1. Microbiome signatures and mechanistic pathways in pediatric obesity: from early-life risk to precision interventions.
1. 儿童肥胖的微生物组特征与机制通路:从生命早期风险到精准干预PMID:日期:2026-10-03The gut microbiota is increasingly recognized as a modulator of metabolic health in children, influencing nutrient absorption, immune tone, epithelial barrier function, and energy homeostasis. This review summarizes current evidence on microbial signatures and mechanistic pathways associated with pediatric obesity and evaluates microbiota-targeted strategies for prevention and intervention. Relevant studies published between January 2000 and February 2026 were identified through searches of PubMed, Embase, and Web of Science using keywords related to pediatric obesity, gut microbiota, microbial metabolites, and microbiota-based interventions. Human studies and mechanistic animal models examining host-microbe metabolic interactions were included. Pediatric obesity is associated with shifts in gut microbial composition, although taxonomic findings are heterogeneous across studies and should not be interpreted as universal biomarkers. Relatively consistent patterns include reduced Bifidobacterium and Akkermansia muciniphila, whereas associations involving Faecalibacterium, Blautia, and lactobacilli are context-, species-, and strain-dependent. Functional alterations include changes in short-chain fatty acid production, bile acid signaling, microbial branched-chain and aromatic amino acid metabolism, and endotoxin-related inflammatory pathways. The enrichment of Gram-negative taxa such as Enterobacteriaceae may contribute to impaired epithelial barrier integrity, lipopolysaccharide translocation, toll-like receptor 4 signaling, chronic low-grade inflammation, and insulin resistance. Early-life exposures, including cesarean delivery, formula feeding, and antibiotic use, are repeatedly associated with altered microbial succession and later obesity risk, although causality remains incompletely defined. Interventional studies indicate that dietary fiber enrichment and selected probiotic strains can improve microbial and metabolic outcomes in some settings, but efficacy remains strain-specific and clinically heterogeneous. Microbial and metabolic signatures are associated with pediatric obesity, converging on pathways of energy harvest, epithelial barrier dysfunction, inflammation, and disrupted host-microbe signaling. Integrative multi-omic and longitudinal studies are required to establish causality and guide the development of personalized, microbiota-based interventions for obesity prevention and treatment in children.

3. Correlation between perinatal asphyxia-induced brain injury, number of enlarged perivascular spaces, and sleep quality in later life.
[英文摘要] Perinatal asphyxia (PA) is increasingly recognized as a significant contributor to neonatal brain injury. Identifying reliable biomarkers for assessing the severity of injury is crucial. Enlarged perivascular spaces (EPVSs), which are considered imaging markers of glymphatic dysfunction are reliable indicators of disease severity and prognosis. However, the application of EPVSs among neonates remains unclear. This study aims to investigate whether numbers of EPVSs can serve as a novel biomarker for the severity of PA-induced brain injury (PABI) and the sleep quality in later life. Neonates (n = 319) with PA were categorized as mild (n = 229) or severe (n = 90). Clinical, amplitude-integrated electroencephalography (aEEG), and cranial magnetic resonance imaging (MRI) data were collected. The sleep quality was assessed using questionnaires via follow-up. Correlation analysis was performed to examine associations between numbers of EPVSs and clinical parameters. Multivariate linear regression analysis was performed to evaluate the independent correlation between numbers of EPVSs and severity of PABI. The correlation between numbers of EPVSs and later sleep quality in infants was also evaluated. There were multiple clinical parameters correlated with numbers of EPVSs. Univariate analysis revealed that physiological status, inflammatory markers, and numbers of EPVSs were related with the severity of PABI. Multivariable regression analysis found that numbers of EPVSs independently related with the severity of PABI [P = 0.0001 in all infants (MRI); P = 0.0248 in mild PA group (MRI); P = 0.0114 in severe PA group (MRI)]. In the severe PA group, numbers of EPVSs were significantly correlated with long-term sleep quality (P = 0.0061, r = 0.3365), a higher number of EPVSs was associated with more severe sleep disturbance. Numbers of EPVSs may serve as a novel imaging biomarker for evaluating the severity of brain injury after PA; this biomarker can predict sleep disorders later in life. Collectively our data suggests that glymphatic dysfunction may contribute to post asphyxia-induced brain injury. Early evaluation of EPVSs could aid in risk stratification and intervention strategies.

