1. A Trans-Atlantic, Expert-Based, Delphi Consensus Document on controversial issues in the management of patients with chronic limb-threatening ischemia.
PMID:日期:2026-08Two recent multicenter randomized controlled trials (RCTs) compared best endovascular treatment versus surgical revascularization for patients with chronic limb-threatening ischemia (CLTI) and reached opposite conclusions. The aim of this international, expert-based Delphi Consensus document was to address these conflicting results and to explore their interpretation and applicability in the real-world clinical practice. A three-round Delphi Consensus document was completed by 51 European and 17 USA-based experts addressing five pre-specified CLTI clinical scenarios and two questions regarding the applicability of the two RCTs. All answers were provided anonymously. The response rate for each round was 100%. Consensus was reached when ≥70% of the participants agreed on a response showing preference for a specific approach. Most participants (43/68; 63.3%) preferred a venous bypass for a patient with segmental occlusions of the lower limb arteries and an available distal target vessel, although this did not reach consensus. In contrast, consensus was achieved (49/68; 72.2%) for a diabetic patient with occlusion of all three infrapopliteal vessels for whom most experts favored angioplasty of any possible tibial artery. In case of previous endovascular recanalization failure, options were divided between repeat angioplasty (27 of 68; 39.7%) and venous bypass (35 of 68; 51.5%). The majority of participants reported that their approach had not changed following publication of the two RCTs (50 of 68; 73.5%) and did not believe that the trials had changed significantly the management of patients with CLTI in their clinical practice (53 of 68; 77.9%). In a subgroup analysis comparing European vs. USA-based physicians, there were minor differences in specific topics. In the present Expert-based Consensus document, considerable heterogeneity was found between USA-based and European participants on several topics related to CLTI and consensus was not always achieved. Endovascular and open surgical techniques should be regarded as complementary strategies, rather than as competing alternatives. Treatment should be individualized based on individual patient anatomy, extent of ischemia, presence of tissue loss, foot infection and conduit availability.

2. Comparative cost analysis of saphenectomy and radiofrequency ablation in the treatment of great saphenous vein incompetence.
PMID:日期:2026-08The aim of this study was to assess the cost differences between two techniques for treating great saphenous vein incompetence: conventional surgerysaphenectomy and radiofrequency ablation. We conducted a retrospective observational cohort study, selecting 40 patients for the radiofrequency cohort and 80 for the saphenectomy cohort. Sociodemographic, baseline, clinical, and ultrasound follow-up data were collected at 10 days and one-month post-intervention. Finally, a cost analysis was performed from the perspective of both the public health system and society, with indirect costs related to sick leave evaluated in the latter perspective. At the 10-day follow-up, hematoma incidence was significantly higher in the saphenectomy group (22 cases, 27.5% vs. 3 cases, 7.5%). The average Visual Analogue Scale pain score was higher in the radiofrequency group (2.1±1.8 vs. 1.1±1.7); however, mean analgesic consumption was higher in the saphenectomy group. From a cost perspective, the expense for the public health system was higher for radiofrequency (3167.2€ ± 123.4 vs. 3066.6€ ± 205.8). However, both sick leave costs (2276.6€ ± 1209.4 vs. 1105.8€ ± 1265.8) and total cost (5357.1€ ± 1246.1 vs. 4286.3€ ± 1294.8) were higher in the saphenectomy group. Radiofrequency ablation is a safe technique, allowing for an earlier return to normal work activities, with a low rate of complications and short- and medium-term efficacy at least equivalent to open surgery. The inclusion of radiofrequency ablation in our health system may yield overall cost savings compared to saphenectomy in the surgical treatment of great saphenous vein incompetence.

3. Impact of previous skin hydration on postsclerotherapy hyperpigmentation: a prospective, randomized, comparative study (SKINCARE Study).
PMID:日期:2026-08Postsclerotherapy hyperpigmentation (PSH) is a frequent and distressing complication. It is a cosmetic concern in aesthetic phlebology, with limited prospective efficacy/safety data. Skin hydration with moisturizer strengthens the skin barrier and may potentially prevent postsclerotherapy hyperpigmentation. This prospective, randomized, comparative study included 55 adult patients with reticular veins and telangiectasias. Patients were randomized to receive a daily moisturizer for 30 days before sclerotherapy or to no hydration. All participants underwent a single session of polidocanol 0.5% sclerotherapy. The primary efficacy outcome was moisturization's ability to prevent PSH, as evaluated by blinded evaluators using photos taken before and after procedures. Secondary outcomes included patient-reported outcomes assessed using the VEINES-QoL/Sym questionnaire. Both groups experienced hyperpigmentation. PSH appeared in fewer patients who received hydration (44% vs. 57%, P=0.423), with no statistically significant difference. No skin alterations occurred at any time point. Nine patients (90%) rated the results as "very good" or "excellent". In this randomized, double-arm, prospective, pilot study, the use of moisturizer 30 days before polidocanol 0.5% sclerotherapy did not reduce PSH.

