Visual attention deficits are common but under-recognized in Parkinson's disease (PD), and we investigate whether theta-frequency (5 Hz) STN-DBS differentially modulates visual attention compared with high-frequency (130 Hz) stimulation in PD patients. Thirty-nine PD patients with bilateral STN-DBS perform an attentional capture task under three stimulation conditions (OFF, low-frequency, high-frequency). Patients are stratified into impaired ( = 21) and normal ( = 18) visual attention groups based on their accuracy in the AC-distractor task during OFF-stimulation, with <90% accuracy defining impairment. Behavioral accuracy and reaction time are analyzed using Bayesian mixed-effects models. P300 amplitudes and latencies are measured using electroencephalography. In the impaired group, 5 Hz stimulation significantly improves accuracy in the distractor condition compared with OFF (BF > 1,000), whereas 130 Hz stimulation reduces accuracy (BF = 122.8). This behavioral pattern is paralleled by enhanced P300 amplitude and prolonged latency under 5 Hz stimulation, and suppressed P300 amplitude under 130 Hz stimulation. No significant stimulation effects are observed in the normal group. Theta-frequency STN-DBS selectively improves visual attention in PD with pre-existing attentional deficits, likely by enhancing top-down attentional resource allocation, while high-frequency stimulation impairs performance.

People with Parkinson's disease (PwPD) may experience speech changes early in the disease process, yet it is unclear how they perceive them. Moreover, in PD specifically, the relationship between speech, cognition, and swallowing is not well-established. Therefore, this study aimed to determine the association between self-perceived speech impairment, clinician-perceived speech impairment, cognitive impairment, and swallowing impairment in PD. Data were obtained from the Parkinson's Progressive Markers Initiative. We included baseline visit data from 1072 PwPD. An ordinal logistic regression was performed to determine what clinical measures were associated with self-perceived speech impairment. The ordinal logistic regression found that self-perceived speech impairments were significantly associated with clinician-perceived speech impairment (Wald χ[3] = 44.039, < 0.001), self-reported swallowing impairment (Wald χ[3] = 61.863, < 0.001), cognitive impairment (Wald χ[4] = 32.293, < 0.001), and being male (Wald χ[1] = 11.520, < 0.001). This preliminary work showed potential relationships between self-perceived and clinician-perceived speech impairment, swallowing impairment, and cognitive impairment, demonstrating the multidimensional nature of self-perceived speech impairment. More research using more extensive assessment methods is needed to further elucidate these relationships in PD.

This study aimed to examine diffusion alterations indexed by the diffusion tensor image analysis along the perivascular space (DTI-ALPS) in patients with Parkinson's disease presenting with subjective cognitive decline (PD-SCD), and to investigate their associations with impairments across specific cognitive domains. This is a cross-sectional study. A total of 607 eligible participants were enrolled and categorized into three groups: healthy controls (HC), Parkinson's disease with normal cognition (PD-NC), and PD-SCD. The DTI-ALPS index was calculated as a diffusion-based metric reflecting water diffusivity along the perivascular space relative to adjacent fiber tracts. To ensure the robustness of the findings, Region of Interest (ROI) delineation was performed and validated using both manual anatomical delineation and JHU atlas-based methods. Statistical analyses included inter-group comparisons, Receiver Operating Characteristic (ROC) curves to assess predictive performance for PD-SCD, and partial correlation analyses between ALPS indices and cognitive domains. The DTI-ALPS indices were significantly lower in the PD-SCD group compared to both PD-NC and HC groups, exhibiting a significant stepwise decline (HC > PD-NC > PD-SCD). ROC analysis showed that the left ALPS index (AUC = 0.6503) and the mean ALPS index (AUC = 0.6452) had modest discriminatory performance in differentiating PD-SCD from PD-NC. Partial correlation analysis revealed that ALPS indices were specifically and significantly associated with attention and working memory deficits. Notably, the side of motor symptom onset in PD did not significantly influence ALPS values. High consistency was observed between the manual and atlas-based ROI methods. In this cross-sectional cohort, PD-SCD was associated with a lower DTI-ALPS index, and this diffusion alteration was related to attention and working-memory performance. These findings suggest that the DTI-ALPS index may serve as a potential diffusion-based marker for early cognitive risk stratification in PD; however, its precise biological basis and longitudinal significance require further investigation.

