2. Longitudinal Growth in Children With Epidermal Differentiation Disorders.
PMID:日期:2026-09-30Children with epidermal differentiation disorders (EDDs) frequently exhibit impaired growth, yet longitudinal growth trajectories across multiple EDD genotypes have not been systematically evaluated. To characterize longitudinal growth outcomes, including height, weight, and head circumference, in children with EDDs and evaluate genotype-specific associations. This longitudinal cohort study of children with EDDs used registry-based survey data and pediatric growth measurements through age 18 years. Data were collected from June 2022 to June 2025 and analyzed in September 2025. Participants were recruited through the National Registry for Ichthyosis; growth data were analyzed using US Centers for Disease Control and Prevention/World Health Organization pediatric growth standards. A total of 135 individuals aged 30 years or younger from 129 kindreds with confirmed EDDs and 26 unaffected siblings from 20 kindreds were included. Participants were classified into 5 genotype-based groups: autosomal recessive nonsyndromic-EDDs (nEDD), SPINK5-syndromic EDD (sEDD), keratinopathic-nEDDs, FLG-nEDD, and STS-sEDD. The primary exposure was EDD genotype. Secondary exposures included supplemental caloric intake, gastrostomy tube use, preterm birth, and neonatal intensive care unit admission. Primary outcomes were longitudinal height, weight, and head circumference percentiles from birth to age 18 years. Growth trajectories were analyzed using linear mixed-effects models, comparing EDD subtypes with population norms at monthly points within prespecified age intervals (age 0-24, 25-48, and >48 months). Among 135 participants (62 female [45.9%] and 73 male individuals [54.1%]; mean [SD] age, 10.5 [6.8] years; 13 Asian [9.6%], 7 Black [5.2%], 9 Hispanic [6.7%], and 89 White individuals [65.9%]), mean weight and height were less than the 50th percentile during the first 24 months of life (weight, 31.4th percentile; P < .001; height, 39.0th percentile; P = .02), with no significant difference from the population median thereafter. Growth impairment depended on genotype: individuals with SPINK5 variants exhibited persistent deficits at age 25 to 48 months (weight, 17.7th percentile; P = .002; height, 6.8th percentile; P < .001), whereas early deficits associated with ABCA12 and KRT10 variants attenuated over time and were no longer statistically significant. The results of this cohort study suggest that growth impairment in EDDs depends on genotype and is most prominent during the earliest months of life. Routine growth monitoring should be standard of care, particularly for children with high-risk genotypes, such as SPINK5.

3. Best Practices for Managing Pseudofolliculitis Barbae: An Expert Consensus Delphi Study.
PMID:日期:2026-09-23Pseudofolliculitis barbae (PFB) is a chronic inflammatory disease caused by penetration of a shaved, curved hair shaft into the skin. Standardized recommendations for diagnosing and managing PFB are needed. To establish expert consensus recommendations outlining best practices for diagnosing and managing PFB. A 3-round Delphi consensus study was conducted using a panel of board-certified US dermatologists with expertise in PFB. Consensus statements addressing diagnostic criteria, behavioral modification, topical and systemic therapies, procedural interventions, and occupational considerations were developed and evaluated using a 9-point Likert scale. Consensus was predefined as greater than or equal to 75% agreement. Forty statements associated with diagnostic criteria, medical and procedural management, and occupational considerations reached consensus. Behavioral modification of hair removal practices as first-line therapy, use of targeted topical and systemic treatments, and procedural interventions, such as laser hair removal, were supported. No agreement was reached regarding a standardized grading scale, the use of oral isotretinoin for refractory disease, superficial chemical peels, or the use of multiblade razors. Using a Delphi consensus study approach, expert consensus guidelines were established for diagnosing and managing PFB. These recommendations provide an evidence-informed framework to support clinical decision-making, guide future research efforts, and help inform policy considerations associated with PFB management.

