    1. The effects of knee brace, biomechanical footwear device, and exercise in knee osteoarthritis: a randomized clinical trial.

    Knee osteoarthritis (OA) poses a significant economic and personal burden globally. To investigate the effectiveness of clinical interventions for knee OA. This randomized clinical trial was conducted at Central Finland Central Hospital. A total of 136 participants with mild-to-moderate knee OA participated in the intervention (60% women, mean age 61 years). The primary intervention lasted 4 months, with a 12-month follow-up. Participants were allocated to: (1) knee brace plus exercise, (2) biomechanical footwear device, or (3) exercise alone. The primary outcome was knee pain intensity measured using the Visual Analog Scale (VAS, 0-100 mm). Secondary outcomes included the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) pain, stiffness, and function subscales, and health-related quality of life assessed using the Short Form 36 Health Questionnaire (SF-36) physical and mental component scores. Knee pain decreased at both 4 and 12 months in all groups. Mean VAS reductions at 4 and 12 months were -48 and -38 mm in the knee brace plus exercise group, -32 and -28 mm in the footwear group, and -35 and -26 mm in the exercise group. A significant between-group difference favoring the knee brace plus exercise group was observed at 4 months (P = 0.009), but not at 12 months (P = 0.067). WOMAC pain scores also showed a between-group difference at 4 months (P = 0.042), favoring the knee brace plus exercise and footwear group over exercise alone, with no difference at 12 months. SF-36 physical component change scores favored the knee brace plus exercise and footwear groups at 12 months (P = 0.003), while no between-group difference was observed for the mental component. All interventions reduced knee pain. Knee brace combined with exercise showed greater short-term benefit, but the difference was not clinically significant. Findings should be interpreted cautiously, and further research is needed. This clinical trial is registered on ClinicalTrial.gov with ID NCT03684850.

    Moderate-to-severe traumatic brain injury (msTBI) is frequently accompanied by persistent sensory changes that can substantially impair quality of life. Theory suggests that these changes may not occur in isolation but instead be closely intertwined with psychological difficulties (eg, anxiety). A better understanding of the interrelationships between sensory changes and psychological problems may help identify mechanisms underlying symptom persistence and inform rehabilitation efforts. We conducted a psychometric network analysis on cross-sectional questionnaire data from 401 individuals with msTBI (mean years post-injury, 12.7; range = 0.4-35.4 years) originally recruited from an inpatient rehabilitation setting. We assessed sensory changes across modalities using a custom checklist and the Somatic Symptom Scale - 8. Guided by theory, 6 psychological problems were selected for analysis and assessed using scales from the Inventory of Depression and Anxiety Symptoms and the Comprehensive Assessment of Traits Relevant to Personality Disorders. Pairwise correlations indicated broad associations between sensory changes and most psychological constructs. When applying more stringent, multivariate network estimation methods, these associations narrowed to a more specific set of unique pathways. Across network models-and after adjusting for all other sensory changes, psychological problems, and covariates (age, sex, injury severity, and time since injury)-up to 10 direct conditional associations were identified between sensory changes and psychological problems, with the most robust links observed between light hypersensitivity and both health anxiety and traumatic avoidance. These findings support a biopsychosocial account in which biological injury mechanisms likely underpin sensory changes following msTBI, while psychological processes may differentially contribute to their expression, amplification, and maintenance. The results suggest heterogeneity across sensory domains, with some changes potentially reflecting relatively greater neurobiological contributions and others relatively greater psychological influences. This highlights the potential value of targeting specific psychological factors within rehabilitation for individuals experiencing sensory changes following msTBI. ACTRN12623001267640.

