1. Morphology of the Middle Mesial Canal in Permanent Mandibular First Molars and Its Relationship to Pulpal Floor Anatomy in a Thai Subpopulation: A Micro-Computed Tomographic Analysis.
PMID:日期:2026-09-08To evaluate the relationship between the middle mesial canal (MMC) and pulpal floor anatomy and to compare root dentine thickness among the mesial canals across MMC types in the Thai subpopulation. A total of 724 mature permanent mandibular first molars were obtained and initially screened using cone-beam computed tomography to identify teeth with MMCs. These teeth (n=49) were rescanned with micro-computed tomography (micro-CT) to analyse canal configurations. Pulpal floor types were observed under a dental operating microscope, and minimum dentine thickness was measured using Fiji ImageJ software. Linear regression assessed relationships among dentine thickness, canal level and MMC type, while chi-square tests evaluated correlations between the presence of MMC and pulpal floor type. In this Thai subpopulation, the confluent MMC was most common (63.3%), followed by fin (28.6%), independent (6.1%) and unclassified (2%) types. Type 2 pulpal floors, characterised by mesiobuccal and mesiolingual orifices connected by an isthmus, were most common (61.3%). The mean vertical distance from the pulpal floor to the MMC orifice was 0.7 ± 1.00 mm, and 90% of MMC orifices were ≤1.5 mm from the floor. Dentine in MMCs was significantly thinner than in MB canals (middle and apical levels) and ML canals (apical levels). No significant differences were observed in dentine thickness among MMC types. There was no statistically significant association between pulpal floor type and the presence of MMC (P=.106), although MMCs were observed more frequently in type 2 floors (30.6%) than in type 1 floors (16.7%). In this Thai subpopulation, the confluent MMC, particularly the subtype with an isthmus, was the most common. No significant association was found between the presence of a pulpal floor isthmus and MMC occurrence. The MMC orifice was located within 1.5 mm of the pulpal floor in 90% of cases. The MMC exhibited the smallest dentine thickness, especially in the middle and apical thirds.

2. Effect of Calcium Hydroxide as an Intracanal Medicament with Aqueous- and Viscous-Based Vehicles in Patients with Pulpal Necrosis and Symptomatic Apical Periodontitis on Post-operative Pain and Periapical Healing Outcome.
PMID:日期:2026-09-07This study aimed to measure the effect of calcium hydroxide with propylene glycol and calcium hydroxide with normal saline as intracanal medicament in patients with pulpal necrosis and symptomatic apical periodontitis on postoperative pain and periapical healing outcome. For this investigation, 50 patients with symptomatic apical periodontitis and pulpal necrosis in permanent mandibular molar teeth were chosen. Depending on the kind of intracanal medication utilised during the procedure, the participants were divided into 2 groups of 25 patients each at random. Group 1: calcium hydroxide and propylene glycol and group 2: calcium hydroxide and normal saline. All patients with pre-operative pain scores between 60 and 144 mm were asked to record the pain on the Heft-Parker visual analogue scale (HP-VAS) from 6, 24, 48, 72, 96 hours and days 5,6,7.td. The healing of periapical lesions was determined at 0 months, 3 months, 6 months, 9 months and 12 months using the periapical index (PAI) score. No statistically significant difference in postoperative pain scores was observed between the groups at baseline and during the 1st-4th days (P > .05). The propylene glycol group demonstrated significantly lower pain scores on the 5th, 6th and 7th days compared to the normal saline group (P < .05). No significant difference in periapical healing was observed at the 3-month and 6-month follow-ups (P > .05); however, statistically significant improvements were observed in the propylene glycol group at 9 months and 12 months (P < .05). Calcium hydroxide used with a viscous vehicle (propylene glycol) demonstrated a greater reduction in postoperative pain after day 4 and improved periapical healing at 9 and 12 months compared to an aqueous vehicle (saline). Although statistically significant differences were observed at specific time points, the clinical relevance of these findings should be interpreted with caution, and further studies are required to confirm these results.