5. Discrimination performance of the Phoenix sepsis score versus pediatric sequential organ failure assessment for predicting in-hospital mortality in critically ill children: a multicenter external validation study.
5. Phoenix脓毒症评分与儿童序贯器官衰竭评估对危重患儿院内死亡预测的区分性能:一项多中心外部验证研究PMID:日期:2026-09-22This study compared the prognostic accuracy of the Phoenix sepsis score (PSS) and the pediatric sequential organ failure assessment (pSOFA) score for predicting in-hospital mortality in critically ill children with suspected infection, with a focus on neonatal and hematology sub-cohorts. This retrospective, multicenter cohort study analyzed data from 2374 children (< 18 years) with suspected infections admitted to 13 intensive care units (ICUs) between 2023 and 2025. Two predefined sub-cohorts were examined: the neonatal sub-cohort (≤ 28 days; n = 576) and the hematology sub-cohort (n = 494). PSS and pSOFA scores were calculated using the worst available physiological and laboratory data within 24 hours of ICU admission. The primary outcome was in-hospital mortality. Prognostic accuracy was assessed mainly via the adjusted area under the receiver operating characteristic curve (AUROC). Compared with the pSOFA score, the PSS demonstrated significantly greater discrimination for mortality (adjusted AUROC: 0.840 vs. 0.817; P < 0.001) in the overall cohort (n = 2374) and the hematology sub-cohort (0.718 vs. 0.682; P = 0.006) but not in neonates (0.915 vs. 0.850; P = 0.222). At the ≥ 2-point threshold, both scores maintained high sensitivity (> 80%) but low positive predictive value (PPV) (≤ 50%) across all cohorts. In the high-event-rate hematology sub-cohort, the PSS had an approximately 5% lower sensitivity for a meaningfully higher PPV than the pSOFA score. In contrast, within the low-event-rate neonatal sub-cohort, both tools showed equivalent sensitivity and similarly low PPVs (< 8%). In this study, the PSS demonstrated superior predictive performance in the overall cohort, whereas no statistically significant difference was observed between the two scores in the neonatal sub-cohort. In the hematology sub-cohort, both scores showed limited discriminative ability.

7. Integrating optimal patient-centered care: patient- and family-reported experiences after complex treatment for rare diseases.
PMID:日期:2026-09-19Although life expectancy after complex treatments for rare diseases during childhood has increased, long-term medical, psychosocial and societal impacts remain for children and their families. Evaluating patient and family experiences is essential for the development and optimization of integrated patient-centered, value-based care pathways. This study aims to evaluate patient- and family-reported experiences across three follow-up care pathways for complex treatment of rare diseases. This cross-sectional study was conducted among patients/families in follow-up care after (1) fetal and/or neonatal intensive care treatment; (2) pediatric hematopoietic stem cell transplantation for non-malignant conditions; and (3) cardiac intervention for congenital heart disease. A combination of validated patient-reported experience measures (PREMs) was used to capture experiences with complete integrated care pathways regarding information provision, integrated care collaboration, communication and comprehensive care. Descriptive statistics were used to analyze quantitative ratings, whereas free-text responses were analyzed using thematic analysis. Between April 2024 and June 2025, 70 families completed the PREMs. Likert-scale ratings, as well as an analysis of 286 free-text responses, revealed positive evaluations of relationship building, the humanistic approach and communication across all pathways. The identified areas of improvement varied across care pathways and included comprehensive (long-term) information provision and screening and support for the psychosocial impact on patients as well as their families. Through shared learning across care pathways, insights from patient and family experiences guide patient- and family-centered, value-based care pathway optimization to ultimately improve outcomes and quality of life for children and their families.