6. Lived Experience of Hirsutism in Transfeminine Individuals.
PMID:日期:2026-09-23Hirsutism is commonly associated with gender dysphoria in transgender and gender-diverse individuals, with many seeking permanent hair removal. Little research has examined how hirsutism impacts life and stigma in gender minority groups. To examine lived experiences of hirsutism and hirsutism treatment in transgender and gender-diverse participants. This qualitative study included semistructured interviews with transgender women and nonbinary adults assigned male at birth across 3 clinical settings in Atlanta, Georgia, from February 7 to June 26, 2023. Self-reported hirsutism. Interviews exploring experiences of hirsutism and hirsutism treatment were analyzed via thematic analysis. Among 18 participants who completed the study, 17 were transgender women and 1 was a nonbinary individual assigned male at birth (mean [range] participant age, 38 [22-68] years). Fourteen participants self-rated clinically significant hirsutism levels, and only 1 reported having a diagnosis of hirsutism from a clinician. Participants described experiences of discrimination and rejection related to hirsutism and admitted avoiding social and romantic interactions in anticipation. Hirsutism was also reported to interfere with feminine gender expression and resulted in gender dysphoria. Many individuals went to great lengths to conceal visible hair in public, keeping razors or tweezers readily available for facial hair removal. Barriers to permanent hair removal included appointment burden, clinician attitude, lack of transgender-inclusive hair removal services, cost, and lack of insurance coverage. In this qualitative study, transgender women and nonbinary adults assigned male at birth described experiencing hirsutism-related stigma and facing barriers to permanent hair removal. Multilevel changes aimed to reduce stigma, educate clinicians on hair-removal options, expand transgender-inclusive hair-removal services, and advocate for insurance coverage for permanent hair removal are required to reduce barriers to access medically necessary permanent hair removal for gender dysphoria.

7. Efficacy and Safety of Ruxolitinib Cream in Patients With Cutaneous Lichen Planus: A Randomized Clinical Trial.
PMID:日期:2026-09-23Lichen planus is a chronic, highly pruritic, inflammatory disease that affects the skin and other tissues. To evaluate the efficacy and safety of ruxolitinib, 1.5%, cream in patients with cutaneous lichen planus. This phase 2, 16-week, randomized, double-blind, vehicle-controlled trial with a 16-week open-label extension was conducted from November 2022 to February 2024. Patients were enrolled from 19 sites in the US and Canada. The trial included adults with predominantly cutaneous lichen planus, an Investigator's Global Assessment (IGA) score of 3 or 4, an Itch Numerical Rating Scale score of 4 or greater, and an affected body surface area of 20% or less were enrolled. Data were analyzed from February to August 2024. Patients were randomized 1:1 to ruxolitinib, 1.5%, twice daily cream or vehicle cream for 16 weeks, after which eligible patients applied ruxolitinib cream twice daily as needed in a 16-week, open-label extension. The primary end point was IGA treatment success (IGA score of 0 or 1 [clear/almost clear skin] with a ≥2-grade improvement from baseline) at week 16. Among 64 randomized patients, 46 (72%) were female, 18 (28%) were male, 18 (28%) were African American/Black, 4 (6%) were Asian, 40 (63%) were White, and the median (range) age was 57.0 (18-78) years. Significantly more patients who applied ruxolitinib, 1.5%, cream vs vehicle achieved IGA treatment success (50.0% [95% CI, 31.9%-68.1%] vs 21.9% [95% CI, 9.3%-40.0%], respectively; response rate difference, 29.4% [95% CI, 7.6%-51.3%]; P = .01; primary end point) at week 16, with further improvement to week 32. Similar results were observed in the proportion of patients who achieved a 4 or greater-point improvement from baseline in Itch Numerical Rating Scale score (week 16, 48.4% [95% CI, 30.2%-66.9%] vs 16.1% [95% CI, 5.5%-33.7%]; response rate difference, 33.5%; 95% CI, 12.2%-54.9%; P = .004; secondary end point). No serious treatment-emergent adverse events or discontinuations due to an adverse event were observed with ruxolitinib cream. The results of this randomized clinical trial suggest that ruxolitinib cream significantly reduced lesions and itch in patients with lichen planus. Therefore, ruxolitinib cream represents a potential treatment option for patients with cutaneous lichen planus. ClinicalTrials.gov Identifier: NCT05593432.
