1. Midlife mental health and frailty up to 35 years later: Insights from the HUNT study.
1. 中年心理健康与虚弱状况至35年后的关联:来自HUNT研究的见解PMID:日期:2026-09-28This longitudinal cohort study aimed to determine whether mental health problems in midlife are linked to frailty after age 70, as measured using two sets of frailty criteria. We used data from 27,958 adults aged 35-60 years in the Trøndelag Health Study (HUNT1, 1984-1986), with frailty outcomes assessed in 9,956 participants aged 70 years and older (HUNT4, 2017-2019). Mental health at baseline was self-reported. Frailty at follow-up was measured using both the Fried criteria and the 35-item HUNT4-Frailty Index. Linear regression models estimated frailty differences by baseline mental health status, adjusted for sociodemographic and lifestyle factors, stratified by age. We applied inverse probability weighting and multiple imputation to reduce selection bias. Mental health problems at age 35-49 at baseline were associated with 0.52 (95% CI: 0.35-0.69) higher Fried scores and 0.066 (95% CI: 0.048-0.084) higher HUNT4-FI scores at follow-up. For ages 50-60, Fried scores and HUNT4-FI scores were higher by 0.30 (95% CI: -0.02-0.63) and 0.037 (95% CI: 0.006-0.068), respectively. We observed that midlife mental health problems were associated with higher frailty levels three decades later, across two commonly used frailty models. These findings underscore the importance of both psychological and physical factors in ageing.

2. Self-rated near- and distance-vision patterns, vision change, and loss of functional independence in older adults: a longitudinal ELSA study.
2. 老年人自评近距与远距视力模式、视力变化及功能性独立丧失:一项ELSA纵向研究PMID:日期:2026-09-25Self-rated vision is often analysed as a single static measure, leaving the relevance of near versus distance difficulty and recent vision change less clear. To examine associations of near- and distance-vision patterns and vision change with next-wave loss of functional independence. We used waves 1-9 of the English Longitudinal Study of Ageing. Current-pattern analyses included 50,991 person-intervals from 12,811 participants; vision-change analyses included 39,143 intervals from 10,668 participants. Impairment was defined as self-rated fair, poor, or blind vision with usual correction. Discrete-time complementary log-log models used participant-clustered robust standard errors. The model adjusted for sociodemographic, behavioural, clinical, and hearing factors; an extended model adjusted for memory, orientation, and depressive symptoms. Near-only, distance-only, and dual impairment were associated with functional-independence loss, with hazard ratios of 1.39 (95% CI, 1.23-1.58), 1.42 (1.22-1.66), and 1.59 (1.37-1.83), respectively. Dual impairment had the largest association with incident instrumental activities of daily living limitation (1.86, 1.53-2.24). Incident, improved/recovered, and persistent impairment were associated with functional-independence loss, with hazard ratios of 1.51 (1.34-1.70), 1.27 (1.11-1.46), and 1.63 (1.39-1.90). Self-rated near- and distance-vision patterns and vision change were associated with next-wave functional loss. Distinguishing visual domain and change may add useful information to functional assessment.

