1. EXPRESS: Normal Aging Limits Cortical Network Reorganization and Behavioral Recovery after Experimental Stroke.
PMID:期刊:日期:2026-10-02Although advancing age is associated with worse recovery after stroke, little is known about how aging influences post-stroke reorganization of neuronal circuits and networks. We used wide-field calcium imaging to compare cortical network dynamics and behavioral recovery after focal photothrombotic stroke in forepaw somatosensory cortex in young (3-month-old) and aged (18-month-old) Thy1-GCaMP6f mice. Both young and aged mice developed significant somatomotor deficits after stroke, but only young mice exhibited substantial behavioral recovery. While young and aged mice showed differences in baseline forepaw-evoked response amplitudes and their trajectories after stroke, the temporal dynamics of these circuits were comparable both before and after stroke between young and aged mice. Relative to aged mice, young mice demonstrated superior baseline measures of hemispheric stimulus-locked fidelity, bilateral coherence, and cortical entrainment, that declined significantly after stroke; this trend was absent in aged mice. Finally, only young mice showed significant recovery of somatosensory and somatomotor functional connectivity after stroke. These results suggest similarities in neuronal network degradation caused by aging and stroke and highlight the vulnerability of neuronal networks in both processes. Our findings also indicate a potential network-level basis for chronic deficits in stroke survivors.

2. EXPRESS: Bayesian networks to estimate prognosis in vascular cognitive impairment and small vessel disease: integrated analyses of interdependent contributors to multiple outcomes.
PMID:期刊:日期:2026-10-02Vascular cognitive impairment (VCI) and small vessel disease (SVD) involve interconnected factors influencing multiple outcomes, also beyond cognitive decline. Bayesian networks (BNs) can help unravel these complex interrelations, which we demonstrate in this proof-of-concept study in the Heart-Brain Connection cohort, including memory-clinic patients with SVD, patients with heart failure, carotid occlusive disease, and reference participants. We trained BNs and jointly modelled cognitive decline (Clinical Dementia Rating (CDR) increase) and major adverse cardiovascular events (MACE) over five years as outcomes in relation to multiple demographic and disease factors and emerging imaging and plasma biomarkers, also considering non-random study discontinuation. Of 566 individuals (median age 68, 64% men), 80 had MACE and 112 experienced CDR increase. Diagnostic group and baseline cognition were key determinants of both outcomes. The BN identified baseline clinical severity as a non-random study discontinuation source. Plasma biomarkers formed an interconnected subnetwork, linked to demographic and vascular factors, but without direct dependencies with outcomes. The trained BN provides individualized inference under partial evidence, informing on outcome probabilities. This proof-of-concept study demonstrates how BNs quantify and visualize the dependency structure underlying prognostic heterogeneity in VCI and SVD, including non-random study discontinuation and positioning of emerging biomarkers.

3. EXPRESS: Association between Neurovascular Network Dysfunction and Poor Sleep Quality in Patients with Cerebral Small Vessel Disease.
3. EXPRESS:脑小血管病患者神经血管网络功能障碍与睡眠质量差之间的关联PMID:期刊:日期:2026-10-02Poor sleep quality is frequently reported among patients with cerebral small vessel disease (CSVD), but its neurovascular correlates remain unclear. We examined exploratory associations between multidomain neurovascular network (NVN) measures and poor sleep quality in patients with CSVD. In this cross-sectional analysis nested within a prospective CSVD registry, 102 patients with CSVD underwent heart rate variability, cerebral haemodynamic and resting-state electroencephalography assessments. Poor sleep quality was defined as a Pittsburgh Sleep Quality Index (PSQI) global score >5. Analyses were primarily adjusted for age and sex, with false discovery rate (FDR) correction applied across all evaluated NVN variables. Fifty-three participants had poor sleep quality. Among 108 evaluated NVN variables, 12 showed nominal unadjusted between-group differences and 13 showed nominal age- and sex-adjusted associations with poor sleep quality. Neither crude nor adjusted associations survived FDR correction. In secondary exploratory model analyses, the multidomain NVN model showed an apparent area under the curve (AUC) of 0.795 and a bootstrap optimism-corrected AUC of 0.758. PSQI-defined poor sleep quality in CSVD was accompanied by exploratory differences across autonomic, haemodynamic and neuroelectrical NVN domains. Longitudinal studies with non-CSVD controls and objective sleep assessments are needed.