8. Association between prenatal exposure to acid-suppressive medications and subsequent risk of autoimmune diseases in offspring: a nationwide birth cohort study in South Korea.
PMID:日期:2026-09-17Although acid-suppressive medications are commonly used during pregnancy and may influence immune development in offspring, studies examining their association with pediatric autoimmune diseases remain limited. Therefore, we aimed to investigate the association between prenatal exposure to acid-suppressive medications and the risk of autoimmune diseases in offspring, using a nationwide birth cohort from South Korea. This large-scale, nationwide mother-child cohort study in South Korea was conducted from January 1, 2009, to December 31, 2023, using data from the National Health Insurance Service of Korea. We evaluated the risk of autoimmune diseases in offspring following prenatal exposure to acid-suppressive medications, including histamine-2 receptor antagonists (H2RAs) and proton pump inhibitors (PPIs). The outcome was defined as the occurrence of systemic autoimmune diseases, autoimmune endocrine diseases, autoimmune thyroid diseases, autoimmune hepatitis, and other autoimmune conditions, classified according to International Classification of Diseases, 10th Revision codes (ICD-10). After applying propensity score-based overlap weighting, we used Cox proportional hazards models to estimate the risk, presenting adjusted hazard ratios (aHRs) with 95% confidence intervals (95% CIs). Sensitivity analyses were conducted by medication type, pregnancy trimester of exposure, time-dependent risk, and through various stratifications. In addition, we accounted for within-family clustering in the sibling comparison analysis with each mother treated as a separate stratum. Among the 3,012,992 mother-child pairs initially identified, a total of 2,777,119 children met the inclusion criteria. Of these, 507,845 pairs were exposed to prenatal acid-suppressive medications, and the mean follow-up duration was 9.6 years [standard deviation (SD): 2.4]. In the overlap-weighted cohort (404,550 pairs exposed and 404,551 pairs unexposed), prenatal exposure to acid-suppressive medications was modestly associated with increased risks of systemic autoimmune diseases (aHR: 1.11, 95% CI 1.01-1.21) and other autoimmune diseases (aHR: 1.09, 95% CI 1.05-1.12), such as lupus erythematosus, psoriasis, and rheumatoid arthritis. However, the high numbers needed to harm (37,716 and 6102 per year, respectively) indicate that the absolute excess risks were small, with insignificant sibling-comparison findings (aHR: 0.89, 95% CI 0.77-1.03, and aHR: 0.97, 95% CI 0.92-1.02, respectively). For any autoimmune disease, first-trimester exposure was associated with a slightly elevated risk (aHR: 1.09, 95% CI 1.06-1.13). Modest associations were observed between prenatal exposure to acid-suppressive medications and pediatric autoimmune diseases; however, the low absolute excess risks and null findings from the sibling comparison analysis, which provides stronger control for unmeasured shared familial factors, indicate limited clinical concern. Nevertheless, exposure during the first trimester may warrant careful clinical consideration.

10. Safety of sirolimus in pediatric vascular anomalies: a BKMR-PopPK exposure-response framework for individualized risk and dosing.
10. 西罗莫司在儿童血管异常中的安全性:基于BKMR-PopPK暴露-反应框架的个体化风险与给药方案PMID:日期:2026-09-17Vascular anomalies are rare, heterogeneous disorders that threaten the health of children. Sirolimus is an important drug therapy, but its high incidence of adverse drug reactions (ADRs) may cause treatment discontinuation and therefore poor disease control. Although prior studies suggest a strong link between sirolimus exposure and ADRs, the quantitative relationship remains unclear. This study aimed to systematically quantify the relationships between steady-state trough sirolimus concentrations (C) and three ADRs: myelosuppression, abnormal liver function, and dyslipidemia. We retrospectively analyzed the data using Bayesian kernel machine regression (BKMR) to characterize associations of C and its determinants with ADRs, and further developed an integrated BKMR-population pharmacokinetic (PopPK) framework for quantitative risk assessment and prediction. A total of 257 patients were involved in this study (766-892 blood samples). BKMR results showed the quantitative exposure-response relationships between C and ADRs, identifying disease complexity and duration of treatment as key risk modifiers. We recommend routine C monitoring within safety windows: 5.0-7.5 ng/mL for simple lesions or milder disease; 8.0-10.2 ng/mL for complex or mixed vascular malformations. Stricter ADR surveillance is warranted in complex malformations, and during long-term therapy (> 6 months), vigilance for myelosuppression and dyslipidemia through regular screening is also warranted. This is the first study to quantify the relationship between C and ADRs in pediatric vascular anomalies. The integrated BKMR-PopPK framework provides a tool for translating pharmacological insights into practice and advancing model-informed precision dosing (MIPD). These findings support therapeutic drug monitoring (TDM)-based target ranges, addressing a knowledge gap and reducing reliance on empirical dosing. This paradigm also informs safety evaluation and early-phase trials in other pediatric rare diseases with limited samples.
