1. Personalizing mycophenolate therapy: genetic determinants and practical limited sampling strategy for pediatric lupus nephritis.
PMID:日期:2026-10-02To identify determinants of mycophenolic acid (MPA) pharmacokinetics (PK) in pediatric patients with lupus nephritis (LN), and to develop a limited sampling strategy (LSS) for estimating MPA-AUC. A single-center observational study enrolled biopsy-proven LN receiving mycophenolate mofetil (MMF) at least 7 consecutive days. Univariate analysis were performed to assess associations between MPA PK parameters and demographics, genetic polymorphisms, clinical variables. LSS models for predicting MPA-AUC were developed via multivariate stepwise regression analysis. Statistically significant correlations were observed between MPA-AUC and MMF dose, plasma albumin level, estimated glomerular filtration rate, and genetic polymorphisms in HNF1A (rs56097722) and CYP2C8 (rs1058932). CES2 (rs11075646) was significantly associated with MPA half-life (T₁/₂). Single timepoint MPA concentrations demonstrated poor predictive performance for AUC(R = 0.194-0.553). The optimal LSS model was achieved with four timepoints: MPA-AUC = 2.935 + 0.716·C + 1.305·C + 3.176·C + 4.705·C (R = 0.945). This model demonstrated good predictive performance (MPE = 1.84%, MAPE = 9.47%, RMSE = 12.53%). The LSS model was validated internally and externally for stability and applicability. Reduced MPA exposure was associated with MMF doses < 20 mg·kg⁻·day⁻, hypoproteinemia, preserved or mildly impaired renal function, and HNF1A rs56097722 C/C or CYP2C8 rs1058932 G/G genotypes. CES2 rs11075646 C/C tended to correlate with a shorter MPA T₁/₂. The validated LSS models are recommended for estimating MPA-AUC in pediatric LN, which are expected to simplify the therapeutic drug monitoring process.

2. Effectiveness of Lactobacillus-supplemented eradication therapy for Helicobacter pylori: a systematic review and meta-analysis of randomized controlled trials.
2. 乳酸杆菌补充根除治疗幽门螺杆菌的有效性:随机对照试验的系统评价和荟萃分析PMID:日期:2026-09-26Helicobacter pylori eradication therapies supplemented with Lactobacillus spp. may improve eradication rates and reduce adverse events by modulating gut microbiota and immune responses. This meta-analysis aimed to evaluate the efficacy and safety of Lactobacillus-supplemented Helicobacter pylori eradication therapy. We systematically searched for randomized controlled trials (RCTs) comparing standard Helicobacter pylori eradication therapy with and without Lactobacillus in PubMed, Embase, and Cochrane Library up to August 2026. The primary outcome was eradication success, and the secondary outcome was adverse events. We calculated pooled risk ratios (RR) with 95% confidence intervals (CIs) using a random-effects model. We also estimated the number needed to treat (NNT) for eradication success and conducted subgroup analyses by antibiotic therapy type, duration of antibiotic treatment, Lactobacillus strain type, Lactobacillus dosage, period of Lactobacillus use, probiotic formulation, geographical region, and age range. This review was registered in PROSPERO (CRD42024611138). Thirty-two RCTs (4,010 patients) were included. Lactobacillus-supplemented eradication therapy improved eradication success (RR 1.10; 95%CI 1.05-1.14; NNT 13) and reduced the risk of adverse events (RR 0.70; 95%CI 0.58-0.84). Subgroup analyses showed significant differences in eradication success by antibiotic regimen and in adverse event risk by age group. Improved eradication success with Lactobacillus-supplemented eradication therapy was observed only with triple therapy regimens, whereas adverse event risk was reduced only among adults. Lactobacillus-supplemented eradication therapy was associated with a 10% relative increase in Helicobacter pylori eradication success and a 30% relative reduction in treatment-related adverse events compared with the control group.

