Rest intolerance, the experience of negative emotions when attempting to rest, is an emerging public health concern, yet its population-based epidemiology remains poorly understood. This study systematically investigated the prevalence, distribution, and health correlates of rest intolerance among adults in China, with particular focus on its manifestation in highly educated and younger populations. We analyzed cross-sectional data from 24,405 adults in the 2024 Psychology and Behavior Investigation of Chinese Residents (PBICR) survey. Rest intolerance (Rest Intolerance Scale-8 [RIS-8]), mental health (Patient Health Questionnaire-9 [PHQ-9], Generalized Anxiety Disorder-R3 [GAD-R3], and Perceived Stress Scale-4 [PSS-4]), and quality of life (EQ-5D-5L or EuroQol visual analog scale [EQ-VAS]) were assessed. Cutoffs were derived via cluster analysis and latent profile analysis, followed by regression and moderation analyses. The overall prevalence of high rest intolerance was 14.94% (3645/24,405). Notably, rates were significantly elevated among younger adults and those with higher educational levels. Rest intolerance was significantly associated with adverse mental health outcomes (anxiety: β=0.406; depression: β=0.402; and perceived stress: β=0.440; all P<.001) and reduced quality of life (EQ-5D: β=-0.115 and EQ-VAS: β=-0.117; both P<.001). Moderation analyses further revealed that these adverse associations were stronger among highly educated and younger individuals. Rest intolerance affects 14.94% of adults in China, with highly educated and younger populations constituting particularly high-risk groups. The condition demonstrates robust associations with adverse mental health outcomes and diminished quality of life, especially among these populations. These findings underscore the need for targeted psychological interventions and workplace health policies addressing rest-related distress in contemporary performance-oriented societies.

Digital mailbox platforms established by authorities for secure communication with the general populace are increasingly available for distribution of surveys. While such solutions constitute an attractive way of easily distributing surveys, nonresponse remains a critical problem, warranting measures to increase participation. We aimed to investigate questionnaire characteristics predicting participation and the effect of reminders among men in a survey distributed through a national digital mailbox solution. This study used an internet-based survey among men aged 45 to 70 years; the survey was sent out in 2 waves through a digital mailbox used by Danish authorities. Predictors for response were analyzed using logistic regression. Of 24,000 invitations, 22,288 participants accepted, with 6756 (30.3%) completing the survey. We found some evidence suggesting a rule of thumb that reminders may add another half to the response rate achieved in a preceding invitation, with 21% and 10% of responses obtained before and after the reminder, respectively (in the first wave). Longer questionnaire variants (odds ratio [OR] 0.77, 95% CI 0.66-0.88; <.001 [longest vs shortest variants]), questionnaires with attached informational material (OR 0.92, 95% CI 0.85-0.98; =.02[with vs without material]), or with potentially distressful emotional content (OR 0.91, 95% CI 0.85-0.98; =.008 [most vs least distressful content]) tended to yield lower response rates. Furthermore, older age was associated with a higher response rate when comparing the oldest (65-70 years) to the youngest (45-55 years) age groups (OR 2.49, 95% CI 2.31-2.70; <.001). We confirmed some previous findings obtained with other modes of survey distribution on the association between response rates and questionnaire characteristics, showing that it is important to pay attention to specific characteristics when designing internet-based surveys distributed through digital mailboxes. Nonresponse may constitute a considerable and increasing challenge, and reminders may help substantially increase participation.

