Prevention and management of venous thromboembolism in pregnancy: cutting through the practice variation. 妊娠期静脉血栓栓塞症的预防和管理:消除实践差异
There is clinical practice variation in the area of prevention and management of venous thromboembolism (VTE) in pregnancy. There are limited data and differing recommendations across major clinical practice guidelines, especially relating to the role of postpartum low-molecular-weight heparin (LMWH) for patients with mild inherited thrombophilia and those with pregnancy-related VTE risk factors. This chapter explores the issues of practice variation and related data for postpartum VTE prevention. Controversial topics of VTE management in pregnancy are also reviewed and include LMWH dosing and the role of anti-Xa level monitoring, as well as peripartum anticoagulation management around labor and delivery.

Epidemiology, risk factors and sequelae of venous thromboembolism. 静脉血栓栓塞症的流行病学、危险因素和后遗症
The aim of this review was to discuss the epidemiology, risk factors and sequelae of venous thromboembolism (VTE). VTE has an incidence of 1-2 per 1000 people annually. The risk of VTE increases with age and is highest in Caucasians and African Americans. Combined oral contraceptives (COC), especially the third-generation COCs, have been strongly implicated in VTE. Hospitalized patients, especially patients with underlying malignancy and undergoing surgery, have a host of risk factors for VTE. Thrombophilia can predispose an individual to VTE but indiscriminate testing for thrombophilia in patients presenting with VTE is not indicated. VTE can have serious chronic sequelae in the form of post-thrombotic syndrome (PTS) and chronic thromboembolic pulmonary hypertension (CTPH). The risk of PTS and CTPH is increased with recurrent deep vein thrombosis and pulmonary embolism, respectively. Mortality from VTE can be as high as 21.6% at one year. Patients who had an episode of VTE have a high risk of subsequent VTE and this risk is highest in patients who had a first VTE event associated with malignancy. A good understanding of the epidemiology and risk factors of VTE will enable the treating medical practitioners to identify patients at risk and administer appropriate VTE prophylaxis to prevent the long-term consequences of VTE.

Prevention of venous thromboembolism in the cancer surgical patient. 癌症手术患者静脉血栓栓塞症的预防
Venous thromboembolism is a common complication in patients with malignant disease. One of the environments in which patients can present with symptomatic thromboembolic disease is in the postoperative period. Operation in the patient with cancer increases the risk of thromboembolic complications some two to three fold. A variety of methods have been evaluated for the prevention of thromboembolic disease in cancer surgical patients. The most extensively investigated are the pharmacologic methods, including low-dose unfractionated heparin and low molecular weight heparin. These agents are recommended for the prevention of thromboembolic disease during hospital stay. For selected high-risk populations, extended thromboprophylaxis into the postdischarge period is also recommended.

Mechanisms and treatment of venous thromboembolism in patients with brain cancer: a narrative review. 脑癌患者静脉血栓栓塞症的机制和治疗:叙述性综述
Malignant brain tumors, including both primary brain cancer and metastatic brain cancer, are associated with a markedly increased risk of venous thromboembolism (VTE). Anticoagulation is challenging in this population, as these patients are not only at risk of thrombotic complications but also at high risk of bleeding. In this review, we examine current knowledge on the incidence, risk factors, pathophysiology, and management of brain cancer-associated VTE. In primary brain cancer, particularly in glioblastoma, expression of the procoagulant proteins podoplanin and tissue factor by tumor cells was found to be an important pathophysiological driver of hypercoagulability. Expression of these prothrombotic factors was found to be dependent on the genetic profile of the brain tumor. Clinical data on the treatment of VTE in brain cancer are limited and mostly based on observational studies. The risk of intracranial hemorrhage during anticoagulation remains a key concern. Data from retrospective studies suggest that direct oral anticoagulants may be associated with a lower bleeding risk than low-molecular-weight heparins. Pharmacological thromboprophylaxis in the ambulatory setting is not routinely recommended, largely due to the lack of trial data in this population. Future studies are needed to improve risk prediction, to clarify the underlying mechanisms of brain cancer-associated VTE, and to define safe and effective treatment strategies.

Development of a machine learning-based predictive model for venous thromboembolism risk assessment in orthopaedic patients with routine prophylaxis. 基于机器学习的预测模型用于接受常规预防的骨科患者静脉血栓栓塞症风险评估的开发
Despite the use of conventional preventive measures, the long-term risk of the development of venous thromboembolism (VTE) in orthopaedic patients remains high in a high-risk patient population. Accurate risk assessment is critical; however, existing assessment tools appear to have certain limitations, and machine learning (ML) models appear to have higher predictive accuracy. Develop an ML model with clinical features to predict VTE in orthopaedic patients on standard prophylaxis. We used 147 clinical variables with XGBoost and CatBoost models for VTE risk prediction, comparing their performance with the Caprini score. Both internal and external validations were conducted to assess the model's efficacy. SHapley Additive exPlanation (SHAP) values were employed to improve interpretability and accurately evaluate predictive efficacy. Using 8182 patients (153 VTE cases), XGBoost and CatBoost achieved internal Area Under the ROC curves (AUCs) of 0.941 and 0.937. In external validation (2121 patients; 31 VTE cases), AUCs were 0.888 and 0.902. They outperformed traditional methods with high accuracy, balanced sensitivity and specificity. SHAP analysis showed feature importance and VTE correlation across algorithms. This study used two models with clinical features to improve VTE risk prediction accuracy in orthopaedic patients under conventional prevention. The models identified VTE risk factors and highlighted key preventive measures.

