1. Multifunctional CD38 in the pathogenesis and treatment of B-cell lymphomas.
PMID:日期:2026-06-15B-cell lymphoma is a group of non-Hodgkin lymphomas with high heterogeneity. Despite advancement in therapeutics, the clinical outcomes of B-cell lymphomas need to be improved. CD38, a transmembrane multifunctional protein, is widely expressed across various B-cell lymphoma subtypes. Research indicates that CD38 plays multiple pathogenic roles in B-cell lymphomas, including regulation of proliferation, apoptosis, immune modulation, primarily through its enzymatic activity and its influence on signaling pathways in malignant lymphoma cells and immunosuppressive cells, making it a valuable therapeutic target. In recent years, CD38-targeted therapies, such as anti-CD38 antibodies and CD38-directed CAR-T cell therapies, have been explored for B-cell lymphomas. This review summarizes the knowledge on CD38 molecular functions, data of CD38 heterogeneous expression and prognostic role in B-cell lymphomas (from indolent to aggressive subtypes), anti-CD38 antibody efficacy and mechanisms of action, current understanding of the treatment resistance mechanisms, frontier advances in CD38-targeted therapies, and the challenges of novel targeted and immunotherapies in clinical applications, thereby seeking to provide a deep understanding of the role of CD38 in the pathogenesis and treatment of B-cell lymphomas. CD38-targed therapy is especially promising as a novel and optional treatment for CD20-negative lymphomas, for which standard treatment has not been established. Although the efficacy of anti-CD38 antibodies in most other B-cell lymphomas is low in previous clinical trials, in contrast to the remarkable efficacy of daratumumab in multiple myeloma, novel therapeutic strategies and combination therapies in ongoing clinical trials have the potential to overcome the counteracting actions and to improve the clinical outcomes of patients with relapsed/refractory B-cell lymphoma.

2. Therapy-related B-cell acute lymphoblastic leukemia after treatment for multiple myeloma.
PMID:日期:2026-04-25Changing landscape of treatment has significantly improved survival in multiple myeloma (MM) in last two decades by using proteasome inhibitors (PIs), immunomodulatory drugs (IMiDs), monoclonal antibodies, and autologous stem cell transplantation (ASCT). With the associated improvement of overall survival, cumulative exposure to these agents have led to an increased incidence of secondary malignancies. Therapy-related myeloid neoplasms are more frequent than therapy-related B-cell acute lymphoblastic leukemia (t-B-ALL). We describe four patients who developed t-B-ALL following treatment for MM. All had received novel agents, three had lenalidomide maintenance, and two had undergone ASCT. Latency from MM diagnosis to t-B-ALL ranged from 3 to 7 years. Age ranged from 57-63 years with pancytopenia being the presentation. Bone marrow aspirates revealed Myeloperoxidase negative blasts with B-lineage immunophenotype. Cytogenetic and molecular findings were variable. At B-ALL diagnosis, clonal plasma cells or monoclonal band were absent, supporting clonal independence. Patients were treated with vincristine, anthracycline, and steroid-based regimens and achieved measurable residual disease negativity in two patients, however, only one patient alive at 21 months of follow-up and the remaining three succumbing within seven months of diagnosis. Therapy-related B-ALL is a rare but aggressive secondary malignancy in MM, commonly detected when presented with cytopenias. Prognosis of this entity is poor, in comparision to de novo B-ALL, underscoring the need for vigilant clinical follow-up, an urgent need to identify factors that predict the development of therapy-related malignancies and exploration of tailored therapeutic strategies.

