2. mTOR/Autophagy//NF-κB Axis Drives Benzalkonium Chloride-Induced Inflammatory Response in Corneal Epithelial Cells.
PMID:期刊:日期:2026-09-17Benzalkonium chloride (BAK), a quaternary ammonium antimicrobial agent widely utilized in ophthalmic formulations, is associated with chronic ocular surface toxicity. BAK induces corneal epithelial inflammation and autophagic impairment, yet their interrelationship remains unclear. Mouse corneal epithelial cell lines (MCECs) were subjected to short-term BAK exposure (10-20 μg/mL, 15 min + 8 h recovery). TLR4/MyD88/NF-κB pathway activation and proinflammatory cytokines (Il-1α, Il-1β, Il-6) were assessed. Autophagic flux was evaluated via LC3-II/I ratio, p62 accumulation, GFP-RFP-LC3 autolysosome formation assay, and lysosomal function. Pharmacological interventions with rapamycin (mTOR blockade) and TAK-242 (TLR4 inhibition) were used to dissect pathway interactions. Short-term BAK exposure dose-dependently activated the TLR4/MyD88/NF-κB pathway and elevated proinflammatory cytokines. Concurrently, BAK inhibited autophagic flux through mTOR hyperactivation, evidenced by reduced LC3-II/I ratio, p62 accumulation, impaired autolysosome formation, and lysosomal dysfunction. Both rapamycin and TAK-242 significantly attenuated BAK-induced MyD88/NF-κB activation and inflammation. Critically, BAK activates parallel TLR4-MyD88 and mTOR pathways that synergistically amplify NF-κB signaling. These results unveil mTOR as a therapeutic target for mitigating BAK-induced corneal damage, thereby proposing mTOR suppression or autophagic activation as viable interventions for BAK-associated ocular surface disorders. Particularly at lower concentration (0.001%) and shorter exposure (15 min) of BAK.

3. {"_":"Real-World Dosing Intervals for Long-Acting Anti-VEGF Intravitreal Injection Therapies in nAMD and DME: Academy IRIS Registry Analysis.","sup":["®"]}
PMID:期刊:日期:2026-09-10To determine real-world dosing intervals for long-acting intravitreal anti-vascular endothelial growth factor (anti-VEGF) therapies in patients with neovascular age-related macular degeneration (nAMD) and diabetic macular edema (DME). Retrospective observational cohort study in American Academy of Ophthalmology IRIS Registry (Intelligent Research in Sight) patients with nAMD or DME treated with aflibercept 8 mg (aflibercept) or faricimab. Mean injection intervals during a ≤24-month period beginning 3 or 4 months after the initial injection were evaluated and descriptively contrasted with those in phase 3 clinical trials. The mean dosing interval was 5-8, 9-12, and ≥13 weeks in 27.5%, 45.6%, and 25.8% of aflibercept-treated patients with nAMD ( = 40,065); 27.0%, 43.4%, and 28.3% of aflibercept-treated patients with DME ( = 13,045); 40.5%, 36.1%, and 20.8% of faricimab-treated patients with nAMD ( = 132,697); and 42.2%, 31.7%, and 22.5% of faricimab-treated patients with DME ( = 39,849). Dosing intervals were consistent between treatment-naïve and previously treated patients. In aflibercept phase 3 studies, 77% (nAMD) and 89% (DME) of patients achieved a 16-week dosing interval. In faricimab phase 3 studies, 45% (nAMD) and 52% (DME) of patients achieved a 16-week dosing interval. Extension of injection intervals to 16 weeks is greatly reduced in real-world experience compared with phase 3 clinical studies. This discrepancy could be explained by protocol-mandated restrictive rescue criteria used in the registration studies, allowing injection only after vision loss and significant fluid recurrence. Current longer-acting anti-VEGF medications offer an extension to only 5-12 weeks for most patients in the real world.