4. EXPRESS: High-resolution three-dimensional characterization of clot evolution in experimental intracerebral hemorrhage using diceCT.
PMID:期刊:日期:2026-09-25MRI is widely used to monitor experimental intracerebral hemorrhage (ICH), but T2*-weighted lesion volume may reflect both clot components and blood degradation products during hematoma evolution. We utilized diffusible iodine-based contrast-enhanced CT (diceCT) to examine diceCT-visible clot changes in relation to MRI-defined lesions. ICH was induced in 102 mice via collagenase or autologous blood infusion. DiceCT was performed at 4 hours, 1, 3, 7 and 28 days post-ICH. In paired MRI-diceCT cohorts, diceCT-visible clots were compared with T2*-weighted MRI lesions. DiceCT enabled volumetric assessment of clot components and erythrolysis-associated intralesional hypodense regions after ICH. High-resolution imaging revealed vascular-associated clot extensions toward the brain surface. Multimodal analysis revealed an early shift in hemorrhagic lesion composition: the fraction of the MRI-defined T2* lesion represented by diceCT-visible clot decreased markedly by day 7 (16.92% in the autologous blood model and 2.59% in the collagenase model). DiceCT provides high-resolution ex vivo visualization of clot structures after experimental ICH. Combined MRI-diceCT analysis indicates that MRI-defined lesions and diceCT-visible clot structures diverge over time, supporting the use of complementary imaging readouts to characterize hematoma evolution and persistent blood-derived signals after ICH.

5. EXPRESS: NEUROPROTECTIVE EFFICACY OF AN OMEGA-3 DIGLYCERIDE EMULSION IN A PIGLET MODEL OF NEWBORN HYPOXIC-ISCHEMIC BRAIN DAMAGE.
5. 快报:Omega-3甘油二酯乳剂在仔猪新生儿缺氧缺血性脑损伤模型中的神经保护疗效PMID:期刊:日期:2026-09-23Acute injection of omega-3 fatty acids (n-3) formulated in a novel diglyceride (DG) lipid emulsion is neuroprotective after hypoxic-ischemic encephalopathy (HIE) in neonatal rodents. the efficacy of acute n-3 DG emulsion treatment was tested in 2-3-day-old piglets subjected to bilateral carotid artery clamping and 8% O₂ exposure for 30 min. Animals were randomly assigned to normothermia or hypothermia (HT, 34 ± 0.5 °C), then to n-3 DG or vehicle administration 30 min post-HI. Brain activity was assessed by amplitude-integrated EEG over 6 h. Brain injury was analyzed by TUNEL, GFAP and IBA-1 staining, ¹H-NMR spectroscopy (Lactate/NAA, Glutamate/NAA), and Western blot (caspase-3, TNF-α, IL-1β, protein carbonylation). n-3 DG was well tolerated, with no hemodynamic or respiratory side effects. HT alone moderately improved brain activity and only reduced inflammation, whereas n-3 DG treatment partially restored early EEG recovery, attenuated cell death and neuroinflammatory markers, and limited protein carbonyl formation and GFAP+ cell loss. Notably, n-3 DG efficacy was preserved under HT conditions. These results support the neuroprotective potential of n-3 DG emulsion in a piglet model of HIE, with promising implications for clinical translation.