Multimorbidity defined as the co-occurrence of two or more chronic comorbidities, is becoming increasingly burdensome and is a big challenge for healthcare systems all over the world. Venous thromboembolism (VTE) is a potentially lethal disease and is the third most common cardiovascular disease. Multimorbidity is closely associated with VTE, and the VTE risk is approximately fourfold higher in individuals with multimorbidity compared to those without. Notable and consistent evidences show a significant association between multimorbidity and VTE. Plausible mechanisms for the observed associations between multimorbidity and VTE have been outlined, including higher prevalence of identified VTE risk factors, organ function and coagulation function disorders, reduced physical activity, older age, low cognitive level of VTE, and complications following the multimorbidity. Worse therapeutic and prophylactic anticoagulation efficacy, and safety are suggested by the studies, and the VTE recurrence and bleeding risk are higher in patients with multimorbidity compared to those without. Management of the therapeutic and prophylactic anticoagulation for VTE in patients with multimorbidity is difficult, and a balanced and detailed evaluation of the risks of VTE and bleeding is needed, and antiplatelet medications, increased doses or alternative direct oral anticoagulants (DOACs), thromboelastography (TEG), and physical activity may be helpful.

Overuse of venous thromboembolism prophylaxis among hospitalized patients with liver disease. 肝病患者住院期间静脉血栓栓塞症预防的过度使用
Patients with liver disease are at risk of venous thromboembolism (VTE); however, little is understood regarding the safety and efficacy of VTE prophylaxis in patients with cirrhosis. We examined the application of a VTE risk assessment model in VTE prophylaxis decision-making in a closed cohort of hospitalized patients with liver disease. Sequential patients admitted to an inpatient hepatology service at a tertiary care center were evaluated for need for VTE prophylaxis. Risk assessment by IMPROVE was compared with current practice patterns of VTE prophylaxis. Rates of bleeding and clotting events were noted. 98 patient encounters were included in our analysis. 76% of patients received VTE prophylaxis in practice. IMPROVE recommended use of VTE prophylaxis in 19% of patients. Patients who received VTE prophylaxis that was not warranted had significantly lower risk of clotting compared with patients in whom VTE prophylaxis was warranted per IMPROVE. Application of IMPROVE risk assessment would significantly reduce VTE prophylaxis use among hospitalized patients with liver disease. Our findings challenge the "one-size-fits-all" current practice pattern of VTE prophylaxis. Future studies are needed in large cohorts of hospitalized patients with liver disease that include clinical outcomes of bleeding and clotting risk.

Dysfunctional prothrombin residue Arg596 associated mutation has been found to precipitate venous thromboembolism (VTE). In the current study we investigated the prevalence of Arg596 associated mutations in Chinese patients with VTE and explored the functional impact of Arg596Gln mutation on coagulation function in affected patients. Prothrombin clotting activity was measured in 267 unrelated patients with unprovoked VTE. Patients with moderately decreased activities underwent further analysis of the gene. Prothrombin amidolytic activity and antigen levels were detected in mutation carriers. Specific family members were investigated about their VTE histories and clinical phenotypes. The thrombin generation test (TGT) was used to evaluate thrombin function and antithrombin resistance assay was applied to assess the extent of impaired antithrombin inhibition of mutation carriers. Two heterozygous mutation carriers of prothrombin Arg596Gln were identified, both of whom had moderately decreased clotting activities but normal amidolytic activities and antigen levels. Among the families of the two probands, nine out of 13 mutation carriers experienced episodes of VTE. TGTs showed that patients had elevated endogenous thrombin potential and prolonged start tail time. Thrombin generation could be inhibited in the presence of thrombomodulin. The thrombin Arg596Gln variant in patients' plasma presented strong resistance to antithrombin inhibition. Prothrombin Arg596Gln mutation is a risk factor for Chinese patients with VTE due to its moderately decreased clotting activity but strong resistance to antithrombin inhibition. Prothrombin clotting activity screening and its encoding gene sequencing should be considered in patients with VTE when other established risk factors are absent.

Retrospective evaluation of venous thromboembolism: Are all transient provoking events the same?
Venous thromboembolism (VTE) provoked by transient risk factors has traditionally been classified as a single entity with lower risk of recurrence. We evaluated the association between different categories of transient provoking factors and the relative risk of recurrence. Retrospective evaluation of VTE events in non-cancer patients from July 2011 to December 2012 at two tertiary institutions in Australia with a minimum follow-up of 24 months. A total of 747 VTE cases were identified, and following exclusion of cases with mortality within 30 days of presentation (n=26), unprovoked cases (40.2%) had a higher risk of recurrence (4.6 vs 2.3/100 event-years, P=.01). Provoking factors included surgery (40.4%), injury (16.7%), medical-related factors including non-surgical hospitalisation or active infection (22.0%), travel (13.2%) and oestrogen related (6.5%). Air travel had the highest recurrence rate of 5.9/100 event-years, comparable to unprovoked VTE. VTE provoked by surgery showed lower recurrence rate at 1.8/100 event-years (P=.03). 62.5% of patients with provoked VTE recurred with an unprovoked event. Transient provoking factors for VTE are heterogeneous with varying potency and should not be considered a single entity. The high recurrence rate after travel-provoked VTE suggests that it is a "minor," if not negligible provoking factor with higher thrombotic predisposition.

Venous thromboembolism (VTE) is the third most common cardiovascular disorder, affecting up to 5% of the population. VTE commonly manifests as lower-extremity deep venous thrombosis (DVT) or pulmonary embolism. Half of these events are associated with a transient risk factor and may be preventable with prophylaxis. Direct oral anticoagulants are effective and safe and carry a lower risk for bleeding than vitamin K antagonists. Many patients with VTE will have a chronic disease requiring long-term anticoagulation. Postthrombotic syndrome affects 25% to 40% of patients with DVT and significantly impacts function and quality of life.