4. Lower intraluminal thrombus load in patients with abdominal aortic aneurysms and blood type O.
PMID:日期:2026-07-07Abdominal aortic aneurysm (AAA) is a major cause of mortality among older men. Current clinical practice primarily determines the indication for elective AAA repair based on the maximum aneurysm diameter. However, this approach may not adequately capture the complexity of individual rupture risk, and intraluminal thrombus (ILT) has been associated with increased growth and rupture risk. In other vascular beds, non-O blood types are correlated with an increased risk of thrombosis. This study investigates the association between ABO blood type and ILT volume in AAA patients. A cross-sectional analysis of patients with infrarenal AAAs from the Copenhagen Aortic Cohort (COACH) assessed AAA diameter, AAA volume, and ILT volume using three-dimensional ultrasound and 3D-CEUS, respectively. Patients were categorized into blood type O and non-O groups for analysis. ILT volume was compared between the groups. In total, 296 patients under surveillance for AAA with a median AP diameter of 43 [IQR 38-48] mm, and blood type O (N.=101) and non-O (N.=195) were included. Patients with blood type O had a 4.6% lower ILT volume per 10 mL AAA volume than patients with other blood-types (P=0.003), after adjusting for AAA volume and other known covariates. Blood type O is associated with a lower thrombus load in patients with AAA. This finding aligns with prior evidence linking non-O blood types to elevated vWf and Factor VIII levels, both important in generating thromboses. Future longitudinal studies are needed to explore the relationship between blood type and AAA progression.

5. Dose-dependent effects of rosuvastatin in deep vein thrombosis: a retrospective cohort study.
PMID:日期:2026-06-15Deep vein thrombosis (DVT) remains associated with substantial morbidity despite anticoagulation therapy, with 20-30% experiencing recurrence and up to 50% developing post-thrombotic syndrome (PTS). Statins possess anti-inflammatory properties that may benefit DVT outcomes, but evidence for adjunctive use remains limited. We conducted a retrospective cohort study of 255 patients with first-episode proximal DVT diagnosed between January 1, 2020 and December 31, 2020, with follow-up through December 31, 2024. Using multinomial propensity scores, we matched patients 1:1:1 who were receiving rosuvastatin 20 mg (N.=85), rosuvastatin 10 mg (N.=85), or no statin (N.=85) for ≥3 months before DVT diagnosis. All patients received guideline-directed anticoagulation. Primary outcome was composite of DVT recurrence and/or PTS. We used Cox regression for time-to-event analyses and mixed-effects models for biomarker trajectories. After median follow-up of 42 months (IQR 36-48), the primary outcome occurred in 31.8% of rosuvastatin 20-mg group, 38.8% of 10-mg group, and 47.1% of controls (HR=0.67, 95% CI 0.50-0.90, P=0.008 for 20 mg vs. control). Both rosuvastatin groups showed greater reductions in inflammatory markers compared to controls. Complete vein recanalization at 48 months was observed in 74.1%, 65.9%, and 58.8% respectively (overall P=0.045; pairwise 20 mg vs. control P=0.095, Table II). Safety profiles were similar across groups, with no excess bleeding or muscle-related events. In this hypothesis-generating study, rosuvastatin use was associated with reduced DVT complications, with suggestions of dose-dependent effects. These associations require confirmation in prospective randomized trials before clinical recommendations can be made.

7. Automated three-dimensional ultrasound surveillance after endovascular aortic repair.
PMID:日期:2026-06Accurate measurement of the residual sac after endovascular aortic repair (EVAR) is essential for effective surveillance, guiding detection of complications and decisions regarding reintervention. While two-dimensional ultrasound (2D-US) remains standard-of-care, it is limited by operator variability. Three-dimensional ultrasound (3D-US) with modeling software has demonstrated improved precision but remains resource-intensive. The introduction of three-dimensional Software-Assisted Ultrasound (3D-SAUS) offers an automated, on-cart alternative for more streamlined clinical use. This study compares 3D-SAUS with 2D-US in measuring maximum residual sac diameter in EVAR patients, focusing on variability and clinical applicability. Patients previously treated with EVAR and following US-based follow-up were scanned by three or four experienced ultrasound operators. The maximum anterior-posterior residual sac diameter was measured for all patients using 2D-US and 3D-SAUS. Outcomes were median and limits of agreement (LoA) with 95% confidence intervals for both modalities. Fourteen EVAR-treated patients underwent 44 paired 2D-US and 3D-SAUS scans. Median maximum residual sac diameters were 50.0 mm with 2D-US and 50.3 mm with 3D-SAUS. LoA were 3.4 mm for 2D-US and 3.6 mm for 3D-SAUS, with overlapping 95% confidence intervals (2.2-4.6 mm and 2.5-4.7 mm, respectively). The difference in LoA was not statistically significant (P=0.61). Automatic technical success was achieved in 79% of the available 3D scans. 3D-SAUS demonstrated comparable residual sac diameter measurement precision and variability to expert 2D-US in post-EVAR patients. These findings support the potential of 3D-SAUS to enhance reproducibility and standardize ultrasound-based follow-up.