    Cognitive impairment is highly prevalent after stroke, and effective long-term rehabilitation strategies remain limited. Mindlenses Professional is a medical device that combines prism adaptation, delivered through wearable prismatic glasses, with tablet-based digital cognitive training using serious games. To evaluate the efficacy of Mindlenses Professional, compared with standard paper-and-pencil cognitive rehabilitation, on multiple cognitive outcomes in participants with stroke over a 6-month follow-up. In this randomized controlled clinical trial, 60 participants with stroke underwent 10 rehabilitation sessions over 2 weeks. Neuropsychological assessments were performed at baseline (T0), immediately after treatment (T1), and at 3- and 6-month follow-ups (T2-T3). Composite scores were computed for visual perception, hemineglect, language, memory, and executive functions. Longitudinal changes were analyzed using linear and generalized linear mixed-effects models, with group, lesion side, and time as fixed factors. Compared with standard rehabilitation (control group), the use of Mindlenses Professional induced greater and sustained improvements in hemineglect and language. The experimental group showed significant advantages for hemineglect at 3 months and for language at all posttreatment time points. In the memory domain, significant gains emerged from 3 months onward and further increased at 6 months. Executive functions improved in both groups, with a differential temporal pattern influenced by lesion side and a marked long-term advantage in participants with right-hemisphere stroke treated with Mindlenses. Visuoperceptual performance remained stable in the experimental group, whereas it declined over time in the control group. These findings support the integration of prism adaptation and digital cognitive training as a safe, feasible, and promising approach for long-term poststroke cognitive rehabilitation.     Over half of stroke survivors present with a cognitive deficit, often resulting in poorer outcomes and decreased quality of life. To provide an overview of randomized controlled trials (RCTs) of poststroke cognitive rehabilitation, we (1) examined study design and participant characteristics, including trends over time, sample sizes, methodological quality, sex distribution, time poststroke, and intervention type; and (2) recorded and compared outcome measures (OMs). This study provides an overview of English-language poststroke cognitive rehabilitation RCTs, based on data from 3 systematic reviews (neglect, aphasia, and general cognitive impairment). MEDLINE, Embase, CINAHL, PsycINFO, Scopus, and Web of Science were searched for English RCTs published up to December 31, 2024. For each systematic review, screening, data extraction, and quality assessment were conducted by 2 independent reviewers. Data extractions from the 3 systematic reviews were pooled into a single dataset for analysis. A total of 596 RCTs were analyzed, with a median sample size of 36 and 42% female participants. The number of RCTs and sample sizes increased steadily over time. However, there was no improvement in methodological quality, measured by PEDro scores, and female recruitment declined over time. Most RCTs targeting general cognition (54%) and neglect (82%) were conducted in the acute and subacute poststroke phases, whereas the majority of aphasia RCTs (57%) were conducted in the chronic phase. Technological interventions, including non-invasive brain stimulation and computerized training, have become more common in recent years. OMs used across trials showed substantial heterogeneity. Despite substantial growth in poststroke cognitive rehabilitation research, methodological quality, sex representation, and OM standardization have not improved in parallel. Persistent OM heterogeneity and misalignment with clinical guidelines limit knowledge translation, highlighting the need for adequately powered trials, and the development of aligned core outcome sets to support clinical decision-making.

    7. Psychometric properties of the System Usability Scale for digital rehabilitation technologies: a COSMIN systematic review.

