1. Key revisions in the 2023 guidelines for the diagnosis and treatment of acute encephalopathy in childhood.
PMID:日期:2026-09-23New literature has accumulated since the publication of the guidelines for the diagnosis and treatment of acute encephalopathy in childhood 2016 (GL2016), necessitating the revision of these guidelines. In 2020, the working group (WG) of the Japanese Society of Child Neurology began work on publishing a revised version of the GL2016. The WG formulated guidelines using the methodologies recommended by the Medical Information Network Distribution System (Minds). GL2023, published in 2023, includes clinical question 1 (CQ1), based on Minds 2020, and general and specific sections on acute encephalopathy (an update to GL2016) based on Minds 2007. In CQ1, early target temperature management at 36 °C within 24 h was weakly recommended for pediatric cases meeting certain criteria to reduce the risk of progression to acute encephalopathy with biphasic seizures and late reduced diffusion and its sequelae. GL2023 provides updated recommendations that may improve the diagnosis and management of pediatric acute encephalopathy.

2. Third-line tocilizumab for refractory pediatric anti-NMDA receptor encephalitis: a single-center case series.
PMID:日期:2026-09-21Tocilizumab has been proposed as a third-line therapy for refractory autoimmune encephalitis; pediatric Latin American data remain limited. To evaluate the efficacy and safety of tocilizumab in pediatric patients with severe or refractory anti-N-methyl-d-aspartate receptor (NMDAR) encephalitis. We retrospectively reviewed pediatric patients with definite anti-NMDAR encephalitis treated with tocilizumab after inadequate response to previous therapy. Six patients (four males, median age at onset 4.5 years). All had severe disease (modified Rankin Scale [mRS] 4-5; Clinical Assessment Scale in Autoimmune Encephalitis [CASE] ≥10). All received first-line therapy, five received rituximab before tocilizumab. Tocilizumab was initiated 2-3 months after the last rituximab infusion. At 12 months, all achieved favorable outcomes (mRS 2; CASE ≤4). Adverse events included varicella-zoster virus infection, mild cytopenia, and dyslipidemia. Tocilizumab was associated with favorable outcomes and acceptable safety in this small pediatric series of refractory anti-NMDAR encephalitis. The CASE scale was a sensitive and reliable instrument for longitudinal functional assessment.

3. Effect of Doctor Car dispatch on neurological outcomes in children with status epilepticus: A single-Center retrospective cohort study.
PMID:日期:2026-09-13Status epilepticus (SE) requires prompt treatment because prolonged seizures increase the risk of neurological injury. We investigated whether a physician-staffed ambulance (commonly referred to as "Doctor Car" in Japan) improves neurological outcomes in pediatric SE. We conducted a medical record-based retrospective study of children aged ≤15 years with SE transported to a tertiary emergency center between December 2014 and November 2024. The primary endpoint was neurological sequelae at hospital discharge. Secondary endpoints included time to antiseizure medications (ASMs) administration, seizure duration, seizure cessation within the International League Against Epilepsy (ILAE) t threshold, and mechanical ventilation. Propensity score matching, subgroup analysis according to the t threshold, and Bonferroni correction were performed. Twenty-one patients in the Doctor Car group and 127 in the non-Doctor Car group were analyzed, with 17 matched pairs. Doctor Car dispatch was associated with shorter time to ASMs administration and shorter seizure duration before and after propensity score matching. Patients whose seizures ceased within the ILAE t threshold were more likely to receive Doctor Car dispatch and earlier ASMs administration. Neurological sequelae were associated with prolonged seizure duration and delayed seizure cessation after ASMs administration, but not with time to ASMs administration. Neurological sequelae did not differ significantly between the groups. Doctor Car dispatch was associated with earlier prehospital ASMs administration, shorter seizure duration, and increased likelihood of seizure cessation within the ILAE t threshold in pediatric SE. These findings support the role of physician-staffed ambulances in improving prehospital seizure management and may contribute to reducing neurological injury.

