Various anthropometric and dietary indicators influence insulin resistance (IR), and lipid biomarkers may play a pivotal mediating role in these relationships. To investigate the mediating effects of lipid biomarkers on the relationships between anthropometric, dietary indicators, and IR, and the moderating role of sex in these associations. A hospital-based cross-sectional study was conducted with 169 participants. Parallel mediation models assessed the mediating effects of lipid biomarkers (triglycerides, HDL, LDL, and total cholesterol) on the relationships between anthropometric (BMI, waist circumference, waist-to-hip ratio, and waist-to-height ratio) and dietary indicators (at least one starchy staple, at least one fruit, consumed all five recommended food groups (ALL-5), global dietary recommendations (GDR) score, and noncommunicable disease risk/protect scores) and IR. Moderated mediation analyses evaluated the moderating effect of sex. Statistical analyses were bootstrapped. The cumulative effect of lipid biomarkers fully mediated the relationship between WHtR and IR (Indirect Coef. = 0.65; 95% CI [0.02, 1.70]). TG significantly mediated the associations between WHR and IR (indirect Coef. = 0.55; 95% CI [0.11, 2.63]), BMI and IR (indirect Coef. = 0.76; 95% CI [0.17, 2.27]), and WC and IR (indirect Coef. = 0.72; 95% CI [0.04, 2.63]). Furthermore, LDL-C (Indirect Coef. = 1.99; 95% CI [0.24, 9.61]) and HDL-C (Indirect Coef. = 0.55; 95% CI [0.03, 2.57]) mediated the relationships of starchy staple and fruit intake with IR, respectively. Sex moderated the direct effect of WC on IR (interaction Coef. = 5.11, = 0.002) but did not moderate the indirect pathways involving lipid biomarkers. These findings elucidate the intricate interplay between body composition, diet, lipid biomarkers, sex, and IR, providing insights for developing targeted prevention and intervention strategies to combat IR.

Imagine a surgeon's critical decision: Should the gallbladder be removed now, along with the planned bariatric surgery, or risk the complication and necessity of a second surgery later? This clinical dilemma is central to treating morbidly obese patients, who face a high prevalence of gallstone disease exacerbated by rapid postoperative weight loss. The best approach to managing existing gallstones in bariatric candidates remains debated, with debate focusing on whether combining laparoscopic cholecystectomy (LC) with bariatric surgery is both safe and advantageous. In this pilot study, we provide randomized evidence to guide this decision. In this prospective randomized controlled pilot study, 58 morbidly obese patients with ultrasound-confirmed gallstones were randomly assigned to two groups: Group I ( = 30) received bariatric surgery and LC; Group II ( = 28) had bariatric surgery only, with LC delayed for symptoms. The primary outcomes were clearly defined as operative time, intraoperative complications, and postoperative morbidity, providing a focused measure of safety and efficacy. Secondary outcomes included hospital stay, pain, and follow-up gallstone symptoms. Baseline demographics and comorbidities were similar across groups. Operative time was longer in Group I (98.93 ± 11.58 min) than in Group II (75.18 ± 11.26 min, < 0.001). An extra port was used in 20% of Group I patients, compared with none in Group II ( = 0.012). No significant differences were observed in bleeding, bile leakage, postoperative complications, or hospital stay. Group I reported higher pain scores ( < 0.001). During follow-up, 79.3% of Group II developed symptomatic gallstones, requiring later cholecystectomy. Concomitant LC during bariatric surgery in morbidly obese patients with pre-existing gallstones is demonstrated to be safe and feasible, with acceptable increases in operative time and postoperative pain. The high rate (79.3%) of symptomatic gallstone development in patients who did not undergo concomitant cholecystectomy supports adopting routine concomitant LC to prevent future morbidity, thereby influencing clinical decision-making and standard practice. Orlistat is a potent lipase inhibitor utilized as a preventive agent for obesity and fat absorption control. Existing literature presents conflicting findings regarding its impact on lipid parameters. This systematic review followed the PRISMA guidelines and was registered in PROSPERO (ID: CRD42024550889). A comprehensive search of PubMed, Scopus, Web of Science, and Cochrane Register of Controlled Trials was conducted for studies published before January 19, 2025. Eligible studies included randomized controlled trials (RCTs) evaluating orlistat in adults (≥ 18 years) with dyslipidemia. Furthermore, the Grading of Recommendations, Assessment, Development, and Evaluations assessment tool was employed to analyze the certainty of evidence or each outcome. A total number of 1369 participants, with 682 in treatment and 687 in control categories, were included in our study. Orlistat reduced body mass index (BMI) (SMD [95% CI]: -0.30 [-0.58, -0.03], value (heterogeneity) = 0.026), and also it was associated with a decrease in high-density lipoprotein cholesterol (SMD (95% CI): -0.31 [-0.48, -0.13], value (heterogeneity) = 0.436). Changes in waist circumference (WC) and triglycerides (TGs) did not reach statistical significance in the primary analysis (WC: SMD [95% CI] -0.1562 [-0.3138; 0.0015], = 0.0%, -value (heterogeneity) = 0.7572; TG: SMD [95% CI] -0.1668 [-0.7979; 0.4642], = 97.7% value (heterogeneity) < 0.0001); however, after publication-bias adjustment using the trim-and-fill sensitivity analysis, meaningful reductions were discovered for WC (SMD (%95CI): -0.1712 [-0.3248; -0.0176], = 0.0%, value (heterogeneity) = 0.7696) and TG (SMD (%95CI): -0.8900 [-1.6619; -0.1181], = 97.9%, value (heterogeneity) < 0.0001). The secondary analysis demonstrated that follow-up duration accounted for 30% of TG heterogeneity, suggesting a small but significant decline in orlistat's TG-lowering effect over time (slope: -0.1239; 95% CI: -0.2355, -0.0123; value = 0.0295). No significant changes were observed in other parameters of the study. Besides, gastrointestinal issues were the most frequently reported adverse events among the studies. Our findings suggest that orlistat meaningfully reduces BMI but is associated with decreased HDL-C, which may be undesirable given HDL-C's protective role in cardiovascular health. Evidence for reductions in TG and WC is uncertain: the primary meta-analysis showed no statistically significant effects, whereas trim-and-fill sensitivity analysis suggested potential reductions. No significant short-term impact on TG was observed, though a modest reduction may emerge with prolonged use.