Age-related declines in lower-limb somatosensory function may contribute to impaired motor control and increased fall risk in older adults. Although plantar sensory stimulation has been reported to improve postural control, its effects on neural processing remain unclear. This study investigated whether repeated plantar electrical stimulation modifies lower-limb somatosensory processing in older adults using somatosensory evoked potentials (SEPs). 83 older adults aged ≥65 years were assigned to either a plantar electrical stimulation group (PES, = 37) or a non-intervention control group (CON, = 46). The PES group performed home-based plantar electrical stimulation for 8 weeks (30 min/session, ≥2 days/week). SEPs elicited by tibial, peroneal, and median nerve stimulation were recorded before and after the intervention. SEP latencies and amplitudes were analyzed using mixed-design ANOVA, and ANCOVA was performed for components showing baseline group differences. In the PES group, P37 latency elicited by tibial nerve stimulation and P37 and N50 latencies elicited by peroneal nerve stimulation were shorter at Post than at Pre, whereas no significant changes were observed in the CON group. ANCOVA confirmed that the group effect remained significant for P37 latency during tibial and peroneal nerve stimulations after adjustment for baseline values. Significant negative correlations were observed between baseline latencies and intervention-related changes, although these findings should be interpreted cautiously because change scores include baseline values. No significant intervention effects were observed for SEP amplitudes during lower-limb stimulation, and median nerve SEPs showed no comparable latency shortening. Repeated plantar electrical stimulation was associated with shortened early SEP latencies elicited by lower-limb nerve stimulation. No comparable latency shortening was observed in median nerve SEPs, suggesting that the observed changes were more prominent in lower-limb than in upper-limb somatosensory processing. These findings suggest that repeated plantar electrical stimulation may be associated with changes in the temporal characteristics of lower-limb somatosensory processing in older adults. Further studies should determine whether these neurophysiological changes are associated with improvements in balance, mobility, and other functional outcomes.

Aging is associated with alterations in white matter structure and decline in bimanual performance. These parallel changes may be interrelated, as previous studies have linked cerebral white matter properties to both motor and cognitive function. The cerebellum plays a central role in complex motor coordination and higher-order processes, yet age-related differences in cortico-cerebellar pathways remain insufficiently understood. This study therefore examined two major cortico-cerebellar tracts - the dentato-rubro-thalamo-cortical tract (DRTT) and the cortico-ponto-cerebellar tract (CPCT) - to assess age-related differences in white matter properties and their association with bimanual motor performance. Diffusion Magnetic Resonance Imaging data from 46 younger and 46 older healthy adults were analyzed using a state-of-the-art fixel-based analysis approach to derive fiber-specific metrics reflecting micro- and macrostructural axonal properties. An overall reduction in fiber density and cross-section was observed in older adults, primarily driven by macrostructural differences; alongside localized increases in these structural properties in specific regions. No significant associations were found between cortico-cerebellar white matter properties and bimanual motor performance. These findings corroborate the general patterns of age-related white matter decline while underscoring the importance of within-tract analyses. Although cortico-cerebellar pathways demonstrate age-related structural differences, their contribution to age-related alterations in bimanual coordination performance in healthy aging may be limited or obscured by tract-level averaging masking regional effects and the multifactorial nature of bimanual motor coordination, which reflects the interaction of distributed neural and peripheral systems. The observed regional increases of fiber-specific measures in older adults may reflect age-related adaptations. Yet, their functional significance remains uncertain and warrants further investigation.

