1. Congenital diaphragmatic hernia: a comprehensive review of pathogenesis, genetics, clinical features, and risk stratification.
PMID:日期:2026-08-12Congenital diaphragmatic hernia (CDH) is a rare and complex developmental disorder characterized by diaphragmatic defects, pulmonary hypoplasia, and persistent pulmonary hypertension, leading to significant neonatal morbidity and mortality. This review summarizes current knowledge on the pathogenesis, genetic background, clinical features, and risk stratification of CDH. The etiology is multifactorial, involving genetic mutations, chromosomal abnormalities, and environmental influences, with key molecular pathways including the retinoic acid and Sonic Hedgehog signaling pathways. Advances in pre-natal diagnosis, particularly the ultrasound-based lung-to-head ratio and magnetic resonance imaging-derived total fetal lung volume, have improved prognostic assessment. Post-natal risk stratification using clinical parameters and scoring systems further guides management and predicts outcomes. Despite improvements in neonatal intensive care and surgical techniques, long-term morbidity remains a major concern. A comprehensive understanding of CDH pathophysiology and the integration of pre-natal and post-natal risk assessment are essential for optimizing individualized management strategies and improving survival and long-term outcomes.

2. Oral cavity cancer: retrospective analysis of a tertiary level hospital.
PMID:日期:2026-07-03To describe the clinical characteristics, treatment patterns, and survival outcomes of patients with oral cavity cancer treated at a tertiary care center. A retrospective study was conducted on 224 patients with oral cavity cancer treated between 1988 and 2023. Clinical, pathological, and therapeutic data were collected, evaluating survival through Kaplan-Meier curves and Cox regression models. The median overall survival was 92.3 months, with a better prognosis in early stages (111.6 months in stage I) and in patients treated surgically. Non-SCC tumors showed significantly greater survival. The use of induction chemotherapy followed by radiotherapy notably improved survival in advanced disease (203.9 months). Surgery was confirmed as the most effective strategy in the early stages. Age, functional status, histology, and clinical stage at diagnosis are relevant prognostic factors. Surgery remains the cornerstone of treatment, and in locally advanced disease, induction chemotherapy is an option.

3. Volumetric computed tomography Hounsfield unit analysis: a superior method for opportunistic osteoporosis screening and assessment.
PMID:日期:2026-06-17This study investigated the correlation between two-dimensional and volumetric Hounsfield unit (HU) measurements from computed tomography (CT) and T-scores from dual-energy X-ray absorptiometry (DXA), assessing their interchangeability for opportunistic osteoporosis (OP) screening. A retrospective analysis included 97 patients (15 males, 82 females; mean age 66.48 ± 9.97 years) who underwent lumbar CT and DXA within 7 months. Two-dimensional HU values were measured from axial images (L1-L4), whereas volumetric HU values were obtained using 3D segmentation. Correlations with DXA T-scores were analyzed, and agreement between methods was assessed using Bland-Altman analysis and intraclass correlation coefficients (ICC). Both HU methods significantly correlated with DXA T-scores (p < 0.001), with volumetric HU showing stronger correlations (r = 0.572 vs. r = 0.532). Osteoporotic patients had lower HU values, with volumetric HU offering better differentiation. Bland-Altman analysis revealed a systematic bias, with two-dimensional HU underestimating values. ICC showed excellent agreement, with volumetric HU achieving higher reliability (ICC = 0.963 vs. ICC = 0.918). Volumetric HU measurements provide a more reliable and sensitive assessment of trabecular bone density, making them a superior method for opportunistic OP screening and treatment monitoring.

4. Pre-operative differentiation of atypical parathyroid tumors and parathyroid adenomas: Is it possible?
PMID:日期:2026-05-28This study aimed to determine whether atypical parathyroid tumors (APTs) can be differentiated from benign parathyroid adenomas (PAs) preoperatively and to compare their biochemical characteristics, post-operative complications, and long-term outcomes. A retrospective analysis was conducted on patients who underwent parathyroidectomy between 2010 and 2022. Fifty patients with APT and one hundred with PA (control group) were included. Demographic data, biochemical parameters, complications, and follow-up outcomes were compared. Pre--operative serum calcium and parathyroid hormone (PTH) levels were significantly higher in APT patients. Receiver operating characteristic analysis identified Ca > 11.5 mg/dL and PTH > 180 pg/mL as thresholds for distinguishing APTs. Multivariate analysis confirmed these as independent predictors. APT patients had a higher rate of concurrent thyroid surgery and more frequent complications such as transient nerve injury and hungry bone syndrome. No significant difference was found in persistent disease or recurrence after an average follow-up of 4.8 years. Pre-operative Ca and PTH levels may help distinguish APTs from PAs. Patients with APT should be informed about the increased risk of complications and concurrent thyroid surgery, and appropriate preventive strategies should be considered.

