3. Utility of E-Nose in Paediatric Respiratory Diseases Using Machine Learning Approach: A Pilot Study.
3. 使用机器学习方法在儿童呼吸系统疾病中使用电子鼻:一项试点研究PMID:日期:2026-10-01To assess the feasibility of an electronic nose (e-nose) for volatile organic compounds (VOCs) to classify respiratory diseases and establish a protocol for use. We analysed exhaled air samples with Cyranose 320 nose from 67 children (14 healthy, 34 asthmatics and 19 other respiratory diseases) (mean age 11.4 years). Three samples were collected for each patient. We used machine learning models to generate an algorithm to classify children according to disease. There were significant differences in 30/32 sensors between plastic and tedlar bags. The Cronbach alpha was <0.5 in all cases, so that the consistency of the measurements was low. An analysis was performed to distinguish between healthy and asthmatics, < .05 in 27/32 sensors. The algorithm predicted asthma disease with high sensitivity (over 99%) and accuracy (over 96%). The negative and positive predictive values were over 90%. The e-nose seems a valuable tool for screening asthma disease in the paediatric population in clinical practice.

4. Effect of Preferred Language on Pediatric Rapid Response Activation and Subsequent Intensive Care Utilization.
PMID:日期:2026-10-01Health care disparities stemming from language discordance have been well established, leading to increased morbidity and mortality. We sought to examine the effect of patient preferred language on time to rapid response activation and subsequent intensive care utilization. We sought to evaluate the association between patient preferred language and time to rapid response (RR) activation, intensive care unit (ICU) transfer rate, and intensive care utilization in a quaternary pediatric hospital. We conducted a retrospective observational study of pediatric patients with rapid response activation from January 2020 to June 2023. Demographic and clinical data, including time to rapid response, ICU transfers, and use of mechanical ventilation, non-invasive ventilation, and vasoactive support, were analyzed. Statistical comparisons were performed between English-speaking and language other than English (LOE) patients, with subgroup analyses for Spanish-speaking patients. Of 374 unique patients, 25% were using LOE. Time to rapid response activation was not significantly different between language groups. However, Spanish-speaking patients were more likely to be transferred to the ICU ( = .0047) and had longer hospital stays (10 vs 7 days, = .0425). While language preference did not impact time to rapid response activation, Spanish-speaking patients had increased ICU transfers and prolonged hospital stays. These findings highlight the need for improved language-concordant care to mitigate disparities in pediatric critical care outcomes.

6. Virtual Reality Education for Hospitalized Pediatric Patients Improves Intrinsic Motivation: A Prospective, Randomized Crossover Study.
PMID:日期:2026-10-01Hospitalized children experience educational disruption and reduced motivation during prolonged admissions. Despite the availability of in-hospital schools, many cannot participate due to illness severity or isolation precautions. This pragmatic, randomized crossover trial evaluated whether bedside virtual reality (VR) lessons improve intrinsic motivation among inpatients aged 5 to 25 years unable to attend the hospital school. Participants completed both VR-based educational sessions and standard of care (SOC) conditions on consecutive days. The primary outcome was intrinsic motivation, with secondary outcomes including educational self-efficacy, well-being, self-esteem, and parental experience. Of 156 enrolled patients, 100 were analyzed (mean age 11.5 [SD = 4.1 years]; 43.9% female). Intrinsic motivation scores were higher in the VR condition than SOC ( = .0048), and parental satisfaction was also greater ( = .0008). Bedside VR education represents a feasible, acceptable, and scalable adjunct to inpatient learning, supporting intrinsic motivation and caregiver satisfaction during hospitalization. 7. Pediatric Invasive Group A Streptococcus Characteristics Before, During, and After COVID-19.
PMID:日期:2026-10-01Pediatric invasive Group A Streptococcus (iGAS) infections two years before, during, and a year after the Coronavirus Disease 2019 (COVID-19) pandemic were analyzed in an urban hospital in the Northeastern United States. Invasive GAS was diagnosed in 38 patients. There were as many infected children post-COVID as in the prior 4.5 years combined. The post-pandemic surge was most striking in late spring. The median age was youngest during the post-pandemic year at 2.3 years. During and after the pandemic, about half the children had concurrent virus detection, and two-thirds had no known chronic comorbidities, highlighting potential for significant morbidity in healthy and young children. The spectrum of iGAS disease manifestations was similar among the 3 time periods, with soft tissue infections predominating overall. Of post-pandemic GAS isolates, 67% were 1 and 33% were 12. Preventive efforts of this grave disease at all ages should continue.

8. Lichen Sclerosus: A Common but Often Overlooked Condition in Pediatric Patients.
PMID:日期:2026-10-01Lichen sclerosus (LS) is a chronic inflammatory dermatosis that is frequently underdiagnosed in pediatric patients, especially girls presenting with persistent urinary symptoms. We aimed to characterize the clinical features and treatment outcomes in children with LS who presented with urinary symptoms. This retrospective study evaluated 88 children diagnosed with LS at a pediatric nephrology center, all of whom presented with urinary symptoms. Diagnosis was based on clinical and dermatoscopic findings. The mean age at diagnosis was 6.0 years, with a median delay of 8 months. Dysuria (79.5%) and pyuria (87.5%) were the most common symptoms, although only 45.5% had confirmed urinary tract infections. Among 60 patients followed (mean duration = 16.8 months), topical pimecrolimus resolved symptoms in 88.3% and improved skin lesions in 66.6%. Lichen sclerosus should be included in the differential diagnosis for children with urinary symptoms. Clinical awareness and early diagnosis can lead to avoiding unnecessary investigations.

9. Home Observation for Measurement of the Environment: Collecting Data Virtually.
PMID:日期:2026-10-01This study examined the feasibility and the reliability of a virtual assessment of the home environment using the widely used measure of the home environment, the Home Observation for Measurement of the Environment (HOME) Inventory. A convenience sample of 53 families were assessed with the virtual and in-person HOME. Children were ages 6 to 36 months and lived in 2 states in the United States. When examining HOME dimensions, the mean level of agreement was not statistically different from 85% except for the Involvement dimension. After having received both an interview in-person and virtually, most parents reported preferring the virtual (63.3%) over the in-person interview (36.7%). Results from this study show that the HOME Inventory can be administered virtually for many families and that the data collected is useful for research and applied purposes.

10. The Impact of Parental Education on Atopic Diseases in Children Aged 10 to 12 Years: A Cross-Sectional Study.
10. 父母教育对10至12岁儿童特应性疾病的影响:一项横断面研究PMID:日期:2026-10-01The objective of this study was to examine the association between parental education and the occurrence of asthma, allergic rhinitis, and eczema in children, while accounting for potential confounding factors. Data were collected through questionnaires completed by 379 children aged 10 to 12. Maternal, paternal, and combined parental educational levels were assessed. Logistic regression models were used to evaluate associations with the odds of atopic diseases, adjusting for confounders. High parental educational level was significantly associated with a lower occurrence of allergic rhinitis in children (adjusted odds ratio [aOR] = 0.52, 95% CI: 0.27-0.99, = .048), particularly when mothers had high, and fathers had low educational levels (aOR = 0.30, 95% CI: 0.13-0.71, = .006). In addition, children with highly educated fathers were less likely to develop eczema (aOR = 0.56, 95% CI: 0.33-0.93, = .028). These findings suggest that parental education influences atopic diseases in children, reflecting the complex role of social determinants of health.