3. Antifungal pulse therapy for onychomycosis: a narrative review.
PMID:日期:2026-09-25Onychomycosis is a common chronic fungal infection of the nail unit that may be associated with significant functional impairment and impaired quality of life. Systemic antifungal drugs remain the cornerstone of treatment for moderate-to-severe disease, with continuous oral terbinafine and pulse itraconazole being the most widely used systemic antifungals. Pulse regimens involve intermittent administration of relatively high doses of antifungals with the aim of reducing cumulative drug exposure, treatment costs, and treatment burden while maintaining efficacy. However, the comparative effectiveness of pulse and continuous regimens remains unclear. This narrative review summarizes the current evidence regarding antifungal pulse therapy for onychomycosis. A literature search was conducted using PubMed and Scopus databases to identify relevant randomized trials, comparative studies, systematic reviews, meta-analyses, observational studies, and case reports/case series published between March 2001 and April 2026. Evidence relating to treatment regimens, pharmacologic rationale, efficacy, and safety was reviewed and synthesized. The available evidence suggests that pulse therapy is an effective treatment option for onychomycosis. Among the currently available pulse regimens, itraconazole has the strongest supporting evidence and has demonstrated clinical and mycological outcomes comparable to continuous therapy in selected patient populations. Modified pulse schedules, prolonged regimens, and combination approaches incorporating topical antifungal agents have also shown promising results. In contrast, evidence supporting pulse terbinafine is less consistent, with several randomized controlled trials favoring continuous terbinafine, particularly in dermatophyte toenail onychomycosis. Fluconazole administered in weekly regimens appears to be less effective than terbinafine- or itraconazole-based regimens and is generally considered a second-line option. From a safety perspective, pulse and continuous regimens demonstrate safety profiles comparable to those of continuous therapy, with gastrointestinal adverse effects being the most frequently reported events. The current evidence supports continuous terbinafine as the preferred first-line systemic treatment for dermatophyte onychomycosis, whereas pulse itraconazole remains an important alternative for patients who are unable to tolerate terbinafine, have contraindications to its use, or have failed previous therapy. Further well-designed randomized studies with standardized outcome measures and long-term assessment of recurrence are required to better define the optimal role of pulse antifungal therapy in onychomycosis management.

5. Antidepressant use and the risk of atrial fibrillation and ventricular arrhythmia: A systematic review and meta-analysis.
5. 抗抑郁药使用与心房颤动和室性心律失常风险:系统综述与荟萃分析PMID:日期:2026-09-24Antidepressants (ADs) have been associated with adverse cardiovascular effects, including cardiac arrhythmias. Evidence on the association between AD use and the risk of atrial fibrillation (AF) and ventricular arrhythmia/sudden cardiac death (VA/SCD) remains inconsistent. This systematic review and meta-analysis aimed to evaluate the relationship between AD use and the risk of AF and VA/SCD. A detailed search was conducted in PubMed, Scopus, and Web of Science till June 2026. Cohort and case-control studies comparing AD users with non-users and reporting AF, VA, or SCD as outcomes were included. Pooled odds ratios (ORs) with 95% confidence intervals (CIs) were calculated using a random-effects model. Twelve studies with 9,619,278 participants were included. AD use was associated with a significantly increased risk of VA/SCD (9 studies, 6,373,522 participants, OR = 1.34, 95% CI: 1.15-1.55; I² = 90.5%). The association remained significant for both SSRIs and TCAs. For AF, no overall significant association emerged (3 Studies, 3,245,756 participants, OR = 1.75, 95% CI: 0.84-3.62; I² = 98.1%). However, sensitivity analysis excluding one influential study showed a significant association with AF (2 Studies, 18,109 participants, OR = 1.27, 95% CI: 1.07-1.50) and eliminated heterogeneity (I² = 0%). This meta-analysis suggests an association between AD use and an increased risk of VA/SCD, while evidence for AF remains uncertain because the results were influenced by a single study. Future large observational studies that better address confounding by indication and adequately adjust for depression are needed to provide more robust evidence.