In the postpandemic era, trends in HIV, syphilis, and hepatitis C virus (HCV) infections, as well as the prevalence of coinfections among high-risk groups, may have shifted; however, relevant empirical studies are limited. This study aimed to assess the seropositivity rates and coinfection rates for HIV, syphilis, and anti-HCV; and to explore the correlates of HIV, syphilis, and anti-HCV positivity among voluntary counseling and testing (VCT) clients at Centers for Disease Control and Prevention (CDC)-affiliated clinics in Zhejiang Province from 2022 to 2024. This retrospective study analyzed routine data extracted in January 2025 from the National HIV Information Prevention and Control System. The Cochran-Armitage trend chi-square test was used to analyze trends in positivity rates over the years. Logistic regression was used to identify relevant indicators for positivity for HIV, syphilis, and anti-HCV in 2024. The HIV positivity rates decreased from 2.462% (743/30,182) in 2022 to 1.664% (681/40,916) in 2024 (χ for trend =53.6; P<.001). The syphilis positivity rates were 1.378% (414/30,049), 1.502% (675/44,951), and 1.62% (647/39,929) in 2022, 2023, and 2024, respectively (χ for trend=6.8; P=.03). The anti-HCV positivity rates were 0.309% (77/24,950), 0.387% (161/41,593), and 0.504% (192/38,098) in 2022, 2023, and 2024, respectively (χ for trend=14.8; P<.001). In 2024, the positivity rates for dual HIV-anti-HCV coinfection, dual HIV-syphilis coinfection, and triple HIV-syphilis-anti-HCV coinfection were 0.011%, 0.203%, and 0.005%, respectively. Multivariable logistic regression analysis showed that age ≥35 years and HIV testing history were associated with HIV, syphilis, and anti-HCV positivity; compared with proactive consultation, outreach consultation was associated with lower odds of HIV positivity (adjusted odds ratio [aOR] 0.096, 95% CI 0.060-0.153), but higher odds of syphilis positivity (aOR 1.241, 95% CI 1.033-1.491) and anti-HCV positivity (aOR 2.837, 95% CI 1.882-4.276); compared with consulting for casual heterosexual behavior, consulting for homosexual behavior was associated with higher odds of HIV positivity (aOR 1.909, 95% CI 1.514-2.408) and lower odds of anti-HCV positivity (aOR 0.031, 95% CI 0.011-0.082). From 2022 to 2024 in Zhejiang Province, the HIV and anti-HCV positivity rates among VCT clients at CDC-affiliated clinics declined, while the syphilis positivity rates increased. The coinfection positivity rates for these 3 infections remained at low levels. Individuals reporting homosexual behavior constituted a high-risk group for HIV positivity yet exhibited a lower positivity rate for anti-HCV. Among CDC-affiliated VCT clients, older age, lower educational attainment, and prior HIV testing history were associated with higher infection positivity rates, suggesting that these characteristics might help refine targeting of VCT and prevention efforts within this setting.

Urban communities affected by chronic violence face persistent challenges in accessing primary health care (PHC). Although violence is widely recognized as a determinant of health inequities, there exists limited evidence on how direct exposure to violent events is related to individuals' perceptions of difficulty in accessing PHC services in highly vulnerable urban environments. The study aims to examine how sociodemographic characteristics and exposure to violent events are associated with perceived difficulty in accessing PHC in standardized hypothetical mental health scenarios among residents of a community affected by chronic urban violence in Rio de Janeiro. A cross-sectional survey was conducted between October 8 and November 1, 2024, with 1006 adult residents who completed 4 PHC vignettes and reported past-year exposure to violent events. Participants completed a structured questionnaire that included sociodemographic information, past-year exposure to interpersonal violence, and evaluations of 4 hypothetical vignettes describing different clinical or social situations. Vignette responses were recorded using an ordinal Likert-type scale ranging from "very easy" to "extremely difficult." Descriptive analyses were performed for all variables, and associations between sociodemographic characteristics and violence indicators were examined using Pearson chi-square tests, with effect sizes estimated using the coefficient or Cramér , as appropriate, and their respective 95% CIs. Differences in vignette ratings across sociodemographic groups were assessed using Kruskal-Wallis tests. To explore the underlying relationships among violence indicators, vignette responses, and sociodemographic variables, exploratory factor analysis and multiple correspondence analysis were conducted. Violence exposure was frequent. Threats of harm were reported by 27.7% (157/567) of Black participants and 30.5% (72/236) of non-Black participants. Slapping was reported by 20.8% (118/567) of Black individuals and 28% (66/236) of non-Black individuals, while being pushed or restrained affected 21.2% (50/236) and 26.7% (63/236), respectively. Significant sex differences emerged for being punched or kicked (female 17.2% [75/435] vs male 23.9% [88/368]; =.02). Kruskal-Wallis tests indicated few differences in vignette-based perceptions across groups, except for race in the alcohol-dependence vignette (²=7.384, =.006). Factor analysis identified 2 dimensions: violence variables loaded strongly on one factor (0.83-0.96) and vignette responses on another (0.66-0.83). Multiple correspondence analysis results showed that forms of violence contributed mainly to Dimension 1 (9%-15%) and vignette categories to Dimension 2 (7%-12%). Exposure to violence was widespread, with differences in some forms of violence observed between men and women. Violence-related variables and vignette-based perceptions of difficulty accessing PHC were predominantly represented by distinct dimensions in the exploratory multivariate analyses. Strengthening PHC in violence-affected urban settings requires addressing structural barriers that shape residents' access to health care.

