3. A Silent Killer in an Unlikely Place: Necrotizing Fasciitis of the Breast.
3. 隐匿部位的沉默杀手:乳腺坏死性筋膜炎PMID:日期:2026-09-20Primary necrotizing fasciitis (NF) of the breast is a rare, life-threatening soft tissue infection that is frequently misdiagnosed due to its non-specific early presentation and uncommon anatomical location. We report a case of primary NF of the breast to highlight the diagnostic challenges posed by atypical risk factors and the critical importance of timely surgical intervention and antibiotic cover. A 66-year-old female presented with breast pain, erythema, and systemic symptoms in the setting of hidradenitis suppurativa (HS) being treated with adalimumab and long-standing tobacco use. Clinical assessment, laboratory investigations, and computed tomography (CT) imaging were performed, followed by surgical exploration. Initial concerns included abscess formation, cellulitis, or a HS flare. However, clinical instability and CT imaging demonstrated a gas-filled structure in the medial left breast with a developing gas-filled collection. Surgical exploration revealed extensive necrotising fasciitis involving the medial quadrant of the inner breast and underlying fascia, necessitating emergent surgical debridement and broad-spectrum antimicrobial therapy. The patient required further debridement and wound management, followed by delayed reconstruction. This case illustrates how underlying risk factors, particularly immunosuppression, may predispose patients to atypical presentations of NF and reinforces the importance of early surgical and antibiotic intervention when clinical suspicion is high.

4. Preventing Surgical Site Infections: Ethical and Operational Imperatives for the Global Health Agenda.
PMID:日期:2026-09-14Surgical site infections (SSIs) remain a major cause of morbidity, mortality, and inequity worldwide. Many SSIs are preventable and represent an important indicator of surgical quality and patient safety.Evidence and implementation experiences on SSI prevention were reviewed, focusing on infection prevention and control (IPC), antimicrobial stewardship (AMS), antimicrobial resistance (AMR), quality improvement, surveillance, safety culture, human factors, and emerging technologies.Multimodal, context-adapted IPC interventions can substantially reduce SSI incidence without major capital investment. Persistent gaps in infrastructure, guideline implementation, surveillance, and environmental controls remain important challenges. AMR further emphasizes the need to integrate AMS into surgical safety frameworks. Artificial intelligence and objective diagnostic technologies may improve surveillance and early SSI detection.SSI prevention is both a clinical necessity and an ethical imperative. Global surgery strategies should integrate IPC, AMS, surveillance, safety culture, and innovative technologies to advance equity, quality, and safety in surgical care worldwide.

7. {"_":"Two Rare Cases of Mesh Infection Due to Following Laparoscopic Inguinal Hernia Repair.","i":["Mycobacterium abscessus"]}
PMID:日期:2026-08-30Mesh infections after laparoscopic inguinal hernia repair are rare, particularly those caused by , a multi-drug-resistant nontuberculous mycobacterium. We describe two cases of infection following total extraperitoneal inguinal hernia repairs. Both patients developed delayed groin pain and fluid collections. Cultures confirmed , requiring mesh explanation and prolonged multi-drug antibiotic therapy. Despite complications, both achieved infection resolution after approximately 12 months. Investigation suggested contamination of laparoscopic instrument sheaths as the infection source, prompting changes in sterilization protocols. The pathogen's biofilm formation and resistance to sterilization contributed to persistence. Early recognition, mesh removal, and multi-disciplinary care are essential for successful management of mesh infections. Strict adherence to sterilization protocols is critical in preventing such rare but serious complications.

8. Extent-Guided Staged Management of Localized Late-Onset MSSA Sternal Infection after Bilateral Lung Transplantation: A Case Report.
PMID:日期:2026-08-30Sternal infection after lung transplantation is uncommon but life-threatening; late-onset cases may be difficult to diagnose when signs are subtle. A 38-year-old female developed methicillin-susceptible sternal infection with bacteremia 7 months after bilateral lung transplantation. Computed tomography (CT) showed a localized abscess around the sternal wires. CT and intraoperative findings confirmed that infection was confined to extrathoracic peri-sternal tissues without communication with the thoracic cavity. This guided debridement, drainage, wire removal, antimicrobial therapy, and negative-pressure wound therapy (NPWT) with instillation and dwell time followed by standard NPWT. After infection control and adequate granulation, delayed primary closure was performed 60 days after debridement without flap reconstruction. She was discharged on day 77, without recurrence, 8 months after closure. Careful assessment of infection extent may help identify patients with localized infection suitable for staged management, permitting delayed primary closure without flap reconstruction.