4. Management of Neuropathic Ocular Pain Associated with Dry Eye Disease in a 5-Year Prospective Cohort.
PMID:期刊:日期:2026-09-03This study aimed to assess the long-term management and 5-year evolution of patients with neuropathic ocular pain (NOP) associated with chronic dry eye disease (DED). Fourteen patients with NOP underwent a 5-year follow-up. Baseline and 5-year assessments included questionnaires on dry eye (Dry Eye Questionnaire 5 [DEQ-5] and Ocular Surface Disease Index), pain (Brief Pain Inventory [BPI]), anxiety/depression disorders (Hospital Anxiety and Depression Scale), and a dedicated questionnaire on NOP characteristics (Neuropathic Ocular Pain Questionnaire), which was developed for this study. Treatment tolerance and effectiveness were rated on a 0-5 scale. Clinical examination, meibography, tear film analysis, corneal nerve density, and inflammation assessment using confocal microscopy (IVCM) were performed at both time points. Patients were predominantly women (93%) with a mean age of 41 ± 17 years. High rates of comorbid anxiety/depressive disorders (85.7%) and diffuse chronic pain (35.7%) were observed, supporting the need for multimodal care. Over 5 years, significant improvements occurred in mean neuropathic pain scores ( = 0.005), BPI scores ( = 0.003), and DEQ-5 scores ( = 0.041). Clinical parameters remained unchanged, except for a significant decrease in IVCM inflammation score ( = 0.004). All patients received topical ophthalmic treatments; 78.6% were treated with systemic neuropathic pain relievers, and 64.3% received psychological care. The most effective treatments included artificial tears, cyclosporine eye drops, scleral lenses, oral therapies, and autologous serum eye drops. This study provides the first long-term overview of therapeutic strategies for NOP associated with DED. Findings support a multidisciplinary approach using specific questionnaires and personalized, long-term treatments. However, controlled prospective studies are needed to establish the efficacy of specific therapeutic strategies.

5. {"_":"Cytoprotective Potential of -Acetylcysteine in an Model of Retinal Oxidative Injury.","i":["N","In Vitro"]}
PMID:期刊:日期:2026-08-21Sodium iodate (NaIO) is a well-established oxidizing agent that induces oxidative stress (OS) in ARPE-19 cells, while antioxidants such as -acetylcysteine (NAC) have demonstrated cytoprotective effects in retinal degeneration models. This study establishes an NaIO-induced OS disease model to investigate pathways involved in RPE damage and restoration, providing a platform for preclinical screening of antioxidant molecules. The cytoprotective effect of NAC on NaIO-induced OS in RPE cells was evaluated using CMFDA cell viability assay (2',7'-dichlorofluorescin diacetate), JC-1 staining, colony-forming unit assay, scratch assay, PI/Annexin V staining, and gene expression analysis related to OS, inflammation, the cell cycle, and autophagy. NAC treatment significantly decreased ROS levels and stabilized mitochondrial potential, increased cell viability and colony-forming ability, cell adhesion and migration potential. Furthermore, NAC prevented NaIO-induced cell death and modulated the expression of genes associated with OS, inflammation, cell cycle, and autophagy. The results showed successful establishment of a reproducible OS-based retinal degeneration model in ARPE-19 cells. Additionally, NAC exhibited significant cytoprotective activity against OS in RPE cells, highlighting its therapeutic potential as an antioxidant agent against retinal degeneration by reducing ROS levels, restoring mitochondrial membrane potential, and attenuating cell cycle dysregulation and inflammation.

6. Drug Delivery Research Related to Posterior Segment Eye Diseases: A Bibliometric Analysis of Global Trends and Therapeutic Hotspots.
PMID:期刊:日期:2026-08-19Drug delivery to the posterior segment remains constrained by ocular barriers, short intraocular residence, repeated administration, and safety concerns. We mapped the global research structure of drug delivery research related to posterior segment eye diseases and identified major contributors, knowledge foundations, and therapeutic hotspots. English-language articles and reviews published from 1999 to 2024 were retrieved from the Web of Science Core Collection using a predefined topic search combining drug-delivery and posterior segment terms. Bibliometrix, VOSviewer, and CiteSpace were used to analyze publication growth, contributors, journals, highly cited articles, cocited references, keyword co-occurrence, citation bursts, thematic evolution, and conceptual structure. The dataset contained 4,055 publications and showed sustained growth, with a higher output phase after 2020. The United States and China were the leading national contributors, while the University of Helsinki, University of Eastern Finland, Johns Hopkins University, and Emory University were prominent institutional contributors. Keyword, burst, and thematic analyses indicated a shift from permeability, pharmacokinetics, and sustained-release platforms toward macular degeneration, anti-VEGF-related therapy, injection burden, nanoparticles, long-acting systems, and clinically oriented formulation strategies. Research has shifted from barrier characterization, pharmacokinetics, and sustained-release platforms toward macular degeneration, anti-VEGF-related therapy, injection burden, long-acting systems, and clinically oriented formulation strategies. These bibliometric signals indicate research activity rather than clinical effectiveness.

