1. Cardiovascular-Kidney-Metabolic Overlap in Patients Hospitalized for Heart Failure.
PMID:日期:2026-09-30Among patients with heart failure (HF) in contemporary US clinical practice, the burden of overlapping cardiovascular-kidney-metabolic (CKM) conditions and the associated impact on outcomes remain poorly characterized. To evaluate the prevalence of CKM multimorbidity and the impact on clinical outcomes in patients hospitalized for HF across the spectrum of ejection fraction (EF). This was a retrospective cohort study of patients hospitalized for HF at US hospitals participating in the Get With The Guidelines-Heart Failure (GWTG-HF) registry. The analysis included patients hospitalized for HF in the GWTG-HF registry between 2019-2025 and Medicare beneficiaries hospitalized for HF in the GWTG-HF registry between 2019-2023. Presence of up to 4 additional CKM conditions including atherosclerotic cardiovascular disease, chronic kidney disease, diabetes, and obesity (body mass index ≥30; calculated as weight in kilograms divided by height in meters squared). In-hospital mortality, 1-year postdischarge all-cause mortality, and 1-year postdischarge HF readmission. This study included 725 205 patients (median [IQR] age, 72.0 [61.0-82.0] years; 386 886 male [53.3%]) hospitalized for HF between 2019 and 2025 and 123 271 Medicare beneficiaries (median [IQR] age, 80 [73-87] years; 64 375 female [52.2%]) hospitalized for HF between 2019 and 2023. Among 725 205 patients hospitalized for HF, 521 739 (71.9%) had 2 or more overlapping CKM conditions, and a minority of patients (48 732 [6.7%]) had HF only. CKM overlap was greatest in HF with preserved EF (HFpEF). Compared with HF only, increasing number of overlapping CKM conditions was associated with stepwise increases in in-hospital mortality (4 CKM conditions: HF with reduced EF [HFrEF] adjusted odds ratio [aOR], 5.08; 95% CI, 4.43-5.83; HF with mildly reduced EF [HFmrEF] aOR, 2.37; 95% CI, 1.68-3.34; HFpEF aOR, 2.05; 95% CI, 1.77-2.38), and this association was strongest in HFrEF (P for interaction <.001). Among Medicare beneficiaries hospitalized for HF, compared with HF only, increasing CKM multimorbidity was associated with increasing risks of postdischarge all-cause mortality (4 CKM conditions: HFrEF adjusted hazards ratio [aHR], 2.10; 95% CI, 1.93-2.29; HFmrEF aHR, 1.38; 95% CI, 1.17-1.63; HFpEF aHR, 1.09; 95% CI, 1.03-1.17) and HF readmission (4 CKM conditions: HFrEF aHR, 2.33; 95% CI, 2.12-2.56; HFmrEF aHR, 2.20; 95% CI, 1.82-2.66; HFpEF aHR, 1.91; 95% CI, 1.77-2.06), with steepest increases in risk among patients with HFrEF (P for interaction <.001). In this contemporary cohort of US patients hospitalized for HF, most patients had multiple additional overlapping CKM conditions. Increasing CKM multimorbidity was independently associated with stepwise increases in mortality and readmission risk. Although these associations spanned the EF spectrum, the prognostic impact of CKM multimorbidity was greatest in HFrEF and least in HFpEF.

4. Global, Regional, and National Burden of Heart Failure, 1990-2023: A Systematic Analysis for the Global Burden of Disease Study 2023.
4. 1990-2023年全球、区域和国家心力衰竭负担:2023年全球疾病负担研究的系统分析PMID:日期:2026-09-30Heart failure (HF) is a common condition with substantial geographical variation in its prevalence and underlying etiologies. Consistent and comparable data on the burden of HF within populations are needed to inform health system priorities. To estimate the global burden of HF. This cross-sectional study estimates disease burden using Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) methodologies to integrate multiple information sources. Data come from 204 countries and territories from 1990 to 2023 representing the global population with prevalent HF. HF and underlying etiologies. Estimates with 95% uncertainty intervals (UIs) for the prevalence and years lived with disability (YLDs) for HF by age, sex, location, year, and underlying etiology. Globally, there were 56.0 million (95% UI, 49.9-62.2 million) estimated cases of HF in 2023. The estimated prevalence of HF generally increased with age for all locations. While ischemic heart disease and hypertensive heart disease were the 2 most common etiologies globally among older adults, there was substantial variation in etiological pattern by location and age. Congenital heart anomalies and rheumatic heart disease (RHD) were common etiologies in children and young adults, while the proportion of HF due to alcohol use peaked in middle adulthood. Location-specific estimates highlighted regions with higher burden of HF due to specific etiologies, including RHD (south Asia, east Asia, Oceania, southeast Asia, central sub-Saharan Africa, eastern sub-Saharan Africa, southern sub-Saharan Africa, and western sub-Saharan Africa), alcohol use (eastern Europe, Australasia, the Caribbean, and central Europe), and Chagas disease (southern, Andean, tropical, and central Latin America). Heart failure is a significant global public health burden, and the impact is likely to increase as populations age. However, most HF etiologies can be prevented by interventions that reduce exposure to modifiable risk factors, such as elevated blood pressure, excessive alcohol consumption, and tobacco use. Etiology- and location-specific estimates can provide necessary information for public health officials to determine how best to prioritize interventions to reduce HF disease burden.