6. Masking effect in the Spanish spontaneous reporting database FEDRA.
6. 西班牙自发报告数据库FEDRA中的掩蔽效应PMID:日期:2026-09-21This study aimed to analyse the potential masking effect of drug-event combinations (DECs) with extreme reporting rates and to evaluate the impact on disproportionality analysis and therefore in signal detection. An algorithm is proposed, based on the approach established by Juhlin et al., that identifies influential outliers and excludes them through six unmasking strategies to recalculate the Reporting Odds Ratio (ROR). This study was performed in the Spanish spontaneous reporting database FEDRA. The dataset included reports from 1 January 1981 to 17 February 2025 excluding those in which the suspected drug was a vaccine (ATC group J07). A total of 287,145 DECs were analysed. Of these, 0.4% (1,211 out of 287,145) were considered influential outliers. Almost 21% (81,371 out of 389,262) of the FEDRA reports included an influential outlier. About 14% (494 out of 3,447) of the drugs and 5.9% (576 out of 9,745) of the adverse drug reactions in FEDRA were part of an influential outlier. Regarding the disproportionality analysis, the proportion of DECs whose lower limit of the 95% confidence interval of their ROR increased after the different strategies ranged from 14.6 to 48.1%. The study demonstrates the existence of a masking effect in FEDRA caused by certain highly reported DECs. Their exclusion according to the proposed unmasking strategies could reveal additional SDRs that may otherwise remain masked during routine signal detection activities. This study aimed to analyse how certain highly reported drug-event combinations (DECs) may obscure relevant safety signals. To address it, a method was developed to identify and remove these extreme DECs to get a clearer picture. The analysis was conducted using data from the Spanish spontaneous reporting database FEDRA, excluding vaccines (ATC group J07). A total of 287,145 DECs were assessed, of which 0.4% were identified as influential outliers. Approximately 14.3% of the drugs and 5.9% of the adverse drug reactions in FEDRA were involved in these extreme DECs. Between 14.6% and 48.1% of the DECs showed an important increase in their lower limit of the 95% confidence interval of their reporting odds ratios. In conclusion, the study demonstrates that certain highly reported drug-adverse drug reaction pairs can hide other important issues. By removing these extreme DECs, the potential to detect true safety signals increases.

7. Case-based pharmacotherapeutic learning structured around the prescribing optimization method and the WHO 6-step model: Medical students' views from an exploratory questionnaire study.
7. 基于病例的药物治疗学习:围绕处方优化方法和WHO六步模型构建——来自一项探索性问卷调查的医学生观点PMID:日期:2026-09-19Pharmacotherapy is a core competence for physicians and case-based learning is a valuable teaching method for developing this competence. This study explored medical students' views on structuring pharmacotherapeutic education around either the Prescribing Optimization Method (POM), starting from the medication list, or the World Health Organization (WHO) 6-Step Model, starting from the patient's presentation. Students who had experienced both the POM and the WHO 6-Step Model, featuring cases involving multiple medications, during an elective pharmacology clerkship at the University of Bologna, were invited to complete an anonymous questionnaire. Participants rated their agreement with a statement regarding the usefulness of each structure for developing pharmacotherapeutic knowledge (1 = strongly disagree, 5 = strongly agree), indicated their preferred structure, and provided comments explaining their choice and contexts in which each structure was considered useful. All students participating in the clerkship shared their views (n = 27). Their ratings of each structure's usefulness for developing pharmacotherapeutic knowledge were 4.3 ± 0.8 (POM) and 4.1 ± 1.1 (WHO). Thirteen students preferred the POM, often referring to its comprehensive medication focus in the context of polypharmacy, and the value of such guidance in the undergraduate learning process. Twelve students preferred the WHO 6-Step Model, often referring to its alignment with clinical practice and the learning it offers for their future professional role. Students perceived both structures as useful for pharmacotherapeutic learning; future studies should examine whether these approaches improve prescribing competence or therapeutic reasoning performance.

8. Efficacy and safety of zilebesiran in adults with hypertension: a systematic review and meta-analysis of randomized controlled trials.