7. Molecular Mechanisms of Hypoxia-Driven Scleral Remodeling in Myopia Progression.
PMID:期刊:日期:2026-08-18Myopia, driven by pathological axial elongation, is a leading cause of visual impairment. This review synthesizes and critically evaluates the evidence for the "scleral hypoxia theory," focusing on the role of hypoxia-inducible factor-1α (HIF-1α) signaling in scleral remodeling during myopia progression. A comprehensive literature search was conducted on PubMed to 2026, focusing on scleral hypoxia, HIF-1α signaling, extracellular matrix (ECM) remodeling, and their interplay with inflammatory and metabolic pathways. Evidence was categorized into 3 tiers: direct scleral/myopia evidence, indirect ocular evidence, and extrapolative evidence from other tissue systems. Converging evidence from human genetic studies, animal models, and cell-based experiments supports the scleral hypoxia theory. Reduced choroidal blood perfusion creates a hypoxic microenvironment in the sclera, stabilizing HIF-1α and activating downstream matrix metalloproteinases (MMPs), which promotes ECM degradation, scleral thinning, and biomechanical weakening, facilitating axial elongation. The HIF-1α pathway exhibits extensive crosstalk with transforming growth factor-β, AMP-activated protein kinase (AMPK)/Sirtuin 1 (SIRT1)/Peroxisome proliferator-activated receptor gamma coactivator-1 alpha (PGC-1α) metabolic signaling, oxidative stress, and inflammatory pathways. Emerging evidence suggests potential roles for macrophage polarization (M1/M2) in scleral remodeling. Notably, the majority of direct mechanistic evidence is derived from a limited number of landmark studies, and independent replication across diverse models is needed. The hypoxia-HIF-1α-MMP signaling axis is a promising candidate mechanism in myopic scleral remodeling, with translational implications for both pharmacological and nonpharmacological strategies. However, key gaps remain, including the need for independent replication, longitudinal human evidence, and direct experimental validation in scleral fibroblasts. Future multiomics and targeted interventional studies are required to establish causality and translate these findings into clinical applications.

8. Association of Meibomian Gland Orifice Changes with Intense Pulsed Light Treatment Response in Meibomian Gland Dysfunction.
PMID:期刊:日期:2026-08-10To investigate epidemiological factors and clinical manifestations associated with longitudinal treatment response to intense pulsed light in patients with meibomian gland dysfunction (MGD). This study was conducted on 392 eyes (392 patients) undergoing 4 sessions of IPL. Treatment response was evaluated using measurements of meibomian gland expressibility (MGE) and meibum quality (MQ). A generalized estimating equation (GEE) was applied to evaluate associations between epidemiological factors, clinical parameters, and longitudinal changes in MGE and MQ. In adjusted GEE models, lower MGE and MQ grades indicated better meibomian gland function. Therefore, negative coefficients reflected improvement, whereas positive coefficients reflected a less favorable response. For MGE, worse baseline grade was associated with greater improvement after 4 sessions ( -0.74, < 0.001), although final MGE grades remained higher among eyes with higher baseline MGE grades. Telangiectasia across the meibomian gland orifice and keratinization were associated with less favorable MGE response ( = 0.52 and 0.30; < 0.001 and 0.040, respectively). For MQ, worse baseline grade was associated with greater improvement ( = -0.85, < 0.001), whereas telangiectasia across the orifice was associated with less favorable response ( = 1.45, < 0.001). After 4 sessions of IPL, worse baseline MGE and MQ grades were associated with greater longitudinal improvement. However, residual dysfunction remained, particularly for MGE among eyes with more severe baseline disease. Meibomian gland orifice changes, particularly telangiectasia across the orifice and keratinization, were associated with less favorable longitudinal response and may help inform prognostication and individualized treatment planning.