5. Relationship between lipid profile and liver enzymes in cholelithiasis, cholesterolosis, and cholesterol polyp cases.
PMID:日期:2026-05-22The aim of this study is to investigate the relationship between liver enzymes and lipid profiles in cholecystectomy cases with cholecystitis, cholesterolosis, and cholesterol polyps. The cholecystectomy cases were divided into 3 groups according to their pathological diagnosis; cholesterolosis and cholelithiasis (Group 1), cholesterolosis (Group 2), and cholesterolosis and cholesterol polyp (Group 3). The groups were subjected to statistical analysis based on age, gender, cholesterol, triglyceride, low-density lipoprotein, high-density lipoprotein (HDL), alkaline phosphatase, gamma-glutamyl transferase (GGT), and C-reactive protein values. Of the 227 cases included, 39 (17.2%) were male, and 188 (82.8%) were female. HDL value was significantly lower in men than in women (p = 0.00), while GGT (p = 0.00), aspartate aminotransferase (p = 0.01), and alanine aminotransferase (p = 0.00) values were significantly higher in men. No significant differences were detected in comparisons between pathological diagnostic groups (all p > 0.05). No statistically significant findings were found regarding lipid profile and liver enzyme levels in cases of cholesterolosis, cholelithiasis, and cholesterol polyps. Although it is predicted that the HDL level being significantly lower in men than in women will increase the risk of polyp, no significant difference was detected between the pathological diagnosis groups in terms of gender. Studies containing larger case series are needed to reveal this relationship more clearly.

6. Impact of prolonged surgical mask versus KN95 respirator use on tear film stability in healthcare workers.
PMID:日期:2026-05-22This study aimed to evaluate and compare the effects of prolonged daily wear of a surgical mask (SM) versus a KN95 respirator on tear film stability in healthcare workers. Forty-eight healthcare workers (96 eyes) were randomly assigned to wear either an SM or a KN95 respirator. Corneal fluorescein staining (FS), tear film break-up time (BUT), Schirmer I test (SIt), and the ocular surface disease index (OSDI) questionnaire were assessed at baseline without a mask (T0), after 8 h of continuous wear (T1), and after 1 week of daily wear (≥ 8 h/day, T2). In the SM group, the FS score at T2 was significantly higher than at T0 and T1 (both p < 0.05). In the KN95 group, the FS score at T2 was significantly higher than at T0 (p < 0.05). BUT decreased significantly and progressively from T0 to T2 in both groups (all p < 0.0167). No significant changes in SIt or OSDI scores were observed over time within either group. Furthermore, no significant differences in FS, BUT, SIt, or OSDI scores were found between the two mask groups at any time point. Prolonged daily use of both SM and KN95 respirators over 1 week negatively affected tear film stability in healthcare workers, as indicated by a significant reduction in BUT and altered FS scores. The similar magnitude of effect for both mask types suggests that the impact is related to mask wear in general rather than to a specific design. These preliminary findings warrant confirmation in larger, controlled studies with longer follow-up periods.

7. Evaluation of dentists' knowledge and attitudes regarding ankyloglossia in children.
PMID:日期:2026-05-18Ankyloglossia, known as tongue-tie, is a common congenital malformation that restricts the movement of the tongue. It is seen in 4.8% of newborns and is more prevalent in males compared to females. This study aims to evaluate the knowledge and attitudes of dentists in Turkey regarding ankyloglossia. Our survey was conducted online with dentists in Turkey who examine and treat patients. The data were analyzed using IBM Statistical Package for the Social Sciences V.21 software. Frequency distributions and percentages were examined, and relationships between nominal variables were assessed using the χ2 test. A significance level of p < 0.05 was accepted. The results showed that 76.14% of the participants could recognize ankyloglossia in children, and 87.14% believed it to be a congenital anomaly. In addition, 91.49% of the participants thought that ankyloglossia negatively affects breastfeeding, and 98.34% believed that it could cause speech disorders. Our study emphasizes the importance of increasing awareness of ankyloglossia and the significance of early diagnosis and treatment.