8. zilebesiran在成人高血压中的疗效和安全性:随机对照试验的系统评价和荟萃分析PMID:日期:2026-09-16Hypertension is a major modifiable risk factor for cardiovascular disease, yet global control rates remain low. Zilebesiran, an investigational RNA interference therapy, targets angiotensinogen to modulate the renin-angiotensin-aldosterone system. While previous reviews exist, updated evidence is needed to better characterize its efficacy and safety. A systematic search of PubMed, Cochrane Library, Google Scholar, Embase, and ClinicalTrials.gov was conducted using MeSH and free-text terms. Two reviewers independently extracted data and assessed risk of bias using RoB 2.0. A random-effects model was applied in RevMan 5.4, with heterogeneity evaluated using I² statistics. Four studies involving 1,412 adults with mild to moderate hypertension were included. The primary outcome, placebo-adjusted change in office systolic blood pressure (SBP), showed a pooled mean difference (MD) of -7.16 mmHg (95% CI: -13.63 to -0.70). Secondary outcomes demonstrated significant reductions in 24-hour ambulatory SBP (MD: -9.92 mmHg; 95% CI: -18.74 to -1.10), daytime SBP (MD: -9.76 mmHg; 95% CI: -19.46 to -0.06), nighttime SBP (MD: -9.95 mmHg; 95% CI: -16.71 to -3.19), and 24-hour ambulatory diastolic blood pressure (DBP) (MD: -7.27 mmHg; 95% CI: -11.13 to -3.41). In subgroup analysis by follow-up duration, the reduction in daytime SBP was significantly greater at 3 months than at 6 months (P = 0.02), suggesting a potential attenuation of the antihypertensive effect over time. Zilebesiran markedly reduced angiotensinogen levels (MD: -1.00; 95% CI: -1.15 to -0.85). Overall adverse events were significantly more frequent with Zilebesiran compared with placebo (OR: 1.46), including increased risks of hyperkalemia and injection-site reactions, while serious adverse events did not differ significantly between groups. Overall, zilebesiran produced significant and consistent BP reductions, especially in ambulatory measures, with a favorable safety profile. However, evidence remains limited by small sample sizes and reliance on placebo comparators, and larger trials with active antihypertensive controls are needed to validate long-term efficacy and safety.

9. Real-world pharmacogenomics-guided antihypertensive treatment response in clinical practice underrepresented population: implications for precision prescribing and medication safety.
PMID:日期:2026-09-11Pharmacogenomics-guided therapy is a promising approach for optimizing antihypertensive treatment response; however, robust real-world data from South Asian and particularly Pakistani populations remain scarce. Understanding genotype-drug response relationships in this underrepresented population is essential to inform context-specific precision prescribing strategies. To evaluate associations between pharmacogenomics variants and antihypertensive therapy response using electronic health record (EHR) data from Khyber Pakhtunkhwa (KPK) patients, and to assess the incremental predictive value and practical limitations of genotype-based stratification in routine clinical care. This retrospective cohort study analyzed 1,247 hypertensive patients treated at Kuwait Teaching Hospital (KTH), Peshawar, between January 2023 and August 2025. Hypertensive adults initiating antihypertensive monotherapy with at least 12 months of EHR follow-up and available genotyping results were included to minimize misclassification of treatment response. Five pharmacogenomics variants (CYP3A5 rs776746, ADD1 rs4961, NEDD4L rs4149601, ADRB1 rs1801253, and ACE rs4340) were genotyped using TaqMan assays. Primary outcomes were systolic blood pressure (SBP) reduction and target BP achievement (< 140/90 mmHg), analyzed using ANOVA, multivariable logistic regression, and receiver operating characteristic (ROC) curve analysis. Multiple testing was addressed using a Bonferroni-corrected significance threshold (α = 0.002) for