    The System Usability Scale (SUS) is widely used in the rehabilitation field to assess the satisfaction dimension of digital technologies, although it was not specifically developed for this context. This systematic review aims to synthesize and appraise the psychometric properties of the SUS in the context of rehabilitation digital technologies, identify its strengths and limitations, and provide evidence-based recommendations for researchers and clinicians. We assessed methodological quality using the COSMIN Risk-of-Bias checklist (4-point scale), applying the "worst score counts" principle. The certainty of evidence was synthesized using a modified GRADE approach, downgrading for risk of bias, inconsistency, imprecision, and indirectness. We included 33 studies. Hypothesis testing for construct validity (n = 27) was rated Sufficient (+) with high certainty, showing expected correlations with satisfaction (rs = 0.38 to 0.49) and task performance (r = 0.91). Internal consistency (n = 8) was rated Sufficient (+) with reliability coefficients (α or ω) ranging from 0.63 to 0.91; however, evidence was not graded due to structural inconsistencies. Structural validity (n = 3) yielded an indeterminate result due to conflicting Confirmatory Factor Analysis indices (CFI from 0.816 to 0.960) and a lack of assessment for local independence and monotonicity. Notably, content validity assessment was entirely absent across all included studies. While the SUS demonstrates robust construct validity, uncertainty regarding its structural validity in rehabilitation contexts suggests that the total sum score cannot be fully trusted as a unidimensional measure. Furthermore, the lack of evidence for content validity is a major gap, as the scale's relevance for patients with cognitive or communication impairments remains unverified. Clinicians should interpret SUS results with caution, triangulating them with qualitative data to ensure specific clinical goals of effectiveness and efficiency are met.     Exercise is central to the nonpharmacological management of axial spondyloarthritis (axSpA), but the comparative effectiveness of different modalities remains uncertain. To compare seven exercise modalities in adults with axSpA, generate probabilistic rankings, and assess certainty of evidence. We searched PubMed, Embase, Web of Science, the Cochrane Library, CNKI, Wanfang, VIP, and CBM from inception to November 1, 2025, for randomized controlled trials of structured exercise in adults with axSpA. Interventions were classified as mobility exercise, strength exercise, aerobic exercise, Pilates, Tai Chi, Qigong, or yoga. Co-primary outcomes were the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), Bath Ankylosing Spondylitis Functional Index (BASFI), and Bath Ankylosing Spondylitis Metrology Index (BASMI). Secondary outcomes were visual analog scale (VAS) pain, Ankylosing Spondylitis Disease Activity Score based on C-reactive protein (ASDAS-CRP), 6-minute walk test (6MWT), erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and Ankylosing Spondylitis Quality of Life (ASQoL). We performed frequentist random-effects network meta-analyses and assessed certainty using CINeMA. Fifty-nine randomized trials involving 3404 participants were included. Heterogeneity was moderate to high for most outcomes. Compared with usual care, Pilates showed the largest effects on BASDAI (MD -1.72, 95% CI -2.79 to -0.66), BASFI (MD -1.65, 95% CI -2.74 to -0.56), and VAS pain (MD -9.60, 95% CI -13.81 to -5.38). Yoga also reduced VAS pain (MD -3.38, 95% CI -6.13 to -0.63). Qigong was the only modality associated with significant reductions in both ESR (MD -10.71, 95% CI -14.55 to -6.87) and CRP (MD -6.99, 95% CI -10.52 to -3.46). No modality significantly improved BASMI, ASDAS-CRP, 6MWT, or ASQoL. Rankings varied by outcome, and few active modalities differed significantly. Certainty was low to moderate for most comparisons. Exercise interventions improved disease activity, physical function, and pain compared with usual care. Pilates and Qigong showed the most favorable outcome-specific effects, but no modality was definitively superior. Exercise selection should be individualized. PROSPERO (CRD420251231497).     The questionnaires currently used to assess physical functioning in low back pain (LBP) are the Oswestry Disability Index (ODI), the Roland-Morris Disability Questionnaire (RMDQ), and the Quebec Back Pain Disability Scale (QBPDS). Whereas those tools had traditionally been validated according to the classical test theory, Rasch analysis - based on the item response theory - offers a more refined item-level assessment. However, despite recommendations, reporting standards for Rasch analyses remain inconsistent. This review aimed to describe the use of Rasch analysis to validate these tools and assess their reporting quality. A systematic search of PubMed, Embase, Google Scholar, and Cochrane was conducted without time restrictions, including studies in English or French analyzing Rasch models for the ODI, RMDQ, or QBPDS. Keywords included terms related to "low back pain," "validation," and "Rasch analysis." Of 107 eligible studies, 23 (22%) applied a Rasch analysis. The ODI (k = 11) and the RMDQ (k = 10) were more frequently analyzed than the QBPDS (k = 2). Most studies (87%) had Rasch analysis as a primary objective. Reporting rates varied widely. The software used (91%), unidimensionality testing (83%), and fit statistics (78%) were well documented, whereas response dependency (30%) and person fit tests (17%) were rarely reported. Although recommended to validate questionnaires of physical functioning in LBP, Rasch analysis is underused and inconsistently reported. Variable terminology and software output complicate comparisons. Insufficient critical appraisal tools hinder the adoption of methods. Standardized reporting guidelines and enhanced expertise are needed to improve the integration of Rasch into psychometric assessments. This systematic review and meta-analysis was registered in PROSPERO (registration number: 1157921, date of registration: 29 September 2025).