4. Prescribing patterns of migraine pharmacotherapy in children and adolescents: descriptive study using a large-scale claims database.
PMID:日期:2026-09-12Treatment options for pediatric patients are more limited than for adults. Data on pediatric migraine treatment practices, overprescription, and long-term prescription patterns are limited. The study aimed to describe the incidence of overprescription of acute medications and prescription patterns in children and adolescents. A descriptive study was conducted using data from the JMDC claims database between April 2012 and March 2025. We included patients with migraine aged 6-17 years. The cumulative incidence of overprescription was estimated for acute medications. The longitudinal prescription patterns of acute and preventive medications were summarized on Days 0, 30, 60, 90, 180, 270, and 365 using Sankey plots. Analyses were stratified by age group (6-11 and 12-17 years). The study included 27,074 pediatric patients with migraine; 6495 were aged 6-11 years, and 20,579 were aged 12-17 years. The 5-year cumulative incidence of overprescription was 4.86% for NSAIDs, 1.55% for triptans, and 3.13% for acetaminophen. Overprescription was more frequent in adolescents than in children (NSAIDs 5.56% vs 2.46%). At Day 0, 40.1% of patients received analgesics alone, 13.1% received triptans/ergotamine alone, 13.0% received combination therapy, and 24.4% received prescriptions including preventive medications. By Day 365, these figures had decreased to 3.7%, 1.0%, 0.8%, and 9.1%, respectively. Among pediatric patients with migraine, overprescription was more common in adolescents than in younger children. All prescription regimens decreased over time, but preventive medications appeared relatively persistent compared with acute medications.

5. Electroclinical and neuroradiological evolution in pediatric and adult patients with Rasmussen encephalitis.
PMID:日期:2026-09-08Rasmussen encephalitis (RE) is a rare progressive inflammatory disorder characterized by drug-resistant focal epilepsy and unilateral cerebral atrophy. This study aimed to evaluate the longitudinal electroclinical, neuroradiological, treatment, and surgical characteristics of pediatric and adult patients with RE followed at a tertiary referral center. Fourteen patients (10 females) with a median follow-up of 75.5 months were retrospectively reviewed. Clinical characteristics, serial electroencephalography (EEG), brain magnetic resonance imaging (MRI), immunotherapy regimens, surgical interventions, and seizure outcomes were analyzed. Median age at symptom onset was 102 months. Focal motor seizures were the initial presenting symptom in 86% of patients, and epilepsia partialis continua developed in 57%. Serial EEG demonstrated progression from focal epileptiform abnormalities to regional and hemispheric slowing confined to the affected hemisphere. Left-hemispheric involvement was observed in 71% of patients in this relatively older cohort. Initial MRI frequently demonstrated cortico-subcortical T2-weighted and fluid-attenuated inversion recovery (FLAIR) hyperintensities preceding progressive cortical atrophy. Serial MRI revealed heterogeneous radiological evolution, including diffuse hemispheric and limited regional cortical atrophy patterns. Despite immunotherapy, most patients showed progressive disease and refractory seizures. Hemispherectomy resulted in seizure freedom in two of three operated patients. RE demonstrated heterogeneous electroclinical and neuroradiological evolution across pediatric and adult patients. The coexistence of an older age profile and a predominance of left hemisphere involvement may suggest age-related phenotypic variability. Early MRI abnormalities may precede overt hemispheric atrophy, highlighting the importance of longitudinal evaluation for timely diagnosis. Although immunotherapy may provide temporary stabilization, surgical treatment appears to provide the most favorable seizure outcomes in selected patients.

