Evaluation of the efficacy of immunotherapeutic strategies aimed at achieving durable antiretroviral treatment (ART)-free control within a clinical trial relies on analytical treatment interruptions (ATIs). ATIs involve stopping ART to assess post-intervention efficacy with frequent viral load monitoring. These trials rely on participant commitment but also involve uncertainty about the timing and experience of viral rebound, transmission risk, and impacts on personal and sexual relationships. Understanding participants' lived experiences is critical to participant involvement and retention and to designing ATI studies that are ethical, feasible, and acceptable. We conducted a qualitative study involving semi-structured interviews with 20 cisgender men living with HIV enroled in the phase 2 double-blinded randomised placebo-controlled RIO trial of HIV-specific broadly neutralising antibodies between January and May 2025 in the United Kingdom. We transcribed, coded and analysed the interviews using thematic analysis. The participants described being motivated by scientific altruism but had worries about stopping ART. During the ATI, participants found breaking their daily pill-taking routine challenging, yet a majority described feelings of liberation thereafter. The participants preferred once weekly viral load testing, as it reassured safety, which was coupled with support from researchers, who buffered anxiety during periods of viral rebound. ATIs had a notable impact on personal relationships, including avoidance of dating, reduced intimacy and sexual pleasure while off ART. Easy access for partners without HIV to free PrEP, PEP, and condoms was welcomed by all participants. Restarting ART was difficult following the ATI, but was resolved with pre-trial preparation supporting adherence. The participants with lengthy ATIs remained enthusiastic but were challenged by uncertainty that required adjustment of their life plans. Finally, participants expressed mixed views regarding the use of placebo arms in trials but accepted a second ATI period after receiving broadly neutralising antibodies open-label. Participants described being part of an ATI trial as both liberating and demanding. The findings indicate that acceptability is influenced by predictable, frequent weekly HIV viral load monitoring, timely results communication, and access to free prevention, with clear concerns about relationship impacts during viral rebound, and clearly communicated pre-defined ART restart criteria.

Long-acting injectable cabotegravir plus rilpivirine (LA CAB+RPV) is a highly effective, well-tolerated antiretroviral regimen, but real-world adverse event (AE) data remain limited. We characterised the type and frequency of AEs associated with LA CAB+RPV in people with HIV in routine clinical practice. This multicentre, prospective, observational study (SCOLTA project, Italy) enrolled people with HIV initiating LA CAB+RPV. Clinical characteristics, AEs and discontinuations were systematically collected. A centre-level survey assessed AE management and prevention practices. In our cohort 598 people with HIV were enrolled across 25 centres. Forty-two participants (7%) experienced overall 51 AEs, of which 37 (88%) led to discontinuation of LA injectable therapy. Injection site reactions (ISRs) were the most frequently reported AEs (29.4%). Central nervous system (CNS)-related AEs accounted for 7.8% of events and resulted in treatment discontinuation in 75% of cases. In multivariable analysis, unknown HIV acquisition mode was independently associated with an increased risk of AEs. Age ≥50 years showed a consistent trend across all models and was the only significant independent predictor in sensitivity analyses restricted to people with HIV with known HIV acquisition mode. Among the 23 centres responding to the survey, 15 (65.2%) reported a reduction in injection-related events following specific training of staff administering LA CAB+RPV. LA CAB+RPV showed a lower AE rate than pivotal trials, despite more AE-related discontinuations. CNS-related AEs were uncommon, supporting good tolerability, but led to discontinuation when they occurred. Older age was independently associated with increased AE risk.