3. Venetoclax-based low-intensity therapy in pediatric AML: A viable option for chemotherapy-intolerant patients.
PMID:日期:2026-02-15Childhood acute myeloid leukemia (AML) comprises 15-20% of pediatric leukemias and carries promising survival rates (60-70%) in high-income countries with use of intensive chemotherapy. This is associated with high treatment-related mortality in low- and middle-income countries, especially during induction, due to malnutrition, infections, and insufficient resources. Venetoclax, a BCL-2 inhibitor effective in older adults unfit for standard therapy, has shown potential in pediatric relapse AML. Here, we describe two children with newly diagnosed AML who developed life-threatening toxicities with intensive chemotherapy and were subsequently treated with venetoclax-based low-intensity regimens. Both patients achieved complete remission, attained measurable residual disease negativity, and maintained durable responses with minimal toxicity, allowing largely outpatient management. These cases suggest a promising, feasible alternative for chemotherapy-intolerant patients.

4. Immunological, hematological and biochemical benefits of adjuvant nigella sativa to pharmacotherapy in immune thrombocytopenic purpura patients.
PMID:日期:2026-02-15Patients having acute and chronic immune thrombocytopenic purpura (ITP) suffer from thrombocytopenia, decreased T regulatory lymphocytes (causing immunological tolerance) and abnormal biochemistry (increased inflammatory mediators, autoimmunity, and oxidative stress). Nigella sativa is a prophetic medicine remedy rich in pharmacological ingredients exerting anti-inflammatory and thrombocytosis effects that may improve the biochemistry of ITP patients. To evaluate the benefits of nigella sativa to ITP patients. Ethical committee approval and patients' consents were gained and complied with the declaration of Helsinki. Children's parents or legal guardians provided informed consent. 15 acute ITP patients received steroids or Eltrombopag olamine (revolade) for one month then complete blood count was done in addition to counting CD4 T helper cells and Treg cells using flow cytometry. Then, same patients continued the same treatments plus adjuvant 2 grams/day grinded nigella sativa seeds (taken orally added to foods or drinks) for the next month followed by doing the same blood tests. Another 15 patients having chronic ITP received same treatments and investigations. In acute ITP patients on steroids or revolade, platelets count was low (29.26±5.88 × 10/cc) by the end of the first month and significantly increased to (275±33 × 10/cc) (P<0.001) upon adding adjuvant nigella sativa treatment for one month. Likewise, platelets count was low (28.86±5.88 × 10 platelets/cc) in chronic ITP patients by the end of the first month and significantly increased to (267±26 × 10 platelets/cc) (P<0.001) upon adding adjuvant nigella sativa treatment for one month. Nigella sativa treatment significantly increased the percentage of CD4 T-helper lymphocytes (P<0.05) and Tregs (P<0.01) in both acute and chronic ITP patients. Adjuvant nigella sativa potentiated pharmacological treatments, exerted potent thrombocytosis effects and increased immunological tolerance in acute and chronic ITP patients possibly due to its anti-inflammatory, immunomodulatory and biochemical effects. Nigella sativa is a recommended adjuvant natural remedy to ITP patients. This study is registered among our hematological studies using nigella sativa in treating hematological diseases in ClinicalTrials.gov. NCT02816957.

5. Incidence and mortality of heparin-induced thrombocytopenia in critically Ill patients.
PMID:日期:2026-02-15Heparin is widely used as an anticoagulant in hospitals. Ventilated patients on heparin are at a higher risk of developing heparin-induced thrombocytopenia (HIT). This study aims to examine the effect of HIT on mortality in ventilated patients. We used available ICD-10 codes from 2016 to 2020. Patients with specific ICD-10 codes for HIT and ventilator dependence were included. The incidence of HIT in ventilated patients was calculated, and mortality rates were compared between HIT-positive and HIT-negative ventilated patients. For the year 2020, given the presence of COVID, additional multivariate analysis was performed to adjust for baseline characteristics and confounding factors. We found no significant increase in mortality in patients on ventilation with HIT compared to those without HIT for the years 2016-2019 in the pre-COVID era. However, there was a significant increase in mortality among ventilated patients with HIT in the year 2020 during the first year of the COVID pandemic before vaccination (odds ratio [OR] = 2.03, 95% confidence interval [CI] = 1.26-3.26, P = 0.004). However, after adjusting for age, gender, race, and COVID-19 diagnosis, only COVID-19, but not HIT, showed a significantly higher risk of mortality in ventilated patients with HIT compared to no HIT (OR = 3.80, 95% CI = 3.47-4.15, P < 0.001). These findings suggest HIT is not associated with increased mortality among ventilated patients. However, in the year 2020, COVID-19 infection increased the mortality of ventilated patients by almost 4-fold regardless of HIT. This data suggests that HIT is not an independent risk for mortality in ventilated patients.