variant-drug class associations, and model discrimination was internally validated using bootstrap resampling (1,000 iterations). Mean patient age was 63.1 ± 11.5 years; 661 (53.0%) were female. Baseline SBP/DBP were 157.1 ± 18.8/91.7 ± 11.1 mmHg. Calcium channel blockers (CCBs) produced greater SBP reduction than other classes (22.32 ± 8.78 vs. 19.68 ± 8.67 mmHg, t = 4.43, p = 1.0 × 10; overall drug-class ANOVA F = 5.27, p = 3.3 × 10, Bonferroni-significant). The CYP3A5 rs776746 *3/*3 genotype was associated with enhanced CCB response (23.96 ± 8.79 vs. 19.78 ± 8.17 mmHg, p = 9.7 × 10), and the ADD1 rs4961 GG genotype with superior diuretic response (24.02 ± 8.84 vs. 17.66 ± 8.69 mmHg, p = 1.7 × 10); both associations remained significant after Bonferroni correction. On direct recalculation from raw genotype counts, four of five variants deviated from Hardy-Weinberg equilibrium (CYP3A5 p = 0.0058, NEDD4L p = 0.0146, ADRB1 p < 0.0001, ACE p = 0.0379), while ADD1 remained in equilibrium (p = 0.554). In multivariable logistic regression, older age (aOR = 1.019, 95% CI 1.007-1.031), lower baseline SBP (aOR = 0.915, 95% CI 0.906-0.925), and absence of diabetes (aOR = 0.670 for diabetes, 95% CI 0.499-0.901) were independent predictors of target BP achievement; CCB therapy (aOR = 1.362, 95% CI 0.975-1.902) and favorable CYP3A5/ADD1 genotypes (aOR 1.14 each) showed positive but non-significant trends. The model achieved an AUC of 0.847, with a bootstrap-derived optimism-corrected AUC of 0.841 (95% CI 0.823-0.866). Real-world EHR data from Khyber Pakhtunkhwa reveal significant genotype-treatment response associations for CCB and diuretic therapy in an underrepresented South Asian population. However, the extensive Hardy-Weinberg disequilibrium observed across four of five variants indicates that population stratification and/or genotyping considerations must be more fully accounted for, and together with the observational design and lack of external validation, these findings should be regarded as hypothesis-generating rather than sufficient grounds for routine clinical implementation of pharmacogenomics-guided antihypertensive therapy.

10. Response to alemtuzumab-based therapy and long-term survival after allogeneic transplantation in T-Cell prolymphocytic leukemia: a systematic review and meta-analysis of proportions.
10. T细胞幼淋巴细胞白血病对基于阿仑单抗治疗的反应及异基因移植后的长期生存:一项比例的系统评价与荟萃分析PMID:日期:2026-09-10T-cell prolymphocytic leukemia (T-PLL) is a rare, aggressive neoplasm with poor outcomes. Alemtuzumab and allogeneic hematopoietic cell transplantation (allo-HCT) are the mainstays of treatment, yet estimates rest on small single-arm studies, and no synthesis has pooled both outcomes. We quantified response to alemtuzumab-based therapy and long-term survival after allo-HCT. We performed a systematic review and meta-analysis of proportions. MEDLINE, CENTRAL, ClinicalTrials.gov, Google Scholar, were searched from inception to June 2026. Studies reporting response to alemtuzumab-based therapy or survival after allo-HCT in adults with T-PLL were eligible. Sensitivity analyses assessed robustness, while publication bias (Egger's test), risk of bias (JBI), and certainty (GRADE) were assessed. Seven studies (200 patients) informed the primary analysis. The pooled overall response rate was 81.9% (95% CI 66.6-91.1; I²=78%) and complete response rate 58.5% (95% CI 39.6-75.2; I²=76%); heterogeneity was explained by line of therapy (frontline 85%, relapsed/refractory 51%; p<0.0001). Across three independent registries (363 patients), long-term survival after allo-HCT was 30.3% (95% CI 25.8-35.3; I²=0%). Alemtuzumab-based therapy was associated with high response rates in T-PLL-approximately 85% frontline-and approximately one-third of transplanted patients achieved long-term survival. These pooled values are descriptive benchmarks derived from low-certainty, single-arm evidence and require prospective confirmation.