Advances in antiretroviral therapy (ART) have extended the life expectancy of people living with HIV, increasing the prevalence of comorbidities and polypharmacy burden. To compare comorbidity burden and polypharmacy among people living with HIV and matched people without HIV stratified by age, sex, and race/ethnicity. This retrospective study used claims data from Optum's de-identified Clinformatics® Data Mart Database (01/01/19-12/31/24). Adults (≥18 years) with ≥1 ART claim or HIV diagnosis code during the 2024 calendar year (index date: earliest ART or HIV claim) were matched 2:1 to adults without HIV by age group, sex, race, region, and insurance type. Continuous enrolment of 12 months pre-index (baseline) and ≥30 days post-index was required. Baseline comorbidities and polypharmacy were compared between the two cohorts. . A total of 26,078 people living with HIV were matched to 52,156 people without HIV. Mean age was 58.9 years, 21.3% were female and 31.3% were Black. Mean comorbidity burden was higher in people living with HIV vs without HIV overall (3.70 vs 3.14) and across age groups and sex (female: 4.40 vs 3.85; male: 3.33 vs 2.78) (all < 0.001). Multimorbidity (≥2 non-HIV comorbidities) and polypharmacy (≥5 non-ART drugs) prevalence were significantly higher among people living with HIV and increased with age and among females. Similar patterns were observed across racial/ethnic groups. People living with HIV had greater comorbidity and polypharmacy burden than matched people without HIV across demographic subgroups. These findings underscore the need for comprehensive comorbidity assessment and comedication review to minimise drug interactions and support optimal treatment decisions for people living with HIV.

As people living with HIV live longer, secondary chronic conditions are increasingly common. Although nutrition important in HIV care, limited data describe nutrient-related dietary patterns among people with HIV and specific chronic conditions. To conduct an exploratory assessment of researcher-developed nutrient-related food-frequency proxy measures among people with HIV by secondary chronic condition status. This cross-sectional study included 64 people with HIV receiving services from community-based organizations in St. Joseph County, Indiana ( = 36), and Washington, DC ( = 28). Dietary information was assessed using selected items from the National Institutes of Health Diet History Questionnaire. Proxy measures for selected nutrient-related dietary exposures were derived using a researcher-developed scoring approach and expressed as percent Daily Value (%DV). Non-parametric tests compared demographic characteristics and proxy measures by secondary chronic condition status and type. Participants with secondary chronic conditions had higher reported healthcare access ( = 0.025) and perceived relevance of nutrition ( = 0.036). The composite %DV-based relative intake score did not differ by condition status. Participants with cancer-related conditions had lower vitamin D-related ( = 0.039) and calcium-related proxy scores ( = 0.037) than participants without secondary chronic conditions, lower calcium-related scores than those with non-cancer conditions ( = 0.024), and lower vitamin C-related scores than those with cardiometabolic conditions ( = 0.024). These preliminary findings suggest possible differences in selected nutrient-related food-frequency proxy measures across chronic condition subgroups. Because key clinical and socioeconomic factors were not assessed, observed differences cannot be attributed to comorbidity status. Larger studies using validated dietary assessment methods, biomarkers, and comprehensive HIV-related clinical measures are needed.

Chemsex increases HIV risk, but the Dutch substance landscape is evolving beyond traditional drugs (e.g. GHB/GBL, methamphetamine) to include novel synthetic cathinones. This study mapped the spatial distribution of sexualised substance use and identified distinct chemsex-user profiles among men who have sex with men (MSM) in the Netherlands. Data from 1413 Dutch MSM in the EMIS-2024 survey (December 2023-July 2024) were analysed. Bayesian spatial ecological regression estimated the regional posterior prevalence and relative risk (RR) of chemsex engagement and its association with regional self-reported HIV prevalence. Latent-class analysis (LCA) identified substance-use patterns. Logistic regression assessed associations between HIV, chemsex engagement, and user patterns. Overall, 19.8% of MSM reported chemsex. Regional prevalence was highly heterogeneous, ranging from 25.2% (RR = 1.27) in GGD-Amsterdam to 16.4% (RR = 0.83) in GGD IJsselland. Amsterdam and unexpected border regions (e.g. GGD-Drenthe) showed significantly higher RRs for chemsex engagement and the broader sexualised substance use. Substance use varied widely, with a notable presence of novel synthetic stimulants (13.9%) and ecstasy (21.4%). LCA revealed two distinct profiles: a 'Traditional Substance-Moderate use' class and a 'Broader-Polysubstance' class using a broader range of sexualised substances. Both individual and ecological analyses confirmed a strong association between chemsex engagement and higher self-reported HIV prevalence. Chemsex among Dutch MSM is geographically heterogeneous and increasingly features broad polysubstance use. Its consistent link to higher self-reported HIV prevalence at individual and regional levels highlights the need for geographically tailored harm-reduction strategies.