6. Assessing glucose 6-phosphate dehydrogenase activity in children with acute lymphoblastic leukemia and its relationship to disease activity.
PMID:日期:2025-08-15Glucose 6-phosphate dehydrogenase (G6PD) activity of red blood cells (RBC) may be helpful as a prognostic factor and a probable predictive indicator of disease activity in children with acute lymphoblastic leukemia (ALL). This cross-sectional, case-control study was performed on almost 133 pediatric ALL cases from 2016 to 2020 in an oncology hospital. Patients with a history of blood transfusion within the last three months, acute hemolytic crisis, any other type of enzyme deficiency like pyruvate kinase and hexokinase, and chronic liver disease were excluded. The G6PD activity in RBC was measured using the spectrophotometric method. In addition, the G6PD activity was assessed in 133 normal individuals as a control group. According to the kit, the G6PD <1.5 IU/g of Hb level was recognized as severely deficient. The correlation of G6PD activity with disease activity and other parameters in ALL patients was determined using the Pearson correlation test. Data were measured by an independent t-test and a one-way ANOVA test. The mean G6PD activity of RBC in the control (n=133) and patient group (n=128) was 9.1±2.08 IU/g of Hb and 11.12±3.8 IU/g of Hb, P<0.001, respectively. There was a significant difference in the G6PD activity of RBC in patients' blastic and non-blastic phases, t (128) =-2.48, P=0.014. The G6PD activity of RBC is higher in childhood ALL than in the control group. Moreover, the G6PD activity of RBC in the blastic phase of leukemia was higher than that of patients in remission.

7. Rosai-Dorfman-Destombes disease in an elderly man. A case of necrosis in the lymph node.
PMID:日期:2025-08-15Rosai-Dorfman-Destombes disease (RDD), often associated with autoimmune disease, is rare in the elderly. We report a 79-year-old Japanese man with RDD, who had been treated for non-specific interstitial pneumonia (NSIP) for longer than 6 years. Detecting RDD lesions was evaluated with imaging studies, including PET-CT, and the RDD diagnosis was based on the pathological findings of a biopsied lymph node. Findings and Clinical course: PET-CT revealed an FDG-avid 5.5 cm-sized mass (SUVmax; 7.12) at the right lung and enlarged lymph nodes at the bilateral supra-clavicular area (SUVmax; 10.12). A supraclavicular lymph node biopsy confirmed the diagnosis of RDD; however, it was characterized by lymph node necrosis, which is rarely noted in the RDD tissue. Three months after the RDD diagnosis, the patient developed cold agglutinin disease (CAD), causing severe anemia, for which packed red blood cell transfusions and sutimlimab therapy were planned; however, the patient died of presumable NSIP exacerbation. RDD occurs even in the elderly. RDD in this case was associated with autoimmune NSIP and CAD. The presence of necrotic foci in the biopsied lymph node does not contradict the diagnosis of RDD.