6. EXPRESS: Visual gamma stimulation causes prolonged enhancement of low-frequency blood flow oscillations across superficial cortical regions in mice.
6. EXPRESS:视觉伽马刺激引起小鼠浅层皮质区域低频血流振荡的长时间增强PMID:期刊:日期:2026-09-17Gamma entrainment using sensory stimuli (GENUS) uses 40Hz-pulsed sensory stimuli to entrain neural activity in the gamma band (30-150Hz). However, the effect of GENUS on low-frequency vascular oscillations has not been fully explored. The objective of this study is to elucidate the effect of GENUS on vasomotion in healthy mice and potential confounds for future application in disease studies. Head-fixed, awake C57Bl/6 mice (n=18; 9M 9F) aged between 18 to 60 weeks were subjected to white light of either 40Hz visual flicker (GENUS), or constant stimulus (control). Blood flow was imaged using laser speckle contrast imaging (LSCI) before, during, immediately after 1 hour of stimulus, and 30min after the stimulus termination. A linear mixed-effects model showed that GENUS enhanced the magnitude of 0.2-0.4Hz blood flow oscillations by 38% during stimulation and by 30% at 30 minutes after stimulation compared to control when controlled for age, sex, and other factors. The effect on vasomotion was distributed across the surface of many cortical regions not limited to visual areas. These results support the exploration of GENUS for increasing vasomotion in therapeutic contexts.

7. EXPRESS: The granular Hypoperfusion Intensity Ratio (gHIR): A Novel Perfusion-Weighted Metric to Assess the Cerebral Infarct Growth Rate.
7. EXPRESS:全脑低灌注强度比(gHIR):一种评估脑梗死生长速率的新型灌注加权指标PMID:期刊:日期:2026-09-17We introduce a novel perfusion-imaging metric, the granular hypoperfusion intensity ratio (gHIR), to assess the infarct growth rate (IGR). The gHIR was calculated using an equation that weighted the proportions of ischemic volumes across multiple Tmax thresholds (2s to 18s) to yield a value from 0 to 1, with higher values indicating a faster IGR. gHIR was compared with four established IGR metrics: ASPECTS decay, multiphase CT-angiography collaterals, HIR, and rCBF-IGR. The primary outcome was a good functional outcome (90-day modified Rankin Scale ≤2), assessed with multivariable logistic regression. We evaluated prognostic performance with the c-statistic and determined the optimal gHIR cutoff using the Youden Index. Of 672, 570 (85%) were included: the median age was 77 years [IQR=67-83]; 290 (50.8%) were female; and 272 (47.7%) had a good functional outcome. A good functional outcome was associated with gHIR (adjusted odds ratio [aOR] 0.42 [95%CI=0.34-0.52] per 0.1-unit increase). The AUC of the gHIR for predicting a good functional outcome was 0.75 (95%CI=0.71-0.79). The optimal gHIR cutoff for a good functional outcome was ≤0.30. The gHIR showed a strong association with outcomes and superior prognostic performance compared with other standard CT-based methods for assessing IGR.

8. EXPRESS: Longitudinal Circulating miRNA Profiling Identifies Pharmacodynamic Biomarkers of Remote Ischemic Conditioning in Acute Ischemic Stroke: A Translational Sub-Study of the Randomized TRICS-9 Trial.
8. EXPRESS:纵向循环miRNA谱分析鉴定急性缺血性卒中远端缺血预适应的药效学生物标志物:随机TRICS-9试验的转化亚研究PMID:期刊:日期:2026-09-17Remote ischemic conditioning(RIC) is a promising neuroprotective strategy for ischemic stroke, but reliable pharmacodynamic biomarkers confirming its biological activity are lacking. In this translational sub-study of the multicenter randomized phase II TRICS-9 trial, we profiled five circulating microRNAs (miR-181b,miR-150,miR-143,miR-223, and let-7a) in 42 acute stroke patients assigned to RIC or standard medical therapy at 24 hours and 72 hours post-randomization.At 24 hours, miR-143,miR-150, and let-7a were significantly lower in RIC patients, with miR-143 and miR-150 showing strong pharmacodynamic biomarker performance in receiver operating characteristic analysis. At 72 hours, miR-181b was significantly elevated in RIC patients, while let-7a and miR-223 remained suppressed.Longitudinal paired analysis revealed significant within-patient temporal increases in miR-181b,miR-150, and miR-143 from 24 to 72 hours in RIC patients consistent with a biphasic molecular response to conditioning. These findings identify miR-143, miR-150, let-7a, and miR-181b as candidate pharmacodynamic biomarkers of RIC biological activity. Importantly, none of the candidate miRNAs demonstrated significant correlation with stroke severity, discharge NIHSS, or 90-day functional outcome, consistent with their role as pharmacodynamic rather than prognostic markers. These results provide mechanistic support for translational convergence between preclinical and clinical RIC neuroprotection research and highlight the need for external validation in larger outcome-linked cohorts.