6. Pediatric pseudotumor cerebri syndrome: Clinical characteristics, management and outcomes in a single-center cohort.
PMID:日期:2026-09-07Pediatric pseudotumor cerebri syndrome (PTCS) differs from adult disease in epidemiology, risk factors and clinical presentation. Female predominance and obesity are typically reported after puberty, while secondary forms are more frequent in younger children. Due to limited data on prepubertal patients, this study aimed to analyze clinical features, management and outcomes of pediatric PTCS, focusing on metabolic factors and disease course. We retrospectively analyzed 14 children and adolescents diagnosed with PTCS in a pediatric center between 2015 and 2025, according to Friedman criteria. Data included demographics, pubertal status, body mass index, presenting symptoms, secondary causes, treatments and outcomes over 12 months. Additional post-study follow-up was performed when available. Female predominance was observed regardless of pubertal status. All patients were overweight or obese at diagnosis. Headache was the most frequent presenting symptom (85.7%), often accompanied by visual symptoms. Papilledema was present in all patients. Secondary PTCS occurred in three patients. Treatment included weight loss and acetazolamide in all patients. At 12 months, complete clinical and ophthalmologic remission was achieved in 64.3%. Recurrences occurred in three patients, both early and late: two required surgical interventions. PTCS should be suspected in overweight or obese children and adolescents presenting with persistent headache or visual symptoms, regardless of pubertal status. Early diagnosis, weight control and regular ophthalmologic monitoring are essential to reduce the risk of permanent visual impairment. Long-term follow-up is necessary, as relapses may occur beyond the first year after diagnosis. Prospective studies are warranted to confirm these findings.

7. Neurocognitive outcomes in pediatric brain arteriovenous malformations: a systematic review.
PMID:日期:2026-09-03Pediatric brain arteriovenous malformations (AVMs) are an important cause of hemorrhagic stroke and long-term morbidity. Neurocognitive outcomes remain incompletely characterized. To synthesize reported neurocognitive, educational, and occupational outcomes in individuals diagnosed with brain AVMs before 18 years of age. PubMed, Scopus, Web of Science, the Cochrane Library, and ScienceDirect were searched from January 1995 through June 5, 2026, according to a prospectively registered International Prospective Register of Systematic Reviews protocol (PROSPERO; CRD420261352472). Studies reporting at least one neurocognitive, educational, or occupational outcome in individuals diagnosed with a brain AVM before 18 years of age were eligible irrespective of management strategy. Because of clinical and methodological heterogeneity, findings were synthesized narratively. Twelve studies involving 287 individuals diagnosed in childhood were included. Participant ages at assessment, follow-up periods, study designs, and neurocognitive instruments varied substantially. Reported intelligence quotient (IQ) scores ranged from 81 to 124, although scores from different instruments and developmental stages were not directly comparable. Across studies, reported outcomes included attention or cognitive difficulties, need for educational support, reduced academic performance, and limitations in school or work participation, while other participants maintained average or above-average functioning. Occupational outcomes were reported in only a small number of long-term follow-up studies. No comparative neurocognitive benefit of a specific management strategy could be established. Reported neurocognitive and educational outcomes after pediatric brain AVM diagnosis were heterogeneous, and evidence concerning occupational outcomes was sparse. Prospective studies using age-appropriate, standardized longitudinal neurocognitive assessments are needed.

8. Cytokine profiles in children with PCR-confirmed HHV-6 infection according to seizure occurrence.
8. 根据癫痫发作情况,PCR确诊HHV-6感染的儿童的细胞因子谱PMID:日期:2026-08-30Although cytokines are contributors to febrile seizures, previous studies have largely involved pathogen-heterogeneous populations. Human herpesvirus 6 (HHV-6) is a common cause of febrile seizures and is useful for studying pathogen-specific inflammatory responses. This retrospective study included children with polymerase chain reaction-confirmed HHV-6 infection using stored plasma samples obtained within 72 h of hospital presentation, classified according to seizure status. Plasma cytokine levels were measured using a multiplex immunoassay. Multivariable linear regression was used to identify the cytokines associated with seizures, with Benjamini-Hochberg false discovery rate (FDR) correction across 15 cytokines. Correlations between cytokine levels and hematologic parameters were evaluated. Cerebrospinal fluid (CSF)-to-plasma cytokine ratios were analyzed. Sixty patients were included (37, without seizures; 23, seizures), with no significant differences in baseline characteristics between the groups. Plasma interleukin-8 (IL-8) showed an association with seizures (adjusted fold change 1.84, p = 0.03), but this association did not remain statistically significant after FDR correction (q = 0.45). Inverse correlations between IL-8 (and to a lesser extent macrophage inflammatory protein-1 alpha) and neutrophil and platelet counts were observed in the non-seizure group only. In the paired analyses (n = 8), the CSF-to-plasma ratios of IL-8 and monocyte chemoattractant protein-1 exceeded 1.0, with a trend toward higher IL-8 ratios in the seizure group after adjusting for age. Although exploratory, findings from this pathogen-homogeneous cohort suggest that IL-8 is a candidate cytokine associated with both seizure occurrence and an altered relationship with neutrophil and platelet counts, warranting further study.