Many patients have been treated by physical therapists after an infection with COVID-19. The policy changes and unique situation of the COVID-19 pandemic necessitate a retrospective analysis. This study examined physical therapy provision for adult patients recovering from COVID-19 in primary care in the Netherlands over the first 2.5 years of the pandemic. The primary aim was to examine changes in the amount of health care provision and associated factors. Furthermore, we aimed to gain insight into the patient population and changes over time, as well as the use of advised outcome measures in physical therapy after COVID-19. We conducted a retrospective cohort study using routine health care data from the electronic health records of physical therapy practices in Dutch primary care. In total, 71,152 COVID-19 treatment trajectories initiated between March 1, 2020, and December 18, 2022, were included. Multilevel generalized linear mixed model analyses examined changes over time in the patient population and in the amount of physical therapy within treatment trajectories (ie, duration in weeks and number of consultations). Outcome measures were analyzed using descriptive statistics. Completed treatment trajectories had an overall duration of 24 weeks with 28 consultations, with a decreasing trend in the amount of physical therapy provided over time (<.001). Also, a parabolic effect of age (<.001) was found, showing a maximum number of consultations and duration of the treatment trajectory among older patients aged 60 to 79 years, with smaller quantities for younger and older patients. Treatment trajectories for women were 1.5 weeks longer than those for men (β 1.5, SE 0.12; <.001) but did not include more consultations (=.76). In 83% (59,184/71,152) of treatment trajectories, a baseline score on at least one of the included advised outcome measures was registered. Median (IQR) baseline scores were 420 (340-490) m on the 6-minute walking test (n=27,035); 13 (10-17) seconds on the 5-times sit-to-stand test (n=2337); 5.8 (5-6.3) points on the Fatigue Severity Scale (n=995); 8 (7-10) points on the Patient-Specific Functional Scale (PSFS; n=55,074); 10 (9-12) points on the Short Physical Performance Battery (n=4665); and 29 (22-38) and 30 (24-40) kg of grip strength, respectively, on the left (n=12,306) and right (n=12,293) hand. Difference scores could be examined for the 6-minute walking test, grip strength, and the PSFS. This study demonstrates the potential of routinely collected health care data to describe real-world physical therapy provision for COVID-19 in Dutch primary care. Patients showed severe perceived fatigue, reduced functional capacity, and lower-limb strength at the start of physical therapy. The amount of physical therapy declined over the first 2.5 years of the pandemic, differed by age and gender, and was generally lower than the amount reimbursed by the Dutch government through basic health insurance.