3. Age- and sex-related distribution of 30-second chair stand test performance across adulthood: Findings from the INSPIRE-T cohort.
3. 30秒坐立测试在成年期各阶段的年龄和性别相关分布:来自INSPIRE-T队列的研究发现PMID:日期:2026-09-23Physical performance is an important indicator of health status across adulthood. In relatively high-functioning populations, bounded instruments such as the Short Physical Performance Battery (SPPB) may show ceiling effects, limiting discrimination at the upper end of the scale. The 30-second Chair Stand Test (30s-CST) provides a repetition-based measure of lower-extremity performance, but its distribution across the broader adult lifespan, particularly in younger adults, remains insufficiently characterized. This study aimed to characterize age- and sex-related 30s-CST performance across adulthood and to contextualize its distribution using other commonly used performance-based measures. This cross-sectional analysis included 1012 community-dwelling participants aged 20-102 years with available 30s-CST completion status; 268 (26.5%) were unable to perform the conventional 30s-CST. Among participants with available measurements, 77.2% achieved the maximum SPPB score of 12/12. In addition, 85% completed the 5-STS in <11.59 s, 91% met the sex-specific handgrip strength thresholds, and 85.8% had a usual gait speed ≥1.0 m/s. Among the 744 participants who completed the 30s-CST, the number of repetitions ranged from 4 to 40, with a mean of 15.8 ± 4.7 repetitions. The 30s-CST showed a broad distribution of performance across adulthood among test completers, with generally lower performance in older age groups. In parallel, a large proportion of the cohort achieved the maximum SPPB score. These findings highlight differences in the distribution of these performance-based measures in a relatively high-functioning population. NCT04224038 (2019-10-16).

5. Population-based appendicular skeletal muscle mass equation: Weak association between predicted and bioimpedance-derived changes during stroke and hip-fracture rehabilitation.
5. 基于人群的四肢骨骼肌质量方程:在卒中与髋部骨折康复期间,预测值与生物电阻抗推导值变化之间关联较弱PMID:日期:2026-09-23A population-based appendicular skeletal muscle mass (ASMM) equation was developed against dual-energy X-ray absorptiometry (DXA), primarily in community populations. We examined agreement with repeated clinical bioelectrical impedance analysis (BIA)-derived estimates in older rehabilitation inpatients. We analyzed 554 patients after stroke and 296 after hip fracture, without repeat admissions. Standardized BIA was performed within 7 days after admission and within 7 days before discharge; nurse-measured anthropometry was obtained within 7 days of BIA. The equation was applied without refitting, with conventional BIA-derived ASMM as the primary comparator. We examined correlations and mean differences in within-person change and sensitivity analyses for data quality, incomplete follow-up, and comparator definition. Admission correlations were 0.899 and 0.839 after stroke and hip fracture, respectively. Change correlations were 0.198 (95% CI, 0.082-0.309) and 0.227 (0.078-0.388), respectively. Mean differences in change (equation minus BIA) were -0.41 kg (95% CI, -0.53 to -0.28) and -0.14 kg (-0.39 to 0.10). Sensitivity analyses for data quality, incomplete follow-up, and comparator definition yielded similarly weak change associations. Internal-coherence analyses did not establish the reliability of BIA-derived change. Equation-predicted and BIA-derived changes were weakly associated in these selected rehabilitation patients. Population transportability, measurement-method differences, nonconcurrent assessments, and comparator uncertainty could not be disentangled. These findings do not establish longitudinal inaccuracy of the original DXA-based equation.

6. Physical function-dependent calf circumference thresholds for predicting 1-year mortality in older patients hospitalized with heart failure: a multicenter prospective cohort study.
PMID:日期:2026-09-21Calf circumference (CC) reflects muscle mass and nutritional status but may be confounded by peripheral edema in heart failure (HF). The Short Physical Performance Battery (SPPB) is a validated predictor of adverse outcomes in HF. This study developed and temporally validated a simple risk-stratification model combining SPPB and CC to predict 1-year mortality in hospitalized older patients with HF. We analyzed data from the Japanese multicenter J-Proof HF Registry, comprising 7362 patients aged ≥ 65 years. Participants were divided chronologically into training (n = 4417) and temporal validation (n = 2945) cohorts. Decision-tree analysis using SPPB and CC identified cutoff values and defined three mortality risk groups. Model performance was assessed using Kaplan-Meier analysis and multivariable Cox regression. Patients were classified into low (SPPB ≥10 and CC ≥ 25.9 cm), intermediate (SPPB ≥10 and CC < 25.9 cm orSPPB 7-9 and CC ≥ 25.3 cm), and high (SPPB 7-9 and CC < 25.3 cm or SPPB < 7) risk groups. In the temporal validation cohort, 1-year survival differed significantly across groups (log-rank P < 0.001), with stepwise increases in adjusted mortality risk. Findings were consistent across sex subgroups. The combined SPPB-CC model provides a simple approach for stratifying 1-year mortality risk in older patients hospitalized with HF. This bedside tool may help identify patients who could benefit from further nutritional and rehabilitation assessment.