8. Acute erythroid leukemia in a child: insights from a rare case.
PMID:日期:2025-08-15Acute erythroid leukemia (AEL) is an extremely rare subtype of acute myeloid leukemia (AML), accounting for less than 1% of AML cases. It predominantly affects older adults and is characterized by a proliferation of erythroid precursors, typically constituting >80% of bone marrow components, with ≥30% being proerythroblasts and often associated with poor prognosis. We present the case of a 15-year-old female who developed de novo AEL, an unusual presentation in a pediatric patient. The patient presented with fever, increased vaginal bleeding, leukocytosis, severe anemia, and 87% erythroid cells in peripheral blood smear. Bone marrow analysis revealed 90% erythroid precursors, 60% of which were proerythroblasts. Immunophenotyping confirmed AEL, showing positive expression of CD71, CD235a, CD36, and negative myeloid markers. Next-generation sequencing identified mutations in ARID1A and ATM genes, without mutation. The patient was treated with AML induction therapy (7+3 regimen). This report highlights a rare case of de novo AEL in a pediatric patient, emphasizing its clinical presentation, the diagnostic challenges posed by its rare occurrence and overlapping features with other hematological disorders, and the critical role of a comprehensive diagnostic evaluation, including morphology, flow cytometry, and genetic studies, in achieving timely diagnosis and appropriate management. Further research is needed to understand the molecular landscape better and identify optimal therapeutic strategies for pediatric AEL.

9. Prognostic value of platelet volume indices in heart failure: a systematic review and meta-analysis.
9. 血小板体积指数在心力衰竭中的预后价值:系统综述和荟萃分析PMID:日期:2025-08-15Heart failure (HF) is a complex condition with a substantial global prevalence and clinical burden. Prognostic biomarkers are essential for effective management. This study systematically reviews and synthesizes the prognostic value of mean platelet volume (MPV) and platelet distribution width (PDW) in HF patients. A comprehensive search was conducted in Medline, Scopus, Web of Science, and Embase databases. Eligible studies were identified, screened, and selected according to pre-defined criteria. Data extraction and quality assessment were performed by independent reviewers. Meta-analyses were conducted using random-effects models, and heterogeneity was assessed using statistics. Publication bias was assessed using Egger's regression and Begg's tests. From 12471 records, 21 studies were included. MPV showed significant predictive value, with pooled hazard ratios (HR) of 1.49 (0.67-3.32) and odds ratios (OR) of 1.71 (1.5-1.91) for mortality and morbidity. The pooled mean difference in MPV values between the affected and unaffected subjects was 0.66 (0.21-1.1, P=0.008). MPV was positively correlated with NT-proBNP levels (pooled coefficient: 0.13, P=0.028). The pooled area under the curve for MPV in prognosticating adverse outcomes was 0.75 (0.69-0.82). However, PDW did not show significant prognostic value (HR: 1.56, OR: 1.11). MPV is a useful prognostic marker in HF, associated with increased mortality and morbidity. Prognostic significance of PDW remains unclear, requiring additional research. The application of MPV can improve risk stratification and management of HF, but further research with larger populations and diverse settings is essential to confirm these findings and establish clinical reference ranges.

10. A systematic review of ABCG8 mutation and sitosterolemia.
PMID:日期:2025-06-15Sitosterolemia is a rare inherited condition caused by elevated levels of plant sterols in the plasma, characterized by mutations in ABCG5 and ABCG8 genes. A scarce occurrence in this condition are hematological abnormalities such as hemolytic anemia, stomatocytosis, and macrothrombocytopenia. We conducted a meta-analysis and systematic review to answer these questions regarding patients who have hemolytic anemia and ABCG8 mutation. 13 reports were shortlisted for the final analysis (Observational studies-6, case series-4, case reports-3). Descriptive statistics were utilized to study the patient characteristics. From the 13 reports that we found in available literature, we identified 19 cases of ABCG8 mutation and anemia. From the random-effects proportions model, the chance of this event occurring among patients with sitosterolemia was 6.8% [0.068, 95% Confidence Interval (CI) 0.016-0.120, P=0.010] (I 24.68%) (14/145). Thrombocytopenia and stomatocytosis were frequently reported. Splenomegaly and xanthomas were other common associations. To the best of our knowledge, we provide the first report of the prevalence of anemia, specifically in patients with sitosterolemia caused by a mutation in the ABCG8 gene. At 6.8%, this is an extremely rare occurrence in an already infrequent disease.