Routine surveillance is important for understanding hand, foot, and mouth disease (HFMD) after monovalent enterovirus 71 (EV71) vaccine introduction, but interpretation is difficult when etiologic typing is sparse and the COVID-19 pandemic overlaps with the postvaccine era. This study described surveillance-derived changes in HFMD burden and typed serotype composition in Anhui Province, China, after EV71 vaccine introduction and during the COVID-19 pandemic. We conducted a retrospective ecological surveillance study using HFMD records from January 2008 to December 2023. The primary analysis used negative-binomial interrupted time series (ITS) counterfactual models for all reported HFMD cases and typed EV71 monthly counts. Vaccine-era models were trained using data from January 2013 through May 2016; the previous specification covering January 2010 through May 2016 was retained as a sensitivity analysis. COVID-19-period models were trained using data from January 2008 through December 2019. Secondary analyses assessed spatial, temporal, and age patterns. Vaccine coverage histories, individual vaccination records, specimen submission metadata, and laboratory platform-change metadata were unavailable; therefore, models estimated calendar-based surveillance deviations rather than vaccine effectiveness. Anhui reported 1,292,579 HFMD cases from 2008 to 2023, including 47,990 (3.7%) typed cases. EV71 declined from 45.8% (8479/18,534) of typed cases before vaccine introduction to 3.4% (578/16,930) during COVID-19, while other enteroviruses increased from 29.6% (5487/18,534) to 81.8% (13,848/16,930). There were 123 deaths from 2008 to 2023, corresponding to 9.52 deaths per 100,000 reported cases; no deaths were recorded from 2020 to 2023. In primary vaccine-era ITS models, typed EV71 counts remained below the predicted trajectory in 2017 to 2019: 470 observed vs 659 predicted cases (95% prediction interval [PI] 467-933) in 2017, 181 vs 531 (95% PI 379-798) in 2018, and 63 vs 427 (95% PI 305-662) in 2019. All reported HFMD cases were not uniformly below the vaccine-era counterfactual. During the COVID-19 pandemic, all reported HFMD cases remained below prediction through 2023, with the largest relative difference in 2022 (28,385 observed vs 175,601 predicted; 95% PI 111,465-268,202). The proportion of reported children aged ≥4 years increased from 19% (50,503/266,132) during 2017 to 2019 to 29.1% (44,133/151,795) during 2021 to 2023 (odds ratio 1.75, 95% CI 1.72-1.78; P<.001). Typed EV71 surveillance signals declined after EV71 vaccine introduction, and overall HFMD burden was strongly disrupted during the COVID-19 pandemic. However, sparse typing, unavailable vaccination and testing policy metadata, and the aggregated other enteroviruses category limit causal interpretation. These findings should be read as surveillance signals, not as direct estimates of population-level serotype replacement or individual-level vaccine protection.

Health information-seeking behavior (HISB) is a critical component of personal health management. Health literacy (HL) shapes whether and how people seek health information, but its role across age groups remains underexplored. Korea's highly digitalized information environment and rapidly aging population provide an important context for examining age-group differences in HL and HISB. This study examined age-group differences in the association of HL with 2 related stages of HISB-HISB experience and, conditional on engagement, source-based HISB type-and assessed whether the roles of HL, education, and health-related factors differed across age groups. Cross-sectional data from the 2020-2021 Korea Health Panel and 2021 HL Supplementary Survey (N=7910) were analyzed. HL was measured using the European Health Literacy Survey Questionnaire-16-item (HLS-EU-Q16). HISB type was classified as source-based "active seeking" or "passive acquisition" according to the respondents' top-ranked information source. Age-stratified multivariable logistic regression was used for HISB experience and type, and sensitivity analyses examined age×HL interactions and potential selection into the HISB type sample. Mean HL scores were lower in older age groups (14.10, SD 2.94 in ages 19-44 years vs 8.34, SD 4.65 in ≥65 years). HISB prevalence was highest in the youngest group (1072/1608, 66.7%) and lowest in the oldest group (1052/3487, 30.2%). Among those who engaged in HISB, 92.7% (993/1072) of the youngest group and 48.2% (507/1052) of the oldest group were classified in the active-seeking category. Primary age-stratified and interaction analyses showed that HL was significantly associated with HISB experience and type among adults aged 45-64 and ≥65 years, but not among those aged 19-44 years, whereas education and health-related factors were more salient among younger adults. The HL-HISB type association was sensitive to modeling selection. Age-group patterns were evident in HISB experience and source-based type. HL was a more prominent correlate among middle-aged and older adults, while education and health-related factors were more salient among younger adults. Age-tailored strategies that also consider structural, motivational, and digital-access factors alongside HL are needed.