This randomized controlled trial aimed to evaluate the impact of intermittent fasting (IF) during resistance training (RT) on body composition, muscular strength, and the testosterone:cortisol ratio in obese males. Twenty obese males (aged 20-30, BMI 30-36 kg/m) were selected from eligible volunteers and randomly assigned to control (regular diet) and IF (4:3 IF) groups. All subjects participated in RT 3 days/week for 8 weeks. Forty-eight hours before and after the protocol, blood sampling and anthropometric measurements were done in a fasting state, and data were analyzed at a significance level of < 0.05. The IF/RT group lost twofold more weight and fat, had higher arm and chest measurements, and had less waist circumference than the C/RT group. The testosterone levels and muscle strength improved with RT, and there was no difference between the C/RT and IF/RT groups. It appears that intermittent fasting may enhance the efficacy of RT in obese males and is unlikely to have any detrimental effects on muscular strength or the anabolic index. The global obesity epidemic challenges health systems, driving people to seek metabolic and bariatric surgery (MBS), especially laparoscopic sleeve gastrectomy (LSG). Many MBS centers have limited resources for patient education, creating knowledge gaps that lead patients to search online. AI chatbots, such as ChatGPT, can provide reliable medical information, though concerns about accuracy and completeness remain. The study involved four fellowship-trained minimally invasive surgeons (MISs), nine fellows (MIFs), and two general practitioners (GPs) in the MBS multidisciplinary team from March 1, 2024, to March 30, 2024. Seven AI chatbots were selected, including ChatGPT 3.5 and 4, Bard, Bing, Claude, Llama, and Perplexity, based on their public availability on December 1, 2023. Forty patient questions regarding LSG were sourced from social media, MBS organizations, and online forums. Experts and chatbots answered these questions, with their responses evaluated for accuracy and comprehensiveness on a 5-point scale. Statistical analyses compared groups' performance. Chatbots demonstrated a higher overall performance score (2.55 ± 0.95) compared to the expert group (1.92 ± 1.32, < 0.001). Among chatbots, ChatGPT-4 achieved the highest performance (2.94 ± 0.24), while Llama had the lowest (2.15 ± 1.23). Expert group scores were highest for MISs (2.36 ± 1.09), followed by GPs (1.90 ± 1.36) and MIFs (1.75 ± 1.36). The readability of chatbot responses was assessed using Flesch-Kincaid scores, revealing that most responses required reading levels between the 11th grade and college level. Furthermore, chatbots exhibited fair reliability and reproducibility in response consistency, with ChatGPT-4 showing the highest test-retest reliability. AI chatbots generated accurate and comprehensive answers to common bariatric patient questions, suggesting promise as a scalable aid for patient education. However, readability often exceeds recommended levels, performance varies by model, occasional inaccuracies occur, and medicolegal considerations remain unresolved. Accordingly, chatbots should complement clinician counseling, and future work should improve readability and reliability and evaluate real-world safety and impact.