9. National survey of patients with neuromuscular disease and COVID-19 in Japan.
PMID:日期:2026-08-30Coronavirus disease 2019 (COVID-19) poses particular risks for patients with neuromuscular disorders, but its severity and outcomes in this population remain unclear. We investigated the clinical features, transmission routes, treatments, and prognostic factors of COVID-19 in patients with neuromuscular disorders in Japan. A retrospective questionnaire survey was submitted to 13 neuromuscular disease centers to identify patients diagnosed with COVID-19 between December 2019 and October 2022. The collected data comprised demographics, underlying diseases, living situation, activities of daily living (ADLs), vaccination status, respiratory and cardiac management, symptoms, treatments, and outcomes. We evaluated whether pre-illness respiratory status, cardiac function, vaccination history, and age could be risk factors associated with prognosis after COVID-19 infection. In total, 155 patients were identified. Duchenne muscular dystrophy was the most frequent neuromuscular disease (35%), followed by myotonic dystrophy (14%). The mean patient age was 30.7 years, and 70% of patients lived at home with family. Meanwhile, 54% of patients were wheelchair users, and 8% were bedridden. The vaccination rate was 56%. Fever was the most common symptom, and 5% of patients were asymptomatic. Antipyretics were the most frequent treatments; adults mainly received antivirals. Overall, 87% recovered fully, but two patients died. Of the examined factors, only higher age at onset was predictive of worse prognosis (p = 0.0019). COVID-19 symptoms and transmission routes in patients with neuromuscular disease resembled those in the general population. Advanced age was associated with poorer outcomes, underscoring the need for careful monitoring in these subgroups.

10. Comparison of oral prednisolone and ACTH therapies in infantile epileptic spasms syndrome: using BASED and E-Chess scores.
PMID:日期:2026-08-29This study aimed to compare the efficacy of adrenocorticotropic hormone (ACTH) and oral prednisolone therapies in children with infantile epileptic spasms syndrome (IESS), utilizing the Burden of Amplitudes and Epileptiform Discharges (BASED) score to assess electrographic severity and the Early Childhood Epilepsy Severity Scale (E-Chess) to evaluate clinical outcomes. This retrospective cross-sectional study included 40 children aged 1 to 16 months with IESS. Children received either ACTH (n = 23) or high-dose oral prednisolone (n = 17). Electroencephalographic severity was assessed before treatment and on day 28 post-treatment using the BASED score. Clinical epilepsy severity was evaluated at one year post-treatment using the E-Chess score. Both treatment groups showed significant reductions in BASED scores (overall cohort p < 0.001; ACTH group p < 0.001; prednisolone group p = 0.007). However, there was no statistically significant difference between the ACTH and prednisolone groups regarding post-treatment BASED or E-Chess scores. While clinical factors such as gender, age, and etiology were not associated with favorable outcomes, a lower post-treatment BASED score was significantly associated with a better clinical prognosis (p = 0.007). Notably, a higher number of pretreatment anti-seizure medications (ASMs) was a strong predictor of poor clinical outcome (OR: 5.474, 95% CI: 1.849-16.207; p = 0.002). High-dose oral prednisolone and ACTH therapies demonstrated comparable efficacy in reducing electrographic and clinical seizure burden in children with IESS. Improvement in epileptiform activity, as reflected by reductions in the BASED score, is strongly associated with better clinical outcomes. Additionally, a high pretreatment medication burden may serve as an early indicator of a refractory clinical course.
