2. Regulatory T cells in autoimmune liver diseases.
2. 自身免疫性肝病中的调节性T细胞PMID:日期:2026-10-01Autoimmune liver diseases (AILDs), including autoimmune hepatitis, primary biliary cholangitis, primary sclerosing cholangitis and IgG4-related sclerosing cholangitis, are characterised by immune dysregulation that adversely affects the hepatocytes or bile ducts. Regulatory T cells (Tregs), which are pivotal for immune tolerance, display altered function and abundance across these disorders, with notable differences between peripheral and hepatic environments. This review synthesises current evidence on disease-specific Treg alterations across AILDs, integrating findings from human studies and preclinical models. We highlight heterogeneity in Treg abundance, phenotype and function across disease subtypes, stages and tissue compartments, while examining the influence of the local immune microenvironment and gut-liver axis on Treg stability and function. We further evaluate current and emerging Treg-targeted therapeutic strategies, ranging from restoration of Treg abundance and function to disease-specific modulation of Treg responses, together with the major challenges to their clinical translation. By integrating these findings, we provide a disease-dependent and context-dependent framework for understanding Treg dysregulation in AILDs and identify key priorities for future mechanistic and translational research.

4. Clinical impact of prospective circulating tumour DNA testing for minimal residual disease in colorectal cancer: the INTERCEPT programme experience.
PMID:日期:2026-10-01The impact of circulating tumour DNA (ctDNA) detection, which informs minimal residual disease (MRD) after curative treatment, remains debated especially for stage IV patients. The study objectives were to evaluate the prognostic role of MRD and to estimate lead time (LT) at different timepoints when adopting longitudinal MRD testing. INTERCEPT, is a prospective, observational cohort study of stage II-IV patients with colorectal cancer receiving curative-intent treatment who are longitudinally monitored using tissue-informed MRD assays as part of routine clinical care. Study results confirm the prognostic value of MRD, also beyond 12 months from surgery, with higher recurrence incidence in MRD-positive stage II-III patients than in MRD-negative stage IV patients (p<0.0001). Adjuvant therapy in stage IV disease showed limited durable benefit in both MRD-positive and negative patients. Clinical management was associated with increased use of more intensive second-line imaging (MRI, positron emission tomography; p<0.0001) when ctDNA was detected. Concurrent macroscopic disease detection was observed in 26% of cases and when excluded, LT from MRD detection to radiographic recurrence varies during surveillance (p=0.007). Multitimepoint testing improves sensitivity for recurrence (79.9% vs 48.5%) and may enable earlier, potentially curative interventions. ctDNA testing provides a framework for risk stratification and defines temporal windows that may inform future interventional strategies. Prospective interventional studies are required to define clinical utility and optimal implementation.

8. Hypothermic oxygenated and normothermic machine perfusion mitigate innate systemic and hepatic inflammation after liver transplantation.
8. 低温氧合机器灌注和常温机器灌注减轻肝移植后的先天性全身性和肝脏炎症PMID:日期:2026-09-29Immune-mediated injury drives ischaemia-reperfusion injury (IRI) following liver transplantation. While hypothermic oxygenated (HOPE) and normothermic machine perfusion (NMP) improve clinical outcomes, their immunological effects in humans remain unclear. To examine hepatic and systemic immune responses across preservation strategies in a randomised controlled clinical setting. In this mechanistic substudy of the HOPE-NMP randomised controlled trial (NCT04644744), we analysed immune responses in n=47 recipients of extended criteria donor livers from brain-dead donors randomised to static cold storage (SCS), end-ischaemic HOPE or end-ischaemic NMP. Liver biopsies were obtained at organ arrival and early IRI. Blood samples were collected preoperatively, during reperfusion and on postoperative days 1, 2, 3 and 7. Analyses included spectral flow cytometry, multiplex immunofluorescence with spatial analyses, cytokine profiling, cellular bioenergetics and mass spectrometry. In human liver allografts, both HOPE and NMP reduced neutrophil infiltration during early IRI, without altering intrahepatic spatial distribution. Machine perfusion induced phenotypic modulation of graft-infiltrating myeloid cells, characterised by downregulation of pro-inflammatory and adhesion markers. Peripheral lymphocyte numbers, particularly of NK cells and T cells, normalised faster after HOPE, compared with SCS and NMP groups, correlating with reduced liver injury. Absolute and relative systemic neutrophil numbers were lower in HOPE-recipients. Machine perfusion decreased the inflammatory activation of circulating myeloid and innate lymphoid cells, while inhibitory immune checkpoints were upregulated postoperatively. In a randomised controlled trial using human samples, HOPE and NMP differentially modulate hepatic and systemic immune responses, attenuating neutrophil-driven inflammation and promoting immune regulation after liver transplantation.

10. Expert consensus on clinical trials for use of systemic therapy in patients with hepatocellular carcinoma undergoing surgical and locoregional therapies.
10. 肝细胞癌患者接受手术和局部治疗后系统治疗临床试验的专家共识PMID:日期:2026-09-24Advances in systemic therapy for advanced hepatocellular carcinoma (HCC) have fuelled a growing interest in evaluating these therapies in earlier stages of disease. However, trial design remains challenging due to the marked clinical and biological heterogeneity of early-stage and intermediate-stage HCC. A multidisciplinary group of cancer experts was convened by the HCC-Live Consortium to discuss inclusion criteria, risk stratification, treatment arms and appropriate endpoints of trials evaluating perioperative and combination therapies in patients with HCC. Consensus statements were defined as those obtaining >70% approval by panellists. The panel achieved consensus regarding nine consensus statements for neoadjuvant or perioperative systemic therapy and nine statements for combination locoregional plus systemic therapy. Statements emphasised recruiting carefully selected patient populations, employing treatment strategies and duration that balance efficacy with safety and using clinically relevant endpoints. Trials evaluating neoadjuvant/perioperative approaches should include patients at the highest risk of recurrence, with a recommended primary outcome of 2-year event-free survival using restricted mean survival time. Trials evaluating combination therapies should include a homogeneous patient population and use an appropriate comparator arm that aligns with clinical practice. Specifically, systemic therapy was the recommended comparator arm for patients with larger tumour burden and locally advanced disease. The recommended primary outcome for combination trials was progression-free survival and overall survival, with an alpha-recycling strategy. Secondary endpoints are essential to inform clinical applicability. Optimal design of HCC trials is important to rigorously evaluate the role of systemic therapy in patients undergoing surgical resection or in combination with locoregional therapy.
