1. Thumb Reconstruction Using a Modified Masquelet Technique Following Crush Injury: A Case Report.
PMID:日期:2026-03Traumatic crush injury of the thumb is devastating and often resulted in poor functional outcome. Various reconstruction options available according to surgical expertise. Masquelet technique is well-established in the long bones of lower limb. Only a handful of cases reported for thumb, especially in Asia region. We described a case of crush injury of right thumb following a trauma. The bony defect was initially filled with antibiotic spacer and subsequent reconstruction with tricortical iliac graft, a modified Masquelet two-stage technique. It is a versatile tool which provide satisfactory functional outcome and hand appearance.

3. Outcome Analysis of Single Stage Total Knee Arthroplasty with Long Tibial Stem in Complex Primary Osteoarthritis with Tibia Stress Fracture.
PMID:日期:2026-03The treatment of severe osteoarthritis of knee with proximal tibia stress fracture is daunting. There is no palpable treatment guideline for this condition. This study evaluates the functional and radiological outcomes of single staged long tibial stem total knee arthroplasty. In this retrospective cohort, 10 patients were enrolled with severe osteoarthritis of knee with tibia stress fracture from 2020 to 2023. All patients were operated with posterior stabilising long tibial stem total knee arthroplasty. Pre-operative and post-operative functional and radiological assessments were evaluated. The follow-up period ranged from 16 to 28 months, with a mean of 21 months. 15.2° varus was the average HKA angle (SD 3.68°). Knee range of motion increased from 106° (range 90 - 115°) to 127° (range 120 - 130°), while FFD improved from 16° (range 5 - 30°) to 1.5° (range 0 - 5°). Both the mean Knee Society score, and its functional score increased from 21.5 (range 5-35) to 78 (range 65-90) (p<0.001) and from 21.7 (range 4-42) to 89.3 (range 82-95) (p<0.001), respectively. All patients had fracture union during a follow-up period of three to six months (mean 3.90 months). Stress fracture in not uncommon in elderly osteoarthritic patients, long tibial stem total knee replacement provides safe and sublime result. Extension rod provides better restoration of alignment and facilitates fracture healing.

4. Prevalence of Symptomatic Reherniation After Lumbar Discectomy Using a Bone-Anchored Annular Closure Device and Associated Contributing Factors: A Meta-Analysis.
PMID:日期:2026-03The primary issue following lumbar discectomy for disc herniation is the risk of reherniation in the post-operative period. Many surgical techniques have been proposed to treat disc reherniation, however, the optimal one remains variable. This meta-analysis aimed to investigate the prevalence of symptomatic reherniation after using a Bone-anchored annular closure device following lumbar discectomy and the contributing factors. Identification of published literature was performed on PubMed, Google Scholar, Scopus, and Web of Science databases. Studies published until 14 February 2024 reported the prevalence of symptomatic reherniation after using a Bone-anchored annular closure device following lumbar discectomy and the associated contributing factors. A random effects model was used to conduct Bayesian frequentist network meta-analysis and pair-wise meta-analysis, with the assessment based on standardised mean difference (SMD) and 95% confidence interval (CI). Eleven studies published in 2012 - 2022 recruiting a total of 5195 patients were included in the meta-analysis. The prevalence of reherniation in ACD and control groups was 23.2% (95% CI: 18.2% - 28.1%) and 36.4% (95% CI: 28.2% - 44.5%), respectively. The moderator effect of sample size is significant for pooled data of the ACD group (p-mod=0.002), but not for the control group (p-mod=0.278). After the adjustment with sample size, the prevalence rates were 13.6% (95% CI: 6.2% - 21.1%) and 29.6% (95% CI: 14.9% - 33.2%) for ACD and control groups, respectively. Comparatively to lumbar discectomy alone, using a Bone-anchored annular closure device following lumbar discectomy decreased the symptomatic reherniation rate and post-operative complications, as well as the necessity for subsequent surgeries.