Reproductive life stages, menopausal symptoms, and hormonal exposures are increasingly recognized as potential determinants of cognitive outcomes in women. However, these associations remain largely understudied in Latin American and Caribbean (LAC) populations. In this systematic review, we identified 18 observational studies published between 2003 and 2025. Of these, 16 were conducted across 13 LAC countries and two were conducted among latina women in the United States. Evidence was organized across four reproductive life stages domains: reproductive history, menopausal hormone therapy use, type of menopause and menopausal symptoms, and postmenopausal factors. Most LAC studies were cross-sectional and demonstrated substantial methodological heterogeneity. Across studies, shorter reproductive span, earlier menopause, higher parity, surgical menopause, and greater menopausal symptom burden were generally associated with poorer cognitive performance or higher odds of mild cognitive impairment. Some studies suggested favorable associations between menopausal hormone therapy use and cognitive outcomes. Postmenopausal factors, such as sarcopenia and obesity, as well as a longer duration of menopause, were predominantly linked to poorer cognitive outcomes. Only one population-based cohort evaluated incident dementia, underscoring the scarcity of longitudinal evidence in the region. Overall, the evidence suggests that reproductive life stages in LAC women are meaningfully associated with cognitive outcomes, reflecting the influence of hormonal transitions, menopausal characteristics, and postmenopausal health within distinctive regional contexts. However, the predominance of cross-sectional designs and limited regional representativity, coupled with a lack of integrated physiological indicators, reveal a substantial knowledge gap. Longitudinal and regionally representative cohorts incorporating standardized assessments of reproductive life stages alongside comprehensive cognitive measures are essential to understanding how reproductive life stages shape cognitive outcomes in LAC women. : The systematic review was registered in the Open Science Framework (OSF). White matter hyperintensities (WMH) are key neuroimaging markers of cerebral small vessel disease, associated with cognitive decline and stroke risk. Accurate quantification is essential yet manual segmentation is time-consuming and variable. We systematically evaluated AI performance for automated WMH segmentation across disease contexts and identified factors influencing model performance. Following PRISMA guidelines, we searched PubMed, Embase, Web of Science, Scopus, and IEEE Xplore from database inception to February 3, 2026 (PROSPERO: CRD420261355769). Studies employing AI-based WMH segmentation and reporting Dice similarity coefficient (DSC) were included. A random-effects meta-analysis was conducted, along with subgroup analyses, meta-regression, sensitivity analyses, and publication bias assessment. 26 studies comprising 4,288 participants contributed to quantitative syntheses: 13 evaluated WMH segmentation in general cohorts, and 13 evaluated AD or CSVD-specific cohorts. In the general cohort meta-analysis, the pooled DSC was 0.78 (95% CI, 0.74-0.82). Disease-specific pooled DSC estimates were 0.75 for AD and 0.78 for CSVD. Exploratory meta-regression suggested an association between scanning parameters and DSC ( = 0.042), whereas magnetic field strength showed a borderline association ( = 0.052). AI demonstrates promising performance for automated WMH segmentation, with U-Net-based architectures showing superior accuracy. However, substantial heterogeneity highlights the need for standardized imaging protocols and robust external validation. Future multicenter studies are essential to improve model generalizability and facilitate clinical translation.

Gait impairment in cerebral small vessel disease (CSVD) is heterogeneous and may not be fully explained by conventional structural MRI markers. We investigated the associations of neuroimaging burden, cognitive function, and oculomotor function with gait performance in CSVD and explored potential indirect associations between CSVD burden and gait impairment. We included 65 individuals with CSVD and 35 control participants. All participants underwent quantitative gait assessment, standardized neuropsychological evaluation, and oculomotor recording. Patients with and without gait disorder were compared after adjustment for the total CSVD load score. Correlation analyses assessed associations among neuroimaging markers, cognitive and oculomotor measures, and gait parameters. Exploratory mediation analyses examined cognitive and oculomotor measures as potential statistical indirect pathways between total CSVD load and gait performance. Compared with control participants, patients with CSVD had poorer gait, cognitive, and oculomotor performance ( < 0.05). Despite comparable total CSVD load, patients with gait disorder showed worse executive function and visuospatial ability, prolonged saccade latency, reduced fixation stability, and lower anti-saccade accuracy (FDR-adjusted < 0.05). Total CSVD load and perivascular spaces (PVS) burden were associated with cognitive, gait, and oculomotor measures. In the exploratory mediation analyses, Trail Making Test-B performance showed a significant indirect association between total CSVD load and gait impairment, with no significant residual direct association. Trail Making Test-A performance, fixation stability, and saccade latency, also showed significant indirect association, while the corresponding direct associations remained significant. Executive and oculomotor dysfunction were associated with gait impairment in CSVD and may partly account for the association between total CSVD burden and gait performance. These exploratory findings require confirmation in larger longitudinal studies.