Mobile exposure notification apps (ENAs) can play a significant role in future pandemics. Identifying lessons learned and areas for improvement from such apps, particularly within specific social and cultural contexts, is therefore crucial. This study aimed to evaluate the Finnish ENA "Koronavilkku," according to chosen key performance indicators to assess the ENA's use and performance during the COVID-19 pandemic in Finland, and to identify strengths and improvement areas. Using available data on Finland's ENA, we defined metrics for four key performance indicators: ENA use extent, enablers and barriers to use, the ENA's influence on user behavior, and overall acceptability. We performed an evaluation study combining system data and data from a survey of 4061 respondents conducted in April 2022. A sentiment analysis was performed on open-ended responses. Koronavilkku ENA peak coverage reached approximately 53% of active smartphone users shortly after launch. Use declined over time, particularly among individuals diagnosed with COVID-19. User engagement remained high-among ENA users who received a token, the proportion that entered their token into the ENA was more than 50% during the whole study period. Males, individuals older than 34 years, and those living alone were less likely to use the ENA, while those with higher education and households with children were more likely to be ENA users. Almost 70% (64,642/94,280) of SARS-CoV-2-positive users reported not receiving a token from health authorities. Users reported higher adherence to pandemic guidance than nonusers, with prevalence ratios ranging from 1.24 (95% CI 1.19-1.29) to 1.65 (95% CI 1.45-1.91). Descriptive analyses suggested higher reported adherence among partially compliant users after exposure notifications, whereas little change was observed among noncompliant individuals. Overall acceptability was relatively high, with 68% of respondents using it at least once. However, 49% of those users eventually discontinued use. Our sentiment analysis of open-ended feedback suggests that users expressed more positive attitudes toward the ENA compared to nonusers. However, a substantial portion of the feedback reflected neutral or negative sentiments. Based on observed ENA use trends, we recommend maintaining long-term public interest and engagement in ENAs during prolonged emergencies. Tailored communication should be developed encouraging uptake among demographic groups with lower adoption rates. Initial studies during app rollout could inform these strategies. A major barrier to the ENA's performance was token issuance; earlier and more systematic automation of token distribution in Finland could have enabled more timely exposure notifications and reduced bottlenecks. Future ENAs should define evaluation criteria during development, with periodic assessments measuring effectiveness and informing improvements. A deeper analysis of open-ended feedback provided for the Koronavilkku ENA using advanced language models could provide additional insights into user perceptions and concerns. By addressing these areas, future digital contact tracing tools may improve uptake, acceptability, and integration into pandemic response efforts.

Pretreatment drug resistance (PDR) poses an increasing threat to the effectiveness of antiretroviral therapy and treatment-as-prevention strategies. Zhejiang Province, an economically developed region in eastern China with high population mobility, experiences complex HIV-1 subtype dynamics and evolving resistance patterns. However, recent population-based evidence integrating HIV genetic subtypes, PDR prevalence, and associated factors remains limited. This study aimed to characterize HIV-1 subtype distribution, estimate the prevalence of PDR, and identify associated demographic, virological, and prophylaxis-related factors among newly diagnosed individuals in Zhejiang Province from 2022 to 2024. We conducted a cross-sectional study among adults newly diagnosed with HIV-1 in Zhejiang Province between 2022 and 2024. Demographic, epidemiological, laboratory, and exposure prophylaxis information was collected through the provincial HIV surveillance system. Partial Pol gene sequences were amplified and sequenced to determine HIV subtypes and drug resistance mutations. PDR was interpreted using the Stanford University HIV Drug Resistance Database. Logistic regression models were applied to identify factors associated with PDR. Among 8798 newly diagnosed individuals, the predominant subtypes were CRF07_BC (n=3913, 44.5%), CRF01_AE (n=2524, 28.7%), and CRF08_BC (n=954, 10.8%), with significant heterogeneity across age groups, sexes, transmission routes, and diagnosis years (<.001). Overall PDR prevalence was 5.7% (n=504), with resistance mainly to nonnucleoside reverse-transcriptase inhibitors (n=352, 4.0%). The most frequently detected mutations were K103N, M184V, and M46L. Multivariable analysis showed that higher baseline CD4 cell counts, diagnosis in 2023 and 2024, and infection with the CRF01_AE subtype were independently associated with increased odds of PDR. Pre-exposure prophylaxis was not associated with overall PDR; however, drug-specific analyses demonstrated significant associations between exposure prophylaxis and pretreatment resistance to emtricitabine (<.001) and lamivudine (<.001). HIV-1 genetic diversity in Zhejiang Province remains complex and dynamic, accompanied by an increasing prevalence of PDR. Although exposure prophylaxis was not linked to overall PDR, its association with emtricitabine-specific resistance highlighted the need for strengthened HIV-1 testing prior to prophylaxis initiation and enhanced resistance surveillance, but they require confirmation in future studies.