People with HIV have an increased risk of heart failure (HF) compared to people without HIV. Guideline-directed medical therapy (GDMT) is the cornerstone of HF management, but little is known about GDMT prescription in this high-risk population. To evaluate GDMT intensity and identify patient factors associated with optimal HF therapy among people with HIV and systolic HF. We conducted retrospective analyses of the PATHWAYS study, a multi-institutional study of underrepresented racial and ethnic minorities with HIV. Adult patients with systolic HF (by ICD code) were included. A composite GDMT score was constructed using dosage-weighted prescription of GDMT drugs, ranging from 0% to 100% (with 100% indicating optimal therapy). Among 451 PWH with systolic HF, the median age was 54 years (IQR = 47-62), 34.6% were female, and 96.6% were Black. In the year after HF diagnosis, 85.8% of patients were prescribed any GDMT, and 45.5% of patients attended a cardiology clinic visit. The mean GDMT score was 30% (SD = 22%). In multivariable analyses, lower GDMT scores occurred among patients with older age (adjusted mean ratio [AMR] = 0.96, 95% CI: 0.93-0.99) and severe renal disease (AMR = 0.84, 95% CI: 0.71-0.99). Higher GDMT scores occurred among patients with diabetes mellitus (AMR = 1.33, 95% CI: 1.16-1.53), prior myocardial infarction (AMR = 1.17, 95% CI: 1.02-1.35), and those who attended a cardiology clinic visit (AMR = 1.29, 95% CI: 1.13-1.47). People with HIV and HF had low GDMT scores, suggesting suboptimal intensity of HF care. Cardiology clinic attendance was associated with higher GDMT scores.

As progress toward the UNAIDS 95-95-95 goals has transformed HIV into a chronic, manageable condition, cardiometabolic disease has emerged as a critical determinant of long-term health among people living with HIV. Despite effective antiretroviral therapy, people with HIV remain at elevated risk for hypertension, diabetes, and cardiovascular disease, while HIV care models often prioritize viral suppression over integrated cardiovascular prevention. This study aimed to explore the social and structural determinants that shape cardiovascular preventive health behaviors among people with HIV in the Southern United States to inform the development of interventions that promote sustained health outcomes. We conducted a qualitative descriptive study with twenty people with HIV aged ≥ 35 years, each living with at least one cardiometabolic condition, recruited from three HIV clinics in the Southern United States. Photo-elicitation integrated with semi-structured interviews allowed participants to document and interpret their experiences related to diet, physical activity, lifestyle behaviors, and health monitoring. Thematic analysis mapped findings to five determinant domains: economic stability, neighborhood and environment, social and community context, education, and healthcare access and quality. Participants (median age 57; predominantly African American/Black; most living with HIV for more than 10 years) identified three intersecting, modifiable barriers: the high cost of healthy food, unsafe or poorly designed environments limiting physical activity, and family or peer norms reinforcing unhealthy behaviors. These findings highlight the need to embed food insecurity screening, context-tailored activity counseling, family engagement strategies, and structural supports within HIV care to advance equitable cardiovascular prevention for people with HIV.

