groups: d-FMR and proportionate (p-) FMR (effective regurgitant orifice area [EROA]/LV end-diastolic volume [LVEDV] >0.20 or ≤0.20 mm/mL, respectively). Of the 114 FMR patients included, those with d-FMR (n=66) were more likely to be female and had a significantly larger EROA than patients with p-FMR (n=48), despite a smaller LVEDV index and lower tenting height. Metrics of LV dyssynchrony and etiologies were similar between the 2 groups, but the d-FMR group had a higher proportion of pseudo-prolapse (28.8% vs. 8.3%; P=0.007), extreme mitral annular dilatation (>700 mm/m; 53.0% vs. 29.2%; P=0.011), and deep indentation (24.2% vs. 4.2%; P=0.004). Although these factors were prevalent in the population, no patient with MR due purely to any of these factors underwent TEER. After adjustment by age, sex, and LVEDV index, the number of factors present was associated with d-FMR (odds ratio 4.15; P<0.001). Similar results were obtained when using an EROA/LVEDV cut-off of 0.14 mm/mL. Additional morphological factors were associated with greater MR severity beyond LV size and with residual MR after TEER, supporting a phenotype-based interpretation of FMR disproportionality.

9. Endomyocardial Biopsy and Clinical Outcomes in Acute Myocarditis - Multicenter Cohort Study.
9. 心内膜心肌活检与急性心肌炎的临床结局——多中心队列研究PMID:日期:2026-09-10Endomyocardial biopsy (EMB) remains the gold standard for diagnosing acute myocarditis (AM); however, its prognostic value has not been fully established. We investigated the prognostic impact of EMB, with a focus on fulminant myocarditis (FM). We included 841 AM patients across 7 hospitals who were categorized having undergone EMB (n=268) or not (n=573). The primary outcome was in-hospital death; the secondary outcome was a composite of all-cause death, cardiac transplantation, or left ventricular assist device implantation. In-hospital mortality was lower in the EMB group (6.3% vs. 11.5%; P=0.019), with a similar reduction in the EMB group among patients with FM (11.3% vs. 23.7%; P=0.002), despite a higher rate of extracorporeal membrane oxygenation. During a median follow-up of 447 days, the composite outcome was also lower in the EMB group among patients with FM (P=0.031). In multivariable analyses, EMB was independently associated with lower in-hospital mortality in both the overall cohort and the FM subgroup. Outcomes did not differ substantially across histopathologic subtypes, although patients with giant cell myocarditis had a worse prognosis. In patients with AM, EMB was associated with lower in-hospital mortality and improved long-term outcomes, particularly among those with FM. Further well-designed prospective studies are warranted to evaluate the clinical impact of EMB and its potential role in improving outcomes.