9. EXPRESS: caADAMTS13 reduces neutrophil-mediated vascular inflammation without exacerbating bleeding in experimental intracerebral haemorrhage.
PMID:期刊:日期:2026-09-16Targeting thromboinflammation through the von Willebrand factor (VWF)/ADAMTS13 axis is a promising therapeutic target for acute ischaemic stroke (AIS). Constitutively active ADAMTS13 (caADAMTS13) is a novel thrombolytic that improves outcomes in experimental AIS, with enhanced thrombolytic efficacy compared to current licensed AIS treatments. Current thrombolytics are limited by narrow therapeutic window and the requirement for definitive imaging to exclude intracerebral haemorrhage (ICH). This study aimed to assess whether caADAMTS13 is safe in ICH, evaluating its potential use when stroke subtype is unknown, including pre-hospital settings. In zebrafish larval and murine ICH models, both hyperacute (1h) and delayed (12 or 24h) caADAMTS13 administration did not exacerbate haemorrhage. Early cerebrovascular VWF accumulation (4h) and acute neutrophil recruitment (24h) were unchanged, while vascular neutrophil deposition was reduced by 80% in mice 7 days post-ICH. Delayed caADAMTS13 treatment reduces brain neutrophil recruitment by 37% and injury by 32%, improving ICH outcomes in zebrafish larvae. Together, these findings support the development of caADAMTS13 as a thrombolytic suitable for use in suspected stroke prior to brain imaging to maximise benefit from ultra-early treatment or in regions where rapid access to brain imaging is not possible.

10. EXPRESS: Diagnostic Accuracy of Noninvasive Translesional Pressure Ratio Based on Magnetic Resonance Imaging for Intracranial Atherosclerotic Stenosis: A Validation Study.
10. 简报:基于磁共振成像的无创跨病变压力比对颅内动脉粥样硬化性狭窄的诊断准确性:一项验证研究PMID:期刊:日期:2026-09-16The anatomic stenosis severity of intracranial atherosclerotic stenosis (ICAS) does not reliably reflect lesion-specific hemodynamic impairment, and invasive pressure-wire assessment is technically challenging in the cerebral circulation. This study aimed to develop and validate MRI-derived pressure ratio (M-PR), a noninvasive functional assessment framework for ICAS. Compared with time-of-flight MRA (TOF-MRA)-based lumen reconstruction, high-resolution vessel wall imaging (HR-VWI) could provide more accurate vascular geometry for CFD, while phase-contrast MRA (PC-MRA) could further improve patient-specific boundary conditions. On this basis, we developed two M-PRs using the same PC-MRA-derived boundary conditions but different sources of vascular geometry: TOF-MRA for TOF-PR and HR-VWI for VWI-PR. Correlation with wire-based PR was assessed using Pearson or Spearman analysis, and agreement was evaluated by Bland-Altman analysis. Subgroup analyses stratified by stenosis severity were performed. In 42 patients with symptomatic ICAS (mean stenosis, 75.3% ± 7.8%), VWI-PR showed strong correlation (r = 0.87, P < 0.001) and close agreement (mean difference, 0.002 ± 0.09), whereas TOF-PR showed weaker correlation (r = 0.37, P = 0.016) and poorer agreement (-0.17 ± 0.25). TOF-PR underestimated PR in severe stenosis, whereas VWI-PR remained stable across stenosis grades. These findings support VWI-PR as an accurate hemodynamic biomarker for functional assessment of ICAS.