7. Comparative effects of activity-based interventions on quality of life among community-dwelling older adults: A systematic review and Bayesian network meta-analysis.
7. 基于活动的干预措施对社区居住老年人生活质量影响的比较效果:一项系统综述与贝叶斯网络荟萃分析PMID:日期:2026-09-21Meaningful activity participation may support healthy aging and quality of life (QoL) in community-dwelling older adults. We compared interaction-based social, exercise-based physical, and self-directed individual activities. Thirteen English- and Korean-language databases were searched through May 2026 for RCTs of community-dwelling older adults with a study-sample mean or median age ≥65 years. Interventions were classified using a theoretically informed operational framework. Risk of bias was assessed with RoB 2. Pairwise and Bayesian network meta-analyses estimated standardized mean differences (SMDs); certainty was assessed using GRADE, with SUCRA rankings considered secondary. Seventy-one RCTs (12,159 participants) were included. The overall pairwise effect favored activity-based interventions (SMD=0.59, 95% CI 0.45-0.73), but heterogeneity was substantial and the prediction interval crossed the null; certainty was very low. Category-specific pairwise estimates were 0.68 (0.27-1.08) for self-directed, 0.61 (0.43-0.78) for physical, and 0.46 (0.21-0.72) for social activities, all with very-low-certainty evidence. In the network meta-analysis, low-certainty evidence suggested that all three categories may improve QoL versus no treatment: self-directed SMD=0.78 (95% CrI 0.25-1.32; SUCRA 82.5%), physical 0.73 (0.46-1.00; 79.6%), and social 0.40 (0.11-0.69; 37.6%). Excluding high-risk studies yielded an overall pairwise estimate of 0.53 (0.37-0.69); the estimate for physical activities remained 0.61, whereas the estimates for social and self-directed activities were 0.43 and 0.45. Evidence does not establish superiority of one activity category. Findings remain limited by heterogeneity, risk of bias, potential publication bias, and sparse direct active comparisons. Comparative rankings should be considered hypothesis-generating, and higher-quality head-to-head trials are needed.

8. Joint word recall and functional limitation among selected SHARE participants with prior survey-identified stroke versus a stroke-free reference group.
8. 在既往调查确定患有中风的特定SHARE参与者与无中风参照组中,词语回忆与功能性受限的联合情况PMID:日期:2026-09-21Joint word-recall and everyday-function patterns after stroke require comparison with older adults without prior stroke. We compared survey-observed distributions while retaining longer-term observation limitations. We analyzed SHARE Waves 1, 2 and 4-8. T1 was the first post-stroke-positive assessment with valid recall and function, selecting survivors who remained observed and measurable. One country-wave-aligned pseudo-index with valid measures was selected per eligible, explicitly stroke-negative reference participant with no recorded stroke-positive response at or before that assessment. Overlap weighting balanced country-wave, age, sex and education. Four states combined low relative word recall and ADL/IADL limitation; four-year outcomes were secondary. Among 2470 stroke and 83,767 reference participants, overlap-weighted proportions were 57.1% versus 74.4% for neither state (difference -17.3 percentage points, 95% CI -19.3 to -15.3), 7.2% versus 6.8% for low relative recall only (0.3, -0.7 to 1.4), 26.4% versus 15.0% for functional limitation only (11.4, 9.6 to 13.1), and 9.4% versus 3.8% for both (5.6, 4.4 to 6.8). The maximum post-weighting absolute standardized mean difference was 0.00053, and sensitivity estimates were similar. In the secondary 778-person subset with full potential four-year coverage, 28.1% were unavailable. The aligned comparison showed a redistribution toward states containing functional limitation, with similar recall-only proportions. These standardized distributions describe selected observed participants and are neither population prevalence nor causal stroke effects.