3. Staging Classification of Pulpitis: A Conceptual Framework Integrating Preoperative and Intraoperative Diagnostic Criteria.
PMID:日期:2026-08-27Pulpitis is a progressive inflammatory disease of the dental pulp, traditionally classified using histopathological or symptom-based schemes such as 'reversible' vs. 'irreversible'. While these frameworks have aided understanding, they often fail to reflect the dynamic biological state of the pulp that can aid in precise, minimally invasive treatment decisions. The aim of this article is to introduce a clinically meaningful classification of pulpitis that combines preoperative and intraoperative diagnostic assessment criteria to enable objective therapeutic choices. Drawing on the medical staging model used in other progressive diseases, the proposed classification is defined by (i) preoperative clinical and radiographic characteristics, (ii) intraoperative clinical observation of time-to-haemostasis and (iii) evidence-based therapeutic recommendations from conservative vital pulp therapy to pulpectomy. The proposed staging system comprises Stage 0: normal healthy pulp, Stage 1: mild pulpitis, Stage 2: severe pulpitis, Stage 2.1: partial coronal pulpitis, Stage 2.2: complete coronal pulpitis, Stage 2.3: total pulpitis and Stage 3: pulp necrosis. This staging system provides a structured, objective framework for communication with patients, prognostic assessment, research comparability, and health system integration (coding, record-keeping and reimbursement). It also represents a shift from theoretical disease labels towards actionable, biologically driven treatment pathways. The current staging classification provides a structured clinical framework to guide evidence-based treatment selection and aids the clinician to choose the most appropriate therapeutic approach for each case.

4. Antimicrobial Efficacy of Rhamnolipid Biosurfactant Against Multi-Species Endodontic Biofilm: A Comparative In Vitro Study.
PMID:日期:2026-08-26Persistent root canal infections are driven by resilient multi-species biofilms that withstand conventional irrigants. This in vitro study aimed to compare the antimicrobial efficacy of rhamnolipid (RHL) with NaOCl against a multi-species endodontic biofilm. A 4-species biofilm (Enterococcus faecalis, Streptococcus mutans, Fusobacterium nucleatum, Staphylococcus aureus) was developed on human dentine discs over 21 days under anaerobic conditions to simulate infected root canals. Biofilm-laden specimens were treated for 10 minutes with RHL at 1×, 2× and 4× the minimum inhibitory concentration (MIC, 50 mg/mL for E. faecalis). Standard 5.25% NaOCl and phosphate-buffered saline (PBS) served as positive and negative controls. Antimicrobial efficacy was evaluated by viable colony-forming unit (CFU) counts, quantitative real-time polymerase chain reaction (qPCR) for bacterial load (cycle threshold, CT) and confocal laser scanning microscopy. Rhamnolipid demonstrated a concentration-dependent antimicrobial effect. Colony-forming unit analysis showed the greatest bacterial load reduction in the 4× MIC group (40.80 ± 12.48 CFU), followed by NaOCl (1104.0 ± 35.79 CFU), 2× MIC (1372.20 ± 29.70 CFU), MIC (1556.60 ± 25.19 CFU) and PBS (21080 ± 30.00 CFU), with all intergroup differences statistically significant. Confocal laser scanning microscopy revealed intense red fluorescence and minimal green fluorescence in the 4× MIC group, indicating predominant bacterial death. Real Time PCR results corroborated this trend, with highest CT values for S. aureus, followed by S. mutans, E. faecalis and F. nucleatum. Interestingly, F. nucleatum was more susceptible than E. faecalis in the NaOCl and PBS groups, while the reverse was observed in the RHL-treated groups. Rhamnolipid biosurfactant, particularly at 4× MIC, demonstrated superior antimicrobial efficacy over NaOCl against multi-species endodontic biofilms, notably in reducing E. faecalis. Based on previously reported properties of RHLs, including biodegradability and potentially lower cytotoxicity, RHL may represent a promising adjunct or alternative to conventional endodontic irrigants. However, its biocompatibility, tissue interaction and clinical efficacy require further investigation.