8. Effects of information platform-based nursing management on patients with venous thromboembolism: a systematic review and meta-analysis.
8. 基于信息平台的护理管理对静脉血栓栓塞症患者的影响:系统评价与Meta分析PMID:日期:2026-05-27Venous thromboembolism (VTE) is a common yet largely preventable complication among hospitalized patients. In recent years, information platform-based nursing interventions have been increasingly implemented to enhance VTE risk assessment and the execution of preventive strategies. This study aimed to systematically evaluate the effectiveness of information platform-based nursing management in VTE prevention and control. A systematic review and meta-analysis were conducted by searching PubMed, Embase, Web of Science, CNKI, and Wanfang databases from inception to the present. Two reviewers independently performed study selection and data extraction using a predefined form to collect study characteristics, intervention details, and outcome measures. The risk of bias of randomized controlled trials (RCTs) was assessed using the Cochrane Risk of Bias 2.0 tool (Cochrane RoB 2.0), while observational studies were evaluated using the Newcastle-Ottawa Scale (NOS). Statistical analyses were performed using Review Manager version 5.4. Effect sizes were expressed as risk ratios (RRs) with 95% confidence intervals (CIs). Heterogeneity was assessed using the I statistic. Subgroup analyses were conducted to explore potential sources of heterogeneity, sensitivity analyses were performed to test the robustness of the results, and publication bias was evaluated using funnel plots. A total of 19 studies were included, encompassing 108,482 patients and 30,750 observation records. Quality assessment indicated that the included RCTs generally had a low risk of bias, and the observational studies were of moderate to high methodological quality. Meta-analysis showed that information platform-based nursing management was associated with a significant lower incidence of VTE (RR=0.67, 95% CI: 0.60-0.74, P<0.001), a marked higher implementation rate of preventive measures (RR=1.38, 95% CI: 1.26-1.51, P<0.00001), and a significantly higher level of patient satisfaction (RR=1.16, 95% CI: 1.07-1.27, P<0.001). Subgroup analyses suggested low heterogeneity and consistent effects across different study regions. No substantial publication bias was detected. Information platform-based nursing management is significantly associated with better performance in key indicators of VTE prevention and control and demonstrates important clinical value. Further high-quality, multicenter studies are warranted to confirm its long-term effectiveness and feasibility for broader implementation.

9. Outcomes of endovascular revascularization of upper limb thromboangiitis obliterans via palmar arch angioplasty.
PMID:日期:2026-05-12Thromboangiitis obliterans (TAO), or Buerger's disease, is a rare, nonatherosclerotic segmental vasculitis affecting small- and medium-sized arteries, most commonly in the extremities. Upper extremity involvement presents unique therapeutic challenges, particularly in preserving hand function and preventing amputation. We retrospectively reviewed nine male patients with angiographically confirmed upper extremity TAO. Demographics, angiographic findings, procedural details, and clinical outcomes - including revascularization success and amputation rates - were analyzed. Palmar arch angioplasty was attempted in patients with suitable anatomy and critical ischemia. The mean age was 50.3±4.4 years. All patients were active smokers (mean 28.2±4.1 pack-years). Six patients (66.7%) presented with digital gangrene, while three (33.3%) had ischemic ulceration. Radial artery involvement was present in all patients, and ulnar artery involvement in 77.8%. Palmar arch angioplasty was attempted in six patients and technically successful in four (66.7%). Minor amputations occurred in eight patients (88.9%), while no major amputations were required. Patients with successful revascularization experienced improved wound healing and limb salvage. Endovascular revascularization, including palmar arch angioplasty, may represent a feasible limb-salvage approach in selected patients with upper extremity TAO; however, clinical outcomes remain variable, minor amputations are still frequently observed, and the technique should be interpreted as a supportive measure rather than a definitive solution.

10. International Union of Angiology (IUA) Consensus Document on Enhanced Recovery after Surgery (ERAS) in Vascular Surgery.
PMID:日期:2026-05-12Enhanced recovery after surgery (ERAS) is a multimodal perioperative care pathway designed to improve recovery, optimize patient outcomes, and enhance overall quality and satisfaction. Successful ERAS implementation requires continuous interprofessional collaboration among anesthesiologists, surgeons, nurses, nutritionists, physiatrists, and gerontologists, extending from preadmission to discharge. This consensus document, developed by the International Union of Angiology (IUA) Youth Committee and senior experts, outlines best practices for ERAS in vascular surgery. It synthesizes current evidence and expert opinions to provide a structured clinical pathway that enhances patient care and facilitates real-world application. This document has outlined key strategies to enhance perioperative outcomes in vascular surgery patients, a population at high risk for periprocedural complications, particularly cardiac events. The authors attempted to clarify and standardize key definitions and provide general guidance on broadly applying ERAS principles based on existing literature and clinical experience. While ERAS protocols for vascular surgery have been recently published by the Society for Vascular Surgery (SVS), this document offers a complementary perspective, addressing key perioperative steps and providing specific recommendations for both open and endovascular arterial procedures.