9. Sarcopenia index as a prognostic indicator of mortality rates in community-dwelling centenarians: a prospective cohort study.
9. 骨骼肌减少症指数作为社区百岁老人死亡率预后指标:一项前瞻性队列研究PMID:日期:2026-09-21With global population aging rapidly, centenarians underscore the urgency of understanding their health trajectory and mortality determinants. Identifying reliable and accessible prognostic indicators of mortality rates, and exploring the influence of muscle health on healthy longevity, remain critical unmet needs. Traditional sarcopenia assessment tools including dual-energy X-ray absorptiometry (DXA) are costly and poorly accessible in the primary care. Sarcopenia index (SI), calculated from routine blood biochemical indicators, emerges as a promising low-cost alternative. This study aimed to evaluate prognostic value of SI for mortality rates in centenarians. This prospective cohort study enrolled a total of 1002 community-dwelling centenarians from Hainan, China. Baseline data were collected through epidemiological survey and face-to-face interviews. SI was calculated as follows: serum creatinine (mg/dL)/cystatin C (mg/L) × 100. In a multivariate Cox regression analysis, SI was an independent predictor, with each unit increase associated with a 3.7% reduction in mortality rates (p < 0.001). Time-dependent receiver operating characteristic curve demonstrated moderate discriminative ability of SI. It identified the optimal SI cutoff points of 59.3, 63.1 and 64.2 for one-year, three-year and five-year mortality rates, respectively. Restricted cubic spline (RCS) analysis revealed a nonlinear relationship between SI and mortality rates characterized by a sharp decline in risk with an increasing SI before reaching a plateau. SI exhibited moderate but stable predictive performance for mortality rates among centenarians. As an easy-to-obtain surrogate marker rather than a direct diagnostic indicator of sarcopenia, SI demonstrated practical value with higher accessibility for predicting mortality rates among centenarians.

10. Characteristics and predictors of frequent emergency department presentation and patient-reported care experiences among older adults: A retrospective cohort and survey study.
10. 老年人频繁就诊急诊科的特征与预测因素及患者报告的护理体验:一项回顾性队列与调查研究PMID:日期:2026-09-17Drivers of frequent emergency department (ED) presentations and patient-reported care experiences for older adults remain unclear. A retrospective population-based cohort study of adults aged ≥65 years in New South Wales, Australia, using linked hospital and mortality records and patient-reported ED care experiences. Individuals with an index ED presentation in 2018 who survived the ascertainment period were followed for five years. ED presentation frequency during the remainder of 2018 was classified as low (1-3 ED presentations) or frequent (≥4 ED presentations). Demographic and clinical characteristics were compared. Generalised estimating equation and survey logistic regression models identified factors associated with frequent presentation and negative care experiences. There were 336,283 index presentations in 2018; 10.6% were frequent presenters, mostly aged 75-84 years (38.3%). Frequent ED use was more common among those aged 75-84 years in the first four years (ORs 1.06; 95%CI 1.01-1.11 to 1.27; 95%CI 1.23-1.31) and those aged ≥85 years in the first year (OR 1.33; 95%CI 1.28-1.38). Multimorbidity, rurality, living in aged care, and triage below resuscitation were associated with frequent presentations. Female, dementia, osteoporosis, frailty, non-urgent presentation, and hospitalisation were associated with lower odds of frequent presentation. For patient experience (n=47,461), negative ED experience was associated with long waits (≥60 minutes; OR 1.70; 95%CI 1.19-2.42), complications (OR 1.63; 95%CI 1.16-2.28), and poor communication or involvement in decisions, with insufficient treatment information showing the strongest association (OR 5.17; 95%CI 3.48-7.69). Frequent ED use in older adults reflects persistent issues in complex health and social needs, communication and shared decision-making.