5. Antibacterial Efficacy of Bio-C Temp Versus Calcium Hydroxide Against Enterococcus faecalis in Root Canals and Dentine Debris: An In Vitro Study.
PMID:日期:2026-08-14This study evaluated the antibacterial effectiveness of a calcium silicate-based intracanal medicament against Enterococcus faecalis in root canals and dentinal tubules, compared with calcium hydroxide. Thirty-two extracted human mandibular premolars were prepared with WaveOne Gold reciprocating files (Dentsply, Maillefer, Ballaigues, Switzerland) up to Primary size (25.07) and sterilised at 121°C for 20 minutes. Root canals were inoculated with E. faecalis ATCC 29212 (American Type Culture Collection, Manassas, VA, USA) for 21 days. Two infected specimens were examined using scanning electron microscopy (SEM) to confirm biofilm formation. The remaining teeth were divided into 3 groups based on the intracanal medicament applied for 7 days: distilled water (DW), Bio-C Temp (BT) and UltraCal XS (UC). After incubation, bacterial samples from the root canals and dentine debris were collected, and viable bacteria were quantified as colony-forming units (CFUs). Data normality was assessed using the Skewness-Kurtosis test. Statistical analysis included parametric (1-way ANOVA and Tukey's post hoc) or nonparametric tests (Kruskal-Wallis, Dunn's post hoc and Wilcoxon signed-rank), with significance set at P ≤ .05. Scanning electron microscopy analysis showed extensive E. faecalis biofilm formation with penetration into dentinal tubules. Viable bacteria were recovered from all DW samples, whereas positive bacterial growth was detected in only a limited number of specimens in the UC and BT groups. Both UC and BT resulted in significantly lower CFU counts in the root canals and dentine debris samples compared with DW (P < .05). No statistically significant difference was observed between UC and BT. The calcium silicate-based medicament BT exhibited effective antibacterial activity against E. faecalis in both the root canal system and dentine debris samples, suggesting activity within dentinal tubules, with efficacy comparable to that of the calcium hydroxide medicament UC.

6. Artificial Intelligence for Detection and Prevention of Separated Endodontic Instruments: A Scoping Review.
PMID:日期:2026-07-30Given the growing integration of artificial intelligence (AI) into dental diagnostics and clinical decision-support systems, a comprehensive assessment of existing publications addressing AI applications in the context of separated endodontic instruments is highly relevant. Such an assessment is essential for guiding future research priorities, promoting standardisation of reporting frameworks, and facilitating the transition from experimental AI prototypes towards clinically reliable tools aimed at minimising the incidence of intra-canal instrument separation. To critically evaluate the current landscape of AI models addressing endodontic instrument separation and to determine whether available models support a transition from post-separation detection towards predictive and preventive risk-assessment strategies. A qualitative evidence synthesis was conducted to summarise published studies evaluating AI and machine learning approaches related to separated endodontic instruments. This scoping review was structured in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) principles to ensure transparent study selection and reporting. Results were mapped and synthesised descriptively in 3 layers: study mapping, model mapping, and performance mapping. Of the 8 eligible studies, only 2 (25%) were experimental investigations focused on preventive monitoring and prediction of endodontic instrument separation-related changes. Radiology-based detection was the dominant research direction, represented in 6 of 8 studies (75%). For periapical radiograph-based detection of separated instruments, AI-based models reported accuracy values ranging from 0.795 to 0.982, with the highest values observed for Mask Region-based convolutional neural network (accuracy =0.982) and YOLOv8 (accuracy=0.974). Performance was generally lower for panoramic radiographs. In the cone-beam computed tomography (CBCT)-based approach, true-positive detection was reported at 80.8%, although detailed comparative metrics were limited. Most available AI models related to endodontic instrument separation focus on digital radiological images, including both two-dimensional modalities (panoramic and periapical radiographs) and three-dimensional imaging (CBCT), whereas only a few studies have addressed monitoring the functional condition of instruments during their performance to verify potential critical changes related to the risk of separation. Convolutional neural networks were the dominant methodological approach, whereas traditional ML algorithms were used mainly in preventive experimental studies.