Obesity has become a global health epidemic, contributing significantly to numerous chronic diseases, including metabolic disorders, cardiovascular diseases, and certain cancers. This study aimed to assess the prevalence of overweight, obesity, and abdominal obesity and the associated factors in Duhok City, Kurdistan Region of Iraq. A population-based cross-sectional study was conducted in Duhok City, Iraq, including 490 participants aged 18 years and older. A multistage sampling technique was used, and data were collected via structured questionnaires covering sociodemographic and clinical characteristics. Anthropometric measurements were used to classify participants according to their body mass index (BMI) and waist circumference. The overall prevalence of overweight, obesity, and abdominal obesity was 38.4%, 24.3%, and 72.9%, respectively. Factors significantly associated with overweight in multivariate logistic regression included age group 30-44 (adjusted odds ratio [aOR]: 2.356, p = 0.018), married individuals (aOR: 2.040, p = 0.038), and smokers (aOR: 0.562, p = 0.045). Obesity was associated with age group 30-44 (aOR: 2.769, p = 0.030) and age group 45-59 (aOR: 5.384, p = 0.002). Abdominal obesity was significantly associated with gender, where males had lower odds compared to females (aOR: 0.248, p = <0.001), age group 30-44 (aOR: 2.630, p = 0.011), age group 45-59 (aOR: 4.793, p = 0.002), married individuals (aOR: 3.313, p = 0.001), and sleep disturbance (aOR: 1.790, p = 0.025). The findings indicate a high prevalence of overweight, obesity, and abdominal obesity in Duhok City, with various associated factors. Public health initiatives targeting these associated factors are essential to address the growing obesity epidemic in the region. Further studies with broader geographic coverage and larger sample sizes are needed to reinforce these results and inform policy interventions.

High body-mass index (BMI) is the second largest contributor to stroke burden and the fastest-growing risk factor for stroke globally. Given China's rapidly aging population and the increasing obesity epidemic, understanding the long-term trends and drivers of stroke burden is critical. High BMI is a chief risk factor for stroke. We aim to evaluate the long-term trends and population risk on stroke mortality burden caused by high BMI in Chinese population between 1990 and 2023. The decomposition analysis and linear regression were utilized to determine the contribution of population aging and other risk factors to the changing trends in the mortality and disability-adjusted life year (DALYs) of stroke caused by high BMI. In China, the mortality rate of stroke caused by high BMI increased by 1.640/100,000 during the study period of 1990-2023. The increase was decomposed as 1.180/100,000 due to population aging and 0.460/100,000 due to other risk factors, with population aging contributing 72% and other risk factors contributing 28%. The DALYs rate increased by 52.228/100,000, of which 32.999/100,000 was due to population aging and 19.229/100,000 was due to other risk factors. Notably, population aging contributed 63% of the increase. These findings confirm that population aging was the dominant driver of the increased stroke burden attributable to high BMI, whether measured by mortality or DALY. Population aging is the dominant driver of the increasing stroke burden attributable to high BMI in China. Effective burden reduction will require integrated prevention strategies targeting both the elderly population and modifiable metabolic risk factors.