5. Outcomes of Medial Open Wedge High Tibial Osteotomy in a University Hospital.
5. 大学医院内侧开放楔形高位胫骨截骨术的结果PMID:日期:2026-03Medial open wedge high tibial osteotomy (MOWHTO) is one of the modalities to treat unicompartmental knee osteoarthritis (KO). Many studies have shown good outcomes of MOWHTO, but there are no published series of Malaysian patients. The aim of this study is to determine the outcome of MOWHTO in primary medial compartment KO in a university teaching hospital in Malaysia. This is a retrospective study of patients who underwent MOWHTO in our Joint Reconstruction Unit between 2017 and 2022 with a minimum of 12 months follow-up. Data from a total of 15 knees were reviewed. The mean age of patients was 41.8 years, and the mean BMI was 31.7. The mean Kellgren-Lawrence (KL) osteoarthritis grade was 2.87 (±0.52). The mean hip knee angle (HKA) pre-operatively was 12.26° varus and post-operatively was 3.33° valgus. Mean correction of HKA was 13.59°. The results revealed an improvement in all patients, as observed from the significant mean difference between pre-operative (38.53) and post-operative (77.60) Knee Injury and Osteoarthritis Outcome (KOOS) (p-value<0.001) scores. Patients who were above 50 years old showed a significantly better improvement in KOOS score compared to those below 50 (p-value <0.05). There was no significant difference observed between BMI and KOOS score improvement (F-stat=0.580, p-value >0.05). MOWHTO is a good treatment option in medial compartment primary KO with varus deformity among Malaysian population. A larger sample size with a longer follow-up period is needed to draw a definitive conclusion.

6. Comparison of Outcome Between Neoadjuvant Chemotherapy with Limb Salvage Surgery and Amputation with Chemotherapy in Osteosarcoma Patients: A Systematic Review.
PMID:日期:2026-03Osteosarcoma is a rare malignant bone tumour arising from primitive mesenchymal tissue, most often affecting males aged 10-30. Historically, treatment involved amputation or disarticulation, with a 5-year overall survival rate of just 20%. This systematic review included studies from MEDLINE, Cochrane, EMBASE, and Google Scholar. Additionally, reference lists of relevant sources were manually reviewed. The search, conducted through 2021, used terms such as amputation, osteosarcoma, bone cancer, recurrence, and metastasis. Eight relevant studies were thoroughly reviewed, most conducted at single institutions and published between 1990 and 2019, covering data from 1983 to 2015 and involving 1,389 patients. Three studies (646 patients) assessed 5-year disease-free survival (DFS) in limb osteosarcoma, reporting a median DFS of 56.9 months (range: 6-78 months). Five studies (969 patients) analysed local recurrence, showing higher relapse rates with limb salvage surgery (LSS) compared to amputation (73.5% vs. 28.1%). Additionally, six studies (1,141 patients) evaluated 5-year overall survival, with better outcomes in the LSS group (351 survivors) compared to the amputation group (248 survivors). In conclusion, this thorough review makes a valuable contribution to the current understanding of survival and local recurrence outcomes in patients with limb osteosarcoma treated with either LSS or amputation. The findings suggest that LSS is associated with a higher 5-year survival rate compared to amputation.

7. What is the Functional Role of Obturator Externus Muscle? The Case of a Moderate Tear in a Professional Soccer Player: A Case Report.
PMID:日期:2026-03The obturator externus muscle is a pelvic and hip muscle with a complex anatomical arrangement, which is involved in hip stability. This paper presents a new case of an obturator externus muscle tear in a professional soccer player, offering valuable insights into the clinical presentation, diagnostic challenges, and successful conservative management. Additionally, a comprehensive review of the literature on this topic is presented. A 21-year-old player reported anterior hip and adductor discomfort after a game, with no recollection of a specific traumatic event. Clinical examination revealed tenderness and limited range of motion during external hip rotation with flexion. Ultrasonography was inconclusive, but magnetic resonance imaging showed a partial obturator externus muscle tear. The athlete was submitted to a structured exercise program and, despite the injury, continued training and playing without functional limitations. The magnetic resonance imaging, performed after 35 days, revealed a complete recovery. This paper highlights the rarity of obturator externus muscle tears, their potential underreporting, and the diagnostic value of magnetic resonance imaging in identifying these deep-seated injuries. Importantly, the player's rapid recovery and ability to continue playing suggest that conservative management can lead to excellent functional outcomes. The possible reasons explaining the complete recovery, including the role of this muscle in hip stability and individual anatomical variations, are discussed.