7. In Vitro Evaluation of the Effect of Light-Cured Pulp-Capping Materials on Intrapulpal Temperature and Recovery Time During Sequential Light-Curing.
PMID:日期:2026-07-30Thermal insult is one of the factors contributing to pulpal damage during dental procedures, particularly in deep cavities with minimal dentine thickness. This study aimed to evaluate the effect of different light-cured pulp-capping materials on intrapulpal temperature (IPT) changes and thermal recovery time during sequential light curing in deep cavities. Deep Class 1 cavities were prepared for 56 extracted premolars, maintaining a dentine thickness of 0.5 mm. Teeth were divided into 4 groups (n=14) based on the pulp-capping materials: TheraCal LC, Biner LC, izy INO LC and Urbical LC. A digital thermometer was used to measure IPT by inserting a connected K-type thermocouple through the apices to reach the pulp chamber of the teeth that were suspended in a water bath at 37°C. Intrapulpal temperature measurements were recorded during the sequential restorative procedure starting from: baseline T0 (35°C), T1 after curing of each pulp-capping material, T2 after curing of the adhesive agent, T3 after curing the first 2-mm resin composite layer and T4 after curing the final resin composite layer. Intrapulpal temperature changes (ΔT) after each light-curing step were calculated relative to the baseline temperature. Recovery time after completion of the restorative procedure was recorded using an infrared camera. The data were analysed using 1-way ANOVA and post hoc tests at P < .05. The highest IPT change was recorded at ΔT4 for izy INO LC (8.59°C), followed by Biner LC (8.55°C), TheraCal LC (7.11°C) and Urbical LC (6.85°C). A significant difference in IPT value was observed only at ΔT4 (P <.05), with Urbical LC showing significantly lower values than both izy INO LC (P=.046) and Biner LC (P=.049). The fastest recovery times were recorded for TheraCal LC (2.22 minutes) and Biner LC (2.58 minutes), which were significantly shorter than Urbical LC (4.25 minutes) (P < .05). None of the tested pulp-capping materials was able to maintain the IPT increase below the critical threshold during the sequential light-curing procedure. Urbical LC showed lower temperature rise but slower thermal recovery than the other materials.

8. Dynamic Cyclic Fatigue Resistance and Bending Stiffness of Blue-Treated NiTi Rotary Instruments at Simulated Intracanal Temperature: A Comparative In Vitro Study.
PMID:日期:2026-07-30To compare the dynamic cyclic fatigue resistance, bending stiffness and phase transformation characteristics of blue heat-treated instruments, VDW Rotate (VR) and Azur Blue (AzB), at a simulated intracanal temperature of 34 ± 1°C. Fifty-eight new instruments (n=29 per group) were allocated as follows: dynamic cyclic fatigue testing (n=12), bending stiffness evaluation (n=12), differential scanning calorimetry (DSC) (n=1), X-ray diffraction (XRD) (n=2) and energy-dispersive X-ray fluorescence (EDXRF) (n=2). Dynamic cyclic fatigue testing was conducted in an artificial canal (18 mm length, 1.4 mm diameter, 45° curvature, 5 mm radius) at 34 ± 1°C. Time to fracture was recorded and converted to the number of cycles to failure, and fractured segment lengths were measured. Bending stiffness was evaluated with a 3 mm deflection range at 34 ± 1°C. Differential scanning calorimetry, energy-dispersive X-ray fluorescence, X-ray diffraction and scanning electron microscopy were used for metallurgical and fractographic analyses. Differential scanning calorimetry revealed austenitic finish temperatures (AuF) of 35.8°C for VDW Rotate and 39.4°C for AzB. Azur Blue showed significantly higher cyclic fatigue resistance (2039 ± 250 vs. 1609 ± 222 cycles), lower bending stiffness (0.85 ± 0.07 vs. 1.07 ± 0.08 N) and longer fractured fragment length (4.87± 0.44 vs. 3.27 ± 0.75 mm) than VDW Rotate (P < .05). Azur Blue showed higher cyclic fatigue resistance, whereas VDW Rotate showed higher bending stiffness. These differences were more consistently associated with phase transformation behaviour than with external geometry alone.