Behavioral variant frontotemporal dementia (bvFTD) is an irreversible neurodegenerative disorder characterized by progressive changes in personality and behavior. Magnetic Resonance Imaging (MRI) is widely used to detect and assess structural brain alterations associated with the disease. However, due to the low prevalence of bvFTD among neurodegenerative diseases causing the dementia syndrome, conventional machine learning approaches may struggle to capture comprehensive feature representations. Therefore, this study proposes two late fusion frameworks that integrate a 3D convolutional neural network and a multilayer perceptron (MLP) for improved bvFTD diagnosis. A total of 5,928 participants were included, comprising 3,415 healthy controls (HC), 2,276 Alzheimer's disease (AD), and 237 bvFTD, resulting in a class imbalanced setting with bvFTD as the minority class. To address class imbalance, bvFTD data were initially augmented. A 3D-DenseNet was used to extract features from 3D T1-weighted MRI scans, while an MLP-based model was applied to regional brain volumetric measurements obtained from automated MRI-based brain segmentation. Twelve CNN models with different hyperparameter configurations were trained. Models with and without data augmentation, as well as two fusion-based approaches, were evaluated using accuracy, F1-score, and area under the curve (AUC). Both fusion strategies improved accuracy, F1-score, and AUC compared to the baseline model without data augmentation. Notable improvement was also observed for the bvFTD class, with up to a 120% increase in F1-score. In one of the fusion frameworks, an accuracy of 0.95 ± 0.01 was achieved for bvFTD vs. HC classification. The results demonstrate the effectiveness of the fusion-based approaches compared to non-fused models, outperforming several state-of-the-art methods. The proposed frameworks demonstrate that data augmentation and fusion strategies can improve accuracy, F1-score, and AUC, with statistically significant gains. Overall, the frameworks improve diagnostic performance and support the identification of relevant biomarkers associated with bvFTD pathology.

Neuroinflammation is a key driver of Alzheimer's disease (AD). Although the neutrophil-to-lymphocyte ratio (NLR) is an easily accessible peripheral inflammatory marker, yet its longitudinal association with domain-specific cognitive decline and the underlying neuropathological mechanisms during the preclinical and early stages of AD remain unclear. A total of 1,190 non-people living with dementia older adults from the AD Neuroimaging Initiative cohort were included, comprising 405 cognitively normal participants and 785 participants with mild cognitive impairment (MCI). Participants were stratified according to baseline NLR (≥ 3versus < 3). Linear mixed-effects models were used to examined the association between baseline NLR and longitudinal decline across multiple cognitive domains. Cox proportional hazards model assessed progression risk to MCI or AD. Mediation analyses evaluated whether cerebrospinal fluid (CSF) biomarkers, including amyloid-β (Aβ), total tau (t-tau), and phosphorylated tau (p-tau), statistically accounted for the association between NLR and cognitive decline. During follow-up, a high baseline NLR (≥ 3) was independently associated with accelerated decline in global cognition, memory, executive function, language, and visuospatial abilities (all < 0.05), as well as a 20% higher risk of clinical progression per 1-unit increase in NLR (hazard ratio = 1.20, 95% confidence interval: 1.12-1.29, = 0.001). Mediation analyses indicated that CSF t-tau and p-tau, but not Aβ, partially explained the association between NLR and cognitive decline. The observed associations remained robust across sensitivity and subgroup analyses. Elevated NLR was significantly and independently associated with accelerated decline across multiple cognitive domains and an increased risk of clinical progression in older adults at risk for AD. The observed associations with tau pathology are consistent with a potential role for peripheral immune dysregulation in early disease trajectories; however, formal predictive modeling and external validation are required before clinical application.