Tuberculosis (TB) is a major cause of illness and death among people living with HIV. Early initiation of TB preventive therapy (TPT) is crucial for preventing TB infection. As per National AIDS Control Programme (NACP) guidelines, the newer short-course TPT regime-1HP (isoniazid plus rifapentine, once a day for 28 days) is recommended for people living with HIV from May 2024 in India. To determine the coverage and completion of a newer TPT regime among people living with HIV and factors associated with coverage of TPT, and calculate the incidence of active tuberculosis among those who received TPT at subsequent visits until the end of our study period. The secondary data, including the clinical and sociodemographic profiles of all people living with HIV who walked in the ART centre of a tertiary care hospital, were collected over a year from September 2024 after receiving permission from the Institutional Human Ethics Committee. The data were entered into Microsoft Excel and analysed using IBM SPSS v25. Among the 1611 people living with HIV attending the ART centre, 287 were eligible for TPT, of which 217 (75%) started the newer (1HP) regime, and 216 (99.5%) completed it. Only 1 person living with HIV discontinued TPT because of the development of urticaria. There was no significant association between coverage of TPT and the sociodemographic and clinical indicators except area of residence, where it was significantly higher among people living with HIV residing in rural areas as compared to urban areas. The coverage of the newer short-course TPT regime (1HP) was 75%, and the completion rate was 99.5%. TPT coverage was high among people living with HIV residing in rural areas. TB incidence of 0.46% was recorded among those who received the newer TPT regimens.

To achieve consensus on barriers and strategies to improve the engagement of three priority populations - Black and Latino/a/x individuals, cisgender women, and transgender women in HIV cure research. We assembled a panel of 54 experts assigned to six groups in a hybrid Delphi process: (1) HIV community members, (2) biomedical researchers, (3) medical providers, (4) funders and private industry members, (5) bioethicists and regulators, and (6) social scientists. Over 18 months, we conducted four iterative survey rounds and three group discussions to identify barriers and strategies to arrive at a consensus on how to engage these priority populations in HIV cure research. For Black and Latino/a/x populations, the panellists identified inadequate outreach and a lack of accessible educational information as primary barriers and emphasised community-driven engagement and partnerships with trusted leaders as key strategies. For cisgender women, logistical hurdles, caregiving responsibilities and time constraints were identified as major barriers, with flexible trial designs and equitable compensation proposed as solutions. For transgender women, the lack of transgender-focused research design, including misrepresentation and exclusion, was identified as a key barrier, while centring transgender-specific needs in study design achieved consensus as the most effective strategy. Among all four priority populations, investment in outreach, engagement along the research process, better integration of health needs with research, and enhanced incentives are not novel ideas, but remain obviously ignored in a way that has led to underrepresentation of people in HIV cure research, who carry the greatest burden of HIV in the U.S. This paper uses a hybrid Delphi process to identify and reach consensus on key barriers and strategies to engage underrepresented groups: Black and Latino/a/x individuals, cisgender women, and transgender women in HIV cure research across the United States.

Pain is common in persons with HIV, who often report mental health symptoms and substance use, all of which may affect viral suppression. To investigate correlates of self-reported pain and associations with viral suppression among people with HIV. We analysed data from 3422 self-interviews contributed by 1626 unique patients in continuity care in the Johns Hopkins HIV Clinical Cohort between October 2013 and December 2018. Self-reported pain was measured using the EuroQol-5D-3L ("Have you had any pain or discomfort today?"). The correlations included PTSD, anxiety, depressive symptoms, and substance use. Viral suppression (HIV RNA ≤ 200 copies/mL) is routinely collected for clinical care and was abstracted between 9 months prior to 1 week after the self-interview. Pain was more prevalent in patients who were older, female, white, and who reported mental health symptoms or who had a recent or past prescription for an opioid. The prevalence ratio between pain and viral non-suppression was consistent with no association and also a weak positive association (1.07, 95% CI: 0.95, 1.21). Tailored interventions for mental health may improve pain and HIV outcomes.