9. Periapical Lesion Size and Regenerative Materials as Prognostic Factors in Endodontic Microsurgery: A 12-Month Retrospective Cohort Study.
PMID:日期:2026-07-30This study evaluated the influence of initial periapical lesion size and the use of bone graft and/or membrane materials on endodontic microsurgery outcomes, considering other clinical factors. A retrospective cohort study included 127 teeth treated with endodontic microsurgery, with or without adjunctive bone graft and/or membrane placement, between January 2021 and December 2024. Based on the initial Periapical Index (PAI) scores, patients were classified into 2 groups: small lesions (PAI ≤3) and large lesions (LL, PAI ≥ 4). Treatment outcomes were assessed at 12 months according to established criteria. Statistical analyses included chi-square or Fisher's exact tests, t-tests and multivariate logistic regression (P < .05). The overall success rate was 64.6%. The use of bone graft and/or membrane was associated with significantly higher success rates (72.0% vs. 53.8%; odds ratio [OR]=2.34, 95% CI: 1.10-4.98; P=.03), particularly in LLs (69.6% vs. 42.3%; OR=2.90, 95% CI: 1.08-7.78; P = .03). In multivariate analysis, regenerative material use (adjusted OR=2.48, 95% CI: 1.12-5.50; P=.03) and anterior tooth position (adjusted OR=2.71, 95% CI: 1.24-5.93; P=.01) were significantly associated with treatment success. Within the limitations of this 12-month retrospective cohort study, the adjunctive use of regenerative materials and anterior tooth position were associated with higher success rates in endodontic microsurgery. Preoperative lesion size alone was not significantly associated with treatment success.

10. The Influence of Simvastatin on the Osteogenic Differentiation Induced by Calcium Silicate-Based Cements.
PMID:日期:2026-07-30The combination of simvastatin with hydraulic materials might offer added benefits for enhancing mineralisation, reducing inflammation, and supporting pulp healing. Therefore, the aim of the present in vitro study was to assess the effect of hydraulic calcium silicate-based cements in addition to simvastatin on cell viability and osteogenic differentiation. Human osteogenic sarcoma (Saos-2) cells were cultured in presence of 0.1 μM of simvastatin, and with ProRoot MTA (Mineral Trioxide Aggregate) and Biodentine in combination or not with simvastatin. Cell viability was assessed by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay at 24, 48, and 72 hours and immunofluorescence microscopy analysis was conducted after 72 hours. Osteogenic differentiation was evaluated by Alizarin Red Staining (ARS) after 18 days. Moreover, gene expression (RUNX-2, COL1A1, ALP, OCN) was analysed at 12 and 18 days. Statistical analysis was performed using analysis of variance, Bonferroni's test, and Student's tests. Simvastatin at 0.1 μM preserved cell viability comparable to that of the untreated control. Lesser cytotoxicity was reported when simvastatin was combined with Biodentine, rather than ProRoot MTA. Simvastatin alone or in combination with both cements induced the reorganisation of the actin cytoskeleton into lamellipodia. Alizarin Red Staining showed that simvastatin significantly enhanced mineralisation, mainly when combined with Biodentine. Conversely, both cements alone showed moderate effects. Regarding gene expression, analyses revealed that these materials, in addition to simvastatin, influenced differently osteogenic markers, suggesting the involvement of distinct molecular pathways probably due to different chemical composition. Hydraulic calcium silicate-based cements exhibited distinct effects on the expression of differentiation-related genes, while showing a comparable influence on mineralisation nodule deposition. These findings suggest material-specific biological responses when the cements are combined with simvastatin.