9. Smart Responsive Hydrogels for Dry Eye Disease: Microenvironment-Sensing Drug Delivery and Therapeutic Translation.
9. 干眼病的智能响应水凝胶:微环境传感药物输送和治疗翻译PMID:期刊:日期:2026-08-06To review smart responsive hydrogels for dry eye disease (DED), focusing on pharmacological performance, disease relevance, safety, and translational feasibility. A narrative literature search was conducted in PubMed, Web of Science, Scopus, and Google Scholar up to June 11, 2026, using terms for DED, hydrogels, and ocular drug delivery. Studies were selected based on relevance to DED pathophysiology, hydrogel responsiveness, and translational safety. Thermosensitive and ion-sensitive hydrogels are the most practical near-term options because they enable drop-like administration and rely on simple physiological triggers. ROS- and enzyme-responsive systems offer stronger mechanistic links to DED pathology but face challenges in trigger variability, safety, and manufacturing complexity. Photoresponsive platforms allow spatiotemporal control but require device-dependent activation. Multi-stimuli systems integrate retention and disease-triggered release, though their added complexity must be justified by measurable therapeutic benefit. Most DED-specific responsive hydrogels remain at the preclinical stage; human evidence mainly concerns familiar device formats, not the responsive formulations themselves. Smart responsive hydrogels can tailor drug delivery to ocular surface conditions, but evidence maturity varies across platforms. Thermosensitive and ion-sensitive gels are the most clinically plausible near-term candidates. ROS-responsive, enzyme-responsive, and multi-stimuli systems better engage DED biology but need stronger validation. Translation depends on direct comparator studies, phenotype-matched endpoints, long-term ocular safety, scalable sterile manufacturing, and appropriate regulatory strategies.

10. Ocular Tissue Distribution of Ocular Surface Immunoglobulin Following Topical and Subconjunctival Administration: A Novel Strategy for Mitigation of Antibody-Drug Conjugate-Associated Ocular Toxicity.
PMID:期刊:日期:2026-08-06Antibody-drug conjugate (ADC)-associated ocular toxicity is an emerging, sometimes dose-limiting complication. Macropinocytosis has been implicated in off-target ADC uptake by corneal epithelial cells. This study evaluated whether ocular surface immunoglobulin (OSIG) inhibit macropinocytosis in human corneal epithelial cells (HCE-T) and characterized ocular biodistribution and tolerability following topical and subconjunctival administration. Macropinocytosis was quantified in corneal epithelial cells after exposure to vehicle or increasing concentrations of OSIG. Ocular pharmacokinetics were evaluated in New Zealand White rabbits following single topical instillation or subconjunctival injection of 5% OSIG. OSIG concentrations were quantified in ocular tissues, ocular fluids, tears, serum, and submandibular lymph nodes using enzyme-linked immunosorbent assay (ELISA). Local tolerability was assessed after single and repeated subconjunctival injections using clinical observations, Draize scoring, and ophthalmic examinations. OSIG inhibited Dextran Texas Red uptake in a concentration-dependent manner, with near-complete inhibition at 5%. Topical OSIG produced only transient conjunctival exposure and did not achieve sustained corneal concentrations. In contrast, subconjunctival administration resulted in sustained ocular surface exposure, with peak corneal concentration of 11,200.8 ng/g on Day 14 and measurable conjunctival exposure through 28 days. Serum exposure was detectable but lower than ocular surface tissue exposure. Subconjunctival OSIG was generally well tolerated after single and repeated dosing, with findings limited mainly to transient conjunctival redness and chemosis. Two clinical cases of ADC-associated ocular toxicity treated with topical OSIG were also reviewed. OSIG inhibits macropinocytosis in HCE-T and achieves sustained corneal and conjunctival exposure following subconjunctival administration. These findings support further evaluation of subconjunctival OSIG as a local strategy to mitigate ADC-associated corneal toxicity.