8. [Sensitivity and validity of sentinel node biopsy in early-stage cervical cancer at an oncology institution].
PMID:日期:2026-05-13To determine the validity of sentinel lymph node biopsy (SNB) in women with early-stage cervical cancer (CeCa). We carried out a study of 46 patients with CeCa of FIGO stages IA1c-IB2 treated with radical uterine surgery. The technique for the identification of BGC was performed with patent blue dye and technetium-99m. For its validation, a bilateral pelvic lymphadenectomy (BPL) was performed, and all sentinel lymph nodes underwent pathological ultra-staging. We determined the detection rates of SNB, bilaterality, sensitivity, specificity, and negative and positive predictive values. The sensitivity of the SNB was 100%, specificity was 95%, positive predictive value was 75%, negative predictive value was 100%, diagnostic accuracy was 93%, and the false negative rate was 0%. The BGC technique in patients with CeCa in the early stages who undergo radical hysterectomy is a feasible and safe method at Instituto Nacional de Cancerología. The importance of this method is that BPL can be avoided when it migrates bilaterally, reducing the morbidity of the procedure.

9. Traumatic intracranial pseudoaneurysm treated with flow diverters: Case report and meta-analysis.
PMID:日期:2026-01-01It is controversial to treat traumatic intracranial pseudoaneurysms (TIPAs) with flow diverters (FDs). We present an 18-year-old girl who underwent a transcranial carotid-cavernous fistula of ruptured TIPA treated with FDs, and then report the results of the largest meta-analysis aimed to evaluate the safety and effectiveness of flow diversions in this situation. A systematic search of Web of Science databases, Embase, and PubMed was performed for articles published between 2010 and September 2022. According to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, studies reporting FD for TIPA with ≥ 3 patients were included. We assessed the complete occlusion rate, complications related to the operation, and clinical outcome. A meta-analysis based on heterogeneity was carried out using random or common effects models. We included 7 non-comparative studies with a total of 58 target TIPAs. 17 aneurysms were explicitly described as being ruptured. All FDs were successfully released. Overall, 5 (8.6%) aneurysms were treated with overlapped diverters. During a mean angiographic follow-up of 17.3 months, the complete occlusion rate was 86.0% (46/56; 95% confidence interval [CI], 74-95%; I = 7%; p = 0.37). The operation -related complications rate was 9% (95% CI = 1-23%; I = 0.00%; p = 0.52). Two patients died of javascript: void(0); non-aneurysmal hemorrhage. The percentage of good clinical outcomes was 85.7% (95% CI = 78-98%; I = 23.7%; p = 0.25) at the last follow-up. FD provides significant advantages in the treatment of TIPA. Nevertheless, operation-related complications are not negligible. Additional prospective and randomized investigations are needed to determine the long-term effects of FD on TIPA.

10. Effects of breast milk nutrition on chronic adenoiditis, chronic tonsillitis and allergy.
PMID:日期:2026-01-01It is aimed to reveal whether breast milk has an effect against recurrent upper respiratory tract infections and allergies. The study group consisted of 300 children aged 0-15 years who applied to the Ear, Nose, and Throat Outpatient Clinic with complaints of frequent throat infections, snoring, sleeping with an open mouth, and decreased hearing. The control group consisted of 300 randomly selected healthy children between the ages of 0-15 whose routine controls were terminated. Both groups were compared in terms of age, gender, birth weight, duration of breast milk and formula use, eosinophil count in complete blood count, and time of transition to solid food. It was observed that more operation (adenotonsillectomy) were performed in children with lower than normal birth weight (< 2500 g), children who had breastfed for < 6 months, and children who received formula for more than 6 months (p < 0.001). Eosinophil count was found to be higher in the complete blood count of the children in the study group compared to the children in the control group (p < 0.001). Children are protected from recurrent adenotonsillar infections and allergies with breast milk.