5. No-Touch vs Conventional Vein Grafts in Coronary Surgery: Seven-Year Clinical Outcomes of the SWEDEGRAFT Randomized Clinical Trial.
5. 冠状动脉手术中非接触式与大隐静脉桥血管的比较:SWEDEGRAFT随机临床试验的七年临床结果PMID:日期:2026-09-30No-touch saphenous vein harvesting preserves perivascular tissue and may improve graft patency after coronary artery bypass grafting (CABG), but whether it reduces long-term clinical events remains uncertain. To compare 7-year clinical outcomes after no-touch vs conventional saphenous vein harvesting in patients undergoing CABG. This is a prespecified extended follow-up of the SWEDEGRAFT multicenter, binational, randomized clinical trial, which was conducted at 8 cardiac surgery centers in Sweden and 1 center in Denmark. Patients undergoing primary, nonemergent, isolated CABG were eligible for inclusion. The study was conducted between April 20, 2018, and June 3, 2020, and data were analyzed from February 2026 to April 2026. Patients were randomized 1:1 to no-touch or conventional saphenous vein harvesting. The primary outcome was a composite of death from any cause, myocardial infarction, or repeat revascularization. Secondary outcomes were the individual components of the primary composite. Outcomes were ascertained through national population-based registries. A total of 902 patients undergoing primary, nonemergent, isolated CABG were randomized; 900 were included in the modified intention-to-treat population. Among 900 patients, 454 were assigned to no-touch harvesting and 446 to conventional harvesting. Mean (SD) age was 67.0 (7.6) years, 108 patients (12.0%) were women, and 419 patients (46.6%) underwent nonelective surgery. Median (IQR) follow-up was 6.46 (5.92-6.86) years. The primary outcome occurred in 90 patients in the no-touch group and 91 patients in the conventional group (19.8% vs 20.4%; hazard ratio [HR], 0.98; 95% CI, 0.73-1.31; P = .90). The adjusted 7-year risk difference was -0.8% (95% CI, -6.6% to 4.6%). The secondary outcomes did not differ significantly between groups: death (HR, 0.84; 95% CI, 0.53-1.35), myocardial infarction (HR, 0.74; 95% CI, 0.44-1.24), and repeat revascularization (HR, 1.07; 95% CI, 0.73-1.56). Prespecified subgroup analyses showed no heterogeneity in treatment effect. In this 7-year follow-up of the SWEDEGRAFT randomized clinical trial, no-touch saphenous vein harvesting was not shown to significantly reduce the composite of death from any cause, myocardial infarction, or repeat revascularization through 7 years of follow-up. Taken together with the previously reported excess of leg wound complications, these findings do not establish a net clinical benefit of routine no-touch harvesting. ClinicalTrials.gov Identifier: NCT03501303.

9. Multidimensional Aging Trajectories Preceding Cardiovascular Events.
9. 心血管事件前的多维衰老轨迹PMID:日期:2026-09-23Whether declines in functional aging markers precede cardiovascular (CVD) events is unclear. To determine 14-year trajectories of frailty, intrinsic capacity (IC), and physical health-related quality of life (HRQOL) preceding CVD events in later life and to compare these with matched CVD-free controls. This was a case-control study nested within the Aspirin in Reducing Events in the Elderly (ASPREE) cohort. Participants were aged 70 years and older (≥65 years for US racial and ethnic minoritized groups) with no prior CVD event, dementia, or significant physical disability and were recruited in Australia and the US between March 2010 and December 2014. Four controls were matched to each CVD case by age (±1 year), sex, and education. Participants were followed up prospectively until December 2024. Data were analyzed between January and May 2026. The 64-item Frailty Index (FI), Fried phenotype frailty, IC (comprising cognitive, locomotor, vitality, and psychological domains), and 12-item short form physical HRQOL were assessed annually. CVD events were adjudicated by expert clinical panels. Linear mixed-effects models were used to compare the trajectories between cases and controls. This study included 12 015 participants (2403 cases and 9612 controls) with a mean (SD) age of 76.4 (4.7) years; 6190 were male (52%). Compared with matched controls, individuals who went on to experience a CVD event had consistently worse profiles up to a decade beforehand, including higher frailty, lower IC, and poorer physical HRQOL. These differences widened markedly in the 5 years preceding the event. Cases showed faster deterioration over time, with greater increases in frailty (FI, β = 0.78; 95% CI, 0.66-0.91; Fried, β = 0.05; 95% CI, 0.03-0.07), and steeper declines in IC (β = -0.34; 95% CI, -0.44 to -0.24) and physical HRQOL (β = -0.55; 95% CI, -0.70 to -0.40). Patterns were broadly similar across components of the CVD composite, with the exception that divergence occurred earlier for heart failure and fatal coronary heart disease (≥10 years before the event) than for myocardial infarction or stroke. Results of this nested case-control study show that declines in functional aging markers were prominent before clinically apparent CVD, suggesting that they may represent the culmination of progressive multisystem aging and indicating a prolonged window of opportunity for closer clinical evaluation and early intervention.