8. The Efficacy of Povidone-Iodine in Eradicating Staphylococcus aureus Biofilm on Stainless Steel Alloy Implants.
PMID:日期:2026-03is the leading biofilm-forming microorganisms in orthopaedic implant infections. The biofilms formed are difficult to eradicate and resistance to antibiotics. This current study aims to determine the effectiveness of povidone-iodine; an antiseptic solution in eradicating biofilm on stainless steel alloy. In addition to the usual Colony-Forming Unit (CFU) used for verification, Scanning Electron Microscope (SEM) is used to validate the formation and eradication of the biofilms. This is an study where the biofilm is formed by inoculating clinically isolated , incubated for 24 hours onto stainless steel alloy 316L implants. The implants are then irrigated using povidone-iodine solution with varying concentrations (5 and 10%) and durations (30, 60, and 180 seconds). The anti-biofilm effect was evaluated using plating and SEM methods to confirm its effectiveness. The process is repeated after 24 hours of post-irrigation reincubation to detect any rebound growth. No biofilm seen after irrigation with povidone-iodine at 5% and 10% concentrations at 30, 60 and 180 seconds, respectively, in both CFU count and SEM. This result is replicated after 24 hours of reincubation, in assessing for rebound growth. Our study supports that a minimum of 5% povidone-iodine with a minimum irrigation time of 30 seconds are effective at eliminating biofilm on stainless steel alloy implants. Both CFU count and SEM yield similar value in validating the presence of biofilm. Additionally, SEM allows visualisation of the morphology of the biofilm.

9. Analysis of Factors Affecting the Union of Closed Subtrochanteric Femur Fractures treated by Cephalomedullary Nailing.
PMID:日期:2026-03With surgical advancements and improved implants and instrumentation, nailing is the procedure of choice in subtrochanteric fractures. However, failure in achieving reduction of the multidirectional displacement of the fragments prior to passing the nail, contributes to delayed/non-unions at the fracture site leading to implant failures. In this study, we aim to analyse the factors affecting union of closed subtrochanteric fractures treated by nailing. In this retrospective study, closed subtrochanteric fractures treated with cephalomedullary nailing between 2015 and 2019 were included. Demographic, surgical and radiological data were retrieved and analysed. A total of 60 cases were eligible to be included in the study. Majority of patients were male (50), with a mean age of 46.07±16.40 years. Twenty-two fractures were multifragmentary having a separate butterfly fragment. In 27 patients mini-open technique was used to get the anatomical alignment and to hold reduction until fixation. Overall, the mean time for union was 7.63±5.85 months. We had nine delayed unions and eight non-unions. Varus alignment in the coronal plane of more than 8.5° was the only significant factor associated with delayed or non-union apart from loss of medial continuity. We recommend achieving fracture reduction with less than 8.5° of varus malalignment in the coronal plane. Varus malalignment is poorly tolerated in fractures at this region. To achieve this, we suggest having a very low threshold to minimally open the fracture site for reduction of these fractures, which does not have any negative effect on the fracture union.

10. Dual Plating with 4.5mm PHILOS for Managing Challenging Humerus Shaft Non-Unions: A Novel Technique.
PMID:日期:2026-03Fixation failures of humeral shaft fractures are uncommon. When they do occur, revision fixation can be challenging. This study aims to evaluate the clinical and radiological outcomes of a novel surgical technique for managing challenging humerus midshaft fractures and non-unions, specifically assessing union and complication rates. This retrospective study included all consecutive patients who underwent surgical fixation of midshaft humerus fractures using our novel dual plating technique between October 2016 and July 2019. Six patients were included. At follow-up, union was assessed using plain films. Mean age was 50.8 ± 18.0 years, including 4 males and 2 females. All surgeries were revision fixations except for one primary fixation. Mean follow-up period was 6.5 ± 2.3 months. At final follow-up, 5 of 6 patients (83.3%) achieved clinical and radiographic union, with a mean time to union of 6.4 ± 2.0 months. Residual angular deformity at union averaged 1.2 ± 0.8° in the coronal plane and 2.0 ± 2.0° in the sagittal plane. Aside from one case of delayed union, no major complications were reported. The novel dual plating technique can be reliably executed with good results and shows promise as a surgical treatment option for non-union or implant failure. While particularly valuable in revision settings, it may be considered for challenging primary fixations. Further research is needed before recommending its widespread use in primary fracture management.