Chronic knee pain (CKP) is a globally prevalent condition among older adults, yet the predictive value of diverse obesity-related indices for CKP remains insufficiently explored in the Chinese population. This study aims to investigate the longitudinal relationship between abdominal obesity, multiple body shape indices (including weight, body mass index [BMI], waist circumference [WC], visceral adiposity index [VAI], and conicity index [CI]), and the risk of developing CKP using data from the China Health and Retirement Longitudinal Study (CHARLS). We conducted a longitudinal analysis using data from CHARLS Wave 3 (2015, baseline) to Wave 5 (2020). The inclusion criteria were participants aged 55 years or older with complete biomarkers from fasting blood samples and anthropometric data at baseline. We excluded participants who: (1) had missing blood test results or body index data, (2) were lost to follow-up, (3) had missing information regarding knee pain, and (4) reported pre-existing knee pain at baseline (Wave 3). Abdominal obesity and body shape indices were assessed at baseline. The incidence of CKP was defined as reporting knee pain in two consecutive follow-up waves. Logistic regression and restricted cubic splines were employed to evaluate the associations. 889 out of 3288 participants, which is 27.04%, were identified as CKP during the 5-year follow-up period (2015-2020). After adjusting for various confounding factors, the OR for CKP associated with BMI was 1.03 (95% CI: 1.01-1.05; p = 0.020), WC was 1.01 (95% CI: 1.01-1.02; p = 0.011), and people with abdominal obesity was 1.36 (95% CI: 1.15-1.60; p < 0.001). Subgroup analysis demonstrated similar results without any interactions. In restricted cubic spline regression, the relationship between BMI and CKP is nonlinear, and the relationship between WC and CKP is linear. Among middle-aged and older adults, abdominal obesity, BMI, and WC should be regarded as risk factors of CKP, whereas VAI, CI, and body weight should not. Notably, this study revealed a nonlinear relationship between BMI and CKP, contrasting with the linear link observed between WC and CKP. Furthermore, compared to BMI and WC, abdominal obesity demonstrates a higher statistical effect size and more pronounced clinical significance.

To investigate the independent and joint associations of the C-reactive protein-triglyceride-glucose index (CTI) and the Chinese visceral adiposity index (CVAI) with incident stroke. Additionally, we evaluated whether a novel combined index, CTI-CVAI, outperforms the established TyG-CVAI in predicting stroke risk. This cohort study included 8267 stroke-free participants from the China Health and Retirement Longitudinal Study (CHARLS). Cox proportional hazards models, Kaplan-Meier curves, and restricted cubic splines were utilized to analyze associations with incident stroke. Additive and multiplicative interactions between CTI and CVAI were examined. Predictive performance was assessed via receiver operating characteristic (ROC) analysis, comparing CTI-CVAI against TyG-CVAI using DeLong's test and the Boruta algorithm for feature importance. During a mean follow-up of 8.37 years, 816 incident stroke cases were identified. Elevated CTI and CVAI were independently associated with increased stroke risk in a linear dose-response manner, without significant interaction. The novel CTI-CVAI index exhibited a robust association with stroke; participants in the highest tertile had a 2.14-fold higher risk (HR 2.14, 95% CI 1.75-2.62) compared to the lowest. Crucially, CTI-CVAI demonstrated superior predictive accuracy (AUC 0.627) over the established TyG-CVAI index (P = 0.002), a finding supported by consistent results across subgroups. CTI and CVAI are independent risk factors for stroke, and their combination (CTI-CVAI) offers enhanced predictive value, outperforming the TyG-CVAI index. Monitoring CTI-CVAI may serve as a superior strategy for early risk stratification and targeted stroke prevention in middle-aged and elderly populations.

    日期:
    2026

Weight stigma in healthcare settings refers to the prejudiced devaluation of individuals with higher body weight, often manifested through causal attribution comments. This stigma reflects a complex interplay of psychological and behavioral processes that may contribute to outcomes such as medical avoidance. To determine whether weight stigma in healthcare settings is directly associated with internalized weight stigma, uncontrolled eating, and medical avoidance, and whether these associations are mediated through a serial pathway culminating in healthcare avoidance. A cross-sectional study was conducted with 309 healthcare users in Chile (M = 37.72 years; SD = 15.68; 75% women), using self-report measures and structural equation modeling. All direct effects were significant, except for the path between weight stigma and uncontrolled eating. The serial mediation via internalized weight stigma and uncontrolled eating was supported (β = 0.041, p 0.046; 12.06% of the total effect), as was the simple mediation via internalized stigma alone (β = 0.071, p = 0.048; 20.88%). Mediation through uncontrolled eating alone was not significant (β = 0.013, p = 0.239). The total effect of weight stigma in healthcare settings on medical avoidance was significant (β = 0.340, p < 0.001; 63.24%). The model demonstrated good fit indices. Weight stigma in healthcare settings negatively impacts psychological and behavioral health. Addressing weight stigma is essential to reducing avoidance behaviors and improving continuity of care. These findings highlight the importance of implementing public policy reforms and providing training for healthcare professionals.
